1.Assessment of the anesthesia effect of etomidate combined with remifentanil in elderly patients

ObjectiveTo study the anesthesia effect,safety and reliability of etomidate combined with remifentanil in elderly patients. Methods40 patients scheduled for abdominal operation were divided into two groups with 20 cases each: etomidate group(group A)and propofol group(group B).Anesthesia induction: midazolam 0.04mg/kg,remifentanil 1.5 μg/kg(time>60s),atracurium 0.6mg/kg,group A to etomidate 0.2mg/kg(30 ~ 60s),(T1),1 min before trachea intubation(T2),1 min after trachea intubation(T3),5 min after cutting skin(T4),5 min after exploration(T5),5min after cutting ill focus(T6),3min after tracheal extubation(T7),30min after operation(T8).At the same time,observed analepsia condition. ResultsIn group B,SBP and DBP were lower than those in group A at T2,T3,T4 and T5(all P<0.05).In group B,DBP was lower than those in group A at T6 and HR was lower than those in group A at T3 and T7(all P<0.05).In group B,SBP and DBP were lower than before anesthesia at T2,T3,T4 and T5(all P<0.05).In group B,DBP was lower than before anesthesia at T6 and HR was lower than before anesthesia at T2(all P<0.05).In group A,HR was faster than before anesthesia at T7(P<0.05).The time of openingeyes,remove-tracheal tube in B group was less than those in A group(all P<0.05).Both groups without memory about operation,anesthesia was content. ConclusionEtomidate combined with remifentanil anesthesia was milder than propofol group to elderly patients ardiovascular effects.It was better than propofol group for elderly patients.

Aim Tostudytheantidepressanteffectand mechanism of Gross saponins of Tribulus terrestris. Methods Themodelofdepressionwasestablishedby unpredictable chronic mild stress(UCMS),then open filed test (OFT)and tail suspension test (TST)were used to evaluate the behavioral changes.LC-MS/MS method was employed to measure blood neurotransmit-ters.mRNA expressions of IDO,IL-10 and IL-1βwere detected by quantitative PCR method.Hippocampus protein expression was detected by Western blot.Re-sults Comparedwithcontrolgroup,modelgroup's total distance,number of standing and tail suspension fixed time increased significantly (P <0. 05 ),Neuro-transmitter level of 5-HT in the blood was significantly decreased(P<0. 05 ).mRNA expression of IDO and IL-1βwas increased in hippocampus.Protein expres-sion of IDO was significantly increased in hippocampus (P <0. 05 ).Compared with model group,the treat-ment group was significantly decreased in total distance,number of standing and tail suspension fixedtime(P<0. 05).Neurotransmitter level of 5-HT in the blood and mRNA expression of IL-10 in hippocampus were significantly increased after treatment (P <0. 05 ).mRNA and protein expression of IDO were ob-viously down-regulated in hippocampus (P <0. 05 ). Conclusions GrosssaponinsofTribulusterrestriscan obviously improve rat behavior and show antidepressanteffect,which can increase neurotransmitter level of 5-HT in the blood,down-regulate mRNA expression of IDO and IL-1β,and obviously increase protein expres-sion levels of IDO in hippocampus(P<0. 05 ).
5.Analysis of schistosomiasis endemic situation in Jiangling County from 2004 to 2013

Objective To analyze the schistosomiasis endemic situation and understand its transmission trend in Jiangling County from 2004 to 2013 so as to improving the schistosomiasis control. Methods The relevant data of schistosomiasis were collected and analyzed statistically in Jiangling county from 2004-2013. Results The human prevalence of schistosomiasis was decreased from 10.22%in 2004 to 0.63%in 2013 with the descend rate of 93.84% and the notable descend rate was 76.92%from 2009-2013. The cattle infection rate was decreased yearly from 7.59%in 2004 to 0 in 2013. The appearance rate of frames with living Oncomelaniahupensissnails and the infection rate of snails reduce by 17.53%and 100%respectively. Conclusions Af?ter 10?year control the endemic situation of schistosomiasis in Jiangling County decreases and is at a low level. However the risk factors of transmission still exist. Therefore the comprehensive control measures still should be enhanced continuously.
6.Effects of a novel PPARγ agonist on Wnt-β-catenin pathway in ADPKD cystic-lining epithelial cells

Objective To investigate the effects of a novel PPARγ agonist DH9 on Wntβ-catenin pathway in human polycystic kidney cystic-lining epithelial cells (WT9-12).Methods WT9-12 cells were treated with different concentrations of DH9 for 72 hours and the proliferation was assessed by MTT.WT9-12 cells were pretreated with SB216763 or GW9662 for two hours and then treated with DH9 for 72 hours.Western blotting was applied to detect the protein expression of β-catenin,phospho-β-catenin,GSK3β,phospho-GSK3β.Results DH9 could effectively inhibit the proliferation of the cells.60 μmol/L DH9 could facilitate β-catenin down-regulation (P<0.01) and phospho-β-catenin up-regulation (P<0.01).Inhibition of GSK3β by SB216763 could protect WT9-12 cells against DH9-facilitated β-catenin repression in a dose-dependent manner despite phosphorylating deactivation,but PPARγ inhibitor GW9662 couldn't.Conclusions DH9can effectively block the proliferation of WT9-12 cells.The effect may be mediated by facilitating the down-regulation of β-catenin via GSK3β-dependent mechanism.
7.Analgesic Effect of Etomidate Infusion for Radical Mastectomy in Breast Cancer and Its Effect on Hemodynamics and Inflammatory Mediators

Objective:To study the analgesic effect of etomidate infusion for radical mastectomy in breast cancer and its effect on hemodynamics and inflammatory mediators.Methods:From August 2015 to July 2016,84 patients with radical mastectomy were divided into the observation group and the control group according to the order of admission.The observation group was given continued infusion of etomidate,the control group was given continued infusion of propofol.The visual analogue (VAS) at different time points after operation,diastolic blood pressure (DBP),systolic blood pressure (SBP) and heart rate (HR) at the time ofextubation and serum IL-2,IL-10,IL-12 levels at 72 hours after operation were compared between two groups.Results:At the time of extubation,the DBP,SBP and HR were significantly lower in the observation group than in the control group (P<0.05).The VAS scores of observation group were significantly lower than those of the control group at 1,5,10,24 and 48 hours after operation (P <0.05).The serum levels ofIL-2 in the observation group was significantly higher than that of the control group (P<0.05),IL-10 and IL-12 levels were lower than control group (P<0.05).Conclusion:Sustained infusion of etomidate had good analgesic effect in the treatment of breast cancer patients underwent radical mastectomy with little effect on the hemodynamics and inflammatory mediators.
8.The establishment and application of a time-resolved fluoroimmunoassay in detection of HBV large surface protein

Objective To establish a method of TRFIA with high sensitivity and broad detecting range for serum HBV large surface protein (HBV-LP).Methods The monoclonal antibody of HBV-LP was covered on the microwell plate and incubated with HBV-LP in blood sample,then Eu3+ labeled antibody of HBs was added.HBV-LP in standard substance,blood samples of 66 chronic hepatitis B patients and 30 healthy controls was detected by the TRFIA and ELISA.x2 test and linear correlation analysis were used for data analysis.Results The dose-response curve of standard substance with TRFIA had good linear correlation (r=0.999).Normal reference range was established at 0-1.36 mg/L based on the ELISA results of 30 healthy controls.The sensitivity was 0.10 mg/L.The specificity was 100% (30/30).Correlation coefficient between the TRFIA and the ELISA was 0.800 9 (P<0.001).The positive detecting rates of the 2 methods were significantly different (89.4%(59/66) vs 77.3%(51/66),x2 =6.13,P<0.01).The recovery rate for HBV-LP was between 95.93%-107.62%.The effective detecting range(CV<10%) of TRFIA was 1.35-2 764.00 mg/L,and that of ELISA was 10.8-691.0 mg/L.Conclusion The TRFIA was established for HBV-LP detection with higher sensitivity and wider detecting range compared to ELISA.It has potential value for HBV screening and monitoring of antiviral therapy.
9.Histopathological features of EB virus-related human immunodeficiency virus negative plasmablastic lymphoma

Objective To investigate the clinicopathological features of human immunodeficiency virus (HIV) negative plasmablastic lymphoma (PBL) with no-immunosuppression,so as to accelerate the understanding for this group of disease.Methods The histological features of 6 HIV-PBL patients with no-immunodeficiency were retrospectively analyzed.Epstein-Barr virus (EBV) status was detected by in situ hybridization.Then,immunohistochemistry and fluorescence in situ hybridization (FISH) method were used to determine the immunophenotype,latent status of EBV and MYC translocation in PBL,respectively.Results HIV-PBL showed monotonous proliferation of plasmablastoid or immunoblast-like cells.Giant cells and necrosis could be observed,with less reactive cells in the background and higher mitoses.All the cases had EBV infection and type Ⅰ latency status of EBV (LMP1-/EBNA2-),and expressed terminal B-cell differentiation immunophenotype (CD20-/CD3-/CD138+/Kappa or Lambda+).Six HIV-PBL patients were elderly (median age was 69.5 years old),had equal incidence of PBL between male and female and showed high frequency of involvement of extranodal and extraoral lesion sites (6 cases and 5 cases,respectively).Median sutvival was 25.5 months.In addition,3 HIV-PBL patients had IGH/MYC translocations.Conclusions HIV-PBL is a new entity with unique clinical features including no-HIV infection,elderly,EBV positivity,and more involvement in extranodal and extraoral sites.HIV-PBL should be distinguished from HIV+ PBL.
10.Panning and identification of antagonistic active peptides specifically bin-ding to the first and second extracellular membrane loops of rat CCR5 by technique of phage display peptide library

[ ABSTRACT] AIM: To pan the active peptides which specifically bound to the first and second extracellular membrane loops of rat CC chemokine receptor 5 ( CCR5 ) .METHODS: The technique of phage display peptide library was used and binding ability of the peptides was identified.The amino acid sequences of the first and second extracellular loops of rat CCR5 were searched in the protein database and chemically synthesized corresponding linear peptides were used as targets in the biopanning.After 3 to 4 rounds of screening with Ph.D.TM-7 Phage Display Peptide Library were per-formed, the specific phages were collected and primarily identified by ELISA.RESULTS:The sequences of the peptides displayed on the selected phages were GHWKVWL and HYIDFRW, both of them exhibited positive in phage binding ELISA and the binding to phages and targets were concentration dependent and saturable.CONCLUSION:Two antagonis-tic active peptides specifically binding to CCR5 were successfully obtained by the technique of phage display peptide librar-y, and the binding ability to the first and second extracellular membrane loops of rat CCR5 were proved in vitro.
