1.Establishment of HPLC fingerprint and its application in identification of Folium Apocyni Veneti

Objective To establish chromatographic fingerprint of Folium Apocyni Veneti by HPLC and identify Folium Apocyni Veneti from its adulterants.Methods HPLC Gradient elution was applied to establish the chromatographic fingerprint and"Computer-Aided Similarity Evaluation System"was used in data analyses.Results This chromatographic fingerprint method has good precision,stability,and re- peatability.Twelve common peaks were marked.There are notable differences in chromatographic finger- prints between Folium Apocyni Veneti and its adulterants.Conclusion This chromatographic fingerprint method can be used to evaluate the quality of Folium Apocyni Veneti.
2.Role of postoperative chemotherapy in patients with early-stage pulmonary large-cell neuroendocrine carcinoma after resection

Objective:To investigate the efficacy of postoperative chemotherapy in patients with early-stage pulmonary large cell neu-roendocrine carcinoma (LCNEC) after resection. Methods:A cohort of 50 patients who underwent resection and systematic nodal dissection for LCNEC between January 2008 and December 2014 in our institution was retrospectively reviewed. The patients were divided into adjuvant chemotherapy group (32 cases) and non-chemotherapy group (18 cases). Follow-up information was investigated. Results:The median survival and the 5-year survival rate were 48 months and 72.5%for the adjuvant chemotherapy group whereas 29 months and 35.6%for the non-adjuvant chemotherapy group, respectively. Univariate and multivariate analyses using Cox's proportional hazard models showed that postoperative chemotherapy was a signifi cant prognostic factor for OS (P=0.005;hazard ratio=0.281, P=0.008, respectively). Conclusion:Postoperative chemotherapy is beneficial to patients with early-stage pulmonary LCNEC after complete resection.
3.Operative cooperation of arthroscopic reconstruction of anterior cruciate ligament and posterior ligament with autologous tendons

Objective To summarize the operative effect and nursing experience of arthroscopic reconstruction of anterior cruciate ligament (ACL) and posterior ligament (PCL) with autologous tendons. Methods Eleven patients with both ACL and PCL were treated with surgical operation by arthroscopic reconstruction of anterior cruciate ligament and posterior ligament with autologous tendons. Before operation, supplies of surgical articles, mastery of the instrument's properties, procedures, connection mode and precautions were well performed. During the operation, the nursing measure covered also accurate, skilled and agile cooperation. Results The operation was successfully completed, with the average time of (2.7 ± 0.2)h, bleed volume of (32.0 ± 9.8)mL. No infections occurred after surgery. The Lysholm score after operation was higher than that before operation (92.6 ± 3.2 vs. 65.5 ± 3.1;P<0.01). Conclusion Arthroscopic reconstruction of anterior cruciate ligament and posterior ligament with autologous tendons has good clinical therapeutic effect. Preoperative well preparation and expertly operative cooperation are critical for the success of surgery.
4.Drug Combination on in vitro Activities of Multi-drug resistant Pseudomonas aeruginosa

OBJECTIVE To evaluate the in vitro inhibitory effects of antibacterials in combination carbapenem against multiresistant Pseudomonas aeruginosa.METHODS The protocol was designed by checkboard method and the MIC of the two antibiotics in combination was determined by broth dilution method.FIC was calculated according to MIC results.RESULTS The MIC50 and MIC90 of eight kinds of antibiotics were lower to 1-16 times and 1-64 times,respectively.The synergistic and additive antibacterial activity was seen in ceftazidime with amikacin,piperacillin with tobramycin and azlocillin with sulbactam.The lomefloxacine with ampicillin and lomefloxacine with ceftriaxone were not correlated.CONCLUSIONS Antibacterial activity of ceftazidime combined with amikacin against carbapenem-multiresistant P.aeruginosa is the best.
5.Prognostic factors of combined small cell lung cancer after surgical resection

Objective:To investigate the prognostic factors and survival of patients with combined small cell lung cancer (C-SCLC) after they underwent complete resection. Methods:The clinical records of C-SCLC patients who were subjected to complete resection and systematic nodal dissection in one institution between January 2010 and December 2014 were retrospectively reviewed. Results:Sev-enty-eight patients with histologically diagnosed C-SCLC were identified. The most common combined component was large cell neuro-endocrine carcinoma (LCNEC) (n=42), followed by squamous cell carcinoma (SCC) (n=18), adenocarcinoma (AC) (n=10), and adenosqua-mous carcinoma (ASC) (n=8). The overall survival (OS) rate of the entire cohort was 39.1%. Multivariate analyses using Cox's propor-tional hazard models revealed that size [<3 cm vs.>3 cm;hazard ratio (HR)=0.406;95%confidence interval (CI)=0.202-0.816;P=0.011], performance status (<2 vs.>2;HR=0.113;95%CI=0.202-0.631;P=0.013), combined non-small cell lung cancer (NSCLC) components (LCNEC vs. non-LCNEC, HR=3.00;95%CI=0.096-0.483;P<0.001), stage Ⅲ A vs.Ⅰ;HR=0.195, 95%CI:0.063-0.602;P=0.004) and adju-vant therapy (yes vs. no, HR=0.402;95%CI=0.195-0.831;P=0.014) were significant prognostic factors of OS. Conclusion:The mixed NSCLC components within C-SCLC significantly influence survival. Adjuvant therapy is beneficial for patients with complete resection of C-SCLC.
6.Cross-section study of early renal damage in patients with chronic hepatitis B

Objective To investigate early renal damage of chronic hepatitis B (CHB) patients and the risk factors related to their renal function. Methods CHB patients who visited the second people’s hospital but did not receive systemic treatment were enrolled in our study. Those who visited for general check-up with no hepatic findings during the same period were selected as control group. Glomerular filtration rate (GFR) of all the participants were estimated by simplified MDRD equation and CKD-EPI equation (designated as M-eGFR and C-eGFR respectively). Influence factors of eGFR were statistically analyzed. Results In the total 528 cases in CHB group, 88 (16.67%) and 62 (11.74%) suffered declined M-eGFR and C-eGFR respectively. By contrast, 10 (8.77%) and 6 (5.26%) cases in the total 114 cases in control group present declined M-eGFR and C-eGFR ac?cordingly. Percentages of renal function impairment, estimated by both M-eGFR and C-eGFR, were higher in the CHB group than those in control group. The difference was statistically significant (χ2=4.518, P<0.05;χ2=4.156, P<0.05). Multiple linear regression analysis indicated that age, HBsAg and body mass index (BMI) were risk factors of M-eGFR while age, HBsAg, gender and serum albumin were risk factors of C-eGFR. On the other hand, HBV-DNA and HBeAg were not risk factors for M-eGFR or C-eGFR. Conclusion HBV infection can lead to early renal damage. Age and HBsAg are main risk factors of renal function impairment. Therefore, renal function should be scrutinized in CHB patients.
7.Resistance and Gene Detection in Klebsiella pneumoniae isolated from Patients with Cerebral Surgical Incision Infection

OBJECTIVE To analyze the resistance and gene existing in 7 extended-spectrum beta-lactamases(ESBLs)producing Klebsiella pneumoniae strains from patients after cerebral surgical operation.METHODS Susceptibility test,ESBLs screening and nucleotide sequencing for PCR products of ?-lactamase BLA such as CTX-M,TEM,OXA,SHV,GES,PER and VEB were taken.RESULTS seven isolates of K.pneumoniae may have closed relation with CTX-M positive.CONCLUSIONS The little outbreak might be caused by these 7 homologous strains in the same.
8.Clinical outcomes of epidermal growth factor receptor tyrosine kinase inhibitors in re-current adenosquamous carcinoma of the lung after resection

Objective:To investigate the efficacy of epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKIs) in adenosqua-mous carcinoma (ASC) of the lung after resection. Methods:Clinical data of patients suffering from ASC and receiving EGFR-TKI treat-ment at one institution between January 2006 and December 2014 were retrospectively reviewed. Results:A total of 27 EGFR muta-tion-positive patients with ASC subjected to EGFR-TKI therapy were enrolled in this study. EGFR mutations included deletion in exon 19 in 15 cases and point mutation at codon 858 in exon 21 in 12 cases. Of the 27 ASC patients who received EGFR-TKI treatment, 9 exhibit-ed a partial response and 11 manifested a stable disease, and these patients accounted for a disease control rate of 74.1%(20/27). The median overall survival (OS), median progression-free survival, and median relapse OS of the EGFR mutation-positive patients who underwent TKI therapy were 39 months [95%confidence interval (CI)=25.6-52.4], 15 months (95%CI=12.9-17.1), and 19 months (95%CI=0.9-37.1), respectively. The 3-and 5-year survival rates of these patients after operation were 51.9%and 15.3%, respectively. Con-clusion:The survival of EGFR mutation-positive ASC patients treated with EGFR-TKIs was satisfactory. EGFR testing was recommended for ASCs and EGFR-TKI treatment was suitable for ASCs with EGFR-sensitizing mutation.
9.Assessment of patients discomfort and nursing care with the application of vector system in periodontal treatment

Objective The study aimed to compare the vector system with the hand instrument in the pain severity during scaling and root planning.Methods 60 periodontal patients were randomly divided into two groups for supportive periodontal therapy (SPT):the experimental group (using vector system) and the control group (using gracey instruments),with 30 cases in each group.And the painfulness after SPT was evaluated by Visual Analogue Scale (VAS).Results 66% patients felt mild pain during the supportive periodontal therapy with the application of vector system,while 36% patients felt mild pain in the control group.24 patients in the experimental group accepted vector system for SPT and 23 patients in the experimental group felt less fear of the treatment.Conclusions Patients will feel less discomfort with the application of the vector system,therefore better compliance will be reached.And the cooperation of the doctors and nurses has great impact on the treatment effect.
10.Study of anatomy and variation of arteries of ankles in chinese adults by ultrasound

Objective To analyze the normal anatomy and variation of arteries around ankles in Chinese adults in and around Shaanxi province. Methods Aged 18-82 (48±15) years, 522 males and 557 females who came to Xijing Hospital for medical treatment or for a healthy check were randomly recruited to participate in this study. Their arteries around ankles were assessed by high-frequency color Doppler ultrasound. Then the anatomy and variation of the arteries were analyzed. Results There were totally 213 arterial variations in 175 subjects, including variations at single ankle in 137 subjects and variations at both sides in 38 subjects. The rate of arterial variation at ankle in persons was 16.2%(175/1 079) and was higher in females than that in males (21.5%, 120/557,χ2=24.028, P=0.000). The rate of arterial variation at ankle in limbs was 9.9%(213/2 158). There was no signiifcant difference for the rate of arterial variation between the left (5.1%, 109/2 158) and right (4.8%, 104/2 158) ankles (χ2=0.129, P=0.701). In variations of arteries at ankles, there were 131 cases with small posterior tibial arteries and peroneal arteries extending to the bottoms of feet, i.e., posterior tibial arteries substituted by peroneal arteries (typeⅠ, 6.1%, 131/2 158), 69 cases with small anterior tibial arteries and peroneal arteries extending to the dorsums of feet, i.e., anterior tibial arteries substituted by peroneal arteries (typeⅡ, 3.2%, 69/2 158), and 13 cases with both small anterior and posterior tibial arteries, and peroneal arteries extending to both the dorsums and bottoms of feet, i.e., both anterior and posterior tibial arteries substituted by peroneal arteries (typeⅢ, 0.6%, 13/2 158). There was no signiifcant difference in each type of variation rates between left and right sides. The rates of typeⅠvariation were 6.0%(65/1 079) and 6.1%(66/1 079) at the left and right ankles respectively (χ2=0.010, P=0.928);the rates of typeⅡvariation were 3.6%(39/1 079) and 2.8%(30/1 079) at the left and right ankles respectively (χ2=1.213, P=0.271);the rates of typeⅢvariation were 0.5%(5/1 079) and 0.7%(8/1 079) at the left and right ankles respectively (χ2=0.697, P=0.404). Conclusions Ultrasound can be used to depict anatomy and variation arteries around ankles. There is a high prevalence of anatomic variation of ankle arteries in population in and around Shaanxi province. Therefore it will be helpful for diagnosis vascular abnormalities with the knowledge of characteristics of normal anatomy and variation of ankle arteries.
