Objective:To develop a method of near infrared reflectance spectroscopy ( NIRS) for the rapid determination of peflox-acin mesylate for injection .Methods:The quantitative models were established by the collection of NIR spectra of pefloxacin mesylate for injection.The spectra were pretreated with the methods of vector normalization , and the spectral ranges of 9 176.2-8 169.5 cm-1 , 6 051.9-5 716.3 cm-1 and 4 509-3 999.9 cm-1 were chosen.The partial least square (PLS) was used as the regression method .Re-sults:The prediction model was established by the internal cross validation ,and the concentration range was 7.55%-77.69%.The root mean square error of cross validation (RMSECV) was 1.61%, and the correlation coefficient was 0.992 4.Conclusion: The method of near infrared reflectance spectroscopy can be used for the rapid quantitative analysis of pefloxacin mesylate for injection .

Hyperglycemia has been identified as one of the important factors involved in the microvascular complications of diabetes, and has been related to increased cardiovascular mortality. Endothelial damage and dysfunction result from diabetes; therefore, the aim of this study was to determine the response of endothelial cells to stressful stimuli, modelled in normal and high glucose concentrations in vitro. Eahy 926 endothelial cells were cultured in 5 mmol/L or 30 mmol/L glucose conditions for a 24 hour period and oxidative stress was induced by exposure to hydrogen peroxide (H2O2) or tumour necrosis factor- α (TNF- α ), following which the protective effect of the glucocorticoid dexamethasone was assessed. Apoptosis, necrosis and cell viability were determined using an ELISA for DNA fragmentation, an enzymatic lactate dehydrogenase assay and an MTT assay, respectively. High glucose significantly increased the susceptibility of Eahy 926 cells to apoptosis in the presence of 500 μmol/L H2O2, above that induced in normal glucose (P<0.02). A reduction of H2O2- and TNF- α -induced apoptosis occurred in both high and low glucose after treatment with dexametha-sone (P<0.05). Conclusion high glucose is effective in significantly augmenting stress caused by H2O2, but not in causing stress alone. These findings suggest a mechanism by which short term hyperglycemia may facilitate and augment endothelial damage.

Objective:To explore the influence of infusion drop speed on the efficacy of tiopronin for injection combined with sol-vents in the treatment of abnormal liver function. Methods:Totally 74 patients were randomly divided into the observation group and the control group with 37 ones in each. The infusion drop speed was 60-65 drops per minute in the observation group, and that was 30-35 drops per minute in the control group, and the infusion time was controlled in 1h in the observation group and 2h in the control group. The clinical effect and the improvement of liver function were compared between the two groups after a treatment course of 4 weeks. Results:The total effective rate of the observation group(83. 78%) was obviously higher than that in the control group(56. 76%) (P<0. 05) after the treatment, and ALT, AST and TBIL of liver function indices were improved obviously except for ALB in the two group, and the difference was statistically significant between the two groups(P<0. 05). Conclusion:Tiopronin for injection can effectively improve the clinical effect by strictly controlling the time and drop speed of infusion.

Objective To compare the accuracy of diagnostic peritoneal lavage (DPL),abdominal computed tomographic(CT) scanning,and abdominal B ultrasound for the diagnosis of blunt abdominal trauma. Methods Prospective study was made on 61 hemodynamically stable abdominal injury patients.The patients received both CT and B ultrasound before DPL ,A laparotomy was done on the condition that one of the three examinations had positive finding.The surgical findings were compared with that of the diagnostic studies. Results The sensitivity,specificity and accuracy were 97.4%,81.7% and 91.8% for DPL; 97.3% 91.3% ,and 95.1% for CT;and 92.3%,90.9% and 91.8% for B ultrasound.Although the sesitivity,specificity,and accuracy of B ultrasound in the diagnosis of blunt abdominal trauma were similar to DPL and CT, B ultrasound had more advantages than DPL and CT. Conclusions B ultrasound can replace DPL.CT as a supplementary method in the diagnosis of blunt abdominal trauma.

Objective The aim of this study was to determine if isoflavone genistien has protective effects against high glucose-induced cell apoptosis in human aortic endlthelial cells,and investigate the possible mechanism for this protection.Methods Human aortic endothelial cells subjected to normal (5mmol/L) or high glucose (25mmol/L) were treated with genistein at 0,50,100nmol/L.Parallel experiments were performed with 100nM 17b-estradiol,and also in the presence and absence of the pure anti-estrogen ICI-182,780 (100nmol/L).The effects on cell apoptotic DNA fragmentation were determined using cell death ELISA,and the effects on cellular proliferation were determined using tritiated thymidine incorporation assay.Estrogen receptor expression was detected by Taqman quantitative PCR.Results Genistein at 100nmol/L significantly reduced high glucose-induced DNA fragmentation,and reversed cell DNA synthesis inhibition (P＜0.001) after 24 hours' incubation.The effect of genistein was completely blocked by ICI-182,780administration.Estrogen receptor beta,but not alpha was found to be expressed in these cells.Conclusion Isoflavone genistein shows protection against high glucose-induced cell damage through estrogen receptor beta,reducing apoptotic DNA damage and protecting from the inhibition of cell proliferation.

OBJECTIVE To investigate the use of antibiotics in our Outpatient Department during 2006-2007,and to analyze and evaluate the trend and rationality.METHODS The data about consumption quantity and consumption sum of antibiotics in our Outpatient Department during 2006-2007 were retrieved by means of computer management software in the department of pharmacy for the analysis of the clinical use of antibiotics using the method of defined daily dose(DDD).RESULTS Fluoro quinolones,cephalosporins and plant antibiotics have dominated the first places among the antibiotics used in our hospital,and the consumption of cephalosporins kept increasing year by year.CONCLUSIONS The use of antibiotics in our hospital should be further standardized to ensure the drug using safe,efficient and economic in our hospital.

OBJECTIVE To promote clinical rational use of antibiotic prophylaxis through surveying it in Obstetrics and Gynecology Department (OG) by pharmacists. METHODS Data of 223 cases using of antibiotic prophylaxis in OG were retrospectively analyzed. According to unreasonable use of antibiotic prophylaxis,the clinical pharmacists put out reasonable measures and directed treatment in succedent 206 patients. RESULTS The rate of unreasonable use of antibiotic prophylaxis in 223 cases was 100% and rate of infection after operation was 9.87%. OBJECTIVETo observe the clinical effect of pestle needle at Yaoyangguan bazhen and Heche-mingqiang section for bi syndrome (cold dampness type) at low back.

METHODSThirty-one patients of bi syndrome (cold dampness type) at low back were treated with pestle needle at Yaoyangguan bazhen and Heche-mingqiang section. After the treatment of pestle needle, traditional acupuncture was used at Shenshu (BL 23), Dachangshu (BL 25), Huantiao (GB 30), Weizhong (BL 40), Chengshan (BL 57), Kunlun (BL 60), etc., followed by TCM washing at low back and legs. Once daily, five days of treatment were taken as one course, and there was an interval of 2 days between two courses. Totally 4 courses were given. The visual analogue scale (VAS) before and after treatment was observed, and the clinical efficacy was evaluated.

RESULTSThe total effective rate was 90.3% (28/31) in 31 patients; the VAS after treatment was (2.42 ± 0.78), which was significantly, different from (5.59 ± 1.17) before treatment (P < 0.01). CONCLUSION The pestle needle at Yaoyangguan bazhen and Heche-mingqiang section is effective, noninvasive, painless for low-back bi syndrome (cold dampness type), and it is convenient for operation and easily accepted.

Objective To investigate the effect of different doses of dexmedetomidine(Dex)on sevoflurane consumption in patients undergoing laparoscopic oophorocystectomy.Methods Eighty ASA Ⅰ or Ⅱ patients aged 25-50 yr with body mass index 18-25 kg/m2 undergoing laparoscopic oophorocystectomy were randomly divided into 4 groups (n =20): control group (group C),low dose Dex group(group DL),medium dose Dex group(group DM) and high dose Dex group(group DH).Normal saline 20 ml and Dex 0.3,0.6,0.9μg/kg was infused iv over 10 min at 10 min before skin incision in groups C,DL,DM and DH,respectively.End-tidal sevoflurane concentration (ETsev) was recorded before Dex administration(T1 ),skin incision(T2 ),immediately after pneumoperitoneum (T3 ),10 min of pneumoperitoneum(T4 ) and the end of surgery (T5 ).Duration of anesthesia,consumption of sevoflurane,emergence time,extubation time were recorded and restlessness at 10 min after extubation was also recorded.The concentrations of blood glucose and corticosteroid were measured by quickly by glucose analyzer and radio-immunity gefore anethesia induction (T0) and at T3,T4,T5 respectively.Results The consumption of sevoflurane per hour,ETsev at T2-5,concentrations of blood glucose and corticosteroid at T3-5 were decreased gradually in groups C,DL,DM and DH ( P ＜ 0.05).The emergence time and extubation time were shorter and the incidence of restlessness was lower in groups DL,DM and DH than in group C ( P ＜ 0.05 ).Conclusion Dexmedetomidine can reduce the consumption of sevoflurane in a dose-dependent manner in patients undergoing laparoscopic oophorocystectomy.
