1.Primarily using fine needle aspiration for diagnosis of liver tumor at the Huu Nghi Hospital

Fine needle aspiration under the ultrasound has been applied for the first time at the H÷u NghÞ Hospital on 25 patients with liver tumors. The results showed that number of patients with liver carcinoma, abscess and cysts were 18, 2, and 1, respectively; 4 patients were unable to be diagnosed because the samples could not be collected. Only 6 patients felt mild pain during aspiration. No complication happened

2.Situation of the liver cancer in 125 patients through the CT scanner

365 patients with suspected liver tumor received the CT scanner in the X-ray department of the army central hospital 108 during 1996-1998. The results showed that 125 cases found suffering the liver cancer. Most of lesions were single focus and located in the right liver (81%). The lesions accompanied with cirrhosis (29.6%), with cholelitiasis or langitis (9.6%), with kidney stone (34%), 17% cases had invasive image (late stage)

3.Diagnosis of hepatic cancer by needle aspirasion under ultrasound

From May 1997 to May 1998, we conducted punctuation and aspiration of 30 cases of liver tumors under the guidance of ultrasound in order to diagnose by cytology. The result showed that, accurate diagnosis was 95% better than that given by the examination done with ultrasound (83.5%) and FP (52.5% positive). Punctuation and aspiration of liver tumors under the guidance of ultrasound is a simple method, easily to apply with high accuracy and allows quick and early diagnosis at the admission of the patient

4.The value of clinical examination, ultrasound, AFP, CT scanning, fine needle aspiration cytopuncture and combination of these methods in the diagnosis of hepato cellular carcinomas

In this prospective study, 685 patients with hepato cellular carcinomas were diagnosed and treated in the Central Military Hospital 108 for 4 years from Mar 2001 to Feb 2005. The resulst as: hepato cellular carcinomas often occured in males, 7 times higher than in females. The most common symptoms are pain on lower right side (80.9%), fatigue (45.5%), swollen liver (79.6%). 70.9% of patients were detected by ultrasound, 70.6% of patients were detected by computer tomography among which 51.8% with one single tumor; 38.7% with 3 or more tumors; 51% with tumors on right liver. Nearly 70% of patients had high AFP, 26.3% of patients had AFP>1000ng/ml. The accuracy of diagnosis increased to 97.5% by combination of all methods of clinical examination, ultrasound, AFP, CT scanning and fine needle aspiration cytopuncture

5.Application ultrasound technique for diagnosis and diagnostic differentiation of liver tumors and abscesses at 107 Tran Hung Dao Clinic

142 patients underwent diagnostic ultrasonography. Out of these, 100 patients have liver tumor and 42 have liver abscess. The prevalence is higher in male than in female and higher in 20-60 years of old than in other age groups. Liver tumor and abscess are more likely to be seen in right lobe than in left lobe with size of 5-10cm (55.7%). For liver abscess, sound reduction is more common (25.03%), while for liver tumor, sound combination and mixes are common types.

6.Clinical and paraclinical features of primary liver cancer

Primary liver cancer occurs frequently in male. It has a fatal outcome within the first 6 months and deaths are mainly due to coma and cachexia. 3 initial symptoms are often seen: anorexia, pain under the right costal margin and diarrhea. Liver cancer occurs frequently on a cirrhotic liver. Mass cancer and nodular cancer are the prevailing forms. Microscopically, hepatocellular carcinoma is the predominant picture. Metastasis to the liver pedicle nodes is often noted and distant metastasis is uncommon

40 patients of hepatic cellular carcinoma were absorbed the cells by small needle conducted by ultrasound, middle age was 48 ± 6.34, male/female = 1.91/1. The ultrasound result: rate of the site of tumors in the right live, left live was the same, in these, 45% of the patients had one tumor in the live. Using the ultrasound and probe 3,5 MHz with the syringe and small needle, 90% of the samples which were in right sits in the first time; in these, 75% of the slides was seen the hepatic cells. 52.8% of the absorbed samples were looked like the crushed organ of the live; in these, 80.48% of the samples was seen the neoplasm cells. The diagnostic rate of hepatic cellular carcinoma absorbed the cells by small needle conducted by ultrasound was 77.5%. No complications were seen after the operations

10.Impact of variations in fatty liver on sonographic detection of focal hepatic lesions originally identified by CT.

PURPOSE: The aim of this study was to investigate the influence of variations in fatty liver on the ultrasonographic detection of focal liver lesions. METHODS: A total of 229 patients with varying degrees of fatty liver and focal liver lesions and 200 patients with focal liver lesions but no fatty liver were randomly selected for inclusion in groups I and II, respectively. Findings of focal liver lesions identified on computed tomography were taken as the reference, and findings on ultrasonography were compared with them. RESULTS: The number of focal liver lesions in groups I and II were 501 and 413, respectively. The ultrasonographic detection rates of focal liver lesions in groups I and II were 86.8% (435/501) and 94.2% (389/413), respectively. Comparison of the detection of the focal lesions between patients with and without fatty liver or different grades of fatty liver were as follows: mild fatty liver (162/177) vs. liver without fat infiltration (389/413) (P=0.277); mild fatty liver (162/177) vs. moderate fatty liver (190/212) (P=0.604); mild fatty liver (162/177) vs. severe fatty liver (83/112) (P<0.001); moderate fatty liver (190/212) vs. liver without fat infiltration (389/413) (P=0.051); moderate fatty liver (190/212) vs. severe fatty liver (83/112) (P<0.001); severe fatty liver (83/112) vs. liver without fat infiltration (389/413) (P<0.001); and fatty liver (435/501) vs. liver without fat infiltration (389/413) (P<0.001). CONCLUSION: Mild and moderate fatty liver are not significantly associated with the visualization of the lesion, while severe fatty liver usually impairs the detection of focal lesions in the liver. If a patient with severe fatty liver is suspected to have a liver tumor, ultrasonography should only be chosen cautiously in case of a missed diagnosis.
