1.A clinical analysis of cholesterol granuloma following chronic suppurative otitis media

Objective:To investigate the pathophysiological mechanism,clinical menifestation and radiographic diagnosis of cholesterol granuloma following chronic supperative otitis media.Method:Six cases of CG following chronic suppurative otitis media, confirmed by surgery and pathology,were reviewed and analyzed.Result:CG frequently accompanied with other middle ear diseases,and was shown as a high signal intensity on both T1-and T2-weighted images in magnetic resonance imaging(MRI).Conclusion:It was postulated that the obstruction of pneumatized temporal bone air cells,caused by other middle ear diseases such as cholesteatoma and tympanosclerosis,might be the pathophsiological mechanism of CG.The evaluations of computed tomography(CT) and clinical manifestation were limited to distinguish CG from cholesteatoma or other neoplasm,while the MRI can be of great value to characteristic diagnosis.
2.Reflex epilepsy induced by playing Mah-jong:14 cases report

Objective To analyse clinical characteristics of patients with epilepsy induced by playing Mah-jong.Methods The medical records of 14 epileptic patients who were admitted at the Epilepsy Center of PUMC Hospital chief complaint of Mah-jong induced epileptic seizures from 1996 to 2000 were retrospectively reviewed.Results 12 patients showed generalized tonic-clonic seizures and 2 patients showed partial seizures with secondary generalization. Interictally, normal EEG were found in 13 patients, and one patient was discharged. None patients recept anticonvulsant therapy for preventing seizures. Conclusion Mah-jong induced epilepsy, a reflex epilepsy, is probably related to thinking,spatial task and decision making while playing mah-jong. It is benign, and could be easily prevented by stopping playing Mah-jong.
3.Ictal video-electroencephalography monitoring in presurgical evaluation for intractable temporal lobe epilepsy in 15 cases

Objective To assess the value of ictal video-electroencephalography monitoring (IVEEG) in presurgical evaluation for medically intractable nonlesional temporal lobe epilepsy(TLE). Methods The data of presurgical evaluation and postsurgical follow-up from 15 patients with intractable TLE were retrospectively analyzed.Results In this selected patients group, all 15 patients manifested clinically seizures during long term VEEG monitoring, with initial rhythmic activity from one side of temporal lobe in 13 cases. Of these 13 patients, the side of initial ictal focal pattern was concordant with the side of anterior temporal lobectomy or selective amygdalohippocampectomy in all cases and excellent postsurgical outcome were attained in 11 patients.Conclusion IVEEG monitoring may not only help to confirm the diagnosis and seizure type(s),but also more important to correctly localize the side of epileptogenic region in most patients with intractable TLE.
4.Lateralizing significance of figure"4"sign in patients with mesial temporal lobe epilepsy

Objective To investigate the lateralizing value of figure"4"sign,an asymmetrical tonic posturing observed in secondarily generalized tonic-clonic seizures(sGTC),in patients with mesial temporal lobe epilepsy.Methods Presurgical videotapes of 54 sGTC from 33 patients who were seizure-free for at least 4 years after temporal lobectomy were retrospectively reviewed.Attention was paid to the relationship between the extended upper extremity in figure"4"sign and the resected side.Results The "4"sign was observed in 10(30%)of 33 patients and 23(43%)of sGTC.The extended arm in figure "4"sign was always contralateral to the epileptogenic foci(resected side).Conclusion"4"sign has a significant lateralizing value,when present,in this selected homogeneous group of patients.

Objective To determine the distribution of various epilepsies and epileptic syndromes in the epileptic population treated in epilepsy clinics. Methods Totally 1191 patients with seizures visited between April and September, 2004 in epilepsy center of our hospital were analyzed based on International League Against Epilepsy (1989, ILAE) Classification. Results Of the 1191 cases, partial epilepsies and syndromes were 766 cases (64.3%), generalized epilepsies and syndromes were 240 cases (20.2%), epilepsies and syndromes undetermined whether partial or generalized were 79 cases (6.7%), special syndromes were 16 cases(1.3%),other non-epileptic disorders were 90 cases (7.6%). Symptomatic cases were dominant in partial epilepsies and syndromes, and idiopathic cases were dominant in generalized epilepsies and syndromes. Conclusions Most of epileptic patients might be classified accurately. Treatment should be administered as earlier as possible. The distribution of various epilepsies and epileptic syndromes is regular.

8.Features of clinical and EEG changes during follow-up in patients with benign epilepsy of childhood with central-temporal spikes

Objective To investigate the features of clinical and EEG changes during follow up in patients with BECCT. Correlative analysis between the disease duration and some factors were also performed.Methods The clinical and EEG data of 87 recovered patients with BECCT were retrospectively analyzed.Results The EEG and clinical manifestations are mostly characteristic. Clinical seizures disappeared early before the rolandic spikes of EEG in most patients,showing a longest duration of 6 years. There was only the onset age being correlative with the duration of disease in both seizures and abnormal EEG. Medication was withdrawn after there appeared seizures free for more than two years and a recovery of EEG. However medication can also be stopped even at patient′s EEG still remaining abnormal.Conclusion BECCT is an age dependent, idiopathic epilepsy syndrome in childhood, comprehensively recognizing the clinical and EEG features should be helpful in clinical practice.
9.Cognitive function impairment in children with benign epilepsy with centro-temporal spikes:an event-related potential study

ObjectiveTo find the cognitive function impairment in children with benign epilepsy with centro-temporal spikes and to find a sensitive index which can reveal the cognitive function impairment.Methods21 patients with benign epilepsy with centro-temporal spikes participated in this study. 21 normal subjects, matched for age, gender, years of education and family socioeconomic status, served as controls. Subjects were asked to judged weather the two sequentially presented numbers (S1 and S2) in a digit pair were identical or not, while event-related potential (ERPs) were recorded from electrodes on their scalp.ResultsN270 was evoked by S2 of the conflict condition in either controls or patients. Compared with control group, N270 latency was significantly prolonged and the amplitude decreased in the patient group. However, the peak latency and amplitude of P300 were not changed significantly in patient group. ConclusionChildren with benign epilepsy with centro-temporal spikes as a group show cognitive function impairment. Conflict negative N270 can be employed as a method for evaluating cognitive disturbance, which seems subtler than P300 in detecting mild cognitive function impairment.

AIM To study the pharmacokinetic characters following oral administration of colon specific tinidazole tablets in healthy dogs METHOD The plasma and intestine content concentration of tinidazole was determined by RP HPLC method following a single oral dose of 500 mg of two formulations given to each dogs in an open randomized two way crossover design RESULT The main pharmacokinetic parameters of test and reference tablets were as follow: T max : (6 3?1 2) h and (2 4?1 1) h; C max : (46 23?8 93) mg?L -1 and (58 08?14 65) mg?L -1 ; AUC 0-t : (423 21?93 08) mg?h?L -1 and (458 22?112 07) mg?h?L -1 The relative bioavailability of tinidazole colon specific enteric tablets was 92 9%?6 6%. 6 5 h after intake of colon specific tablets, there were a great deal of tinidazole in dogs colon but almost no tinidazole in small intestine CONCLUSION The result demonstrated that the colon tropic effect of the tablets is significant in dogs
