1.TCM Research Review on the Relationship between Asthma and Leukotrienes and Its Receptor

Objective] Summarize the pathogenesis of asthma and leukotriene receptor in latest research situation. [Methods]Check out the past five years CNKI, VIP data, PUBMED data to leukotrienes and asthma for keyword research literature, and classification, summary. [Results]Numerous clinical studies, in vivo and in vitro experiments showed that leukotrienes and their receptors are involved in the pathogenesis of asthma and progress in a multi-cell, multi-session, and interact with a variety of cytokines.[Conclusion]With concept of wholism as the characteristics of the Genomics technologies, more suitable as a primary means of application in the field of medical research has an overall concept for the regulation of traditional Chinese medicine research in the field of drugs to treat asthma leukotriene pathway and provide a new perspective.
2.Distribution of CD4~+ CD25~+ FOXP3~+ regulatory T cells and their subsets in colorectal carcinoma and the clinical significance

Objective: To determine the distribution of CD4~+CD25~+FOXP3~+regulatory T cells (Treg) and Treg subsets in human colorectal carcinoma microenvironment and to explore their correlation with conventional clinico-pathological features.Methods: Frozen sections and Immunohistochemistry (IHC) were used to detect FOXP3~+ Treg in flesh specimen collected from 42 patients with colorectal carcinoma.The number of FOXP3~+ Treg was evaluated in terms of its association with clinico-pathological feature in tumor and peri-cancer tissue.Double staining was performed to determine the expression of ICOS and FOXP~3.Results:The number of FOXP3~+ Treg in the colorectal carcinoma (mean 24.1) was significantly higher than that in peri-cancer tissue (mean 0.7).A higher number of tumor infiltrating FOXP3~+ Tregs was found in the patient groups with poor differentiation,lymphatic metastasis and non-distant metastasis as compared to the patient groups with well differentiation,non-lymphatic metastasis and distant metastasis.The percentage of FOXP3~+ ICOS~+ Treg was higher in colorectal carcinoma(81% ) than that in peri-cancer tissue(10% ).Condusion: Increased FOXP3~+ Treg may influence the occurrence and development of colorectal carcinoma.Our data support the hypothesis that tumor infiltrating FOXP3~+ Tregs attenuate the immune response against cancer and suggest that strategy to overcome FOXP3~+ Treg function may be beneficial in the treatment of human colorectal cancer.
3.Research of relationship between arthrochalasis and developmental dislocation of hip

[Objective] To study the relationship between arthrochalasis and developmental dislocation of hip(DDH).[Methods]One hundred and seventy-six patients with DDH treated from Jaunary 2006 to December 2006 in the Shengjing Hospital of China Medical University were evaluated retrospectively.The average age was 3.54 years old(range,from 6 months to 15 years),with 153 females and 23 males.Eighty-four patients had both,58 left and 34 right.Totally 192 normal children were controls,and the average age was 3.67 years old(range,from 4 months to 15 years).The diagnosis of DDH was depended on clinical manifestations,radiography and ultrasonography.The criteria of arthrochalasia were the standards proposed by Wynne Davies.SPSS version 10.0 for Windows was used to perform statistical analysis.?2 test and spearman test was conducted for obtained data.A P

5.Distribution and expression of PDGF-A in the ligamentum teres of children with developmental dislocation of the hip

[Objective] This study examined the distribution and the expression of platelet-derived growth factor-A (PDGF-A) in the ligamentum teres of children with developmental dislocation of the hip (DDH) and normal persons in order to investigate the roles of PDGF-A in hip joint laxity. [Methods]There were six pairs of joint laxity of children with DDH and normal children (control group) matched to gender and age. Immunohistochemistry (SP method) method was used to detect the location and distribution of the PDGF-A in the ligamentum teres,and to semi-quantify their content.Semiquantitative RT-PCR method was used to detect the PDGF-A in the ligamentum teres at mRNA level. The quantitative analysis of the PDGF-A was performed by professional image software and the results were analyzed with standard statistical methods.[Results]High density expression was observed in the synovial layer with fibroblast regularly arranged parallel to the joint surface in the ligamentum teres. PDGF-A distribution was decreased in the fibrous layer of the ligamentum teres. 6.Analysis of nutritional status in fragile fracture in hospitalized elderly patients

Objective To evaluate the relationship between fragile fracture sites in hospitalized geriatric and nutritional status.Methods On the basis of whether the fracture,293 cases were divided into fracture group(n =140)and normal control group(n =153),51 males(36.4%)and 89 females(63.6%)in 140 fracture patients. Results The age was (74.04 ±8.58)years in the fracture group,that in the normal control group was (74.77 ± 8.14)years,there was statistically significant difference between the two groups(t =-0.743,P >0.05).The femoral neck fracture was 51 cases(36.4%)and intertrochantetic fracture was 57 cases(40.7%),and other was 32 cases (22.8%).The patients were divided into the two groups according to the age:60 ~75 years group(n =62)and >75 years group(n =78),femoral neck fracture was 37.2%,and intertrochantetic fracture was 52.6% in the group that more than 75 years old,which were 35.5% and 25.8%,respectively in 60 ~75 years group,there was statistically significant difference between the two groups(χ2 =18.337,P <0.01).The hemoglobin,serum albumin weight,weight,BMI in the fracture group were (117.44 ±20.25)g/L,(34.96 ±4.95)g/L,(60.81 ±10.63)kg,(22.83 ±3.54)kg/m2 , and those in the normal control group were (136.99 ±17.10)g/L,(37.27 ±5.85 )g/L,(64.47 ±11.23)kg, (24.24 ±3.88)kg/m2 respectively.The commonly used nutritional indicators:hemoglobin(t =-8.908),serum albu-min(t =-3.619),weight(t =-2.837)and BMI(t =-3.218)were lower than the normal control group(all P <0.01 ),serum phosphate was lower than the normal control group,the difference was statistically singnificant (t =-2.047,P <0.05).The blood calcium(t =0.630)and ALP(t =-0.550)had no statistically singnificant differences between the two groups(all P >0.05).Conclusion The proportion of intertrochantetic fracture and femo-ral neck fracture were higher in hospitalized elderly patients,and malnutrition,such as lower hemoglobin,lower serum albumin,lower weight,and lower BMI were,important factors in the fragile fracture in the geriatric inpatients.
7.Research on allogeneic mouse spleen T lymphocyte apoptosis induced by FasL transfected dendritic cells

Objective To establish the mouse bone marrow-derived dendritic cells expressing FasL protein and explore the mechanism of inducing allogeneic mouse spleen T lymphocyte apoptosis. MethodsMouse myeloid DCs were cultured in selective medium zontaining essential cytokines for DC growth in vitro. The mouse DCs were transfected with liposome-mediated FasL gene. The levels of FasL mRNA before and after transfection were assayed by real-time quantitative PCR. The expression levels of FasL protein were assayed by flow cytometry(FCM)and Western blot. Non-transfected DC,empty plasmid transfected DC and FasL transfected DC were infused intravenouslY into allogeneic mouse. After 7 days, the apoptosis in spleen T lymphocytes was evaluated by TUNEL (terminal deoxynucleotidyl transferase-mediated dUTP nick-end-labeling)method and FCM. ResultsCultured in vitro, the mature myeloid DCs from mouse could be obtained.The expressions of FasL mRNA and protein in FasL transfected DCs were significantly higher. Through the detection of spleen T lymphocyte apoptosis with TUNEL,the apoptosis index(AI)was higher in FasL transfected DC(11.67±1.53),compared with non-transfected DC(2.67±0.58)and empty plasmid transfected DC(3.33±0.58),P<0.01. ConclusionA large quantity of myeloid DCs can be obtained through in vitro culture in selective medium. The liposome-mediated FasL gene transfected DCs could successfully express high levels of FasL protein. Intravenous infusion of FasL gene transfected DCs could induce apoptosis of allogeneic mouse spleen T lymphocytes.
8.Passive manipulation under general anesthesia for treating postoperative hip stiffness in 193 children

OBJECTIVE: To present our experience of manipulation under general anesthesia for treating postoperative hip joint stiffness in children.METHODS: 193 children (226 hips) with postoperative joint stiffness were retrospectively studied at the Department of Pediatric Orthopaedics, Shengjing Hospital, China Medical University from January 1978 to December 2005, comprising 53 males and 140 females, aged 1-14 years old. 226 hips with postoperative stiffness from open reduction of developmental dysplasia of the hip (DDH) (155 cases with 188 hips) and capsulotomy for hip sepsis or tuberculosis (38 cases with 38 hips) were retrospectively reviewed. All patients laid on the surgical table supinately under general anesthesia. The assistant fixed the pelvic with palms. The operator manipulated the hip joint with 3 steps: flexed hip joint slowly and gently to 110°-120° or more; then abducted and externally rotated hip joint; adducted and internally rotated the hip. Then 3-step passive manipulation was repeated for once at least. As soon as the patient waked from general anesthesia, Continuous passive motion was applied for the affected hip with the range of motion increasing daily for 1-2 weeks. They were followed averagely 15 months (6-24 months) to evaluate the promoted range of motion in flextion-extension and adduction-abdduction.RESULTS: 186 hips (82.3%) succeeded in manipulation in addition to 34 failure hips (15%) and 6 hips (2.7%) with femoral neck fracture. The successful rate, promoted range of motion in group of ≤6-year-old and group of ≤12-month-duration from primary surgery were significantly higher, femoral neck fracture rate was less than those in group of >6 years of age and group of >12-month-duration from primary surgery.CONCLUSION: Passive manipulation under general anesthesia is a safe, uninvasive and effective method in treating postoperative hip stiffness in children. Younger age (≤6 years) and less duration from the primary surgery (≤12 months) would yield better result.
9.Treatment of spondylolisthesis in 7 children using Texas Scottish Rite Hospital posterior pedicle screw system

AIM:Spondylolisthesis is a common spinal disease for adults but rarely found in children.In this study,the treatment effect of spondylolisthesis in 7 children with posterior pedicle screw system after 1 year was explored.METHODS:①Eight children patients with spondylolisthesis treated in Shengjing Hospital of China Medical University between January 1996 and December 2006 were selected including 4 males and 4 females.The average age was 12.25 years(range,8-14 years).According to Meyerding classification,7 cases were of stage Ⅰand one of Ⅳ.②Texas Scottish Rite Hospital(TSRH) posterior pedicle screw system was provided by Wego Ortho Co.,Ltd.,GB1Z type,titanium,No.200511028009.Of the 8 cases,4 were treated by TSRH posterior pedicle screw system,3 were treated via a Wiltse paraspinal approach with autogenous bone graft placed between the transverse processes of L5 and sacral ala in situ fusion,and 1 was not treated by operation.The informed consent of treatment was obtained from the patients.③The changes in the spinal radioactive imaging before and after operation,and the material and host biocompatibility were observed.RESULTS:①Seven of 8 cases were followed up.The mean time of follow-up was 56.6 months with a range from 13 to 111 months.②Three of 4 cases treated by TSRH were vertebral completely reduction with the stability of instrumentation about 75%(3/4),and the other one was partly reduction(about 85%).One person developed the progress again in the X-ray films at three weeks after operation.Three cases treated by Wiltse paraspinal approach with autogenous bone graft placed between the transverse processes of L5 and sacral ala in situ fusion healed well without the recurrence of lumbosacral spondylolisthesis and nonunion.CONCLUSION:TSRH is an efficient and safety option for the treatment of spondylolisthesis in children.
10.Microporous polysaccharide hemostatic powder to stop bleeding in posterior spinal surgery:An experimen-tal study

Objective Microporous polysaccharide hemostatic powder ( MPPHP) is a most commonly used hemostatic mate-rial, but reports are rarely seen on its application in posterior spinal orthopedic surgery.The present paper aimed to investigate the val-ue of MPPHP in stopping bleeding in posterior spinal surgery. Methods Twenty-four 5-month-old New Zealand rabbits were equally randomized to an experimental and a control group.An 8 cm-long wound was made in the back muscle of animals to establish the model of spinal posterior surgery.MPPHP was applied to the wound of the experimental rabbits but none to that of the controls.Comparisons were made between the two groups of animals in the bleeding time, in vitro coagulation time, intra-operative blood loss, and results of pre-and post-operative blood routine and histopathological examinations. Results Statistically significant differences were observed between the experimental and control groups in the mean bleeding time ([15.36 ±1.42]s vs [347.58 ±32.43]s, P<0.05), in vitro coagulation time ([235.41 ±21.08]s vs [722.16 ±40.35]s, P<0.05), and intra-operative blood loss ([1.61 ±0.24]g vs [2.86 ± 0.37] g, P <0.05).After operation, the experimental animals showed a significantly higher count of erythrocytes than the controls ([5.68 ±0.46] ×106/L vs [4.47 ±0.43] ×106/L, P<0.05)and a higher level of hemoglobin than the latter ([12.67 ±1.18]g/L vs [10.64 ±1.05]g/L, P=0.00).There were no significant differences in the effectiveness and excellence rates of hemostasis between the experimental and control groups (100%vs 83.33%and 83.33%vs 66.67%, P>0.05).Pathological examination revealed mild bleeding and a small number of red blood cells in the muscle interstitium and blood vessel lumen of the experimental rabbits as compared with the controls. Conclusion Intraoperative topical ap-plication of MPPHP can reduce blood loss and achieve rapid and efficient hemostasis in posterior spinal surgery without affecting wound healing.
