1.Application of damage control orthopedics in treatment of severe multiple injuries combined with fractures

Objective To probe the feasibility, indications and efficacy of damage control orthopedics (DCO) in treating severe multiple injuries combined with fractures. Methods A retrospective analysis was done on the clinical data of 31 patients with severe multiple injuries combined with fractures treated by DCO from 1998 to 2008. Results Of all patients, 18 patients were combined with pelvic fractures and hemorrhage. Bilateralis hypogastric artery ligation was performed early in 17 patients and nonage pelvis external fixation was performed in 12. Eleven multiple trauma patients combined with femoral fractures received primary debridement and temporal external fixation for further definitive internal fixation. Two patients with spinal fractures combined with spinal cord compression received primary simple decompression. All patients received resuscitation in ICU for average 4.3 days (1-11 days). There was one death due to hemorrhagic shock. Conclusions DCO well conforms to pathophysiological character of the wounded. Early DCO and later integrated treatment are key to enhancement of the survival rate of the multiple trauma patients.
2.Outcome of internal radiation with non-sealed colloidal phosphate (~(32)P) and macroaggregate albumin in primary hepatic carcinoma

Objective To evaluate the clinical responses and side effects of non sealed colloidal 32 P internal radiation and macroaggregate albumin (MAA) for hepatocellular carcinoma (HCC). Methods Twenty six patients with HCC underwent ultrasound guided intratumoral injection therapy of 32 P colloid and MAA. The clinical responses and toxic effects were analyzed on basis of clinical manifestions and laboratory data, including the tumor size, histopathological changes, serum alpha fetoprotein (AFP) estimation, blood routine, hepatic and renal functions and immunocompetence assays before and after treatment. Results Clinical responses: alleviation of symptoms, decreased AFP levels, fibrosis and complete or partial necrosis within tumor lesions (CR 2, PR 13, MR 2, SD 7 and PD 2). The total response rate was 57.7% and the median survival period was 16 months. Hepatic lesions became resectable in 5 patients for whom successful operation was performed. The 1 ,2 and 3 year survival rates were 88.5%, 76.9% and 38.5%, respectively. No evidence of non sealed radionuclide induced toxic reactions or complications were observed. Conclusions The treatment of internal radiation with 32 P colloid and macroaggregate albumin is effective, simple, safe and applicable with wide indications for primary hepatic carcinoma.
3.Culture and evaluation of rat’s mesenteric lymphatic endothelium in vitro

Aim To establish the technology of isolation rat’s mesenteri c lymphatics and the culture method of their endothelium and confirm the ability o f synthesis nitric oxide and Factor Ⅷ related antigens in the endothelium in vitro. Methods With an operating microscope and microsurgical in struments, rat’s mesenteric lymphatics were isolated and the endothelium were p rimarily cultured and subcultured. Exerting immunohistochemical method, Factor Ⅷ related antigens and nitric oxide synthase were evaluationed. The contents of NO metabolite were measured. Results The rat’s mesenteric lympha tics were isolated and the endothelium were Fusiform shape or polygon, the monol ayer cultures displayed typical cobblestone or pavingstone morphology. Factor Ⅷ related antigens were expressed and their positive rate was 98%. The positive rate of nitric oxide synthase was 83%. Positive reaction products were buffy ar ound nuclear. NO metabolite was (10.26?2.17) mol?L -1(n=7). Conclusion The culture method we introduced was very practical, simple and reliable. The endothelium is in normal structure and function characters. It also certificated that the endothelium in vitro could express nitric oxide synthase and Factor Ⅷ related antigens.
4.Study on the relationship among job stress, job burnout and organizational commitment in ICU nurses

Objective To investigate the current working status and explore the relationship among job stress, job burnout and organizational commitment in ICU nurses. Methods By using cluster random sampling method, 233 nurses from 5 general hospitals were selected to accept investigation by Commitment Questionnaire and Nurses' general situation questionnaire. Results The job stress and job burnout were both significant negatively correlated with organizational commitment (r=-0.33, P<0.01;r=-0.22, P < 0.01).The management issues and interpersonal relationships and decreased personal accomplishment both had significant impact on affective commitment and continuance commitment of ICU nurses (r=0.539, P < 0.01; r=0.387, P < 0.01). The management issues and interpersonal relationships, professional and career issues and emotional exhaustion had significant predictive power on normative commitment(r=0.458,P < 0.01). Conclusion It generally was in a higher level of job stress in ICU nurses, which indicates that hospital mangers should try to eliminate the causes ,assist them to reduce job stress and relieve the job burnout.
5.Relationship between human papillomavirus gene types and cervi-cal diseases

Objective: To investigate the relationship between human papilloma virus (HPV) gene types and cervical diseases. Methods:The subjects included women with HPV who were diagnosed in the Outpatient Department of the Third Affiliated Hospital of Sun Yat-sen University (Guangzhou, China) from March 1, 2010 to January 31, 2011. A total of 21 HPV DNA types were found in the women using DNA flow-though hybridization genotyping technique, and a questionnaire-type survey was conducted on these pa-tients. In this study, 256 cases of normal or inflamed cervix, 34 cases of cervical intraepithelial neoplasia I (CINⅠ), 61 cases of CINⅡor CINⅢ, and 9 cases of cervical cancer were analyzed. Results:The HPV infection rates were 18.4%in the normal cervical or inflam-mation group, 67.6%in the CINⅠgroup, 96.7%in the CINⅡor CINⅢgroup, and 100%in the cervical cancer group. The HPV in-fection rate was higher in the CIN I group than in the normal HPV infection or inflammation group (P<0.001) and was higher in the CIN Ⅱ or CIN Ⅲ groups than in the CIN I group (P<0.001). The main types of infection were HPV16 (44.2%), HPV58 (24.2%), HPV52 (11.6%), and HPV33 (8.4%) in the CIN groups. The main type of infection was HPV16 in the cervical cancer group (88.9%). Conclusion: Cases of cervical lesions are increasing. With this prevalence of HPV, considerable attention should be focused on HPV16-, 58-, 52-, and 33-related infections and cervical lesions. Among them, HPV16 infection is the primary cause of disease progres-sion and carcinogenesis.
6.Clinical Effect of Renal Artery Embolization for Renal Graft Dysfunction

Objective To explore the clinical effect of renal artery embolization for renal graft dysfunction.Methods A total of 17 patients with renal graft dysfunction were treated by embolizing the renal graft artery.Coil embolization was performed on 7 of the cases,and gelfoam combined with coil was used in the other 10.Results Among the 17 patients,14 discontinued immunosuppressant in 3 weeks after the embolization,and 2 discontinued after receiving low-dose cortisones for about 6 months,and showed good recovery during follow-up.The other patient underwent resection of the renal graft because of continuous fever and graft pain.After the embolization,14 cases developed 'post-embolization syndrome'.Coil migration occurred in 1 case during the procedure.One patient experienced angina during the operation,and developed myocardial infraction the day after the embolization.Conclusions Renal artery embolization is safe and effective for patients with renal graft dysfunction.By using the method,open surgery can be avoided.Attention should be paid to potential complications.
7.Safety evaluation of mouse nerve growth factor injection for treatment of children and elderly patients with optic nerve damage

Background The clinical efficacy and safety of mouse nerve growth factor(NGF) for injection have been evaluated by Ⅰ,Ⅱ,Ⅲ stage of clinical trials.But as a clinical drug,its adverse effects for special population should been further studied.Objective This clinical trial was to observe and assess the safety and tolerability of mouse NGF for injection during the application in juvenile and elder patients with optic nerve damage.Methods A multicenter non-randomized controlled trial for the safety evaluation of mouse NGF injection solution was performed in 100 eye centers in China.This study protocol was approved by the Ethic Committee of each drug research center,and written informed consent was obtained from the subject,legal mandatary and guardian prior to this study.Total 2046 patients who met the included criteria of optical nerve damage were enrolled.The patients were divided into the juvenile group(90 cases,< 18 years),youth-middle-age group (1868 cases,1 8-75 years) and older group(88 cases,>75 years).Mouse NGF(30 μg)was intranuscularly injected once per day for 21 days in all the patients.Systolic and diastolic blood pressure,heart rate,intraocular pressure (lOP),laboratory examinations of blood and urine,blood biochemical indexes were recorded before and after injection.All adverse events were evaluated after administration of drug.Results There were 189 (189/2046,9.23%)dropped out cases,including 184 lost cases after 3 weeks of treatment and 5 withdrew cases from adverse responses of drug,with a shedding rate <10% in each group.Total 1857 individuals finished this trial.No significant differences were found in the IOP,heart rate and blood pressure between before and after admninistration of grug in the juvenile group,youth-middle-age group and older group (P>0.05).The incidence rate of the adverse response in the juvenile group,youth-middle-age group and older group was 57.78%,68.18%,60.01% respectively,and the primary local adverse events were pain(48.89%),redness and swelling(59.09%),duration(54.49%),without significant difference in the rate of local adverse events among the three groups(X2 =2.302,P =0.324).The abnormal rates of laboratory results of blood and urine were not significantly different among the three groups(all at P>0.05).The percentage of impaired fasting glucose was 7.46%,23.73%and 7.79% in the juvenile group,youth-middle-age group and older group,respectively,and the percentage in the older group was higher than that in the juvenile group and the youth-middle-age group (x2 =8.685,P =0.005 ;x2 =27.720,P =0.000).Conclusions Injection of mouse NGF is well tolerated in juvenile and elder patients over the age of 75 years.
8.Qualitative study of Balint group cases in teaching hospital

Objective To analyze the negative experience from medical practice of medical students by Balint group case study,and to explore new approach for training doctor-patient communication skills.Methods 10 times of Balint group activities were held in Zhongshan Hospital of Fudan University,with one case each time.Grounded theory approach were applied for analyzing discussion record.Results The cases were occurred in doctor-patient interaction,teamwork,conflict between one's personal life and vocational development planning.Trigger events involved doctor-patient conflict,conflict of values,decision making problem while conffronted with incurable patients,role ambiguity,unfair treatment.Topics of discussion included emotional experience with patients,career development orientation,medical teamwork,clinical education of medical student.Conclusion Through practice of Balint group,medical students in teaching hospital can improve their awareness of complicating psycho-social problem of patients and themselves,as well as handling it.And it can improve the professional identity,professional growth and professional self-identity.
9.A pilot molecular mechanism study on lacrimal gland as a potential human immunodeficiency virus reservoir

Background Acquired immune deficiency syndrome (AIDS) is an infectious disease caused by human immunodeficiency virus (HIV).Highly active antiretroviral therapy (HAART) is an effective treatment for AIDS,but it cannot completely eliminate the viral load in the body for the existence of HIV reservoir.Previous studies demonstrated that HIV could be detected in tears of virus load negative AIDS patients who received effective HAART,suggesting that lacrimal gland is another member of HIV reservoirs.Objective The aim of this study was to explore whether lacrimal gland has a molecular basis of HIV infection and the mechanism of lacrimal gland infection of HIV.Methods Fourteen specimens of lacrimal gland were collected during the surgery from 14 patients with lacrimal gland diseases in Peking Union Medical College Hospital from November 2013 to December 2015,including 13 non-HIV-infected patients and 1 HIV-infected patient.In 13 non-HIV infected patients,lacrimal glands prolapse was in 12 patients with the normal pathological tissue structure and dacryoadenitis was in 1 patient with the histopathological diagnosis of interstitial lymphoid tissue hyperplasia.The clinical manifestation of HIV-infected patient was dacryoadenitis with the histopathological diagnosis of interstitial lymphoid tissue hyperplasia.The paraffin sections of 12 non-HIV-infected specimens and 1 HIV-infected specimen were prepared,and the expressions of CD4,C-X-C chemokine receptor 4 (CXCR4) and C-C chemokine receptor type 5 (CCR5) in lacrimal gland specimens were detected by immunohistochemistry and verified in 1 specimen of non-HIV-infected specimen by immunofluorescence technology.Results Immunohistochemistry showed that CD4 was suspiciously positive expression in non-HIV-infected specimens with the strong background staining.CXCR4 was positively expressed in cytoplasm and nuclei of most lacrimal epithelial cells of lacrimal gland epithelial cells in each specimen,and CCR5 was focally expressed in few lacrimal gland epithelial cells in each specimen.In addition,CD4,CXCR4 and CCR5 were positively expressed in intercellular scattered lymphocytes on the specimens.Immunofluorescence assay showed that CD4,CXCR4 and CCR5 were expressed in the specimens with the red fluorescence,with the linear-and patchy-like distribution mainly in cellular membrane for CD4 or spot-like distribution for CXCR4 and CCR5 in the cytoplasm.Conclusions HIV receptor CD4 and accessory receptor CXCR4,CCR5 are positively expressed in the lacrimal gland epithelial cells,which is the molecular basis of HIV infection and become a potential HIV reservoir preventing HIV eradication.
10.Clinical and imaging analysis of white matter lesions combined with congenital sensorineural hearing loss in children

Objective To analyze the clinical and imaging characteristics of congenital sensorineural hearing loss (CSNHL)children combined with white matter (WM)lesions in order to provide evidence for clinical practice. Methods With referral to the Department of Pediatrics,Peking University First Hospital from November 201 1 to De-cember 201 5,documents of 78 patients of CSNHL combined with WMlesions were collected and analyzed for the clini-cal and imaging characteristics.Results Bilateral severe -profound hearing loss existed in all 78 cases,48.1 %(25 /52 cases)of the patients exhibited gross motor development delay,98.1 %(51 /52 cases)of them had normal cognition development.One hundred percent (61 /61 cases)of patients had abnormal language development.Infection occurring during pregnancy existed in 21 .2%(1 1 /52 cases)of the patients,the premature and smaller for the gestational age in-fants accounted for 28.9% (1 5 /52 cases).The bilateral multiple WMlesions from the brain MRI were in dot to flake sizes with sharp boundary,the intensity of T1 -weighted imaging decreased,T2 -weighted imaging and fluid attenuated inversion recovery increased.Eighty -two point one percent (64 /78 cases)of the patients were found to have the periventricular and subcortical WM involvement.The most frequently affected periventricular region was the posterior horn (91 .9%,68 /74 cases),followed by the anterior horn and temporal horn,and the least with the body involvement. The former three had a combined lesion tendency (55.4% -68.9%).There was an extensive involvement in the sub-cortical WMof parietal,frontal,temporal and occipital lobes respectively(73.5% -88.2%).Subcortical WM involve-ment of multiple lobes was common (accounted for 67.6% -77.9%).The enlargement of bilateral ventricles existed in 37.2%(29 /78 cases)of the patients and cystic changes in the subcortical WM of anterior temporal lobe could be found in 9.0% (7 /78 cases)patients.Calcification in 2 CT cases was reported.Corpus callosum and basal ganglia of all cases were normal.For cases with MRI scans more than once,WMlesions of 96.0%(24 /25 cases)patients became silent or self -restored.Conclusions The clinical presentations of CSNHL combined with WM lesions are mild,not paralleled with their multiple foci.It is considered as demyelination or a delay of myelination.Due to its benign course, it is probably not the contraindication for the cochlear implantation.
