Objective To investigate the clinical effect of chronic dacryocystitis through dacryocystorhinostomy.Methods The clinical data about 104 eyes in 84 chronic dacryocystitis patients who treated by dacryocystorhinostomy were analyzed.Than summarized and analysed the preoperative preparation,operation method and the curative effect after operation.Results The 104 eyes operation were all successful.The patients followed up for 6 to 24 months.96 eyes had no symptom of tear overflows and overflow pus(approximately 92.31%),2 eyes recurrenced after 1 year,then after operation recovered again(approximately 1.92%),4 eyes had symptom of tear overflows,but tear road washing unobstructed (approximately 3.85%),2 eyes lost to follow up(the rate Was about 1.92%).Conclusion The clinical treatment on the chronic dacryocystltis by dacryocystorhinostomy was effective and the recurrence rate wag very seldom.

【Objective】The effect of sinomenine on human cyclo-oxygenase(COX) and interleukin-1?(IL-1?) was observed to explore the anti-inflammatory mechanism of sinomenine.【Methods】Peripheral blood monocytes(PBMC) were isolated and incubated with sinomenine.After incubation,total RNA from PBMC was extracted and the effect of sinomenine on COX and IL-1? expression with or without the stimulation of lipopolysaccharide(LPS) was observed by the method of semi-quantitative reverse transcription polymerase chain reaction(RT-PCR).【Results】Sinomenine had no effect on COX-1 gene expression,had a slight inhibitory effect on COX-2 gene expression and had a strong inhibitory effect on IL-1? gene expression.【Conclusion】 The anti-inflammatory mechanism may be related to the inhibition of inflammatory cytokines.

Objective To compare the genetic difference of Pogostemon cablin from different habitats.Methods Forty-six primers were used to analyze the fingerprint of Pogostemon cablin by Random Amplified Polymorphic DNA,and 6 of them were identifiable.Primer S358 could reflect the tiny polymorphic differences of Pogostemon cablin from different habitats which had 3 same amplified strips.Primers S358,S359,S443 could be used for the identification of Shipai sample from its other species.Results The results of cluster analysis showed that the two samples from Gaoyao were classified as one type,the four samples from Zhanjiang,Hainan,Shipai as another type,one sample from Caizhilin and one from the pharmacy of the First Affiliated Hospital of Guangzhou University of TCM as the the third type,and the sample from the Qingping Traditional Chinese Herbal Market as the fourth type.Conclusion The RAPD analysis can be used for the identification of Pogostemon cablin from different habitats.

Objective To observe the effect of naringin on proliferation and differentiation of 3T3-L1 preadipocyte,and to investigate its possible mechanism.Methods The cultured 3T3-L1 cells were pretreated with naringin,and then MTT method was used to detect the proliferation of the cells,oil red O staining method and spectrophotography were applied to analyze the degree of differentiation.The expression of PPAR? 2 and C/EBP? mRNA associated with adipocytes differentiation were detected by reverse transcription PCR(RT-PCR).Results Naringin suppressed the proliferation and differentiation of 3T3-L1 preadipocytes and down-regulated the expression of PPAR? 2 and C/EBP? mRNA.Conclusion Naringin can inhibit the proliferation of 3T3-L1,and can suppress their differentiation through down-regulating the expression of PPAR? 2 and C/EBP? mRNA.

Objective To better understand students' professional perception,humanistic education status quo and their preference to different humanistic education methods.Methods Questionnaire was conducted for 990 undergraduate students (2006 grade- 2010 grade) at medical school of Shanghai Jiao Tong university.SPSS 17.0 statistical software was used to analyze results of the questionnaire.Results Nearly 45％ students chose to major in clinical medicine out of personal interest.The identification of students for humanistic quality was lower than that for clinical knowledge and skills.More than 50％ students believed doctors were with high humanistic quality and there was a gap between students′ action and their understanding of the medical career identity and medical humanistic cultivation.More than 50％ students preferred activities that train medical and auxiliary skills.Nearly 67％ students also showed a shallow reading habit from regular internet surfing.Conclusion The professional perception of medical students are influenced by both social and historical factors influence.The gap between understanding and action is a consequence of current humanistic educational system.The humanistic educational methodology should conform to the psychological and s~ial characteristics of contemporary students.

Background:For patients with liver cirrhosis and acute decompensation(AD),it is of great clinical importance to predict short-term mortality at admission. It has been reported that CLIF-C OF,MELD and MELD-Na score can accurately predict the short-term mortality,but all these scoring systems are complicated and have limits in their application. Aims:To define a simple and objective scoring system -- simplified MELD score for short-term mortality prediction in HBV-related cirrhotic patients with AD. Methods:A total of 890 consecutive HBV-related cirrhotic patients with AD hospitalized during Jan. 2005 to Dec. 2010 at Shanghai Ren Ji Hospital were enrolled retrospectively. Clinical data and patients’outcome were collected,and simplified MELD score was calculated by using total bilirubin,international normalized ratio and creatinine values at admission. Patients were classified into different prognostic groups according to their 28-day mortalities and simplified MELD score. Kaplan-Meier survival curve was used to analyze the 1-year accumulate survival rate,and ROC curve was used to evaluate the performance of different scoring systems in predicting 28-day mortality. Results:Simplified MELD score at admission could classify HBV-related cirrhotic patients with AD into low,moderate and high 28-day mortality groups and different long-term prognostic groups;the score of low,moderate and high 28-day mortality group was 0-2,3 and 4-6,respectively,and the corresponding mortality was 5. 5% ,19. 8% and 48. 6% ,respectively. Simplified MELD score had the same good performance as compared with the CLIF-C OF,MELD and MELD-Na scores in predicting 28-day mortality,the area under ROC curve was 0. 828,0. 831,0. 828 and 0. 830,respectively. Conclusions:Simplified MELD score can accurately classify HBV-related cirrhotic patients with AD into low,moderate and high 28-day mortality groups at admission. It is convenient for using in clinical practice.

Objective To study the effect of Dahuang Mudan Decoction on experimental colitis induced by trinitrobenzene sulfonic acid(TNBS)in mice and to explore its mechanism.Methods Mice colitis model was established by rectal administration of trinitrobenzene sulfonic acid.Disease activity index(DAI)and histological scores were assessed,the serum level of TNF-? was detected by ELISA.Results Dahuang Mudan Decoction reduced DAI and histological scores in mice,and alleviate inflammation in colonic tissue.It also significantly decreased mice serum TNF-? level.Conclusions Dahuang Mudan Decoction shows positive therapeutic efficacy on mice colitis induced by trinitrobenzene sulfonic acid,and its mechanism may be involved in inhibiting the production of TNF-?.

Objective To screen and diagnose obstructive sleep apnea-hypopnea syndrome (OSAHS) in elderly males by obesity index using receiver operating characteristic(ROC) curves. Methods Data of 402 consecutive elderly male patients who underwent polysomnography from 2001 to 2008 were collected. The relationship between apnea hypopnea index(AHI) and obese indexes such as body mass index (BMI), neck circumference (NC), waist circumference (WC) and waist-to-hip ratio (WHR) were analyzed by Pearson's correlation. ROC curves were used to determine the best cutoff values to screen and diagnose OSAHS, and their priority was compared by area under curve (AUC). A two-tailed P value less than 0.05 was considered statistically significant. Statistical analysis was carried out with SPSS version 13.0. Results (1) AHI was positively correlated with BMI (r=0.241,P<0.001), NC(r=0.201,P<0.001), WC(r=0.210,P<0.001) and WHR(r=0. 097,P>0.05)) in elderly male patients. The area under curve (AUC) of BMI, NC, WC and WHR was 0.61, 0.58, 0.51 and 0.45 respectively, and P value was 0.001,0.060,0.840 and 0. 250 respectively. Only BMI was competent in screening and diagnosing OSAHS in elderly male adults; (2) The optimal value of BMI was 22.0 kg/m~2 in screening OSAHS with specificity 90% and rate of missed diagnosis 10%; (3) The optimal value of BMI was 29.0 kg/m~2 in diagnosing OSAHS with specificity 90% and rate of missed diagnosis 10%. Conclusions BMI more than 22.0 kg/m~2 could be the reference standard to screen OSAHS and BMI more than 29.0 kg/m~2 to diagnose OSAHS in elderly men.

The samples of experimental and control groups were obtained by 1 ∶ 1 matching method and divided into development (242 pairs) and sign (156 pairs) groups.Through a 10-month group and case type intervention,Gesell test was performed for 5 zone development quotients of large motor,fine motor,adaptability,speech and personal-social to determinate the effectiveness of interventions.Before intervention,t values of the development group were 1.07,1.42,0.78,0.62 and 1.67 while those of sign group-1.48,-1.96,1.82,1.78 and-1.73 respectively.The inter-group comparison of various developmental quotient differences had no statistical significance (P ＞ 0.05).After intervention,t values of the development group were 9.85,8.92,3.57,11.21 and 9.30 for inter-group comparison and-31.65,-36.94,-46.26,-37.56 and-49.85 for intra-group comparison.The results of sign group were 10.91,9.61,10.75,12.01 and 14.36 for inter-group comparison and-23.20,-31.75,-44.94,-33.58 and-54.58 for intra-group comparison.Between intra-group and inter-group,the comparison of the developmental quotient differences had statistical significance (P ＜ 0.01).In two experimental groups,the post-intervention developmental quotients in 5 zones are better than those pre-intervention levels.It shows that preventive intervention of infant mental health is effective.

Different varieties of Terminalia chebula Retz. were identified by DNA finger prints, andat the same time, some problems encountered during the application of RAPD for the identification ofTCM were studied. The polymorphic features of DNA finger prints of different species were compared.Results of the study showed that they were differently related in heredity which can provide a basis for thedifferentiation of one species from the other. Means for the solution of problems arised during RAPD ap-plication were suggested. It was concluded that RAPD is an effective method to identify T. chebula from itsconfusable varieties.
