Objective:To study the epidemiology of attention-deficit hyperactivity disorder(ADHD) in pupils of Zhenjiang urban districts. Methods: Questionnaire was designed and employed for parents of 3 698 pupils at schools in Zhenjiang urban districts. ADHD was diagnosed according to the international DSM-Ⅳ scale. Differences were determined by Qi-square test and multiple regression analysis. Results: Prevalence of ADHD was 6.54% in pupils of Zhenjiang urban districts, and was 9.14% in male pupils and 4.01% in female. There was significant difference between 2 gender groups (? 2=34.933,P
2.Clinical analysis of allergic bronchopulmonary aspergillosis: a report of 48 patients

To retrospectively analyze the clinical data of 48 patients with allergic bronchopulmonary aspergillosis (ABPA) at Shanghai Pulmonary Hospital.There were 23 males and 25 females with a mean age of (36 ± 15) years.Their clinical manifestations included wheeze,cough,sputum production,sputum plugs,fever,hemoptysis,weight loss,chest pain,weakness and night sweats.They had a high peripheral blood eosinophilia,a higher serum total IgE,a higher level of aspergillosis fumigatus-specific IgE and positive immediate skin-prick test to aspergillus fumigatus.Aspergillus species were detected in sputum samples of 26 patients.Chest computed tomography (CT) was performed in 48 patients.There were patchy infiltrations (n =45),transient infiltrations (n =40),central bronchiectasis (n =35) and mucoid impaction (n =18).Obstructive ventilation dysfunction was confirmed by lung function test.

0.05).CONCLUSION:Application of DHS,GN and PFN in treatment of intertrochanteric fractures in elderly exhibits no significant difference in efficacy.Gamma nail and PFN can shorten the operation time to reduce the amount of blood loss and postoperative complications.For the stable fracture,all three fixtures can be used,and the DHS is the cheaper;for instable fracture,PFN and GN due to the biomechanical advantages are good choices.

Objective To investigate the distribution relationship of microglia and neuron in rat medullary visceral zone(MVZ) after stomatic nociception. Methods After 2ml formalin(2.5%)injection in rat stomach by tiny plastic tube,Anti-Fos protein(labeling neuron)and anti-OX 42(labeling microglia)single and double immunohistochemical ABC method staining on model medulla oblongata sections in different time point(0.5h,1h,2h and 3h) were used. Results 1.After wakefulness,the animal models appear restlessness and agitation in 1-2h.2.We found hemorrhage and exfoliation in model's gastric mucosa.3.Fos positive neurons and OX 42 positive microglias were observed in MVZ,whose distribution was congruous and appeared clearly character of locations.4.The results of immunochemical diplex staining indicate that many Fos positive neurons were surrounded by intensive OX 42positive microglias.5.OX 42 positive microglia showed a tendency,which increased in 0.5-2h and decreased in 3h.Conclusion Neurons and microglia in MVZ may be involved in visceral nociception transfer and adjustment function.
5.Role of sonic hedgehog signaling pathway in spinal neurons in morphine tolerance in mice

Objective To evaluate the role of sonic hedgehog (SHH) signaling pathway in spinal neurons in morphine tolerance (MT) in mice.Methods Pathogen-free healthy female Kunming mice,weighing 20-25 g,aged 8-10 weeks,were used in the study.MT was induced with morphine 10 mg/kg injected subcutaneously twice a day for 7 consecutive days.The experiment was performed in two parts.Experiment Ⅰ Forty-eight mice were randomly assigned into 2 groups:control group (group C,n =8) and MT group (group M,n=40).The thermal pain threshold (TPT) was measured at 1 day before morphine injection and 1,3,5,7 and 14 days after the end of injection.Eight mice in each group were sacrificed at 2 h after measurement of TPT at each time point after the end of injection in group M or at 2 h after the last measurement of TPT in group C,and the lumbar segment (L4-6) of the spinal cord was removed.Experiment Ⅱ Forty-eight mice were randomly assigned into 6 groups (n=8 each):SHH inhibitor cyclopamine plus MT group (group CP+M),cyclopamine solvent plus MT group (group D1 +M),SHH agonist SAG plus MT group (group SAG+M),SAG solvent plus MT group (group D2+M),MT plus cyclopamine group (group M+CP) and morphine plus cyelopamine solvent group (group M+D1).At 15 min before morphine injection,cyclopamine 10 mg/kg was injected subcutaneously in group CP+M,and SAG 5 mg/kg was injected subcutaneously in group SAG+M.Cyclopamine 10 mg/kg was injected subcutaneously once a day during the 1-3 days after the end of morphine injection in group M+CP.The TPT was measured before injection of morphine,at 30 min after the first injection of morphine every day and at 1-3 days after the end of morphine injection.The animals were sacrificed at 2 h after the last measurement of TPT,and the lumbar segment (L4-6) of the spinal cord was removed for determination of the expression of SHH signaling pathway-related proteins SHH,ptch1,smo,gli1 and gli3 using Western blot.Results Experiment Ⅰ Compared with group C,the TPT was significantly decreased at 1 and 3 days after the end of morphine injection (P<0.05),no significant change was found in TPT at 5-14 days after the end of morphine injection (P>0.05),and the expression of SHH,smo and glil at 1-5 days after the end of morphine injection,of ptchl at 1 and 3 days after the end of morphine injection and of gli3 at 7 days after the end of morphine injection was up-regulated in group M (P<0.05).Experiment Ⅱ Compared with group D1+M,the TPT was significantly increased,the expression of SHH,ptchl,smo and glil was down-regulated,and gli3 expression was up-regulated in group C P+M (P<0.05).Compared with group D2+M,the TPT was significantly decreased,the expression of SHH,ptch1,smo and glil was up-regulated,and gli3 expression was down-regulated in group SAG+M (P<0.05).There was no significant difference in the parameters mentioned above between group M+CP and group M+D1 (P>0.05).The TPT was significantly lower on 3rd-7th days after beginning of morphine injection and 1-3 days after the end of morphine injection than at 30 min after the first injection of morphine in group CP+M (P<0.05).Conclusion The mechanism underlying the development of MT is partially related to activation of SHH signaling pathway in spinal neurons of mice,however,the maintenance mechanism has no marked relationship with it.
6.Effect of lead exposure on function of blood-cerebrospinal fluid barrier and ZO-1 and occludin protein expression in vitro

OBJECTIVE To establish an in vitro blood-cerebrospinal fluid barrier (BCB) model to investigate the underlying mechanism of lead-induced BCB injuries.METHODS The in vitro BCB model was established by Z310 cells.Different concentrations of Pb(AC)2 (2.5,5.0 and 10.0 mmol·L-1) were used for 24,48 and 72 h.Transendothelial electrical resistance (TEER) and flux of FITC-dextran were performed to determine the permeability of the in vitro BCB model.Western blotting and immunofluorescence methods were used to observe the expression of tight junction protein ZO-1 and occludin.RESULTS Compared with control group,Pb(AC)2 2.5,5.0 and 10.0 mmol· L-1 exposure for 48 h to Z310 cells had no significant effect on survival rate and density.TEER in different groups was gradually increasing.At the 12th day after Pb(AC)2 exposure,the values of TEER and flux of FITC-dextran in Pb(AC)2 5 and 10 mmol· L-1 groups were significantly decreased (P<0.05).Western blotting and immunofluorescence images showed that the expression of ZO-1 and occludin were significantly decreased (P<0.05) after Pb(AC)2 exposure for 48 h.CONCLUSION Lead exposure can cause the breakdown of BCB barriers,and this effect may be mediated by reducing the expression of ZO-1 and occludin proteins.
7.Analysis on Heart Rate Variability and Its Influencing Factors of Hypertensive Patients from Different TCM Syndrome

Objective To investigate the syndrome type distribution of hypertensive patients; To analyze the correlation of characteristics of HRV time domain parameters and its influence factors. Methods Totally 515 cases of hypertensive patients were included and were put under syndrome type distribution. Demographic information, laboratory test parameters, risk factors and clinical symptoms were collected for correlation analysis. HRV time domain parameters were recorded by using 24 h ambulatory electrocardiogram. The differences in SDNN, SDNN Index, HRV Index, PNN50, and RMSSD of different TCM syndrome types were compared. Results Among 515 patients: 160 cases with hyperactivity of yang due to yin deficiency syndrome, 136 cases with turbid phlegm and blood stasis syndrome, 83 cases with overabundant liver-fire syndrome, 69 cases with deficiency of kidney qi, and 67 cases with abundant phlegm-dampness syndrome. By comparing different TCM syndromes, the level of SDNN was significantly reduced in the hyperactivity of yang due to yin deficiency syndrome, overabundant liver-fire syndrome,deficiency of kidney qi syndrome compared with turbid phlegm and blood stasis syndrome, abundant phlegm-dampness syndrome (P<0.05); SDNN Index and HRV Index decreased significantly in the hyperactivity of yang due to yin deficiency and overabundant liver-fire syndrome compared with abundant phlegm-dampness syndrome (P<0.05). SDNN Index decreased significantly in the deficiency of kidney qi compared with abundant phlegm-dampness syndrome (P<0.05). The level of PNN50 was significantly reduced in the deficiency of kidney qi compared with hyperactivity of yang due to yin deficiency syndrome (P<0.05). RMSSD decreased significantly in the hyperactivity of yang due to yin deficiency syndrome, deficiency of kindney qi syndrome, overabundant liver-fire syndrome compared with turbid phlegm and blood stasis syndrome (P<0.05). Discriminant analysis showed that SBP, DBP, MBPS, SDNN, SDNN Index, HRV Index, PNN50, RMSSD were correlated with the diagnosis of five syndrome types. Logistic regression analysis showed that the factors including gender (female), insomnia, elevated systolic blood pressure, MBPS, decreased SDNN Index and PNN50 were positively correlated to hyperactivity of yang due to yin deficiency; other factors including gender (female), advanced age, elevated blood pressure, decreased SDNN, HRV Index and RMSSD were positively correlated with turbid phlegm and blood stasis syndrome. And the study also showed that advanced age, family history of hypertension, elevated blood pressure, decreased SDNN Index, HRV Index and PNN50 were positively correlated to abundant phlegm-dampness syndrome. Conclusion HRV time domain parameters can be significantly reduced in the hyperactivity of yang due to yin deficiency, overabundant liver-fire syndrome, and deficiency of kidney qi syndrome. The autonomic nerve function is damaged seriously. Hyperactivity of yang due to yin deficiency syndrome, abundant phlegm-dampness syndrome turbid phlegm and blood stasis syndrome are closely related to the influencing factors that lead to cardiovascular and cerebrovascular events.
8.Effect of MECT on learning memory in depressed rats and its synaptic plasticity mechanism

Objective To explore the effect of MECT on learning memory in depressed rats and its synaptic plasticity mechanism. Methods Fifty SD rats were randomly divided into 5 groups( n =10): MECT (received ECT with intraperitoneal propofol), ECT (received ECT only), propofol (received intraperitoneal propofol), depression, and control. The treatments were given daily for 7 consecutive days. All rats underwent open field and Morris water maze test. The SYP protein and mRNA expressions in rat hippocampus were detected using immunochemistry and RT-PCR, respectively. Results After treatment, the open field scores were much higher in MECT and ECT groups than in propofol and depression groups ( P <0.05); the learning memory was worse in ECT group than in MECT, propofol and depression groups ( P <0.05); the expressions of SYP protein and mRNA was higher in MECT and ECT groups than in propofol and depression groups ( P <0.05), the expressions of SYP protein and mRNA were lower in MECT group than ECT group ( P <0.05). Conclusions Propofol can improve learning memory in ECT-treated depressed rats through attenuation of ECT-induced expression of SYP in rat hippocampus.
9.Effect of different doses of propofol on synaptophysin level in hippocampus of mentally depressed rats after electroconvulsive therapy

Objective To investigate the effects of different doses of propofol on expression of synaptophysin (SYP) mRNA and protein in the hippocampus of mentally depressed rats after electroconvulsive therapy (ECT) .Methods Fifty adult male SD rats weighing 200-250 g were randomly divided into 5 groups (n = 10 each): group Ⅰ control (group C); group D mental depression (group D) ; group Ⅲ , Ⅳ , Ⅴ propofol 90, 110, 130 mg/kg + ECT (group M_1, M_2, M_3). Mental depression was induced by isolation and chronic unpredictable mild stress (CUMS) in group Ⅱ-Ⅴ . Group M_1 , M_2 and M_3 received intraperitoneal (IP) propofol 90, 110 and 130 mg/kg respectively + ECT once a day ×7 consecutive days while group C and D received IP normal saline instead of propofol. The rats underwent open field test and sucrose liquid consumption test the day before and after mental depression was established and one day after treatment. The rats were killed after the last test for determination of expression of SYP mRNA (by RT-PCR) and protein (by immuno-histochemistry) in the hippocampus. Results The ambulation scores, rearing scores, grooming time and sucrose consumption percentage were significantly decreased while the center,grid detention time (CDT) was significantly increased in group Ⅱ-Ⅴ as compared with control group indicating mental depression was successfully induced. The ambulation scores, rearing scores, grooming time, sucrose consumption percentage, SYP mRNA and protein expression were significantly increased while CDT was significantly decreased after treatment in group M_1 and M_2 as compared with group D indicating that propofol 90 or 110 mg/kg combined with ECT was effective for the treatment of mental depression.Conclusion Propofol 90 or 110 mg/kg combined with ECT is effective in treating mental depression while proprofol 130 mg/kg combined with ECT is not. Excessive inhibition of SYP expression in hippocampus by large dose of propofol may explain the mechanism.`
10.Clinical research for patients with diabetes undergoing off-pump coronary artery bypass grafting by endoscopic saphenous vein harvesting

Objective To compare clinical effects and complications of patients with diabetes in off-pump coronary artery bypass grafting(OPCAB) between endoscopic saphenous vein harvesting(EVH) and conventional saphenous vein harvesting(CVH).Methods Sequence comparison analysis the clinical data of 339 patients with DM who underwent OPCAB in our department from Nov 2011 to Nov 2014.269 cases by EVH,70 cases by CVH.Observing two groups of patients with deprecated rate of SVG,intraoperative SVG blood flow and the value of PI,lower limb wound complications such as incision infection,poor healing,lower limb local hematoma and pain.SVG patency rate of part patients was follow-up by CT coronary angiography.Results To compare the two groups of patients by EVH and CVH,the perioperative death was 8 cases in EVH group (2.4%),2 cases in C VH group (2.9%).The deprecated SVG of patients was 3.9% vs 2.9%.The blood flow was (17.36 ±11.24) ml/min vs(17.11 ± 8.37) ml/min,PI was 2.78 ± 2.37 vs 2.22 ± 2.17.The incision infection was 0 vs 4.4%,poor healing was 0.9% vs 8.8%.The lower limb local hematoma was 5.7% vs 1.5%.The visual pain analogue scale(VAS) was 0.53 ± 1.71 vs 1.26 ± 2.13 3 days after operation.The numbness of lower limb was 9.7% vs 22.1%.The Edema of the legs was 8.5% vs 19.1% 7 days after operation.60 cases were follow-up by CT coronary angiography,the SVG patency rate was91.4% vs 94.6% 1 year after operation,83.3% vs 86.1% 2 years after operation,72.2% vs 73.7% 3 years after operation respectively.Conclusion EVH technology for SVG in the patients combined DM has good clinical result,the recent patency rate of SVG is perfect,postoperative limb complications is decreased by EVH.
