The clinic main application of medical ultrasonic techniques are introduced,including ophthalmology,neurology,angiology,cardiopathy,digestive system,urology,gynaecology and obstetrics,and surgery. Four kinds of medical ultrasonic techniques in the value on the clinic diagnosis are important,namely transcranial color -coded duplex sonography(TCCD),tissue harmonic imaging(THI),three dimensional imaging(3D),and extended field Of view(EFV).
2.Clinical application of complete thoracoscopes and laparoscopes combination operation for thoracoabdominal Injuries

ObjectiveTo investigate the feasibility and effect of complete thoacoscopes and laparoscopes combination operation for thoracoabdominal injuries.MethodsFrom March 2007 to March 2011,the clinical data of 36 patients (observation group) with thoracoabdominal injuries who were performed with complete thoracoscopes and laparoscopes combination operation were analyzed retrospectively,and compared with 36 patients (control group) treated with traditional standard operation in the same period.The intraoperative and postoperative situation was compared between two groups.ResultsAll the patients in observation group were successfully performed by complete endoscopic surgery.There were no operative mortality and complications related to operation,such as diaphragmatic hernia,delayed hemopneumothorax.The operative time of observation group was (98 ±38 ) min,operative blood loss was ( 120±45 ) ml,drainage flow within 24 h was(230 ±55) ml,drainage tube duration was(5 ±2) d,postoperative pain vasual analogue scale (VAS) score was (3.31±0.87) scores,hospitalization cost was ( 1 2.3±7.6) thousand yuan,postoperative recovery time of intestinal function was (2.29±1.02) d,hospital stay was ( 10±3 ) d.Those values of control group were respectively ( 135±31 ) min,(220±30) ml,(400±160) ml,(9±3) d,(6.82±1.67) scores,(23.4±8.5) thousand yuan,(5.46 ±2.31 ) d and ( 16±2) d.There were significant differences between two groups(P <0.05).The occurrence rate of cardiopulmonary complication of observation group [ 11.1%(4/36) ] and control group [ 13.9%(5/36) ] had no significant difference (P>0.05).ConclusionsComplete thoracoscopes and laparoscopes combination operation for thoracoabdominal injuries is a safe and mini-invasive treatment with quicker recovery,less cost and morereliable effect.Therefore,it deserves further clinical application.
3.MRI findings of Wallerian degeneration in pyramidal tract after cerebral injury

Objective To analyze the MRI findings of Wallerian degeneration in pyramidal tract after cere-bral injury for improving the diagnosis accuracy.Methods The MRI findings of Wallerian degeneration in pyramidal tract in 25 cases were analyzed in association with the primary cerebral injury.Results All the primary diseases were above the tentorium of cerebellum in 25 cases,MRI in 2 cases demonstrated a continuous thin band -like long T1 or isometric T1 and long T2 abnormal changes in pyramidal tract,hyperintensify on FLAIR and DWI.2 cases showed hypointensify on T1 WI,hyperintensify on T2 WI and FLAIR,isointensity on DWI.21 cases showed hypointensify on T1 WI,hyperintensify on T2 WI and FLAIR,isointensity or hypointensify on DWI with brainstem atrophy.Conclusion MRI is valuable in the diagnosis of Wallerian degeneration in pyramidal tract after cerebral injury.
4.Hydrogen proton magnetic resonance spectroscopy study on early Wallerian degeneration of the pyramidal tract after cerebral infarction

Objective To investigate the feasibility of hydrogen proton magnetic resonance spectroscopy (1 H-MRS)for detection of Wallerian degeneration of the pyramidal tract after cerebral infarction.Methods Multiple metabolic indices including NAA,Cho, Cr of the bilateral cerebral peduncle were detected by using 1 H-MRS in 1 5 patients with unilateral middle cerebral artery infarction and 1 5 age-matched healthy volunteers,NAA/Cr,Cho/Cr were also calculated.Comparing the difference between cerebral infarction group and the control group ,the ROC curve was analyzed.Results The values of NAA/Cr in the ipsilateral cerebral peduncle of cerebral infarction patients were significantly lower than that of the contralateral,and there were significant differences between the two groups(P <0.05).The values of NAA/Cr in the ipsilateral cerebral peduncle of cerebral infarction patients decreased (P <0.05),as compared with that of the control group.The area under the ROC curve was 0.947,the truncation point was 1.63.The NAA/Cr val-ues of the contralateral cerebral peduncle of cerebral infarction patients had no significant differences compared with the control group (P >0.05),the Cho/Cr values of the ipsilateral and the contralateral cerebral peduncle had no significant difference compared with the control group (P >0.05).Conclusion 1 H-MRS can detect Wallerian degeneration of the pyramidal tract after cerebral infarction.
5.Effect of Body Weight Support Training on Cerebral Palsy

Objective To observe the effect of the body weight support training (BWST) on walking ability of children with cerebral palsy (CP). Methods 50 children with CP were divided into the BWST group (n=30) and control group(n=20). All cases were treated with physical therapy, but the BWST group were added with BWST. The scores of D and E dimensions of the Gross Motor Function Measure Scale (GMFM) were used to assess the walking ability before and after the treatment, respectively. Results The scores of D and E dimensions of GMFM improved in both groups after treatment (P<0.01), and that of the BWST group improved more than control group (P<0.05). Conclusion BWST can facilitate the recovery of the walking ability after cerebral palsy.
6.Influence of Xiaokekang on Renal Function in Diabetic Nephropathy Rats

Objective:To observe the changes of glyeemia, urine protein, serum creatinine, blood urea nitrogen and glycosylated hemoglobin in diabetic nephropathy rats and treating effects of Xiaokekang. Methods: Streptozotocin (STZ) was abdominally administered and was feeded by high lipid diet to establish diabetic nephropathy model in rats. Animals were divided into three groups: model group, Xiaokekang treatment group and normal group. Changes in glycemia, serum lipids, serum creatinine, blood urea nitrogen were measured at the 8th, 12th and 16th week after STZ injection. Glycosylated hemoglobin was measured at the 16th weeks after STZ injection. Results: Glycemia, serum lipids, serum creatinine were higher at the 8th, 12th and 16th week compared with normal group. Serum creatinine was higher at the 12th and 16th week, and glycosylated hemoglobin was also highter than that of the normal group. Xiaokekang reduced these changes. Conclusion: Xiaokekang can reduce glycemia, serum lipids and protect renal function.
7.Enhancement of functional expression of wheat peroxidase WP1 in prokaryotic system by co-transforming with hemA and hemL of Esherichia coli.

Wheat grain peroxidase 1 (WP1) belonged to class III plant peroxidase with cofactor heme, which not only has antifungal activity, but also influences the processing quality of flour. In order to enhance functional expression of WP1 in prokaryotic system by increasing endogenous heme synthesis, we constructed a recombinant plasmid pACYC-A-L containing hemA and hemL of Esherichia coli. Then, we co-transformed it into host strain T7 Express with secretive expression vector (pMAL-p4x-WP1) or non-secretive expression vector (pET21a-MBP-WP1), respectively. The MBP-WP1 fusion protein was further purified by amylose affinity chromatography and its peroxidase activity was assayed using 2,2'-azino-bis (3-ethylbenzothiazoline-6-sulfonate) (ABTS) as substrate. At 12 h after induction at 28 degree, the extracellular 5-aminolevulinic acid (5-ALA) production of T7 Express/pACYC-A-L was up to 146.73 mg/L, simultaneously the extracellular porphrins also increased dramatically. The peroxidase activity of functional MBP-WP1 obtained from T7 Express/ (pACYC-A-L + pMAL-p4x-WP1) was 14.6-folds of that purified from T7 Express/ pET21a-MBP-WP1. This study not only successfully enhanced functional expression of wheat peroxidase 1 in Esherichia coli, but also provided beneficial references for other important proteins with cofactor heme.

8.Prognostic Value of a Change in Troponin-Ⅰ Levels in Patients with Sepsis-associated Myocardial Dysfunction

Objective To investigate the expression of cardiac troponin-Ⅰ (cTnⅠ) levels in patients diagnosed with sepsis-associated myocardial dysfunction and explore the relationship between cTn Ⅰ and cardiac systolic and diastolic function.Additionally,we evaluated the prognostic value of cTn Ⅰ as a valuable biomarker.Methods We admitted 65 patients with sepsis.Using echocardiography,the ratio of early diastolic mitral inflow velocity to early diastolic mitral annulus velocity (E/e') and the left ventricular ejection fraction (LVEF) were measured as an evaluation index of left ventricular diastolic and systolic function,respectively.Patients were divided into a cardiac dysfunction and a normal cardiac function group.The cTn Ⅰ level was measured and compared between the two groups,and we determined the correlation between cTn Ⅰ levels and cardiac diastolic and systolic function.Based on assessment of 28-day mortality,cases were divided into a survivor and a death group.A receiver operating characteristic curve was constructed to predict the prognostic value of cTn Ⅰ.Results The cTn Ⅰ level in the cardiac dysfunction group was significantly higher than that observed in the normal cardiac function group (P < 0.05) and showed a positive correlation with E/e'(r =0.421,P =0.008).However,there was no correlation noted between the cTn Ⅰ level and LVEF (P > 0.05).Compared to the survivor group,the level of cTn]Ⅰ was significantly higher in the death group (P < 0.05).The prognostic value of cTn Ⅰ area under the curve was 0.892,with a cut-off value of 0.82 ng/mL (sensitivity =88.0% and specificity =82.5%).Conclusion The cTn Ⅰ level is noted to be significantly elevated in patients with sepsis-associated cardiac dysfunction and shows a positive correlation with left ventricular diastolic function.A cTn Ⅰ level ≥ 0.82 ng/mL can be used as a valuable predictor of mortality in patients with sepsis.
9.Preliminary observation of clinical efficacy of microwave hyperthermia combined with radiochemotherapy for locally advanced gastric cancer

Objective:To preliminarily observe the clinical efficacy of microwave hyperthermia combined with intensity-modulated radiotherapy (IMRT) and chemotherapy for patients with locally advanced gastric cancer.Methods:Forty patients who could not been operated or refused operation were enrolled in this clinical trial, who were confirmed as locally advanced proximal or distal gastric cancer by gastroscopy pathology and imaging. Radiotherapy was delivered by IMRT technology for 5 times per week with a total dose of 46 to 56 Gy (median dose of 50 Gy) in 25 to 28 fractions. Synchronous hyperthermia was given at 42 to 44℃ twice a week, 45 min/time. S-1 or capecitabine-based synchronous chemotherapy was performed, d1-14/3 weeks. The symptom remission rate, adverse reactions, objective remission rate (complete and partial remission) and survival were observed.Results:A total of 40 patients, aged between 56 and 83 years (median age of 71 years), were enrolled in this study. The male-to-female ratio was 7: 1. Among them, 38 cases (95%) showed symptom remission. The most common adverse reactions were grade 1-2 gastrointestinal reactions and leukopenia. The objective remission rate was 87.5%, the 2-year progression-free survival and overall survival rates were 68.6% and 70.5%, respectively.Conclusion:Preliminary findings demonstrate that microwave hyperthermia combined with chemoradiotherapy achieve satisfactory outcomes and yield tolerable toxicity in patients with locally advanced gastric cancer.
10.Analysis of correlation between rs1187929 and post stroke depression (PSD).

Objective This study aims to investigate the potential correlation between the rs1187929 polymorphism of tyrosine kinase B gene and post-stroke depression (PSD). Methods A total of 78 depression patients (PSD) and 112 non-depression patient controls (NPSD patients) were included in the study. All participants were evaluated using the Hamilton Rating Scale for Depression (HAMD) and further divided into PSD and NPSD groups. PSD patients were diagnosed in accordance with DSM-V criteria. The rs1187929 polymorphism of TrkB was genotyped by using PCR product purification and Sanger double termination sequencing. Results The total prevalence rate was 41.05% . The prevalence rate was 42.86% in men and 38.82% in women ( 字2=0.316,P=0.57). The CC, CT and TT frequencies of PSD group were 26.92% , 48.72% and 24.36% respectively. The CC, CT and TT frequencies of NPSD group were 42.86%, 45.54% and 11.61% respectively. Based on the CC genotype, the relative risk of homozygous mutant TT was 3.341 (字2=7.347, P=0.007, OR=3.341),and the T allele frequency in the PSD group was 48.72% which was higher than 34.38% in the NPSD group. The relative risk of allele T relative to allele C was 1.814 (字2=7.798, P=0.005, OR=1.814). Homozygous mutant TT and allele T were risk factors for post-stroke depression (P<0.05). Conclusion There is a correlation between rs1778929 gene polymorphism and PSD in Han population in South Anhui Province, which warrants further large-scale studies.
