2.Effusion quantification in the knee cavity and peripheral synovial bursa: MRI determination

BACKGROUND: MRI is a gold standard for imaging diagnosis of knee joint disease, but it cannot accurately quantitate effusion of knee.OBJECTIVE: To investigate different volume of contrast-medium in the cavity of knee with MRI scanning to provide objective evidence and quantitaive standard for clinical diagnosis of effusion in the cavity of knee.DESIGN, TIME AND SETTING: Quantitative observation was performed in the MRI Room, the First Affiliated Hospital of Chengdu Medical College from October to December 2008.MATERIALS: Ten samples of fresh normal adult knee joint.METHODS: Puncture with the needle retaining in the cavity of knee of normal adult corpse without effusion or pneumatosis was performed, and 1, 5, 10, 15, 20, 30, 40 mL contrast-medium was injected into the cavity. Coronal section, vertical plane and transect were scanned with T2WI, 4 mm thickness and 1 mm interval by MRI.MAIN OUTCOME MEASURES: The distribution of different amounts of contrast agent in the knee cavity and peripheral synovial bursa in knee coronal plane and sagittal cross-sections.RESULTS:?MRI scanning from coronal section: after 10 mL contrast-medium injection, in the center of medial knee joint, high visual signal was seen in the cavity of joint; after 15 mL contrast-medium injection, scanning between femora and patella, high visual signals were observed; after 20 mL contrast-medium injection, scanning by posterior internal and external condyle of knee, high visual signals were found in 80% of internal and external recess of posterosuperior of thighbone.?MRI scaning at 1.5 cm from internal vertical plane of middle knee: after 5 mL contrast-medium injection, high visual signal area appeared in intercondylar eminence, narrowing gradually, and 70% of posterosuperior internal condyle of tibia; after 10 mL contrast-medium injection, high visual signals appeared at the 1/3 point under suprapatellar bursa; after 15 and 20 mL contrast-medium injection, high visual signal appeared in 80% of 1/3 point of suprapatellar bursa; after 30 and 40 mL contrast-medium injection, high visual signal was seen both internal and external recess of posterosuperior knee.?MRI scanning from inframarginal Apex of patella of knee transect: after 5 mL contrast-medium injection, high visual signal appeared in 90% area at 1/3 point posterior articular cavity; after more than 10 mL contrast-medium injection, high visual signal was seen in 70% area of anterior thighbone facet. CONCLUSION: By injecting different volumes of contrast-medium into the knee cavity and peripheral synovial bursa, reference standard can be constructed according to different volumes of effusion, which is significant for diagnosis of effusion of knee cavity and peripheral synovial bursa with MRI.
3.Effect of Water-Soluble Total Alkaloids of Yimucao Injection on Uterine Contractile Activity in Postpartum Uterine Involution

Objective To investigate the effect of Water-soluble total alkaloids of Yimucao Injection(YITA)on postpartum uterine involution.Methods A guinea pig abortion model was established by drug method.0.17 g·kg-1 YITA was intragastrically administered to record the amount of bleeding and the contents of E2 and P in serum of guinea pigs after delivery.The uterine coefficient was calculated and the contents of NO,ET-1 and Ca2+ in uterine tissue were measured.The isolated uterine model and the in vivo uterine model were prepared by guinea pigs.The former was given YITA 0.05,0.1 and 0.2 mg·mL-1,and the latter was intramuscularly injected with YITA 10,20 and 40 mg·kg-1.The changes of uterine smooth muscle(USM)contractile activity,frequency and tension were recorded by biological function test system.Results Intragastric administration of YITA could decrease the uterine coefficient(P<0.001),NO content in uterine tissue and serum P content in guinea pigs with medical abortion(P<0.01),significantly increased ET/NO ratio(P<0.001);it tended to increase Ca2+ content in uterine tissue(P<0.1).In the isolated USM test,50 and 100 mg·L-1 YITA significantly increased USM contractile activity in guinea pigs(P<0.05),and 200 mg·L-1 YITA significantly decreased USM contractile minimum in guinea pigs(P<0.05)in the in vivo USM contractile test,YITA enhanced USM contractile activity,and 40 mg·kg-1 YITA significantly increased USM contractile activity and frequency in guinea pigs(P<0.05);10 mg·kg-1 YITA significantly increased USM contractile frequency in guinea pigs(P<0.05)and tended to increase motility(P=0.0569<0.1);20 mg·kg-1 YITA tended to increase frequency(P=0.0898,0.0699 respectively,<0.1).Conclusion YITA has the effect of promoting uterine involution,and the mechanism is related to the regulation of ET/NO balance and the direct promotion of USM contraction.YITA is an effective substance for uterine contraction and hemostasis with leonurus injection in the treatment of postpartum hemorrhage and uterine involution.
4.Improvement effect of arbutin on myocardial fibrosis model rats and its mechanism

OBJECTIVE To study the improvement effects of arbutin on myocardial fibrosis (MF) model rats and its mechanism. METHODS The network pharmacology was used to predict the potential target of arbutin in improving MF and molecular docking was used to validated. Totally 50 SD rats were given isoprenaline subcutaneously (5 mg/kg, once a day, for 14 consecutive days) to induce the MF model. Modeled rats were randomly divided into model group, captopril group (9 mg/kg), arbutin low-dose, medium-dose and high-dose groups (50, 100, 200 mg/kg), with 10 rats in each group. Another 10 healthy rats were included as normal group. Each group was given the corresponding drugs, once a day, for 28 consecutive days. Twenty-four hours after the final administration, electrocardiograms and heart-related indexes [heart weight index (HWI), left ventricular weight index (LVWI)] of rats were detected; the levels of creatine kinase (CK), lactate dehydrogenase (LDH), N-terminal pro-brain natriuretic peptide (NT-proBNP) and type Ⅰ collagen (Col Ⅰ) and Col Ⅲ were detected in myocardial tissue of rats; the pathological changes of myocardial tissue were observed, and protein and mRNA expressions of adenosine deaminase (ADA) and adenosine kinase (ADK) were detected in the myocardial tissue of rats. RESULTS The results of network pharmacology showed that the main targets of arbutin improving MF were ADA and ADK. The results of molecular docking showed that arbutin bind stably with ADA and ADK. The results of experimental verification showed that compared with model group, the amplitude of ST and T waves in electrocardiogram were improved in administration groups, and the symptoms of atrial flutter were alleviated; HWI (except for arbutin medium-dose group), LVWI, the levels of CK, LDH, NT-proBNP, Col Ⅰ and Col Ⅲ in the myocardial tissue of rats were decreased significantly (P<0.05); the degree of myocardial fibrosis in rats decreased; protein and mRNA expressions of ADA and ADK in the myocardial tissue were significantly increased (P<0.05). CONCLUSIONS Arbutin can improve cardiac fibrosis and cardiac function of MF model rats, the mechanism of which may be associated with up-regulating protein and mRNA expressions of ADA and ADK,influencing the nucleotide metabolism and collagen generation. zhangminghao@hactcm.edu.cn
5.Mechanism of Zhishi Xiebai Guizhitang in Alleviating Myocardial Injury in Rats with Myocardial Infarction Based on TNF/NF-κB Signaling Pathway

ObjectiveTo investigate the effect of Zhishi Xiebai Guizhitang (ZXGT) on isoproterenol (ISO)-induced myocardial infarction (MI) in rats through the tumor necrosis factor/nuclear factor-κB (TNF/NF-κB) signaling pathway. MethodForty-eight SD rats were randomly divided into control group (blank), model group, perindopril group (4 mg·kg-1), ZXGT group (24.4 g·kg-1), ZXGT +inhibitor group (ZXGT, 24.4 g·kg-1, TNF-α receptor inhibitor R7050, 5 mg·kg-1), and an inhibitor group (R7050, 5 mg·kg-1), with eight rats in each group. The rats in each group were orally administered with their respective drugs for 7 days. Additionally, in the ZXGT + inhibitor group and the inhibitor group, R7050 was injected intraperitoneally at a dose of 5 mg·kg-1 on the 6th and 7th days. Except for the control group, all other groups were given intraperitoneal injections of ISO for 2 consecutive days to induce MI in rats. On the 7th day of the experiment, the rats were anesthetized 30 min after ISO injection, and their electrocardiograms (ECGs) were recorded to observe ST-segment elevation. Small animal echocardiography was used to measure global longitudinal strain (GLS) and cardiac synchrony. Blood samples were collected from the abdominal aorta to measure the levels of serum cardiac troponin T (cTnT), creatine kinase-MB (CK-MB), lactate dehydrogenase (LDH), tumor necrosis factor-α (TNF-α), and interleukin-1β (IL-1β). Histopathological changes in myocardial tissue were observed using hematoxylin-eosin (HE) staining. Immunohistochemistry (IHC) was used to detect the expression of TNF-α, NF-κB p65, and p-NF-κB p65 proteins in myocardial tissue. Western blot was performed to measure the expression of tumor necrosis factor receptor 1 (TNFR1), tumor necrosis factor receptor-associated factor 2 (TRAF2), transforming growth factor-beta-activated kinase 1 (TAK1), NF-κB inhibitory protein alpha (IκBα), phosphorylated (p)-IκBα, NF-κB p65, and p-NF-κB p65 proteins in myocardial tissue. ResultCompared with the control group, the model group showed significant ST segment elevation on the ECG (P<0.01), increased GLS, and reduced cardiac synchrony on echocardiography (P<0.01). Histopathological examination revealed extensive myocardial necrosis. Furthermore, the serum levels of cTnT, CK-MB, LDH, TNF-α, and IL-1β were significantly increased (P<0.01), and the expression levels of TNF-α, TNFR1, TRAF2, TAK1, p-IκBα, and p-NF-κB p65 proteins in myocardial tissue were significantly elevated (P<0.01), while the expression level of IκBα was significantly decreased (P<0.01). Compared with the model group, the perindopril group, the ZXGT group, the ZXGT + inhibitor group, and the inhibitor group rats showed a significant reduction in ST-segment elevation on the ECG (P<0.05, P<0.01), improvement in GLS and cardiac synchrony (P<0.05, P<0.01), a decrease in the area of myocardial necrosis, and reduced serum levels of cTnT, CK-MB, LDH, TNF-α, and IL-1β (P<0.01). Additionally, the ZXGT group, the ZXGT + inhibitor group, and the inhibitor group downregulated the increased TNF-α, TNFR1, TRAF2, TAK1, p-IκBα, and p-NF-κB p65 protein expression levels and upregulated IκBα expression levels in the myocardial tissue (P<0.05, P<0.01). No significant differences were observed between the ZXGT group and the ZXGT + inhibitor group or the inhibitor group. ConclusionZXGT can protect against ISO-induced myocardial injury in rats and improve cardiac function, and its mechanism of action may be related to the regulation of the TNF/NF-κB signaling pathway.
6.Improvement mechanism of Pangshi antai zhixue decoction on spontaneous abortion with heat syndrome by regulating NLRP3 inflammasome

OBJECTIVE To investigate the mechanism of Pangshi antai zhixue decoction in the improvement of spontaneous abortion with heat syndrome by regulating the NOD-like receptor protein 3 (NLRP3) inflammasome. METHODS The binding activities of 13 main components in Pangshi antai zhixue decoction with NLRP3, apoptosis-associated speck-like protein , containing a CARD (ASC), and caspase-1 precursor (pro- No.20-21ZY1053) caspase-1) were predicted by molecular docking. Sixty 1-day-old pregnant rats were randomly divided into normal group, model group, dexamethasone group (0.002 g/kg), and Pangshi antai zhixue decoction low-, medium-, and high-dose groups (11.025, 22.05, 44.10 g/kg), with 10 rats in each group. Each group was given distilled water/corresponding medicinal solution intragastrically, once a day, for 12 consecutive days. Except for normal group, other groups were given traditional Chinese medicine for warming yang and mifepristone to establish a model of spontaneous abortion with heat syndrome. 24 h after the last medication, serum levels of triiodothyronine (T3), thyroxine (T4), interleukin-2 (IL-2), IL-4, IL-6, IL-10, and interferon-γ (IFN-γ) were all detected; the abortion rate and uterine coefficient were calculated; the pathological morphology of the pregnant uterus was observed; protein expressions of NLRP3, ASC and caspase-1 were detected. RESULTS The molecular docking results showed that the binding energies of 13 main components of Pangshi antai zhixue decoction with NLRP3, ASC, and pro-caspase-1 were all less than -5 kJ/moL. The animal experiment results showed that compared with normal group, the uterine coefficient and serum levels of IL-4, IL-6 and IL-10 were decreased significantly in model group (P<0.05); the abortion rate and serum levels of T3, T4, IL-2 and IFN-γ as well as protein expressions of NLRP3, ASC and caspase-1 were increased significantly (P<0.05); there were abortion lesions in the pregnant endometrium. Compared with the model group, most of the quantitative indicators mentioned above were significantly reversed in Pangshi antai zhixue decoction groups (P<0.05), and the endometrial miscarriage lesions in pregnancy were improved to varying degrees. CONCLUSIONS Pangshi antai zhixue decoction influences the immune balance between mother and fetus by regulating the formation of NLRP3 inflammasome, down-regulating pro-inflammatory cytokines such as IFN-γ and IL-2, and up-regulating anti-inflammatory cytokines such as IL-4, IL-6 and IL-10, thereby improving spontaneous abortion with heat syndrome.
