Journal of Jilin University(Medicine Edition) 2019;45(5):1128-1133
Objective: To study the therapeutic effect of compound Chinese medicine preparation of wild chrysanthemum and rhizoma drynariae combined with ornidazole in the treatment of chronic periodontitis, to explore the curative effect of this method in the treatment of chronic periodontitis, and to calarify its mechanism. Methods: A total of 112 patients with chronic periodontitis were divided into control group, ornidazole group, Chinese medicine group, Chinese medicine combined with ornidazole group (combination group) according the order of clinic time; there were 28 cases in each group. After basic periodontal treatment of the affected tooth (supragingival scaling and subgingival scaling), the periodontal pockets of the patients in control group were washed with 3% H2O2 and 0. 9%NaCl injection by turns, once a week; the patients in ornidazole group were orally administered with ornidazole one piece at a time, twice a day; the patients in Chinese medicine group were injected with compound Chinese medicine preparation of wild chrysanthemum and rhizoma drynariae into the pocket to make the medicine overflow to the edge of the gingival margin, once a week; the patients in combination group were injected with compound Chinese medicine preparation into the periodontal pocket while orally administered with ornidazole. All patients in various groups were treated for 4 weeks. The periodontal indexes including modified plaque index (mPLI), modified sulcus bleeding index (mSBI), probing depth (PD), and attachment loss (AL) of the patients in various groups were observed before and after treatment; and the levels of tumor necrosis factor-α (TNF-α) and prostaglandin E2 (PGE2) in the gingival crevicular fluid (PISF) of the patients in various groups were detected after one course of treatment. Results: There were no significant differences in the periodontal indexes and the levels of TNF-αand PGE2 in PISF of the patients before treatment between various groups (P>0. 05). Compared with control group, the level of PGE2 in PISF in Chinese medicine group was reduced (P<0. 05); the PD and the PGE2 level in PISF of the patients in ornidazole group were significantly reduced (P<0. 05); the mPLI, mSBI, PD and AL, the levels of TNF-a and PGE2 in PISF of the patients in combination group were significantly reduced (P<0. 05). Compared with ornidazole group, there were no significant differences in the above indicators of the patients in Chinese medicine group (P>0. 05); the mPLI, mSBI, PD and AL, the TNF-α level in PISF of the patients in combination group were significantly reduced (P<0. 05). Compared with Chinese medicine group, the mPLI, mSBI, PD and AL, the levels of TNF-α and PGE2 in PISF of the patients in combination group were significantly reduced (P<0. 05). Conclusion: The traditional compound Chinese medicine preparation of wild chrysanthemum and rhizoma drynariae combined with ornidazole has the best therapeutic effect in the chronic periodontitis, which can improve the periodontal index and reduce the levels of TNF-α and PGE2 in PISF of the patients.

Objective:To investigate the effect of Chinese traditional medicine-Shuanghuangbu combined with Xipayi Mouth Rinse on the periodontal indexes of the peri-implantitis patients, and to detect the curative effect of Shuanghuangbu combined with Xipayi Mouth Rinse in the treatment of peri-implantitis.Methods: A total of 40 patients diagnosed as peri-implantitis were randomly divided into control group and Chinese traditional medicine group (n=20).All of the patients received supragingival scaling and subgingival scaling, and then the peri-implant pocket of the patients in Chinese traditional medicine group were washed with Xipayi Mouth Rinse, while the fine probe slided slowly along the inner wall of peri-implant pocket into the bottom and retained the Shuanghuangbu, and retreated gently and made the medicine overflow the gingival margin. The patients in control group were given 3% hydrogen peroxide and 0.9% sodium chloride injection washed by turns. The indexes of covering modified plaque index (mPLI),probing pocket depth (PPD),modified sulcular bleeding index (mSBI)of the patients in two groups at baseline and after treatment were detected;Whatman# 1 was applied to aspirate the peri-implant sulcus fluid (PISF)which were weighed at baseline and after treatment, and the level of interleukin-1β (IL-1β)in PISF was analyzed by ELISA method. Results:No differences were observed in age, sex, periodontal indexes, weights of PISF and IL-1βlevels of the patients in two groups before treatment (P >0.05).Compared with before treatment, the indexes including PPD,mSBI,mPLI,weight of PISF and IL-1βlevel in PISF of the patients in two groups were decreased after treatment (P < 0.05).After treatment, the differences in mSBI,mPLI,weight of PISF and IL-1β level in PISF of the patients between control group and Chinese traditional medicine group were significant (P <0.05).Compared with control group, the PPD level of the patients in Chinese traditional medicine group was decreased, but the difference was not significant (P >0.05).Conclusion:The curative effect of Chinese traditional medicine-Shuanghuangbu combined with Xipayi Mouth Rinse is obsious, and the treatment can improve the periodontal status and reduce the level of IL-1βin PISF.

Objective To investigate whether 5% strontium-containing brushite bone cements(DCPD) has repair effect on alveolar bone defects in osteoporotic rabbits. Methods Eighteen healthy adult female rabbits were used to establish osteoporosis models and were randomly divided into three groups:the blank control group, DCPD group, doped 5%DCPD group(5%strontium),with 6 rats in each group. In addition to the blank group, rats the other groups were filled with the corresponding bone cements in the bilateral alveolar bone defects. At 4 weeks and 8 weeks after operation,3 rats in each group were killed and given shooting the defect area X-ray.The expression of b-FGF was detected by immunohistochemistry. Results X-ray results showed that the defected of doped 5% DCPD group have been nearly completedat the 8 week,DCPD group partially completed repair, but blank group was not fully repaired. The results of immunohistochemistry showed that the expression of b-FGF was the highest at 4 weeks and decreased at 8 weeks after operation.Expression of b-FGF was significantly different at 4 weeks and 8 weeks among the three groups. Conclusion The 5%strontium-containing brushite bone cements can repair bone defect in osteoporotic rabbits.
