2.Clinical observation onpreventive treatment of primary dysmenorrhea with acupuncture of regulating Qi and relieving pain

Objective To study the effect of preventive treatment of primary dysmenorrhea (belonging to syndrome of stagnation of Qi and blood) by using “acupunctureof regulating Qi and relieving pain”. Methods 60 patientsfrom January 2013 to November 2013(in acupuncture and moxibustion department of Beijing Chinese Medicine Hospital affiliated to Capital Medical University), with Qi and blood stasis type of primary dysmenorrhea were randomly divided into a treatment group (formula of regulating Qi and relieving pain)and a control group(routine acupuncture group), 30 cases in each group. The treatment group was treated with acupuncturing onlieque,fenglong,ligou as the main points while the control group withhegu, diji, sanyinjiao as the main points. All patients received 3 times of acupuncture treatment at the beginning of the week before menstruation and were treated three menstrual cycles altogether. VAS scores, length of pain and CMSSⅡscores during baseline period and three menstrual cycles were observed in order to determine the clinical effects. Results VAS score, length of pain and CMSSⅡscores were significantly reduced than before(P<0.01)[the treatment group in cycle 1 was(5.05±0.98), (14.03±8.37)h, (12.77±3.64), in cycle 2 was (4.18±0.87), (12.53±7.13)h, (12.07±3.69), in cyce 3 was(3.43±0.82), (11.50±6.88)h, (11.33±3.64);the control group in cycle was 1(5.00± 1.05), (14.97±9.19)h、(14.40±3.5), in cycle 2 was(4.25±0.93), (13.83±9.12)h, (13.70±3.43), in cycle 3 was(3.73±0.91), (12.63±9.02)h, (13.03±3.41)];Compared with the control group, VAS scores intreatment groupdecreased more significantly (P<0.05), while reduced level of pain time and CMSSⅡscores showed no significant difference between the two groups (P>0.05). Conclusion Acupuncture of regulating Qi and relieving painmay effectively relieve pain, shortenlength of pain, improve symptoms of dysmenorrhea severity. It has a significant preventive treatment for Qi and blood stasis type of primary dysmenorrhea.
3.Application of Double-universal Tablet Cases in Efficiency Improvement of Automatic Medicine Packaging Machine in Our Hospital

OBJECTIVE:To shorten time consuming for dispensing mode of automatic medicine packaging machine(ATC), and improve the efficiency of ATC. METHODS:In the inpatient pharmacy of our hospital,previous dispensing mode that two ATC matched with one tablet case separately was replaced by new mode,i.e. one ATC(another ATC was on standby)matched with two tablet cases(one of them added medicine beforehand),and the information of drug-position coordinate was equipped. The time con-sumed,average daily number of packages,financial expenses and labor costs were recorded and analyzed to evaluated the effect of new mode. RESULTS & CONCLUSIONS:Compared with previous mode,new dispensing mode improve the efficiency of ATC greatly. In previous mode,two ATCs and four pharmacists work simultaneously and spend 4.5 hours for 3 850 bags of medicine, which cost 4 400 000 yuan;in new mode,only one ATC and three pharmacists work simultaneously,and spend 3.0 hours for 4 015 bags of medicine,which cost 2 350 000 yuan. Therefore,double-universal tablet cases greatly improves the efficiency of ATC and reduce the financial expenses as machine maintenance and labor cost.
4.Cross-section study of early renal damage in patients with chronic hepatitis B

Objective To investigate early renal damage of chronic hepatitis B (CHB) patients and the risk factors related to their renal function. Methods CHB patients who visited the second people’s hospital but did not receive systemic treatment were enrolled in our study. Those who visited for general check-up with no hepatic findings during the same period were selected as control group. Glomerular filtration rate (GFR) of all the participants were estimated by simplified MDRD equation and CKD-EPI equation (designated as M-eGFR and C-eGFR respectively). Influence factors of eGFR were statistically analyzed. Results In the total 528 cases in CHB group, 88 (16.67%) and 62 (11.74%) suffered declined M-eGFR and C-eGFR respectively. By contrast, 10 (8.77%) and 6 (5.26%) cases in the total 114 cases in control group present declined M-eGFR and C-eGFR ac?cordingly. Percentages of renal function impairment, estimated by both M-eGFR and C-eGFR, were higher in the CHB group than those in control group. The difference was statistically significant (χ2=4.518, P<0.05;χ2=4.156, P<0.05). Multiple linear regression analysis indicated that age, HBsAg and body mass index (BMI) were risk factors of M-eGFR while age, HBsAg, gender and serum albumin were risk factors of C-eGFR. On the other hand, HBV-DNA and HBeAg were not risk factors for M-eGFR or C-eGFR. Conclusion HBV infection can lead to early renal damage. Age and HBsAg are main risk factors of renal function impairment. Therefore, renal function should be scrutinized in CHB patients.
5.Comparison and bias analysis of BUN, Cr and UA between two different biochemical analysis systems

Objective To discuss the comparability of blood urea nitrogen (BUN),creatinine (Cr),uric acid (UA) results and provide basis for result concordance in different biochemical systems.Methods The within-run precision and the between-day precision for the median and high values of quality control materials were tested in Beckman LX-20 and Hitachi 7600 chemistry analyzer respectively,and the evaluation criteria were according to the precision requirement of manufacturer and our laboratory.According to American Clinical and Laboratory Standards Institute (CLSI) document EP9-A2,serum BUN,Cr,UA from 40 patients were detected by Hitachi 7600 chemistry analyzer as experimental method and by Beckman LX-20 chemistry analyzer as comparative method.The relative bias ( SE%) between experimental method and comparative method was calculated and the concordance of biochemical analysis system was determined according to EQA requirements and acceptable performance criteria Results The between-run and between-day precision coefficients of variation for BUN,Cr and UA in two biochemical analysis systems were lower than the precision requirement of manufacturer and our laboratory.In Hitachi 7600 chemistry analyzer,the SE% for BUN at three levels of medical decision were 9.4%,2.0% and 0.8%,the SE% for Cr were 7.3%,3.8% and 2.8%,the SE% for UA were 7.9%,1.7% and 1.2%.Two biochemical analysis systems were not comparable in BUN analysis because SE% of serum BUN levels detected by Hitachi 7600 at low medical decision level exceeded the specified range.The remaining items between two biochemical analysis systems were concordant.Conclusion While the same test is detected by more than two systems in one laboratory,it is necessary to do method comparison and bias evaluation for concordance in order to ensure the comparability among test results.

7.Assessment of short-term effect of cardiac resynchronization therapy by speckle tracking strain imaging

Objective To assess the short-term effect of cardiac resynchronization therapy (CRT) by speckle tracking strain imaging(STI). Methods Sixteen chronic heart failure patients(CHF) with CRT were enrolled. The time to peak systolic longitudinal strain and the time to peak systolic radial strain were derived from the LV apical views, and the time to peak systolic circumferential strain was derived from the parasternal short axis. The standard deviation for time to peak longitudinal, radial and circumferential strain in the 18 segments (LS-SD18, RS-SD18, CS-SD18) and the maximal temporal difference of any two segments(LS-dif, RS-dif, CS-dif) were calculated as a strain-derived dyssynchrony index. The difference of LS-SDIS, RS-SD18, CS-SD18 between 1 month,3 months after CRT and baseline (△LS-SD18, △RS-SD18, △CS-SD18) was calculated. The correlation between △LS-SD18,ARS-SD18,△CS-SD18 and the reduction of end-systolic volume at follow-up(△ESV%) were analyzed. Results The LS-SD18, RS-SD18,CS-SD18 and RS-dif were decreased significantly 1 month,3 months after CRT.△RS-SD18 was correlated with △ESV% 3 months after CRT(r = 0. 694). Conclusions The strain-derived dyssynchrony index is great significient for short-term effect of CRT.
8.Quantitative evaluation of short-term effect of cardiac resynchronization therapy by automated motion tracking of mitral annular displacement

Objective To evaluate the tissue motion tracking of mitral annular displaeement(TMAD) method in the assessment of short-term effect of cardiac resynchronization therapy (CRT). Methods Twenty six chronic heart failure (CHF) patients with CRT were detected by TMAD. The maximal displacement of the six points of mitral annulus(DS), the standard deviation of the time to regional peak systolie displacement of the six points (Td-sd) and the maximal temporal difference of any two points(Tds-diff) were compared before and one month after CRT. And the correlation between mean displacement and left ventricular ejection fraetion(LVEF),left ventricular end systolic volume(LVESV), fraction area change (FAC) was analyzed. Results Compared with the pre-operatively, the displacement and the mean displaeement of the six points of mitral annulus were significantly increased(P<0.01),Tds diff and Td-sd were decreased(P<0.05) one month after CRT. TMAD correlated with LVEF,LVESV and FAC (r =0.419, r = 0.529, r = 0.567, P <0.05) one month after CRT. Conclusions TMAD is a new technology to evaluate the short-term effect of CRT.
9.Evaluation of the short-term effect of cardiac resynchronization therapy to reserve left ventricular remodeling in heart failure with echocardiography

Objective To determine which parameters of reverse remodeling of left ventricular could become the effective indexes in evaluating the short-term therapeutic effect of cardiac resynchronization therapy(CRT)in heart failure(HF)patients.Methods CRT was performed in 26 HF patients with dysfunctions of wall motion.Serial echocardiography was practiced at baseline,one and three months after CRT.The parameters including left atrial end-diastolic diameter(LADD),left ventrieular end-diastolic diameter(LVDD),end-diastolic volume (LVEDV),end-systolic volume(LVESV),ejection fraction(LVEF)were measured and compared before and after CRT therapy.Results At one and three months after CRT,respectively,CRT was associated with reduced LADD,LVDD,LVEDV,LVESV and improved LVEF and filling time compared with those at baseline.Moreover,all these parameters had better correlations with the activities of these patients.Conclusions LADD,LVDD,LVEDV,LVESV and LVEF could become the effective indexes in evaluating the short-term effect of CRT in HF patients.The reduced diameters and volumes of left atria and left ventricle are more sensitive parameters than the improved LVEF.
10.Inhibiting effect of alcohol extract from Dioscore bulbifera on gastric cancer cells

Objective To investigate the inhibiting effects of alcohol extract from Dioscore bulbifera on proliferation, colony formation and migration of cancer cell lines. Methods Alcohol extract from Dioscore bulbifera was prepared using Soxhlet extraction. Human gastric cancer cell line MGC803 was treated with different concentrations(0, 60, 120 mg/L)of al?cohol extract from Dioscore bulbifera. In vitro, proliferation, colony formation and migration of gastric cancer cells were detect?ed by MTT, colony formation experiments and Transwell assay respectively. Results The proliferation(day2-day 6, F=29.130, 21.864, 67.826, 36.015, 43.656, P<0.01)and colony formation(F=11.918,P<0.01)of gastric cancer cells were significantly inhibited by administration of alcohol extract from Dioscore bulbifera at both 60 mg/L and 120 mg/L . The migra?tion(F=4.258,P<0.05)of gastric cancer cells were significantly suppressed after cells were treated with120 mg/L alcohol ex?tract from Dioscore bulbifera. Conclusion Alcohol extract from Dioscore bulbifera significantly inhibit proliferation, colony formation and migration of gastric cancer cells.
