1.Principle and progress of surgery for colorectal cancer

Colorectal cancer is one of the most common malignancies in human.Early diagnosis is the key points in improving the prognosis of patients with colorectal cancer.The implementation of standardized operation and comprehensive treatment for advanced and late stage cancer can improve the outcome.Based on the pattern of lymph node metastasis and the concept of complete mesocolic excision,the operation of colonic cancer has entered the era of normalization and standardization.The surgical procedure of rectal cancer (especially low rectal cancer) should be individualized based on the gold standard of total mesorectal excision (TME).For hepatic metastasis of colorectal cancer,hepatectomy is currently the standard and the only potentially curative treatment method.Therefore,how to evaluate the resectability of hepatic metastasis of colorectal cancer is particularly important.Surgeons are the most important participants in the diagnosis and treatment of colorectal cancer,and standardized diagnosis and treatment is the key to improve the prognosis of patients.
2.Immunological effect of high mobility group box-1 protein on regulatory dendritic cells and corresponding mechanism

ObjectiveTo observe the effects of high mobility group box-1 protein ( HMGB1 ) on function of murine spleen IL-10-producing dendritic cells (DCs) subset CD11clowCD45RBhighDCs.MethodsSplenic CD11clowCD45RBhighDCs and CD4 +T cells in Balb/c mice were purified by magnetic beads sorting.CD11clowCD45RBhighDCs were treated with various doses of HMGB1 (0,20,100,500ng/ml).Flow cytometry was used to determine expressions of CD11clowCD45RBhighDC surface molecules including CD40,CD80,CD86,Ⅰ-a/e and Toll-like receptor (TLR) 4.IL-10 level in CD11clow CD45RBhighDC culture supernatants was determined by ELISA method.The CD4+ T cells were divided into four groups: control group (without only treatment),untreated group (HMGB1-untreated CD11clow CD45RBhigh DCs plus CD4+T cells),high-dose HMGB1-treated group (500 ng/ml HMGB1-treated CD11clowCD45RBhigh DCs plus CD4 +T cells),high-dose HMGB1-treated + antibody 1 group (500 ng/ml HMGB1-treated CD11clowCD45RBhigh DCs plus IL-10 antibody and CD4+T cells),high-dose HMGB1-treated + antibody 2 group (500 ng/ml HMGB1-treated CD11clow CD45RBhigh DCs plus homotype IL-10 antibody and CD4 +T cells).IL-4 as well as interferon( IFN-γ) contents in CD4 + T cell culture supernatants were determined by flow cytometry.ResultsHMGB1 could markedly enhance the expressions of CD40,CD86 and TLR4 on CD11clowCD45RBhigh DC surface as well as level of IL-10 secreted by CD11clow CD45RBhigh DCs as compared with the non-HMGB1 treatment.Meantime,the secretion of IL-10 was HMGB1 concentration dependent.IFN-γ level in high-dose HMGB1-treated group was (279 ± 17) pg/ml,which was markedly lower than that in the untreated group [ (963 ± 11 ) pg/ml,P < 0.05 \ ].IL-4 level in high-dose HMGB1-treated group was (372 ± 14) pg/ml,which was significantly higher than that in the untreated group [ (213 ± 10) pg/ml,P <0.05) \].ConclusionsHMGB1 promotes IL-10 production in CD11clowCD45RBhighDCs,induces the differentiation of CD4 +T cells to Th2 cells and downregulates the immune response via activating CD11clowCD45RBhigh DCs.
3.Comparison of the treament to adhesion ileus between laparoscopic surgery and traditional surgery

Objective To compare the treament to adhesion ileus between laparoscopic surgery and traditional surgery.Methods 59 cases of patients with adhesion ileus were selected.32 patients were given laparoscopic adhesion ileus loose solution method.And the other 32 patients were given traditional surgery.Results The operation time(t =12.135,P =0.002),peristalsis recovery time (t =9.053,P =0.001),postoperative infection rate (x2 =8.346,P =0.004),obstruction again (x2 =4.863,P =0.026) and hospital stays (t =4.724,P =0.028) in laparoscopic surgery group was significantly better.The differences between both groups were statistically significant (P <0.05).Conclusion Compared with the traditional open surgery,laparoscopic adhesion ileus loose solution with surgical trauma is small.Operation time and the postoperative recovery time is short,It can be used as the main treatment of adhesion ileus.
4.On compensation strategies ofHuangdi-Neijing from the perspective of Bassnett’s cultural translation theory

Translation is a process not only concerning on the language transformation, but also concerning on the cultural communications. However, due to the diversity between cultures, translation loss is a stumbling block which has to be coped with, or a necessary process called translation compensation. This paper, employing the method of text analysis, attempts to compare two English versions ofHuangdi-Neijing by Veith and Li-Zhaoguo as study cases to discuss the translation compensation strategies from the perspective of Bassnett’s cultural translation theory, thus achieving the optimal translation effect.
5.Diagnosis and treatment of excessive heat accumulated in chest syndrome which belongs to Fu-viscera disease of sanjiao and its application

6.Leptin and chronic renal failure

Leptin,a polypeptide hormone coded by Ob-gene,has the functions of depressing appetite,reducing caloric intake and enhancing caloric consumption,etc.Leptin has a close relationship with kidney,for it is metabolized and eliminated via kidney.Meanwhile,leptin can also directly affect kidney functions,influencing the process of chronic renal failure.Furthermore,hyper-leptinemia is associated with anorexia,malnutrition and cardiovascular diseases.Different dialysis patterns may also play different roles in cleaning leptin.For example,common dialysis could not clear leptin,but other dialysis modes such as high permeability dialysis,hemodiafiltration,and mastic absorption hemoperfusion can alleviate hyper-leptinemia through cleaning leptin partly.For kidney transplantation patients,hyper-leptinemia may affect the immune balance which has been maintained for a long time.
7.A sporadic case of tuberous sclerosis complex caused by a newly found heterozygous missense mutation of c.T1967C in TSC1 gene

This paper reported a sporadic case of a 29-year-old Han female diagnosed with tuberous sclerosis complex (TSC) by next generation sequencing (NGS),one of genetic analysis techniques.She was admitted because of recurrent intractable seizure for 26 years,dizziness and headache for 3 months.Physical examination revealed angiofibromas over her face,shagreen patches in her lower back area,and hypomelanotic macules around her limbs and body.Cranial MRI manifested lesions on lateral ventricles,cerebellar vermis and left temporal lobe with abnormal signal changes on both sides of extensive cerebral cortex.A pathogenic and heterozygous missense mutation,c.T1967C,in exon 16 of her TSC1 gene was found via genetic tests,which has not yet been reported before.
8.Effect of thiopental sodium on the release of glutamate and gamma-aminobutyric acid from rats prefrontal cortical synaptosomes.

To investigate the effect of thiopental sodium on the release of glutamate and gamma-aminobutyric acid (GABA) from synaptosomes in the prefrontal cortex, synaptosomes were made, the spontaneous release and the evoked release by 30 mmol/L KCl or 20 micromol/L veratridine of glutamate and GABA were performed under various concentrations of thiopental sodium (10-300 micromol/L), glutamate and GABA concentrations were determined by reversed-phase high-performance liquid chromatography. Our results showed that spontaneous release and evoked release of glutamate were significantly inhibited by 30 micromol/L, 100 micromol/L and 300 micromol/L thiopental sodium, IC50 of thiopental sodium was 25.8 +/- 2.3 micromol/L for the spontaneous release, 23.4 +/- 2.4 micromol/L for KCl-evoked release, and 24.3 +/- 1.8 micromol/L for veratridine-evoked release. But GABA spontaneous release and evoked release were unaffected. The study showed that thiopental sodium with clinically related concentrations could inhibit the release of glutamate, but had no effect on the release of GABA from rats prefrontal cortical synaptosomes.

9.Protective effect of glycine on the lung injury in acute necrotizing pancreatitis in rats

Objective To investigate the protective effects of glycine on the lung injury in acute necrotizing pancreatitis(ANP). Methods Male Wistar rats were randomly divided into 4 groups: sham operation, ANP group,pretreated group and treatment group(with intravenous glycine 1g/kg). Serum TNF ? level,glutathione (GSH) and myeloperoxidase (MPO) in pulmonary homogenate were determined at 6, 12, and 24h hours after ANP induced. Pulmonary pathology and 3 day survival rate were compared between the groups.Results Compared with ANP group,in glycine pretreated group serum TNF ? level significantly reduced at 6,12, and 24h,while in glycine treatment group serum TNF ? level decreased at 24-hour . In glycine pretreated group or treatment group, the pulmonary GSH content were decreased.In both glycine pretreated group and glycine treatment group, pulmonary MPO activity decreased at 12 and 24h,and pulmonary pathologic severity significantly ameliorated and survival rate increased. Conclusions Although glycine can not increase the GSH synthesis,but giving prophylactic or therapeutic glycine may be effective in improving acute lung injury in ANP, probably through the inhibition of the infiltration and activation of leukocytes.
10.Mechanisms of transcutaneous immunization and its application in allergic diseases

Transcutaneous immunization (TCI) is a novel immunotherapy approach that induces systemic immune responses via topical application of antigens and adjuvants onto the skin.It is a safe and effective method,and is expected to serve as an attractive alternative to traditional vaccination.TCI induces immune responses in different directions,but the exact mechanism of skin immune tolerance is still unclear.Most studies on TCI are based on animal models.However,there are structural differences between animal skin and human skin,so more researches are needed to achieve the translation of TCI from bench to bedside.Not all vaccines are suitable for cutaneous inoculation,so vaccines for TCI should be evaluated for their safety and efficacy.Various reformed vaccine delivery systems are needed to be further explored in related diseases.
