1.Affect evaluation of a new medical posture mat in the prevention of pressure ulcer in pediatric orthopedics

Objective To evaluate the application effect on preventing pressure ulcers of modern medical position pad in orthopaedic. Methods A total of 124 patients with congenital dislocation of the hip, pelvis fractures and femoral fractures were selected between January 2013 and December 2013. They were divided into 2 groups by random digits table method with 62 cases each. The observation group was given modern medical position pad nursing after operation, and the control group was given routine postoperative nursing. Postoperative 2 hours, 4 hours after operation, postoperative 1 day, 2 days, 3 days and before discharge pressure situation, children crushed area skin comfort, families of children with pain on pressure parts skin care satisfaction and turn the situation were analyzed and compared. Results The observation group postoperative 2 hours did not appear the skin red and bruised, there was no crushed in 4 hours, only 4 cases of skin red, no 1 case skin crushed before leaving hospital. Twelve cases of the control group in postoperative 2 hours appeared red skin pressure, 4 hour after crush skin in 4 cases, 6 cases of skin crushed before leaving hospital. The comfortable children for 56 cases, mild discomfort for 6 cases in the observation group,and 31, 23 cases in the control group. The satisfaction was 100.0%(62/62) in the observation group, and 93.5%(58/62) in the control group. The degree of turn the situation pain in the observation group was lower than that in the control group. Conclusions New medical position mat can effectively extend the pediatric orthopaedic congenital dislocation of the hip, pelvis fracture and femoral fracture with roll over time, reduce the stimulation of the skin, sweat for pressure parts effectively prevent the occurrence of pressure sores, improve the crushed area comfort children and families of children with satisfaction, greatly reduce the postoperative pain when rolled over, worth clinical promotion.
2.Research on the Surface Potential Distribution of Spinal Cord Based on Finite Element Analysis.

Finite element analysis can be used to study the change of the structure and the interior field intensity of human and animal body organs and tissues with simulation experiment. We in our research used finite element analysis software to analyze and solve the spinal cord surface potential problems, and investigated the transmission features of signals generated by interneurons in spinal nerves which were related with body motion control and sensory processing. A three dimensional model of electrical source in rat spinal cord was built, and the influence on potential distribution on spinal cord surface caused by position changes of electrical source in transverse direction and dorsoventral direction were analyzed and calculated. We obtained the potential distribution curves of spinal cord surface and found that the potential distribution on spinal cord surface showed monotone. In addition, potentials of some registration points were smaller than that of registration points around.

3.Clinical study on treatment of 32 cases of ulcerative colitis with Chang An Capsules

In order to explore the therapeutic action and mechanisms of Chang An Capsules for treatment of ulcerative colitis, 62 cases were randomly divided into two groups and treated with oral administration of Chang An Capsules and Bu Pi Yi Chang Pill respectively, for 3 therapeutic courses. Clinical observation, pathological therapeutic effects, and changes of enteroscopic sign , serum immunoglobin, complement C3, serum superoxide dismutase (SOD) and lipid peroxide (LPO) before and after treatment were observed. Results indicated that in the treatment group, the total effective rate was 93. 80%, the therapeutic effects for enteroscopy and pathology were 81. 30% and 84. 40% respectively, and serum immunoglobin, complement Cs, serum superoxide dismutase (SOD) activity and lipid peroxide (LPO) were improved or inhibited, with significant difference compared with the control group (P
4.Formula Screening and in vitro Drug Release of Tegafur Thermo-sensitive Gel for Intratumor Injection

Objective:To screen the best formula of tegafur temperature-sensitive gel for intratumor injection and investigate the in vitro drug release behavior. Methods:The drug dose was determined by cytotoxicity experiment. The thermo-sensitive gel was prepared with PLGA-PEG-PLGA and HPMC as the matrix. With the in vitro release as the index, the effects of PLGA-PEG-PLGA and HPMC at different concentrations on gel were investigated. The gelation temperature, viscosity and pH were detected. Results:The best formula was as follows:25% PLGA-PEG-PLGA, 1% HPMC, and tegafur dose of 1 mg·ml-1 . The average gelation temperature was 36. 7℃, the average viscosity was 7550 mPa·s, and the average pH was 7. 2. Conclusion:Tegafur thermo-sensitive gel for intratumor in-jection shows temperature sensitivity and obvious sustained-release property, which provides experimental basis for the further clinical research.
5.Study on Apoptosis-Promoting Mechanism of β-elemene in Anti-liver Cancer

This article was aimed to study the inhibition of cell proliferation and induction of apoptosis by β-el-emene extract from traditional Chinese medicine (TCM) Rhizoma Zedoariae on human hepatocellular carcinoma (HepG2) cells, as well as the effect on expression of apoptosis-related proteins in liver cancer-burdened H22 mice. Human HepG2 cells were cultured in vitro. MTT method was used in the determination of β-elemene on cell prolif-eration. Apoptosis of cells were detected by the Annexin V-FITC/PI double staining method. Through the establish-ment of liver cancer-burdened H22 mice model, the inhibition of cell proliferation and induction of apoptosis-related protein expression by β-elemene on tumor-burdened mice were observed. The results showed that β-elemene inhib-ited HepG2 cell proliferation, which was dose- and time-dependent. Annexin V-FITC/PI method detected early apoptotic cells. Inhibitions of tumor growth on tumor-burdened mice from different groups were different. Inhibition of tumor growth in the high-dose β-elemene group was relatively low, and that of the middle-dose was more obvi-ous. It was concluded that β-elemene can effectively inhibit human HepG2 cells proliferation. The inhibition on pro-liferation of cancer cells was through the inducing of cell apoptosis. It was also related to the upregulation of apopto-sis-related protein Caspase-3 and downregulation of NF-κB P65.
6.Effect of anisodamine on myocardial connexin 43 expression in pig after resuscitation from cardiac arrest

ObjectiveTo investigate the effect of anisodamine on the expression of connexin 43 (Cx43) in swine ventricular myocardium after resuscitation from cardiac arrest.Methods The experiment was conducted on healthy pigs, and they were randomly divided into three groups, namely sham group, epinephrine group (control group) and anisodamine group (experimental group, animals were resuscitated combined with injection of 0.4 mg/kg of anisodamine), with 5 pigs in each group. Model of ventricular fibrillation was reproduced by alternating current challenge, and cardiopulmonary resuscitation (CPR) was performed 8 minutes after cardiac arrest. Left ventricular myocardium was harvested at 24 hours after restoration of spontaneous circulation. The expression and distribution of Cx43 were observed by immunofluorescence, Cx43 mRNA expression was assessed with reverse transcription-polymerase chain reaction (RT-PCR), and the protein expressions of Cx43 and phosphorylation of Cx43 (p-Cx43) were analyzed by Western Blot.Results The positive expression of Cx43 in ventricular muscle was distributed uniformly, mostly at the end-to-end linkage of myocardial cells, with a few side-to-side linkage in sham group. The positive expression of Cx43 in control group was significantly weaker than that in the sham group, and the signal intensity was significantly declined (4.35±2.10 vs. 10.02±3.66,P< 0.01). The positive expression of Cx43 at the end-to-end linkage and side-to-side linkage was irregular in experimental group, and the signal intensity was obviously higher than that in the control group (7.91±2.54 vs. 4.35±2.10,P< 0.05), but it was significantly weaker than that in the sham group (7.91±2.54 vs. 10.02±3.66,P< 0.05). For control group and experimental group, the Cx43 mRNA and protein expressions were significantly lower than those of the sham group [Cx43 mRNA (A value): 0.32±0.05, 0.32±0.03 vs. 0.48±0.07; Cx43 protein (A value): 0.43±0.03, 0.50±0.07 vs. 0.65±0.04, allP< 0.01], and there were no significant differences between experimental group and control group (allP> 0.05). The p-Cx43 protein expression of control group was significantly lower than that of the sham group (A value: 0.22±0.03 vs. 0.37±0.06,P< 0.01), and it was significantly higher in the experiment group than that in the control group (A value: 0.29±0.07 vs. 0.22±0.03, P< 0.01), but there was no significant difference with the sham group (P> 0.05). No significant difference in p-Cx43/Cx43 was found among sham, control, and experiment groups (0.57±0.09, 0.51±0.05, 0.58±0.06, all P> 0.05).ConclusionAnisodamine can improve the abnormal expression of Cx43 in ventricular muscle of pigs with cardiac arrest, which may be related to the protection effect of anisodamine on cardiac conduction.
7.Thymoma-associtated graft-versus-host-like disease with skin involvement: a case report

A 37-year-old male patient presented with persistent generalized itching erythema and papules for more than 1 month.The patient had received surgical treatment for type B3 thymoma in stage Ⅳ.Five months prior to the presentation,he developed myasthenia gravis.He also complained of chronic diarrhea for two years.Physical examination revealed white patches in the oral mucosa as well as scaly erythema of varying size on the face,trunk and extremities.Lamellar scales could be seen after scratching,while removal of scales could not result in the appearance of bleeding points.There was scaly hyperkeratotic erythema of palms and soles.Histopathological examination revealed psoriasis-like epidermal hyperplasia and parakeratosis with multiple dyskeratocytes,and some of the dyskeratocytes were surrounded by lymphocytes.There was a perivascular infiltration with a small number of lymphocytes.Immunohistochemical study showed positive staining for CD3,CD4 (dermis),CD8 (epidermis) and CD45RO,but negative staining for CD20,CD68 and CD30.The patient was diagnosed as thymoma-associated graftversus-host-like disease,myasthenia gravis and mucosal candidiasis.After treatment with tacrolimus and prednisone,the condition was gradually relieved.
8.Accumulation of p53 and cytokeratin-18neo in non-small cell lung cancer and its clinicopathological significance

AIM: To investigate the tumorigenicity of lung cancer by learning the accumulation of p53 and the exposure of cytokeratin 18 neo-epitope(CK-18neo) related to the clinicopathological parameters in non-small cell lung cancer(NSCLC). METHODS: To detect the monoclonal antibodies of p53 and M30 CytoDEATH(specific antibody for CK-18neo) in 62 cases of NSCLC (included 29 cases of squamous cell carcinoma and 33 cases of adenocarcinoma) and 10 cases of control group by adopting immunohistochemistry assay (LSAB). Moreover, the immunoreactivity of p53 was quantitatively evaluated with positive unit (PU). RESULTS: (1) p53 immunoreactivity was positive in 15 of 29 squamous cell carcinoma (51.72%), 15 of 33 adenocarcinoma (45.45%), 30 of 62 NSCLC (48.39%). In 10 control cases was negative. There were significant differences between these groups (P<0.01). (2)In 62 cases of NSCLC, AI% of M30 is 1.10%, and in 29 cases of squamous cell carcinoma is 0.95%, and in 33 cases of adenocarcinoma is 1.24%. In 10 control cases, the AI% is 1.06%. There is not significant difference among these groups . (3) According to the results of Pearson's correlation analysis, we found positive linear correlation between the immunoreactivity of p53(-/+), p53(5 degrees)and p53(PU)(P<0.01). CONCLUSION: Our results suggested that the pathogenesis of NSCLC might be related to the mutation of gene p53 and cell excessive proliferation and insufficient apoptosis.
9.Diagnosis and microsurgery of acute spontaneous spinal epidural hematoma

Objective To investigate the diagnosis and the effect of microsurgery in patients with acute spontaneous spinal epidural hematoma (ASSEH). Method Five patients with ASSEH treated with microsurgery and confirmed pathologically were retrospectively analyzed. Results The main clinical presentations were root pain and palsy. The main manifestations of MRI were long-segment epidural lesion of high intensity in T1 and T2-weighted images without enhancement. With the microsurgery system, laminectomy via posterior approach and hematoma removal were successfully undergone with full recovery in all cases. Conclusions MRI assisted with the main clinical symptoms may aid preoperative diagnosis in symptomatic ASSEH. Microsurgery is an effective method for treating ASSEH. Postoperative (rather than preoperative) spinal DSA is advantageous for exclusion of spinal vascular malformation in treating ASSEH.
10.Pubvaginal fascia sling cystourethropexy treat of type Ⅲ female stress urinary incontinence

Objective To assess the pubvaginal fascia sling cystourethropexy (PV sling) technique and outcomes of the treatment of female type Ⅲ stress urinary incontinence. Methods From October 2005 to January 2008, 9 women presenting with type Ⅲ stress urinary incontinence were treated with Pubvaginal fascia sling cystourethropexy. All the patients underwent 1 h pad test before discharge and were tested with maximum uroflow rate, volume of postal residual urine, maximum urethral close pressure (MUCP) and abdominal leak point pressure (ALPP) after 3 months. Results The operative time ranged from 60 to 90 min (mean, 75 min). Estimated blood loss ranged from 60 to 100 ml (mean, 76 ml). Intraoperative unilateral bladder perforation occurred in 2 cases. One patient with infection of abdominal incision was cured by changed dressings. The urinary catheter was removed 5 days post-operation. 4 patients had normal urination and 1 patient had urine retention. The recovery following intermittent catheterization was 3-12 days. 8 patients got 3-28 months' follow up. 7 patients were cured with 1 h pad test decreased from preoperative 58 g (45-75 g) to postoperative 1 g (0-2 g), mean residual urine was less 65 ml (0-80 ml). The values of Qmax (ml/s),MUCP (cm H2O) and ALPP (cm H2O) were 10. 5±2. 7, 15. 5±3. 4 and 40. 4±8.2 pre-opreative,and 26.5±3.9, 49.8±6.7 and 98.6± 12.2 3 months post-operative. There were significant differences of these parameters between pre-and post-operation (p<0.01). Conclusions PV sling could be a safe and effective surgical procedure fortreatment of type Ⅲ female stress urinary incontinence.
