International Journal of Surgery 2014;41(3):177-180
Objective To explore and summarize the clinical features and treatmentof the nonpuerperal mastitis. Methods Eighty-two cases of nonpuer-peral mastitis from 2003 to 2013 were analyzed retrospectively. All patients were divided into surgery alone group (11 cases)and medication combined surgery group (71 cases),which PDM patients with triple anti-mycobacterial drugs therapy,GM patients with cortical hormone therapy. Results All patients were diagnosed by biopsy. The total duration of simple drug group (triple or hormone) is (11.92 ±6.50) weeks and remission rate after 4 weeks was (89.06 ± 2.25) %.Drugs plus surgery group, the total duration of the group is (6.57 ± 2.30) weeks, and no recurrence. The total duration of the simple surgery group is (17.09 ± 13.11) weeks and remission rate after 4 weeks was (63.64 ± 1.10) %.Comparison of the total duration of the three groups and the cumulative response rate curves were significantly different(P < 0.05).Conclusions Nonpuerperal mastitis requires a combination of clinical and pathological diagnosis.Surgery is not the only treatment option, triple anti-mycobacterial or cortisol hormones for different types of pathology conservative treatment can achieve a satisfactory therapeutic effect.Drug therapy combined with surgery can shorten the duration and reduce the relapse rate,it is an effective treatment for nonpuerperal mastitis.
5.Inhibitory effect of mastoparan-like peptide from wasp (Vespa magnifica) venom on angiogenesis
Chinese Pharmacological Bulletin 2014;(5):715-718
Aim To investigate the anti-angiogenic effect of 12a, the mastoparan-like peptide from wasp ( Vespa magnifica ) venom. Methods The tube for-mation and proliferation of human umbilical vein endo-thelial cells ( HUVEC ) and chicken chorioallantoic membrane ( CAM) model were used to observe the an-ti-angiogenic effect of 12a in vitro and in vivo, respec-tively. Results In the CAM model, 0. 1 μg or 1 μg of 12 a could markedly inhibit angiogenesis induced by 0. 2 μg rh-bFGF with heavy loss of color, decreasing density and obscure frame of the vessels. The inhibi-tion rates of angiogenesis were 60 . 2 % for 0 . 1 μg and 90. 3 % for 1 μg, respectively. Accordingly, in HU-VEC culture experiment, the proliferation and angio-genesis of HUVEC treated by 1 mg·L-1 and 10 mg· L-1 of 12 a were decreased by 55. 4 %, 39. 3 % and 51. 6 %, 26. 7 %, respectively. Conclusion 12a has a significant anti-angiogenic effect in a concentra-tion-dependent manner.

Chinese Journal of Anesthesiology 2014;34(4):492-496
Objective To evaluate the effect of sevoflurane preconditioning on autophagy during ischemiareperfusion (I/R) injury to isolated rat hearts and the role of PI3K/Akt signaling pathway. Methods Healthy adult male Wistar rats, aged 6-8 weeks, weighing 250-280 g,were anesthetized.Their hearts were excised and perfused in a Langendorff apparatus.Eighty-four isolated rat hearts with I/R injury were randomly divided into 7 groups (n =12 each):normal control group (NC group),I/R group, sevoflurane preconditioning group (S + I/R group),normal control plus wortmannin group (NC + W group),I/R plus wortmannin group (I/R + W group),sevoflurane preconditioning plus wortmannin group (S + I/R + W group),and solvent group (S + I/R + D group). In NC group, the hearts were continuously perfused with K-H solution for 180 min.In I/R group, the hearts were perfused with K-H solution for 30 min,and then subjected to ischemia for 30 min followed by 120 min of reperfusion. In S+ I/R group, the hearts were perfused with K-H solution for 10 min,and then with K-H solution saturated with 2.5% sevoflurane for 15 min, followed by 5 min washout with K-H solution before ischemia. In NC + W group, wortmannin (PI3K inhibitor) 15 μg/kg was injected intraperitoneally at 30 min before chest opening, and the other procedures were similar to those previously described in group NC.In I/R + W group, wortmarnin 15 μg/kg was injected intraperitoneally at 30 min before chest opening, and the other procedures were similar to those previously described in group I/R.In S + I/R + W group, wortmannin 15 μg/kg was injected intraperitoneally at 30 min before chest opening, and the other procedures were similar to those previously described in S + I/R group. In S + I/R + D group, dimethyl sulfoxide 1.5 ml/kg was injected intraperitoneally at 30 min before chest opening, and the other procedures were similar to those previously described in group S + I/R.The HR,± dp/dtmax,left ventricular developed pressure (LVDP) and left ventricular end-diastolic pressure (LVEDP) were recorded at the end of equilibration and reperfusion. At the end of reperfusion, coronary effluent was collected to detect lactate dehydrogenase (LDH) activity, and myocardial specimens were obtained to calculate the percentage of myocardial infract size. The expression of autophagy marker LC3-Ⅱ and Akt,phosphor-Akt (p-Akt),mammalian target of rapamycin (mTOR),and phosphor-mTOR (p-mTOR) was determined by Western blot. Results Compared with NC group,no significant change was found in the parameters of hemodynamics in NC + W group, and HR,± dp/dtmax and LVDP were significantly decreased,LVEDP,myocardial infract size, and LDH activity were increased,LC3-Ⅱ expression was up-regulated,and the expression of p-Akt and p-mTOR was down-regulated in the other groups.Compared with group I/R,HR,± dp/dtmax,and LVDP were significantly increased,LVEDP,myocardial infract size, and LDH activity were decreased,LC3-Ⅱ expression was downregulated, and the expression of p-Akt and p-mTOR was up-regulated in S + I/R and S + I/R + D groups, and no significant change was found in each parameter in S+ I/R+ W group.Compared with S + I/R group,HR,± dp/ dtmax and LVDP were significantly decreased,LVEDP,myocardial infract size and LDH activity were increased,LC3-Ⅱ expression was up-regulated,and the expression of p-Akt and p-mTOR was down-regulated in S + I/R + W group, and no significant change was found in each parameter in S + I/R + D group. Conclusion Sevoflurane preconditioning can decrease autophagy of myocardial cells during I/R through activating PI3K/Akt signaling pathway and enhancing mTOR activity in the downstream, thus protecting isolated rat hearts against I/R injury.
7.Cross-talk among unfolded protein response, cancer metabolism, and autophagy in a cancer microenvironment
Chinese Journal of Clinical Oncology 2014;45(11):734-737
Malignant tumor is typically characterized by fast proliferation and invasiveness. As such, tumor blood supply often fails to satisfy tumor growth requirements, resulting in a stressful cancer microenvironment with low oxygen level and insufficient nutri-ents. Cancer cells survive in such stressful environments by activating various signals (such as unfolded protein response), changing metabolic pathways, and undergoing autophagy. Hence, the cross-talk among these mechanisms should be determined to provide new insights into the interdynamics of cancer cells and their microenvironment. Furthermore, results would provide relevant information for the development of novel therapeutic strategies.
8.Development, problems and prospects of the treatment of diabetic macular edema
Chinese Journal of Ocular Fundus Diseases 2013;(1):1-4
Diabetic macular edema (DME) is a common ocular complication of diabetes patients. It mainly involve macular which is closely related with visual function, thus DME is one of the major reasons causing visual impairment or blindness for diabetes patients. How to reduce the visual damage of DME is always a big challenge in the ophthalmic practice. In the past three decades, there are tremendous developments in DME treatments, from laser photocoagulation,anti-inflammation drugs to anti-vascular endothelial growth factor therapy. However, the mechanism of DME development is not yet completely clear; every existing treatment has its own advantages and weaknesses. Therefore DME treatment still challenges us to explore further to reduce the DME damages.

Chinese Journal of Perinatal Medicine 2013;(6):362-368
Objective To explore the glucose and liplid metabolism changes of pregnant rats and the birth weight of their offsprings due to pregnancy malnutrition, including overnutrition and undernutrition. Methods Thirty pregnant Wistar rats were randomly divided into three groups on the day of conception, the high-fat-diet group, normal control group and 50% food-restricted diet group.Increase of body weight and serum level of triglycerides,high-density lipoprotein,low-density lipoprotein, oral glucose tolerance test (OGTT) and insulin release test(IRT) levels during pregnancy were measured. The birth weight of offsprings was recorded within 12 h after birth.One-way ANOVA,LSD or Dunnett T3,and Chi-square test were applied for statistical analysis. Results The body weight at late pregnancy and the level of triglycerides and low-density lipoprotein were significantly increased in the high-fat-diet rats compared to normal control ones [(1.68±0.13) mmol/L vs (0.78±0.08) mmol/L,(0.57±0.04) mmol/L vs (0.35±0.07) mmol/L; both P<0.01].While the level of high-density lipoprotein of high-fat-diet rats was lower than that of the normal control rats [(0.56±0.06) mmol/L vs (1.09±0.08) mmol/L,P<0.05].The level of triglycerides, low density lipoprotein and high density lipoprotein in 50% food restricted group [(0.47±0.06) mmol/L,(0.21±0.06) mmol/L and (0.42±0.05) mmol/L] were lower than the control group (all P<0.05).Impaired glucose tolerance, abnormal OGTT and IRT results were found in both high-fat-diet and 50% foodrestricted diet rats. The fasting plasma glucose level in high-fat-diet group was significantly increased in late pregnancy compared to the control [(6.63±0.53) mmol/L vs (4.90±0.26) mmol/L,P<0.05],while there was no significant difference between rats in 50% food-restricted diet [(4.18±0.26) mmol/L]and control group (P>0.05).There was no statistical difference in fasting serum insulin level among the three groups. The average birth weight was significantly increased in high-fat-diet rats than in the control group [(6.14±0.31) g vs (5.73±0.26) g,P<0.05],and the incidence of macrosomic newborn rats in the high-fat-diet group was also higher than that of the control group [19.2 % (19/99)vs 7.84%(8/102),P<0.05].The birth weight of offspring in 50% food-restricted diet group was decreased with their average birth weight of (4.54±0.23) g,which was lower than that of the control group (P<0.05).The incidence of macrosomic [13.40% (11/76)] and low birth weight newborn rats [15.90% (13/76) vs 3.92%(4/102)] in food-restricted diet group were both higher than in the control group (P<0.05); and smaller number of offsprings was obtained than the control group (76 vs 102,P<0.05),the number of offsprings in high-fat-diet group was 99,which was less than the control group without statistical difference (P>0.05).Conclusions Pregnancy malnutrition, including overnutrition and undernutrition, may affect the lipid metabolism and glucose tolerance of pregnant rats, leading to insulin resistance, and may also contribute to the abnormal birth weight of the offspring, including macrosomia and low birth weight.
10.Silver ion dressing effects on healing of chronic wound
Chinese Journal of Tissue Engineering Research 2013;(42):7494-7500
BACKGROUND:Silver ion dressing can reduce tissue necrosis, accelerate the epithelial formation, relieve the patient’s pain during the treatment, and strengthen various growth factors for wound repair, which is based on the theory of wet healing theory. It has obvious advantages for chronic wound healing, such as anti-infection and promoting wound granulation and epithelial hyperplasia.
OBJECTIVE:To observe the efficacy of silver ion dressing used in healing of chronic wound.
METHODS:A total of 40 patients who had chronic wounds were selected from Tangshan Branch Hospital of Nanjing General Hospital of Nanjing Military Command. Al cases were divided into two groups. The treatment group was treated with silver ion dressing, and the control group was treated with routine dressing. Secretions extracted from wound were used for bacterial culture prior to and 7, 14, and 21 days after treatment. Healing condition and velocity were observed and wound pain was assessed using visual analog scale.
RESULTS AND CONCLUSION: The detection rate of wound bacteria in the treatment group was significantly lower than that of the control group (P<0.05). And the time of healing was shorter in the treatment group than the control group (P<0.05). Wound pain during dressing change was relieved (P<0.05). Medical costs showed no difference between the two groups (P>0.05). No adverse reaction occurred in the two groups. These findings indicate that silver ion dressing used in chronic wound healing can prevent infection and promote wound granulation or epithelial hyperplasia. It can effectively promote wound healing and has no bad reaction.
