1.Analysis of the influencing factors on the time of acute cerebral apoplexy patients visiting the hospital

Objective To study the influencing factors on the time of acute cerebral apoplexy patients visiting the hospital.Methods 64 patients with acute ischemic stroke were researched.The clinical data of all patients were collected.The influencing factors on treatment time were analyzed by univariate and multivariate Logistic regression analysis.Results In the 64 patients, 26 cases of the visiting time≤6 h(40.63%), 38 cases of visiting time>6 h(59.37%).The results of single factor analysis showed that the relationship between the time of treatment,pre-hospital transfer mode (x2=6.650,5.081,all P<0.05),educational level,living location,MRS score,whether or not know the stroke(x2=4.561,6.557,8.788,4.146,all P<0.05);the disturbance of consciousness and symptoms ≥3 symptoms compared with the visiting time had significant difference(x2=5.133,5.926,all P<0.05);in the multiple factors Logitic regression analysis,the degree of education(OR=2.332,95%CI:1.402~3.904),severity of illness(OR=3.956,95%CI:1.934~7.446),understanding of stroke(OR=3.621,95%CI:1.563~8.125),disturbance of consciousness(OR=4.336,95%CI:2.394~11.374),pre-hospital transport mode(OR=5.658,95%CI:2.546~12.554) were the risk factors of treatment time.Conclusion There is a close relationship with extension and various factors of acute ischemic stroke patients,in the future,not only need to improve the pre-hospital emergency care system,but also need to strengthen the public understanding of the disease.
2.A Study on CT Manifestation of Hepatic Abscess

Objective To evaluate CT features of hepatic abscess caused by different etiologies.Methods CT appearances and clinical data of hepatic abscesses were retrospectively studied in 54 cases.Results There appeared low density lesions in hepatic abscess.Target sign,cluster sign and air in the cavity were special CT manifestations of hepatic abscess.Abscess caused by different etiologies had different features,which included infection by blood stream or bile duct abnormalities,amebic liver abscess,and hepatocellular carcinoma complicated with suppurative infection after treated with TAE.Conclusion The CT manifestations of hepatic abscess were characteristic.These manifestations were helpful in diagnosing hepatic abscess not only in location but also in nature and etiology.
3.CT Diagnosis of Mesenteric Panniculitis

Objective To analyze the CT manifestations of mesenteric panniculitis,so that to improve CT diagnosis of mesenteric panniculitis.Methods Seven patients with mesenteric panniculitis,which were diagnosed according to the CT criteria,were analyzed.Results The fatty masses with defined margins and around the root of the jejunum were found in all patients at CT scan,and there were no enhancement after administration of contrast medium.The masses were sharply defined with abdominal and retroperitoneal fat.There were pseudo-capsule in 7, "arcus adiposus sign" in 5. Enlargement of lymph nodes in the masses and retroperitoneum in 2 cases were seen,respectively.4 patients had abdominal operations,one patient was in combination with carcinoma of head of pancrease.Conclusion Certain features of the CT manifestations of mesenteric panniculitis can be found,CT is effective for the diagnosis of mesenteric panniculitis.
4.Cells and cellular interactions in the pathogenesis of idiopathic pulmonary fibrosis

Idiopathic pulmonary fibrosis ( IPF) is a kind of Interstitial lung disease with cause unknown,which is aggressive for its increasing prevalence and high morbidity and mortality.At present the pathogenesis of IPF is not entirely clear, but cells and cel-lular interactions play a decisive role on the alveolar inflammation and fibrosis results.We review IPF related cells of interest ( immune competent cells and fibroblast, fibrocyte, myofibroblast, endothelial and alveolar epithelial cells) , to summarize cells and cellular in-teractions in the pathogenesis of IPF,and discuss new research directions and therapeutic targets.
5.A preliminary study of the role of miR-193a-3p in radioresistance of esophageal cancer cells

Objective To investigate the role of miR?193a?3p in the radioresistance of esophageal squamous cell carcinoma ( ESCC) . Methods MTT assay was used to identify the cell lines with the highest radiosensitivity and radioresistance in four esophageal cancer cell lines exposed to irradiation of 6 MV X?ray. Stem?loop quantitative real?time PCR was used to measure the expression levels of miR?193a?3p, miR?155, and miR?22?3P in the two cell lines. Further studies were performed on miR?193a?3p because of the substantial difference in its expression between the two cell lines. The mimic (3PM) and antagomiR (3PA) of miR?193a?3p as well as siRNA ( si?LOXL4) were synthesized and transfected into cells to elevate and inhibit miR?193a?3p expression. MTT assay and flow cytometry were used to evaluate the effects of miR?193a?3p and its downstream gene LOXL4 on radiosensitivity. Results KYSE510 and KYSE410 were characterized as cell lines with the highest radiosensitivity and radioresistance, respectively. miR?193a?3p had a substantially larger difference in expression between the two cell lines than miR?155 or miR?22?3P (1. 00 ∶ 21. 00). Transfection of 3PM resulted in elevated expression of miR?193a?3p in KYSE510, which had a significantly lower radiosensitivity and a significantly reduced apoptosis ratio by 11. 01% compared with the control group ( P<0. 05) . KYSE410 transfected with 3PA had a significantly higher radiosensitivity ( P<0. 05) . The expression of LOXL4, a downstream gene of miR?193a?3p, was negatively correlated with miR?193a?3p expression. Transfection with si?LOXL4 inhibited the expression of LOXL4, which resulted in a significantly lower radiosensitivity and a significantly reduced apoptosis ratio by 7. 07% compared with the control group ( P<0. 05) . Conclusions miR?193a?3p promotes the radioresistance of esophageal cancer cells probably by regulation of LOXL4.
6.Nursing management with alemtuzumab for prevention of graft rejection

Alemtuzumab (Campath) was successfully injected in 21 kidney transplant patients,7 islet transplant patients and 1 simultaneous kidney and islet transplant patient for either prevention or treatment of graft rejection.Prophylactic administration was successfully completed in all patients without discontinuation.Adverse events were not observed in 11 patients (38%),but hypertension in 18 patients (62%),shivering in 3 patients (10.3%),high fever in 3 patients (10.3%),and bronchospasm in 1 patient (3%),respectively.All complications alleviated after proper therapy.During the prophylactic administration of alemtuzumab,strict,timely and proper ward-management was needed.Care for lung,perineum,skin,diet and psychological nursing were necessary.Neither graft acute rejection nor graft chronic rejection episode occurred in all patients during 6 months to 2 years follow-up.Therefore,long term effects of Alamtuzumab and consequences of lymphocytopenia need further observation.
7.Studies on activity of NK cells in preeclampsia patients.

The activity of the NK cells in patients with preeclampsia was studied to investigate the pathogenesis of preeclampsia. By using MTT and 51Cr releasing technique, the proliferation and killing ability of the NK cells in maternal and umbilical blood from preeclampsia patients (n = 18) and normal third trimester pregnant women (n = 18) were detected. The NK-92 cell line was as the positive control. The results showed that the NK cell counts of umbilical blood in preeclampsia patients and normal third trimester pregnant women were significantly greater than those of maternal blood (both P<0.05). Compared with that in normal third trimester pregnant women, the proliferative ability of the NK cells in preeclampsia patients was apparently increased (P<0.05). Compared with that in maternal blood, the proliferative ability of the NK cells in umbilical blood from both preeclampsia patients and normal third trimester pregnant women was dramatically increased. The killing ability of the NK cells in preeclampsia patients was significantly higher than that in normal third trimester pregnant women (P <0.05). It was suggested that both number and function of the NK cells in preeclampsia women were increased, and that in umbilical blood was greater than that in maternal blood, speculating that the function of the NK cells may affect the maintenance of the maternal and fetal immune tolerance during pregnancy.

9.Analysis of the spiral CT performance of tertiary silicosis by three-dimension thin-slice reconstruction

Objective To study the spiral CT performance of tertiary silicosis by three-dimension thin-slice reconstruction.Methods Sixty patients with tertiary silicosis were selected in the research.All of the patients were given thin slice scan,and the images of patients with tertiary silicosis were reconstructed by workstation,the spiral CT performance was analyzed.Results In all patients,the pulmonary large shadow mainly distributed in the upper middle lung field,accounted for 93.33% (56/60).Big shadow internal signs for calcification,containing air bronchogram,empty.Big shadow around with signs of more complex,with 40 patients with adjacent pleural thickening phenomenon,accounted for 66.67% (40/60).15 cases complicated with tuberculous,accounted for 25.00% (15/60) of the total number of patients.3 cases with lung cancer,accounted for 5.00% (3/60) of the total number of patients.Conclusion The spiral CT performance of tertiary silicosis by three-dimension thin-slice reconstruction is diversified,which included pulmonary large shadows with symmetric distribution,pleural thickening,bullae of lung in subintima,lymph gland swelling and calcification in mediastinum and hilus pulmonis.It can provide reference for the diagnosis of tertiary silicosis.
10.Change in hippocampal voltage-gated Ⅰ sodium channel of Lithium chloride-Pilocarpine epileptic rat model

Objective To research the changes in hippocampal voltage-gated sodium channel of Lithium chloride-Pilocarpine epileptic rat models,including Ⅰ sodium channel α subunit protein (Nav1.1),mRNA of Ⅰ sodium channel alpha subunit protein gene and function of sodium channel.Methods Epileptic rat models of Lithium chloride-Pilocarpine were established.Nav1.1 expression in the hippocampus of experimental rats was detected by immunohistochemical staining method,and the changes in voltage-gated sodium channel function (the current-voltage curves,activation and inactivation curves and the recovery curve) of hippocampus nerve cells were detected by whole cell patch-clamp technique.Results (1) The Lithium chloride-Pilocarpine rat models were successfully reproduced.Three stages of behavior (acute,latent and chronic) of rat models were observed.The blank control group was free of seizure.(2) Immunohistochemistry results:neurons in CA1 and DG regions of hippocampal of epileptic rats were normal,and there was no obvious change in the expression of Nav1.1.In CA3 area,the degeneration and necrosis of neurons were obvious.Staining of Nav1.1 became superficial and even disappeared in these areas,but the normal tissues were enhanced around degenerative and necrotic neurons.Compared with the blank control group,the expression of Nav1.1 in the model group was higher(0.235 ±0.008 vs.O.210 ±0.002),and there was statistically significant difference (t'=-7.426,P < 0.05).(3) The whole-cell patch-clamp technique showed that the sodium current density of the model group increased significantly compared with that of the blank group [(-319.70 ± 28.24) pA/pF vs.(-229.06 ± 26.01) pA/pF,t =8.178,P < 0.05],the threshold value of activation curve decreased (4.15 ± 0.80 vs.4.50 ±0.85,t =11.020,P < 0.05),the threshold value of inactivation curve increased (7.47 ± 0.53 vs.6.24 ±0.31,t =6.940,P < 0.05),and the recovery time after inactivation shortened [(1.36 ± 0.15) ms vs.(1.86 ± 0.21)ms,t =6.712,P < 0.05],and there were all statistically significant differences.Conclusion Repeated seizures can lead to increase Nav1.1 compensatory expression of,and significantly increase sodium channel current density,while the threshold value of activation curve decreases,the threshold value of inactivation curve rises,and the recovery time after inactivation is shortened,which eventually leads to increased neuron excitability and is more likely to cause seizures.
