1.CAP and ISO15189 accreditation requirements of verification and validation tests for clinical molecular diagnostic assays

College of American Pathologists (CAP) and International Standard Organization (ISO)15189 accreditation provides requirements to the verification and validation tests for clinical molecular diagnostic assays.The article will introduce the requirements in these aspects including the differences between the two accreditation systems,the differences between the verification and validation tests,the essential factors and standardized framework of the verification and validation tests for clinical molecular diagnostic assays.
2.Relationship between the Expression of H19 and IGF-Ⅱ Imprinted Gene in Newborn Placenta and the Birth Body Mass

Objective To study the expression of the imprinted gene H19 and IGF-Ⅱ in newborn placenta,and to discuss its influence on the birth body mass of the neonate. Methods The fresh placental tissues from full-term newborn (without trimester of pregnancy complica-tion and placenta and funic abnormality) with normal,high and low birth body mass (12,10 and 8 samples respectively)were collected. The expression of imprinted gene H19 and IGF-Ⅱ mRNA in the placenta were estimated by reakime fluorescence quantitative PCR Results The ex-pression of H19 mRNA in the placenta was negative correlation to the birth body mass (r =-0.403,P = 0.027).The expression of of IGF-H mRNA was positive correlated to the birth body mass (r = 0.444,P = 0.014). The H19 mRNA expression level in the high birth weight neonates (0.21 0.31) was significantly lower than that in the low birth body mass neonates (1.51 2.04)(P= 0.013). But the expression level of IGF-Ⅱ mRNA in the high birth body mass neonates (2.67±3.41) was significantly higher than that in the low birth body mass neonates (0.39±0.33)(P =0.013). Conclusion The expression of H19 and IGF-Ⅱ mRNA was significantly different in the placenta of normal,high and low birth body mass newboms. These two genes may be related to the birth body mass,and there may be some realation-ship between these two genes.
3.Overview of Antitumor Effects and Mechanism of Ginsenoside Rb1, Rg3, Rh2

Ginsenoside is the main component of antitumor effects of traditional Chinese medicine (TCM) Ginseng. Clinical pharmacological studies have shown that Ginsenoside Rb1 can inhibit P-gp which can lead to efflux of drugs, enhance the sensitivity of cells to drugs, reduce the multidrug resistance of the tumor cell, and maintain the immune function of the body to tumor cells. Ginsenoside Rg3 can impact tumor cell protein expression, play a role in cell division, induce tumor cell apoptosis and inhibit tumor growth of blood vessels. Ginsenoside Rh2 may play an antitumor effect by inhibiting tumor cell proliferation and metastasis, inducing tumor cell apoptosis. This article ex-plained the antitumor effects and mechanism of Ginsenoside Rb1, Rg3, Rh2 in order to provide theoretical evidences for the clinical development and application.
4.The Price Regulation of Medical Service:Theoretic model and reform trends in China

Objective: To explore the medical service price regulation of theoretic model and reform trends in China. Methods:Qualitative method. Results:Medical care is the service that mix the nature of public service and private service. The price regulation of medical care includes the regulation of price level and price structure. The asymmetric information, equality, externality and monopoly of the medical care are the reasons to implement the price regulation. Ramsey pricing model, investment rate of return pricing model, the price-cap model, yardstick price regulation model and Feldstein pricing model are the general models to implement the price regulation of medical care. Conclusion: China should reform and modify the current price level and structure of medical care based on the above models.
5.Content Change of Aconitine in Radix Aconiti Kusnezoffii with Different Boiling Time

Objective To investigate the content change of aconitine in Radix Aconiti Kusnezoffii in different boiling time. Methods The chromatographic procedure was carried out with methanol-water-chloroform-diethylamide (70∶30∶3∶0.2) as mobile phase at a flow rate of 1.0 mL/min, the column temperature at 30 ℃, and UV detector at 240 nm. Results There was a good linear relationship of aconitine within 0.08-0.8 μg, r=0.999 8 (n=6), and the average recovery rate was 97.16%. The content of aconitine increased gradually in 0-2 h, then gradually decreased. And the reduction was evident particularly during 2-4 hours. Conclusion HPLC method is rapid, simple, accurate, reproducible and reliable for determining aconitine. The content of aconitine can be decreased effectively by boiling 4 hours.
6.Spiral CT three-dimensional reconstruction in the differential diagnosis value of lung lesions

Objective To evaluate the value of spiral CT in diagnosis and treatment of lung lesions in clini-cal work.Methods A retrospective analysis of 100 patients with pathological diagnosis of lung lesions patients were randomly divided into the control group (CT normal scan)and observation group (CT -D post -processing tech-niques).Lungs lesions,hilar and mediastinal lymph node metastasis sensitivity,specificity and accuracy of lymph node metastasis of both group were analyzed.Results In the control group,the diagnosis of lung lesions showed sensitivity of 75%,specificity of 50% and accuracy of 72%;while in the observation group the sensitivity was 80%,specificity was 65 % and accuracy rate was 85%.The differences between the two groups were statistically signficant(χ2 =3.854,4.604,5.007,all P <0.05).50 cases of pathologically verified as lung cancer patients with hilar lymph node metastasis in the control group showed diagnostic sensitivity of 60%,specificity of 72%,accuracy of 70%;while diag-nosed hilar lymph node metastasis in the observation group showed sensitivity of 76%,specificity of 84 percent and accuracy rate of 84%.The differences between the two groups were statistically signficant(χ2 =5.882,4.196,5.534, all P <0.05).50 cases of pathologically verified as lung cancer patients with mediastinal lymph node metastasis in the control group,showed diagnostic sensitivity of 62%,specificity of 57% and accuracy of 60%;in observation group the sensitivity was 72%,specificity was73% and accuracy was 72%.The differences between the two groups were statisti-cally signficant(χ2 =4.342,5.626,4.583,all P <0.05).Conclusion The spiral CT for diagnosis of lung lesions positioning has good effect,and can be widely used in the diagnosis of lung lesions.
7.Effects of type 2 diabetes mellitus patients with hypertension on lipid metabolism and insulin sensitivity

Objective: To observe the effects of type 2 diabetes mellitus patients with hypertension on lipid metabolism and insulin sensitivity. Methods: A case-matched study of fasting plasma glucose (FBG), total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HIDL), low density lipoprotein cholesterol (LDL), insulin (FIN), C-peptide (FCP) and insulin sensitivity index (ISI) were detected in type 2 diabetes mellitus patients with and without hypertensiorn. Results: Twenty patients with type 2 diabetes mellitus complicated hypertension had higher TG、 FINS and FCP, lower HDL and ISI levels than 20 patients with type 2 diabetes mellitus without hypertension. Conclusion: Type 2 diabetes mellitus with hypertension had more remarkable lipid metabolic disorder and insulin resistance than type 2 diabetes mellitus without hypertension.
8.Quality standard of Rhinitis Compound Spray

AIM: To establish the quality standard of Rhinitis Compound Spray (Caulis Luffae, Herba Ephedrae, etc. ) . METHODS: TLC was used in qualitative identification of Caulis Luffae and Herba Ephedrae. And TLC-scanning was used for assay of oleanolic acid in the preparation. RESULTS: Caulis Luffae, Herba Ephedrae could be detected by TLC. Oleanolic acid had a good linear relationship within the range of 2.0-10.0 ?g (r= 0.999 8 ). The average recovery was 98.91%, and the RSD was 1.71%. CONCLUSION: The method established is stable and reliable. It can be used for quality control of the preparation.
9.Relationship between serf-efficacy and subjective well-being of the elderly in community

Objective To investigate the relationship between self-efficacy(SE)and subjective wellbeing(SWB)of the elderly in community.Methods 212 elderly people underwent a questionnaire survey by the Memorial University of New Foundland Scale of Happiness and the General Self-efflcacy Scale.Results The level of self-efficacy of the elderly in community was(2.39±0.63),of which impact factor was the level of education and average monthly income per person.The level of SWB was(37.31±7.24),of which impact factor was the level of education and occupation.The level of SE and SWB of the elderly sas mdderately positively correhted,r=0.36,P<0.01. Conclusions In the practice of community care services,community nurses should take SE theory as a framework and to improve SWB by a vailety of nursing measures.
