1.The application and clinical significance of impulse oscillometry lung function in children with bronchial asthma

Objective To investigate the clinical application and diagnostic value of impulse oscillometry lung function in children with bronchial asthma.Methods Thirty-two children (19 children in acute attack group,13 children in clinical remission group) with bronchial asthma were selected,and 20 healthy children as control group.The lung function simation by Master Screen impulse oscillometry device was detected.Results The outcome of the R5,R20,R5-R20,the negative of X5 and Fres in acute attack group (21.90±2.70) Hz]were higher than those in clinical remission group [(7.75±0.97),(3.82±0.50),outcome of the R5,R5-R20 and Fres in clinical remission group were higher than those in control group (P<0.05),but there were no statistical significance in R20 and X5 in the two groups (P>0.05).Conclusion Impulse oscillometry can provide various objective indicators to bronchial asthma in acute attack and clinical remission stage.which can be used as a method in detecting pulmonary function and monitoring therapy of bronchial asthmatic children.
2.Analysis of Optical Coherence Tomography Based on Monte Carlo Simulation Model.

A Monte Carlo model for optical coherence tomography of human skin is proposed. The new model includes importance sampling technique which is designed to suit for the multi-layer human skin, new rules for back scattered photon classification are correspondingly proposed. Based on the new simulation model, we analyzed the focusing of Gaussian beam through skin and the maximum detecting depth of optical coherence tomography. The experimental results show that there exists focus distortion when beam propagates in skin, including focus shift and diffusion. Object lens with greater NA will lower the maximum detecting depth of optical coherence tomography.

3.Changes of platelet function after clopidogrel discontinuation in patients with percutaneous coronary interventions

Objective To investigate the changes of platelet aggregation rate and platelet surface CD 41+CD62P+ expression af-ter clopidogrel discontinuation in the patients with percutaneous coronary interventions (PCI)) .Methods The platelet aggregation rates and platelet surface CD41+CD62P+ in 52 PCI patients with oral clopidogrel for near 12 months and discontinuation soon were measured before clopidogrel therapy(T0 ) ,in 1 week after clopidogrel therapy(T1 ) ,1 week before clopidogrel discontinuation(T2 ) , 1 week(T3 ) and 1 month after clopidogrel discontinuation (T4 ) .Results Compared with T0 ,the platelet aggregation rate and the expression of platelet CD41+CD62P+ at T1 were significantly decreased ,the difference showing statistical significance (P<0 .05) , which at T2 maintained the lower levels ;which at T3 were increased ,which at T4 were recovered to those at T0 .The platelet aggre-gation rates at various time points were (44 .20 ± 18 .36)% ,(25 .38 ± 12 .10)% ,(23 .74 ± 8 .15)% ,(51 .79 ± 10 .55)% and(45 .97 ± 16 .42)% respectively ,and the positive rates of CD41+ CD62P+ were(12 .96 ± 11 .48)% ,(3 .93 ± 3 .33)% ,(4 .72 ± 3 .14)% , (13 .90 ± 10 .38)% and(10 .84 ± 8 .13)% ,respectively .Conclusion In the patients treated with 12-month clopidogrel after PCI ,the platelet aggregation rate and the CD41+CD62P+ positive rate are mildly increased at 1 week after clopidogrel discontinuation and gradually returned to the level before discontinuation at 1 month after clopidogrel discontinuation .
4.Clinical significance of prostatic hypoechoic lesions on ultrasonography

Objective To evaluate the clinical significance of prostatic hypoechoic lesions on transrectal ultrasound (TRUS) for prostate cancer detection.Methods Four hundred and thirty-eight patients referred for biopsy of the prostate from August 1999 to August 2004. Every patient received 6-13 biopsy cores of the prostate. If a hypoechoic was identified, the biopsy was taken from this lesion. Correlation between hypo-echoic lesions, isoechoic areas and cancer detection for each core was performed. Results Among 438 patients, cancer was detected in 112(25.6%). Hypo-echoic lesions were seen in 75 (67%) patients with prostate cancer, the isoechoic were as in 37 (33%). A total of 3504 biopsy cores were obtained from 438 patients, and 636 biopsy cores were taken from hypoechoic lesions. Among 636 biopsy cores, 163 (25.6%) were cancer and 473(74.4%)were not. The cancer detective rate in patients with hypoechoic lesions (75/298, 25.2%) was similar to non-hypoechoic lesions (37/140, 26.4%) on TRUS. Aim To investigate the anti-inflammatory molecular mechanism of chlorogenic acid extracted from Lonicera confusa DC in vitro.Methods PM? of a rat was segregated.MTT assay was used to detect the effects of the chlorogenic acid on PM? cells growth activities. PM? was stimulated with LPS for a prolonged period,ELISA was used to detect the level of TNF-?,IL-6 and PGE2 in the supernatant;COX-2 activity was determined by the level of PGE2 in the supernatant.After stimulating PM? with A23187 for a short time,the 6-keto-PGF1? level in the supernatant was measured by radioimmunoassay to express COX-1 activity.Results Chlorogenic acid had no inhibitive effects between 31.25 mg?L-1 and 1000 mg?L-1.The level of TNF-?,IL-6 and PGE2 in drug groups was lower than that of LPS-induced group,and the difference was significant,in a dose-dependent manner.The concentration of 50 mg?L-1 group was ineffective in the expression of TNF-?.Low concentration chlorogenic acid inhibited the expression of 6-keto-PGF1?,while high-concentration induced it.Conclusions The anti-inflammatory effect of chlorogenic acid may be related to inhibiting TNF-?,IL-6 activity and affecting exogenous AA metabolism.
6.Study on the Extraction Technology of Compound Qima Capsules

OBJECTIVE:To optimize the extraction technology of Compound qima capsules. METHODS:With the blood pres-sure lowering of rats as index,pharmacological efficacy test was used to screen the preparation technology(A was whole herb de-coction;B was Gastrodia elata fine powder mixed with other decocted medical materials). The extraction technology was opti-mized by single factor and orthogonal test using the contents of astragaloside and isoflavone grape glycosides and the quality of sol-id as indexes,with added water,decoction time,decoction times as factors;and the verification test was carried out. RESULTS:Pharmacological efficacy test showed that antihypertensive effect of sample by technology B was superior. The optimal extraction condition of other medical materials of technology B was as follows as 12-fold water per time,decocting for 1.5 h,for 3 times. In verification test,average extraction rates of astragaloside and isoflavone grape glycosides were 64.02% and 51.97%,and average value of the quality of solid was 5.69 g(RSD≤1.92%,n=3). CONCLUSIONS:The optimized extraction technology is stable and feasible.
7.Different fixation methods for artificial femoral head replacement: A biomechanical comparison of joint stability

BACKGROUND: Artificial humeral head replacement is an effective method for the treatment of complex proximal humeral fractures, which has received good results in relieving pain. However, the final functional recovery is unpredictable. OBJECTIVE: To compare biomeshanical stability between anatomical and overlapping reconstruction of the greater tuberosity in cadaveric humeral head replacement models.METHODS: Eight pairs of fresh-frozen shoulder cadavers (16 shoulder joints) were match-paired into two groups. Standardized humeral head replacement procedure was performed in all specimens, and anatomical and overlapping reconstruction of thegreater tuberosity was adopted in each group respectively. For overlapping group, the greater tuberosity was reattached to the proximal humeral shaft in an overlapping style, which was achieved by an additional 5 mm bone osteotomized from the medial cortex of the humeral diaphysis. Custom mounting apparatus and fixation jigs were designed for designated shoulder motion.RESULTS AND CONCLUSION: When the shoulder was external rotated to neutral position, the mean displacement of greater tuberosity in the anatomical reconstruction group was smaller than that of the overlapping reconstruction group (P < 0.05). When the gleno-humeral joint was elevated to 30~ and 60~ forward flexion (accounting for 45° and 90° shoulder forward flexion), there was no significant difference of greater tuberosity displacement between the anatomical group and overlapping group. The findings demonstrated that, although overlapping reconstruction can increase the bone healing area between the greater tuberosity and the humeral diaphysis, there may be some loss in mechanical stability as the trade-off. Even though we strictly follow the standardized postoperative rehabilitation protocol after humeral head replacement, prominent displacement between the greater tuberosity relative to the humeral diaphysis was detected. Accordingly, postponing of the postoperative rehabilitation program after humeral head replacement for a decent period may improve tuberosity healing.
8.Quality standard for Yinhua Mangguo Granule

AIM: To establish the HPLC-FPC of Yinhua Mangguo extract(Flos lonicerae japonicae,Folium Mangiferae,Pericarpium Citri reticulate,etc.). METHODS: The gradient elution was adopted with acetonitrile,methanol and 0.05% phosphoric acid as a mobile phase.The detection wavelength was at 238 nm. RESULTS: The components of Yinhua Mangguo extract were well separated on HPLC,and the HPLC-FPC of 10 batches of samples was established.The results showed that this method had a good repeatability,and the 12 common peaks had an exclusive properties. CONCLUSION: The method developed can evaluate the quality of Yinhua Mangguo extract.
9.Study on the Purification Technology of Sanhuang Yishen Formula

OBJECTIVE:To study the purification technology of Sanhuang yishen formula. METHODS:Using retention rate and impurity rate of purified total polysaccharide,astragaloside and calycosin glucoside as index,the purification effects of water extraction and alcohol precipitation method (50%,60%,70% ethanol) and clarifying agent method (101 juice clarifying agent, ZTC natural clarifying agent,chitosan clarifying agent) were respectively detected to screen the purification method;orthogonal test was used to optimize the technology parameters(mass concentration of liquid,amount of clarifying agent and pH of liquid)by the optimized purification method,and the verification test was conducted. RESULTS:The purification was better when using chito-san as clarifying agent with comprehensive score of 98.62;the purified technology parameters were mass concentration of liquid 1 g/mL,1% chitosan solution amount of 2 mL/g,pH 5.1;the average value of retention rate and impurity rate of purified total poly-saccharide, astragaloside and calycosin glucoside in verification test were 79.56%(RSD=1.24%, n=3), 78.11%(RSD=0.97%,n=3),79.46%(RSD=1.03%,n=3)and 32.18%(RSD=1.16%,n=3),respectively. CONCLUSIONS:Using chito-san as clarifying agent shows good purification effect for Sanhuang yishen formula,which is simple. The optimized technology is stable and feasible.
10.Effects of Polysaccharides from Millettia speciosa Champ on Proliferation of Spleen Lymphocyte and Secretion of Cytokine in Mice

Objective To explore the mechanism of immunoregulation by investigating the effects of polysaccharides from Millettia speciosa Champ (MSC) on proliferation of spleen lymphocyte and secretion of cytokine in mice.Methods The effects of MSC polysaccharides on Con A-induced spleen T lymphocyte proliferation were determined by MTT.The contents of TNF-α, IL-6 and PGE2 were determined by ELISA.Results The Con A-induced T lymphocyte proliferation was significantly increased by MSC polysaccharides at the concentrations from 50 to 200 μg·mL-1.Coupled with TNF-α and IL-6 were significantly increased while PGE2 was significantly decreased.Conclusion MSC polysaccharides could increase proliferation of spleen lymphocyte and enhance the immune responses by increasing the secretion of TNF-α and IL-6 in mice.
