1.Study on the Extraction Technology of Compound Qima Capsules

OBJECTIVE:To optimize the extraction technology of Compound qima capsules. METHODS:With the blood pres-sure lowering of rats as index,pharmacological efficacy test was used to screen the preparation technology(A was whole herb de-coction;B was Gastrodia elata fine powder mixed with other decocted medical materials). The extraction technology was opti-mized by single factor and orthogonal test using the contents of astragaloside and isoflavone grape glycosides and the quality of sol-id as indexes,with added water,decoction time,decoction times as factors;and the verification test was carried out. RESULTS:Pharmacological efficacy test showed that antihypertensive effect of sample by technology B was superior. The optimal extraction condition of other medical materials of technology B was as follows as 12-fold water per time,decocting for 1.5 h,for 3 times. In verification test,average extraction rates of astragaloside and isoflavone grape glycosides were 64.02% and 51.97%,and average value of the quality of solid was 5.69 g(RSD≤1.92%,n=3). CONCLUSIONS:The optimized extraction technology is stable and feasible.
2.Studies on the anti-inflammatory molecular mechanism of chlorogenic acid extracted from Lonicera confusa DC in vitro

Aim To investigate the anti-inflammatory molecular mechanism of chlorogenic acid extracted from Lonicera confusa DC in vitro.Methods PM? of a rat was segregated.MTT assay was used to detect the effects of the chlorogenic acid on PM? cells growth activities. PM? was stimulated with LPS for a prolonged period,ELISA was used to detect the level of TNF-?,IL-6 and PGE2 in the supernatant;COX-2 activity was determined by the level of PGE2 in the supernatant.After stimulating PM? with A23187 for a short time,the 6-keto-PGF1? level in the supernatant was measured by radioimmunoassay to express COX-1 activity.Results Chlorogenic acid had no inhibitive effects between 31.25 mg?L-1 and 1000 mg?L-1.The level of TNF-?,IL-6 and PGE2 in drug groups was lower than that of LPS-induced group,and the difference was significant,in a dose-dependent manner.The concentration of 50 mg?L-1 group was ineffective in the expression of TNF-?.Low concentration chlorogenic acid inhibited the expression of 6-keto-PGF1?,while high-concentration induced it.Conclusions The anti-inflammatory effect of chlorogenic acid may be related to inhibiting TNF-?,IL-6 activity and affecting exogenous AA metabolism.
3.Vitreous cavity injection with erythropoietin protects optic nerve in a rat model of diabetic retinopathy

BACKGROUND: Diabetes can lead to many complications. Of them, retinopathy is a common type. To explore the effect of erythropoietin in diabetic retinopathy, it is necessary to establish an animal model of similar pathologic features and high reproducibility in clinical retinal neovascularization.
OBJECTIVE: To investigate the protective effect of vitreous cavity injection of erythropoietin on optic nerve of rats with diabetic retinopathy.
METHODS: Rat models of diabetic retinopathy were established and injected with erythropoietin through vitreous space for 14 consecutive days. Results were compared between the model and normal control groups.
RESULTS AND CONCLUSION: (1) Transmission electron microscope: compared with the model group, retinal ganglion cel nucleus presented uniform red; cel membrane was intact; endoplasmic reticulum exhibited slight swel ing. A smal number of vacuoles were found in mitochondria around the nuclei. (2) Western blot assay and RT-PCR: compared with the model group, Caspase-3 and Bax protein expression was lower in the erythropoietin group (P < 0.05). Caspase-3 protein and Bcl-2 protein expression was significantly higher in the erythropoietin group than in the model group (P < 0.05). Significant differences in nuclear factor κB and Survivn gene expression were detected between the model and erythropoietin groups (P < 0.05). (3) Results verified that vitreous cavity injection of erythropoietin had neuroprotective effect on rats with diabetic retinopathy, which may be associated with the inhibitory effect on apoptosis.

OBJECTIVE To investigate the clinical efficacy of microwave ablation in treating snoring and obstructive sleep apnea hypopnea syndrome(OSAHS). METHODS Eight patients with snores and 48 with OSAHS were included in the study. They all received micro-surgeries including plastic operations on the uvula and soft palate,tonsil ablation and tonsillectomy,inferior nasal concha and adenoid ablation,and de-volume of the tongue root. Symptoms six months before and after surgery were monitored by Polysomnography(PSG). RESULTS In 56 cases, the patients’ throats were widened. 5.Regulatory effects and mechanism about iRGD-functionalized nanomicelles on colorectal cancer multidrug resistance

In recent years, conventional targeting of chemotherapeutic drugs on colorectal tumor tissue is poor in the clinical application. Due to the multidrug resistance of colorectal tumors, penetration and cytotoxicity of conventional drugs greatly reduced on tumor tissue. With the advent of tumor-penetrating peptides, a new and highly effective antitumor drug delivery system has become a research topic of international scholars. This article will briefly describe the research progress of iRGD peptides with the modified nanomicelles drug delivery system on targeted drug delivery and resistance to drug-resistant colorectal tumors in recent years. These studies show that iRGD peptide-modified nanomicelles will be a highly potential anti-drug delivery system.
6.Establishment and reliability and validity test of fluid intake compliance motivation scale for hemodialysis patients

Objective:To establish fluid intake compliance motivation scale for hemodialysis patients and to test its reliability and validity.Methods:With protection motivation theory as the basic framework, the initial scale was established by means of literature analysis, patient interview and expert consultation. From April 2019 to July 2019, a total of 170 patients with hemodialysis in 10 blood purification center in Jiangsu Province were selected for preliminary investigation by convenient sampling method, and the tentative scale was formed through project analysis and exploratory factor analysis. A total of 550 hemodialysis patients were selected for formal testing to evaluate the reliability and validity of the scale.Results:The final scale consisted of 22 items. 7 common factors are extracted by exploratory factor analysis and the cumulative variance contribution rate was 75.949%. The Cronbach's α coefficient of the total scale was 0.928, Spearman-Brown coefficient was 0.841 and the retest reliability coefficient was 0.899. The content validity index of the scale was 0.987. The confirmatory factor analysis model fitted well.Conclusions:Fluid intake compliance motivation scale for hemodialysis patients has good reliability and validity, which can be used to assess fluid intake compliance motivation of patients.
7.Maturity-onset diabetes of the young type 3 complicated with type 5:A case report and literature review

This report describes a case of maturity-onset diabetes of the young(MODY)type 3(MODY3)complicated with type 5(MODY5),including the patient's clinical features,diagnosis,and treatment,and reviews relevant literature.Using next-generation sequencing of MODY(types 1-14)gene exons and Sanger sequencing for verification,the patient and her mother were assessed.Based on the clinical phenotype and genetic test results,the patient was diagnosed as MODY3 combined with MODY5.Treatment included insulin and linagliptin,with monitoring of blood glucose changes.Clinicians should enhance their understanding of MODY clinical phenotypes.In adolescents with diabetes who have congenital pancreatic and renal developmental defects,elevated high-density lipoprotein cholesterol,no spontaneous ketosis,insulin secretion defects,negative pancreatic autoantibodies,no significant insulin resistance,and who are not obese,gene testing should be conducted to screen for MODY.Accurate diagnosis and personalized treatment can aid in achieving glycemic control,improving quality of life,and optimizing reproductive planning.

Objective:To investigate the normal reference values of spinal bone mineral density measured by quantitative computed tomography (QCT) and the differences of bone mineral density (BMD) in different regions of in Chinese adult males.Methods:Men who underwent low-dose CT lung scan for cancer screening in regions of Northeast, North, East, South, Central and Southwest of China from January 2018 to December 2019 were selected. And the lumbar vertebrae BMD values in the male subjects were measured by the QCT system (Mindways Software, Inc.). The mean BMD values and their decline rates were calculated at an age interval of 10 years, and the prevalence of osteoporosis was calculated according to the American College of Radiology spine QCT osteoporosis diagnostic criteria.Results:A total of 50 682 males with a mean age of (50.22±12.79) years (ranged 20 to 98 years) were included in this study. The peak BMD of (173.11±28.56) mg/cm 3 in the healthy Chinese adult male population appeared in the age group of 20 to 29 years and then declined with age. Before the age of 70 years, the BMD was relatively higher in males in South China, and it was lower in Central China and Southwest China, and it was intermediate in Northeast, North and East of China, with statistically significant differences. There was no significant differences in BMD in the males in the two age groups of 70 to 79 years and 80 and older among the regions in China. The overall decline rate of spinal BMD in Chinese males under QCT was about 46.92% over the lifetime, and it declined obviouslyin the 40-49 age group. The overall prevalence of osteoporosis in Chinese male population aged 50 years and above was approximately 11.42%, with the highest prevalence in Southwest China and Central China (14.72% and 13.87%, respectively) and the lowest in North China and South China (8.53% and 7.71%, respectively). Conclusions:A reference of lumbar spine BMD values for healthy males in China based on QCT is established. BMD values were highest in South China and Lowest in Central China.
9.Prognostic significance of metastatic lymph nodes ratio in patients with gastric adenocarcinoma after curative gastrectomy.

BACKGROUNDWe evaluated the impact of the number of metastatic lymph nodes and the metastatic lymph nodes ratio (the ratio between metastatic lymph nodes and total dissected lymph nodes, MLNR) in patients with gastric adenocarcinoma following curative gastrectomy and also analyzed the relationship between the number of removed lymph nodes and prognosis in node-negative gastric cancer.
METHODSFrom January 2005 to December 2010, 1 390 patients who were diagnosed with gastric adenocarcinoma and underwent curative gastrectomy were included. In particular, lymph node metastasis was not present in 515 patients. The number of metastatic lymph nodes and the metastatic lymph nodes ratio were selected for univariate and multivariate analyses to evaluate their influences on the disease outcome. The survival curve was presented according to the number of removed lymph nodes in node-negative gastric cancer using Kaplan-Meier plots.
RESULTSThe overall 5-year survival rate was 54% in this group. Univariate analysis revealed that age category, macroscopic appearance, histological grade, tumor size, depth of primary tumor invasion, number of metastatic lymph nodes, metastatic lymph nodes ratio, tumor, nodes, metastasis-classification (TNM) stage and status of lymphovascular, and vessel invasion have significant impact on survival. The number of metastatic lymph nodes and the metastatic lymph nodes ratio both have significant impact on survival (P < 0.001). However, in multivariate analyses, only the metastatic lymph nodes ratio was identified to be an independent prognostic factor (P < 0.001). The number of removed lymph nodes in node-negative was a strong prognostic factor of survival, the more lymph nodes dissected, the better the survival.
CONCLUSIONSThe metastatic lymph nodes ratio has more significant prognostic value for survival in patients with gastric cancer following curative gastrectomy than the number of metastatic lymph nodes. The number of removed lymph nodes might be an important prognostic factor for gastric cancer without lymph node metastasis.
