BACKGROUND:Rib fractures are frequently treated with analgesic or compression band,which relieves pain but attenuates respiration.Memory metal chest shield as a new product has been used clinically for patients with multiple rib fractures to alleviate the pain.It displays many advantages compared with traditional approaches OBJECTIVE:To compare the effect of the memory metal chest shield versus the traditional fixation for multiple rib fractures.METHODS:A total of 104 patients with multiple rib fractures confirmed by transthoracic X-ray were selected and randomly divided into control group(n=40)treated with the traditional fixation methods(chest strap or a thick pad)and experimental group(n=64)treated with memory metal chest shield.At 1,24,48,and 72 hours after admission,the visual analog scale(VAS)at rest and cough.the incidence of pulmonary complications,and hospitalization were recorded Of them,32 from the experimental and21 from the control group rccepted pulmonary function tests.and their vital capacity was recorded at 1,24,48,and 72 hours after treatment RESULTS AND CONCLUSION:Pain intensity of experimental group was reduced at rest,while significantly reduced in forced breathing(such as cough).Of the 32 patients with use of memory metal sheeting,vital capacity continuously increased during the observation period,but pulmonary complications were significantly less,and the average hospital stay was significantly less than the control group(P＜0.05).Results show that the memory metal shield displayed fixation effect on the area of broken ribs,which significantly reduced pain,increased lung capacity,reduced the incidence of pulmonary complications and shortened the average length of stay.It is a simple and effective method of treating multiple rib fractures .

Objective:To determine the promoting effect of pyrrolidine dithiocarbamate(PDTC),an inhibitor of NF-?B,on the sensitivity of human ovarian cancer cell line SKOV-3 to chemoagent cisplatin and the possible mechanisms.Methods:SKOV-3 cells were treated with various concentrations of cisplatin and PDTC combination for different periods.Then the growth inhibition of SKOV-3 cells were observed by MTT assay,expression of I-?B? and P65 protein by Western blotting assay,and apoptosis of cells by flow cytometry.Results:Separate application of both cisplatin(0.1-10 ?g/ml)and PDTC(10-40 ?mol/L L)significantly inhibited the growth of SKOV-3 cells and caused cell apoptosis(P

A cochlear implant (CI) is a kind of surgically implanted electronic device that provides a sense of sound to a person who has severe to profound sensorineural hearing impairment. However, studies have shown that the performance in music perception for CI users has not been achieved to an ideal level. It is important to evaluate the ability to perceive music using well designed music perception test materials, in order to improve the quality of life for cochlear implant users. This paper reviewed the studies on assessing music perception ability for adult cochlear implant users from the existing literature.

Objective To observe the clinical effect of Yangyin Tongluo Pills combined with acupuncture for the treatment of cerebral infarction sequela. Methods One hundred patients with cerebral infarction sequela were evenly randomized into treatment group and control group. The control group was given acupuncture therapy combined with oral use of clopidogrel, and the treatment group was given acupuncture therapy combined with oral use of Yangyin Tongluo Pills (mainly composed of Fructus Lycii, Flos Chrysanthemi, Radix Rehmanniae, Cortex Moutan, Radix Salviae Miltiorrhizae, Flos Carthami, Semen Persicae, Pheretima, Rhizoma Gastrodiae, Ramulus Uncariae cum Uncis, Spica Prunellae, Radix Glehniae, Radix Ophiopogonis, Fructus Schisandrae Chinensis, Radix Pseudostellariae) . One month constituted a treatment course, and the treatment covered 2 courses. After treatment, the therapeutic effect of the two groups was evaluated, and the changes of National Institute of Health Stroke Scale ( NIHSS) scores and Barthel Index Scale for activities of daily living ( ADL) , and hemorrheology indexes were observed before and after treatment. Results (1) After treatment for 2 courses, NIHSS scores were decreased, ADL scores were increased in the two groups (P<0.01 compared with those before treatment) , and the improvement of the treatment group was superior to that of the control group ( P<0.01) . ( 2) After treatment, hemorrheology indexes were much improved in the treatment group ( P<0 . 05 or P<0 . 01 ) , but remained unchanged in the control group ( P>0.05) . The treatment group had better effect on improving hemorrheology indexes than the control group (P<0.05) . (3) The total effective rate of the treatment group was 90.00% and that of the control group was 64.00%, the overall therapeutic effect of the treatment group being superior to that of the control group (P<0.05) . Conclusion Yangyin Tongluo Pills combined with acupuncture are effective for the treatment of cerebral infarction sequela, and the effect is superior to that of acupuncture combined with clopidogrel.

[Objective]To study the application of allogenic bone spacer in double-door expansive laminoplasty.[Method]From Apr.2003 to Jun.2004,22 patients underwent double-door expansive laminoplasty using allogenic bone spacers in authors department.All patients were followed up for 16～30 months,with an average time of 18.4 months.JOA scoring system was used to evaluate the neurological status.ROM of cervical spine and bone fusion was determined on X-ray and CT at follow-ups.[Result]At final follow-up,JOA scores improved from(8.3?2.6)points before surgery to(13.3?3.1)points.ROM of cervical spine was reduced from(41.3?10.8)? to(23.4?8.7)?.No postoperative instability happened after operation.Bone fusion between allograft and spinous processes was completed in 69.6%,partial in 23.2% and failed in 7.1% of the operated levels.No breakage and displacement of allograft spacers were found.[Conclusion]Double-door expansive laminoplasty using allogenic bone spacers is safe and effective for cervical spondylotic myelopathy.

[Objective]To evaluate the application and effectiveness of circumferential fusion for the treatment of severe lumbar stenosis of the elderly.[Method]Eighty-three elderly patients with severe lumbar stenosis were treated with circumferential fusion through one-stage posterior approach(PLIF+PLF)from November 2001 to May 2004 at author's department.Postoperative complications were observed.All patients were followed-up for 24~42 months.JOA scoring system was used to evaluate the operative outcome.Plain radiographs and CT were used to judge fusion status.[Result]All patients were able to ambulate 12~14 days after operation.Symptomatic deep venous thrombosis(DVT)happened in three patients.Ten patients had worsening pain in the back and lower extremities temporarily.At final follow-up,low back pain was relieved significantly.JOA scores improved from(13.6?3.8)points before surgery to(22.3?4.6)points.Bone fusion was achieved in all operated intervertebral spaces.[Conclusion]Posterior circumferential fusion is one of safe,effective and reasonable treatment choises for elderly patients with severe lumbar stenosis.

Objective To investigate the expression of multidrug resistance gene 1 product P-glycoprotein(P-gp) when nuclear factor kappa B is induced or inhibited.Methods Ovarian cancer cells SKOV-3 were treated with Docetaxel or combinded with pyrrolidine dithiocarbamate(PDTC),the inhibitor of NF-?B.Western blot assay、 flow cytometry assay and MTT assay were used to measure P65 protein,P-gp,apoptosis of cancer cells,and the survival rate respectively.Results Docetaxel increased P65 protein and P-gp expression,while combind use of PDTC reversed this function.Both Docetaxel(≥(1 ?g/mL))and PDTC(≥(10 ?mol/L)) significantly inhibited the growth of SKOV-3 cells,and caused apoptosis.The combined use of Docetaxel and PDTC at low concentrations significantly inhibited the growth of cancer cells and caused apoptosis as compared with those treated with Docetaxel only.Conclusion The expression of P-gp may be ralated to the activation of NF-?B;Inhibition of NF-?B can enhance the chemosensitivity of ovarian cancer cells to Docetaxel.

Objective To understand the characteristics of complications occurred after endoscopic thyroidectomy and its prevention and treatment.Methods Retrospective analysis on the clinical effects and complications of 105 cases of endoscopic thyroidectomy.ResultsThe average operative time was 85 minutes in the 105 cases.Five cases were transferred to open operation because of bleeding and 11 cases transferred for the intraoperative pathology of papillary carcinoma.After operation,subcutaneous ecchymosis occurred in 5 patients,subcutaneous emphysema in 10 cases,neck and anterior chest skin tautness and discomfort in 7 cases,parathyroid injury in 2 cases.No superior laryngeal nerve or recurrent laryngeal nerve injury was found in the 105 cases.ConclusionUnder strict control on operation indications,proficient and meticulous endoscope operative skills,familiarization with the anatomical structure of thyroid,and timely and proper treatment on postoperative complications can reduce and prevent the occurrence of complications.

Objective:To explore the optimum digestion condition by orthogonal design, and determine the chromium content in Carthami flos by atomic absorption spectrometry ( AAS) to provide the experimental basis for the establishment of trace element limit standard. Methods:The conditions of microwave digestion were optimized using L9 (34 ) orthogonal experimental design, and chromi-um was determined by graphite furnace ( GF)-AAS. Results:The optimal digestion conditions were as follows:nitric acid-hydrochloric acid system was with the ratio of 6∶2, the highest resolution temperature was 185 ℃ with the duration of 25 minutes. The chromium content in 15 batches of Carthami flos was with significant differences. The chromium content in 15 batches of Carthami flos was detec-ted withhin the range of 0. 53-88. 15 mg·kg-1, the average recovery was 97. 1% and RSD was 2. 2%(n=9). Conclusion: The method is simple, accurate and reproducible, and can be used in the quality control of chromium in Carthami flos.

Objective To investigate the prognostic value of modified Glasgow prognostic score(mGPS)in patients with gastric cancer.Methods The clinical data of 600 patients with gastric cancerwho underwent surgical treatment were retrospectively reviewed.These patients were divided into themGPS0 group,mGPS1 group,and mGPS2 group according to the standard of mGPS.Postoperative survivals and risk factors that may affect patient's prognosis were analyzed among the three groups.Results There were 46 (7.7%)cases,33 (5.5%)cases and 521 (86.8%)cases in the mGPS0,mGPS1 andmGPS2 group,respectively.Except for gender(P =0.203),age,BMI,CEA,CA199,CRP,Alb,TNM stageand mGPS affected the over all survivals and there were significant differences among the groups(P <0.01).Gender,age,BMI,CEA,CA199 and TNM stage affected the mGPS and there were significantdifferences among the groups(P <0.01).Multivariate analysis identified that age(OR,1.319,95% CI1.068,1.629,P <0.01),TNMstage(OR,2.909,95% CI 2.616,3.234,P <0.01)and mGPS(OR,1.845,95% CI 1.184,2.875,P =0.007)were correlated with the postoperative death rate and they wererisk factors of poor postoperative prognosis(P <0.01).Conclusion mGPS can be used as an early,simple and effective prognostic predictor of postoperative survival for patients with gastric cancer.
