1.Discussing of Mental Rehabilitation of Children with Disability under ICF Theory and Practice

Objective To discuss the mental rehabilitation of disabled children under International Classification of Functioning, Disabil-ity and Health (ICF). Methods The target, the organization of work team, environment support and the core problems of disabled children's mental rehabilitation were discussed based on the concept of ICF. Results The goal of mental rehabilitation was to recover the mental func-tion of disabled children. The multidisciplinary team should include medicine, psychology, education, social work and so on. Supportive en-vironments such as professional workers, families, communities, partner and barrier-free environment were the effective means of disabled children's mental rehabilitation. Respect and equality were the precondition of disabled children's mental rehabilitation, and positive atten-tion should be the working attitude. Conclusion ICF-CY can provide a new perspective and framework for the mental rehabilitation of dis-abled children.
2.Application evaluation of immunophenotyping in monoclonal gammaglobulinemia

Objective To investigate the clinical value of immunophenotyping and quantitative analysis of serum immunoglobulin in the diagnosis and typing of monoclonal gammaglobulinemia.Methods 118 serum samples were collected from the patients with monoclonal gammaglobulinemia and performed the serum protein electrophoresis(SPE),serum immunofixation(IF)electrophoresis, immunoglobulins(Ig)quantitation and serum light chain κ,λdetection.The analysis was conducted aiming at the patients with im-munophenotyping positive and the Ig quantitation negative.Results 56 cases showed immunophenotyping positive and the immuno-globulin quantitation negative,among them,6 cases were free light chain type (κlight chain in 2 cases andλlight chain in 4 cases), 1 case was the non-secretion type;62 cases showed immunophenotyping positive and the Ig quantitation positive.Conclusion The IF technique has an important significance for the diagnosis and typing of monoclonal gammaglobulinemia,its diagnosis can be com-bined with the other laboratory tests of serum Ig and light chain quantitative detection.

World Report on Disability discussed the current situation of persons with disabilities, health care, rehabilitation, support and assistance, barrier-free environment, education, employment, and relevant policy recommendations, adhering to the spirit of Convention on the Rights of Persons with Disabilities, using the International Classification of Functioning, Disability and Health as its theoretical structure.Based on the report, this article discussed the revelations to services of rehabilitation for persons with disabilities and their problems,which were advocating new concepts of rehabilitation services, using multidisciplinary rehabilitation model, and strengthening construction of talents in services of rehabilitation.
5.Long-term result and prognostic analysis of 18FDG PET/CT positioning three-dimensional conformal radiotherapy for stage Ⅲ non-small cell lung cancer

Objective To analyse the long-term result and prognosis of 18 FDG PET/CT positioning three - dimensional conformal radiotherapy ( 3 DCRT ) for stage Ⅲ non - small cell lung cancer. Methods Sixty-four cases with stage Ⅲ non-small cell lung cancer (clinical stage Ⅲa- Ⅲb ) were randomly divided into two groups: PET/CT positioning three-dimensional conformal radiotherapy group (PET/CT group) and the conventional CT positioning three-dimensional conformal radiotherapy group (conventional CT group). In the PET/CT group, the target volume and critical organs were sketched according to PET/CT after fusion of the PET and the CT images; the treatment plan was worked out, then conventional fractionated 3DCRT ( total dosage around 40 Gy) followed by field-shrinked radiotherapy to a total dose of 65 Gy or sowas performed ;in the conventional CT group, the target volume and critical organs were sketched according to CT and 3DCRT were performed to the same total dose; All cases were treated with the TP scheme (paclitaxel 175 mg/m2,d1 ,cisplatin 40 mg,d2-4) adjuvant chemotherapy for 6 cycles after the radiotherapy. Results The followup rate was 100%. The number of patients who completed the 1-,2-and 5-year follow-up were 40,20 and 11 respectively ;The number of patients of the PET/CT group and conventional CT group were 23 and 17,11 and 9,7 and 4 respectively. Target volumes of 13 cases in the PET/CT group were changed. The complete remission and partial remission rates of the two groups were 13% 、66% and 19% 、53% (x2 = 0. 33, P =0. 564), respectively. The 1-,2-and 3-year local control rates of the PET/CT group and conventional CT group were84 % 、66% 、53 % an d72% 、59% 、44% ( x2 = 2.36, P = 0. 124 ) respectively. The1 -, 2-and 3-year survival rates were 72% 、34% 、22% and 53% 、28% 、13% (x2 =2. 46,P =0. 117) respectively. The level-1 and level-2 lungs' and trachea's late radiation injury of the PET/CT group and the conventional CT group were 28% and 53% ( x2 = 4. 14, P = 0. 042 ), respectively. The hilar and mediastinal lymph node recurrence rates of the PET/CT group were lower than those of the conventional CT group, were 3% ,25%(P = 0. 026) and 6%, 28% ( P = 0. 042 ), respectively. The main reason for treatment failure was distant metastasis both in the PET/CT group and conventional CT group,56% and 47% (x2 = 0. 56,P = 0. 453 ),respectively. Conclusions PET/CT, as a method of sketching the target of stage Ⅲ non-small cell lung cancer, can improve the radiation treatment plan, reduce the recurrence rate of hilar and mediastinal lymph nodes, meanwhile it can not improve the long-term survival rate; Distant metastasis was the main reason of failure.
6.Development and validation of a liquid chromatography-tandem mass spectrometry method for the quantification of creatinine-corrected sarcosine in urine

Objective To establish a liquid chromatography-tandem mass spectrometry ( LC-MS/MS) method for the quantification of creatinine-correctedsarcosine in urine for the prostate cancer diagnosis and treatment.Methods It performed the method establishment and evaluation in this study.Random unrine samples were collected from 36 subjects with prostate cancer, 15 subjects with benign prostatic hyperplasia and 76 healthy people receiving medical examination.Urine samples mixed with [ 2 H3 ]-labeled sarcosine were treated by precolumn derivation using dansyl chloride, then analyzed by LC-MS/MSsystem in multiple reaction monitor ( MRM) mode.Sarcosine and creatinine were quantified by the isotope internal standard method and the standard curve was employed with a series of calibration.The limit of detection, precision and recovery were also evaluated in this study.The results of this methodology were compared with those of the enzymatic method.Results Sarcosine could be distinguished against its isomers completely. The linear equation of sarcosine was Y=2.045 6X+0.068 9, R2 =0.994.The limit of detection and limit of quantity were 8 ng/ml and 25 ng/ml respectively.The intraassay and interassay coefficients of variation were both below 6%.The recovery ratio of sarcosine ranged from 96.8%to 105.1%.The results from the ID-LC-MS method correlated with those from enzymatic method (R2 =0.815, P <0.01).Compared to enzymatic method, the average bias of sarcosine was -37.1%.Conclusions It established a LC-MS method for urinary sarcosine quantification with good specificity, sensitivity and repeatability.This method can provide a reliable platform for the diagnosis of prostate cancer.
7.The carbapenem-resistance mechanism and homology analysis of four strains of Klebsiella pneumoniae isolated from different sites of one patient

Objective To study the drug resistance mechanism and homologous relationship of four clinical strains of carbapen-em-resistant Klebsiella pneumoniae isolated from different sites of the same one patient.Methods Four carbapenem-resistant K .pneumoniae strains were isolated from blood,urine,sputum and pelvic drainage of a patient after cystectomy in clinical mi-crobiology lab of Xinhua Hospital in March 2012.① Modified Hodge test was used to detect the expression of carbapenemase.② PCR amplification assay and DNA sequencing were used to detect resistance-related genes.③ SDS-PAGE analysis was per-formed to analyze outer membrane porin components of the four carbapenem-resistant K .pneumoniae isolates.ERIC-PCR as-say was used to analyze the homology of these carbapenem-resistant isolates.Results Modified Hodge test demonstrated that all the four carbapenem-resistant K.pneumoniae expressed carbapenemase.KPC-2 gene was identified in all the four isolates by PCR test and DNA sequencing.SDS-PAGE analysis indicated an outer membrane porin alteration common to all the four iso-lates which is distinct from that of the strain sensitive to antibiotics.The identical DNA fingerprinting of the four strains was confirmed by ERIC-PCR analysis.Conclusions The antibiotic resistance mechanism of the four carbapenem resistant K.pneu-moniae associated with this case includes the expression of KPC-2 and altered outer membrane permeability induced by abnormal expression of outer membrane porin.These four strains belong to one common clonal group.

Objective To investigate the roles of high-resolution magnetic resonance imaging (HR-MRI) in the diagnosis and treatment of vertebrobasilar dissecting aneurysms (VBDA).Methods From January 2016 to December 2016,62 patients with VBDA admitted to Xuanwu Hospital,Capital Medical University using 3.0 T HR-MRI (3D-TOF-MRA,3D-T1WI,and 3D-T1WI+C sequence) and diagnosed by DSA were analyzed retrospectively.They were divided into either an asymptomatic group (n=18) or a symptomatic group (n=44) according to their medical histories.Their clinical features,imaging features,and the relationship with prognosis were analyzed.Results HR-MRI revealed that the double lumen sign/intimal flap accounted for 27.8% (n=5) and 54.5% (n=24) respectively in the asymptomatic group and symptomatic group.The intramural hematoma accounted for 22.2% (n=4) and 43.2% (n=19) respectively in both groups.The enhancement of aneurysmal wall accounted for 33.3% (n=6) and 77.3% (n=34) in the asymptomatic group and symptomatic group.There were significant differences in the above features between the two groups (all P<0.05).In the symptomatic group,18.2% (n=8) of patients had aneurysm recurrence,which was significantly higher than the asymptomatic group (5.6%,n=1).The difference between the two groups was statistically significant (P<0.05).Conclusions HR-MRI can better reveal the lesion characteristics of VBDA.The rates of aneurysmal wall enhancement and recurrence in symptomatic VBDAs are higher,which indicates that HR-MRI can assess the stability of the aneurysmal wall and identify the prognosis to a certain extent.
