1.Progress of neural invasion in pancreatic cancer

Pancreatic cancer is a kind of highly malignant aggressive cancers.Perinearal growth is one of the importcant biological characteristics of pancreatic cancer.Perineural invasion is an independent prognostic indicator of its prognosis.However,the mechanism has not yet completely clear.The pancreatic cancer perineural invasion mechanism research has been the focus of scholars to discuss,however it' s difficult to break through.Therefore,the discussion pancreatic violation of the peripancreatic neural mechanism for early clinical find,early treatment is essential.The mechanisms of pancreatic cancer research,from all kinds of adhesion molecule to the nerve growth factor,related ligand protein path to susceptibility gene expression,have been reported largely,which helps enhance people' s awareness of peripancreatic neural invasion.At the same time,perineural invasion in the early detection of disease,prolonging survival period and improving the quality of life has important significance.Now the progresses of reviews in recent years are discussed as follows.
2.Multidrug-resistance antisense inhibits volume-activated chloride current in non-pigmented ciliary epithelial cells

AIM: To study the relationship between multidrug-resistance (MDR1) gene product P-glycoprotein (P-gp) and the volume-activated chloride current. METHODS:The volume-activated chloride current in bovine non-pigmented ciliary epithelial cells was recorded using a whole cell recording technique. An antisense technique was used to inhibit the expression of MDR1 gene. The immunofluorescence of P-gp was monitored with a real-time laser confocal microscope.RESULTS:P-gp immunofluorescence correlated negatively with the concentration of the human MDR1 antisense oligonucleotide. The antisense oligonucleotide inhibited the volume-activated chloride current specifically and partially. The latency of activation of the current increased and the peak current decreased. 3.Clinical Observation of Early Application of Tirofiban Combined with Different Lipid-regulating Drugs Inter-vention in Elderly Patients with Coronary Heart Disease

OBJECTIVE:To observe short-term efficacy and safety of early use of tirofiban combined with different lipid-regu-lating drugs percutaneous coronary intervention(PCI)in elderly patients with coronary heart disease. METHODS:562 elderly pa-tients with coronary heart disease underwent selective PCI were selected and randomly divided into atorvastatin group and probucol group,with 281 cases in each group. Both group received routine treatment;atorvastatin group was additionally given tirofiban combined with Atorvastatin tablet,and probucol group was additionally given irofiban combined with Probucol tablet. Clinical indi-cators and main adverse cardiac event(MACE)were compared between 2 groups before and after treatment. RESULTS:After treat-ment,3 level of thrombolysis in myocardial infarction(TIMI)in atorvastatin group accounted for 69.4%,which was significantly lower than 95.7% of probucol group,with statistical significance(P<0.05). After 12h treatment,CK,2 groups had lower CK-MB and LVEF and higher cTnI;LVEF decrease of atorvastatin group was more significant,with statistical significance(P<0.05);af-ter 8 weeks of treatment,NO,NO/ET-1,GSH,GSH/GSSG of probucol group were increased significantly,and ET-1,GSSG and Eh decreased significantly;above indexes of atorvastatin group had no significant change,there was statistical significance between 2 groups(P<0.05). The incidence of MACE in atorvastatin group and probucol group were 21.7% and 21.4%,there was no sig-nificant difference (P>0.05). CONCLUSIONS:Tirofiban combined with different lipid-regulating drugs can effectively improve the prognosis of elderly patients with coronary heart disease after PCI operation with good safety,but tirofiban combined with pro-bucol has more obvious therapeutic effect.
4.Osmolarity, cell volume and proliferation in nasopharyngeal carcinoma cells

AIM: To investigate the relationship between osmolarity, cell volume and cell proliferation in nasopharyngeal carcinoma cells. METHODS: MTT method was applied to detect the proliferation ability of the poorly-differentiated nasopharyngeal carcinoma cell (CNE-2Z) under various osmolarity conditions. The flow cytometry was used to analyse cell cycle distribution. Cell volume was obtained by the image analysis of living cells and cell viability was determined by the trypan blue assay. RESULTS: Cultivation of cells under the hypertonic conditions of 370 and 440 mOsmol/L increased cell volume by 8.7% and 27.8% and facilitated cell proliferation by 22.2% and 33.9%, respectively. However, hypotonic incubation of cells with osmolarity of 160 and 230 mOsmol/L decreased cell volume by 12.8% and 4.1% and inhibited cell proliferation by 34.0% and 15.6%, respectively. Cell volume was positively correlated with cell proliferation rate. Long-term cultivation of cells under anisotonic conditions did not significantly alter cell cycle distribution, but hypotonic cultivation decreased cell viability. CONCLUSION: Proliferation of nasopharyngeal carcinoma cells was closely correlated with the osmolarity of culture medium and cell volume. Hypotonic cultivation may inhibit cell proliferation by decreasing cell volume to facilitate cell death mechanisms.
5.The impact of management program for children with diabetes in China on the glycemic control of type 1 diabetic mellitus children and factors associated with glycemic control

Objective To compare the glycemic control of different management approaches in type 1 diabetic mellitus (T1 DM) children,and to evaluate the influence factors associated with glycemic control.Methods This survey included 68 T1DM children from January 2012 to December 2014 in the Affiliated Hospital of Qingdao University of more than 1 year management.Hemoglobin A1c (HbA1c) levels were detected to assess glycemic control.Twenty-three T1DM children who participated in the World Diabetes Foundation(WDF) management program (group A) were compared with 45 T1DM children who were managed by themselves (group B).The clinical data were analyzed to explore the correlations with glycemic control.Results The HbA1c of 68 children was (8.86 ± 2.00)%,and only 21 cases (30.9%) had optimal HbAlc (HbAlc < 7.5%).The level of HbA1 c of group A (7.60 ± 1.57)% was significantly lower than that of group B[(9.40 ± 1.91) %] (t =-4.045,P =0.001).Group A had less fluctuation in blood glucose than group B,which the duration more than 3 years.Duration (F =2.277,P =0.004),frequency of self-monitoring blood glucose (F =9.375,P =0.001),diabetic education (F =3.276,P =0.002),and diabetic diet (t =-2.046,P =0.025) were associated with glycemic control.Conclusions The glycemic control of T1 DM children at the Affiliated Hospital of Qingdao University was at the medium level,most of them haven't reached the target levels.The WDF management program could significantly and continuously monitor the glycemic level.Diabetic education and more frequent self-monitoring blood glucose and diabetic diet are associated with better glycemic control.
6.Design and application of medical banking system in class A tertiary hospital

Objective To design a medical banking system to realize self register,self settlement,self printing and etc so as to enhance the efficiency.Methods The system was composed of a outpatient self-service sub-system,a hospitalization self-service sub-system,an appointment register sub-system by network bank,a cellphone App for self service,a comprehensive supervision and management platform for self-service system and self-service terminals,which had the functions such as hospital outpatient service,hospitalization self service and background supervision and management.Results Fulfilled the requirements of the patient on self register and settlement,and decreased the workloads of medical staffs.Conclusion The satisfaction over hospital service is enhanced greatly,and the system is of great value for practical application.
7.Cognition and need of medical information in clinicians of Sichuan Province

The questionnaire investigation showed that the clinicians in Sichuan Province have a rather good cognition of medical information with a strong desire for medical information.However, measures should be taken to alleviate the contradictions between their information need and access ability, and excellent medical information service should be provided for them at a low cost .
8.Accuracy of18F-FDG PET/CT in Evaluating Mediastinal and Hilar Lymph Node Metastasis of Lung Cancer

PurposeTo investigate the accuracy of18F-FDG PET/CT in diagnosing mediastinal and hilar lymph node metastasis of lung cancer and in guiding surgery. Materials and Methods Seventy-eight pathology-proven lung cancer patients underwent 18F-FDG PET/CT scanning and surgery. Histology was used as gold standard to evaluate the diagnosis of mediastinal and hilar lymph node metastasis.18F-FDG PET/CT was also compared to chest CT.Results The pathological examination confirmed metastasis lymph nodes in 105 out of 231 excised lymph nodes in 18 patients. No lymph node metastasis was found in mediastinum in the other 60 patients with lymph node staging of pN0. Lymph node staging was pN1 in 5 patients, pN2 in 11 patients, and pN3 in 2 patients. In all 78 cases, the sensitivity, specificity, accuracy, positive predictive value and negative predictive value of18F-FDG PET/CT were 80.9%, 94.7%, 91.0%, 85.0%, 93.1%; while 61.1%, 71.7%, 69.2%, 39.2%, 86.0% on chest CT. These were statistically different (χ2=4.325,P<0.05).Conclusion PET/CT is superior to plain chest CT examination as an accurate and efficient method in evaluating lymph nodes staging of lung cancer.
9.Clinical observation on irinotecan as second-line treatment of patients with advanced colorectal carcinoma

Background and purpose:Colorectal carcinoma(CRC) is a major cause of cancer-related mortality. In the 1990s, several fi rst-line phase Ⅲ trails showed a signifi cant improvement in result with the addition of CPT-11(irinotecan, which is a specifi c inhibitor of topoisomerase Ⅰ) to FU-LV combination therapy(FOLFIRI).We observed the survival rate, effi cacy and adverse reaction of the combination chemotherapy of irinotecan plus folinic acid/continuous 5-? uorouracil bimonthly FOLFIRI regimen as second chemotherapy in treating advanced colorectal adenocarcinoma. Methods:Sixty patients with histologically proved advanced colorectal adenocarcinoma whose disease had progressed after treatment with first-line oxaliplatin or other chemotherapeutics agents were included to receive at least 2 cycles of chemotherapy with irinotecan 180 mg/m2 day 1 and folinic acid 200 mg/m2 iv,5-FU 400 mg/m2 iv bolus,days 1 and 2, 5-FU 600 mg/m2 iv infusion over 22 hours, days 1 and 2.Treatment was repeated every two weeks. Results:All patients were assessable for toxicity and 58 patients were evaluable for treatment response. The non-hematological toxicity was mild. Most was grade Ⅰ or Ⅱ. Only two patients experienced grade Ⅲ diarrhea and one patient experienced grade Ⅲ nausea and vomiting. There were no cases with grade Ⅳ toxicity. The most common hematological toxicity was neutropenia. Grade Ⅲ neutropenia were observed in fi ve patients. There was no case of febrile neutropenia. Based on intention to treat analysis, there were no complete responses (CR), 14(24.14%) partial response (PR), and 30 (51.72%) stable disease. the median time to disease progression was 6.09 months. The median overall survival was 9.65 months. Conclusions:Bimonthly irinotecan in combination with folinic acid and 5-fluorouracil was active with acceptable toxicities and a prolonged survival time in retreated colorectal cancer.
10.Optimum Selection of the Extracting Procedure of Rhizoma Corydalis

Objective To choose the bestway of extracting procedure of Rhizoma corydalis. Methods The optimal extraction process is selected with the orthogonal design. The content of corydalis B and the yield of extracts were used to evaluate the factor levels. Results The ideal extraction process is:extracted with 75% alcohol for 2 times, first 6 times amount, soaked 0.5 h, refluxed 1.5 h, then 4 times amount, refluxed 1.5 h. Conclusions This extractive condition is the best way.
