1.Effects of pentoxifylline on expression of uncoupling protein 2 of hepatic mitochondria in rats with nonalcoholic fatty liver

Objective To investigate the effects of pentoxifylline (PTX) on the expression of uncoupling protein 2 (UCP-2) of hepatic mitochondria in rats with nonalcoholic fatty liver disease (NAFLD). Methods Sixty SD rats were randomly divided into 3 groups (each n = 20): normal control group, experiment group and treatment group. The rats in normal control group were given a normal feed. The rats in experiment and treatment groups were given fat-rich feed. Furthermore, the rats in treatment group were given PTX after 4 weeks in fat-rich diet feeding. The expression of UCP-2 in the liver was detected by immunohistochemistry and semi-quantitative RT-PCR. Results The hepatic expression of UCP2 mRNA was higher in experiment group (4.0±0.3) than in normal control group (1.2±0.1). The hepatic expression of UCP2 mRNA was higher in treatment group (3.0±0.2) than in normal control group, but the hepatic expression of UCP2 mRNA was lower in treatment group than in experiment group (F = 160. 67, P< 0. 01). Conclusions The UCP-2 mRNA is expressed in livers of NAFLD, pentoxifylline plays an important role in the reduction of expression of UCP-2 mRNA in lives of NAFLD.
2.Analysis of nutritional risk and nutritional support therapy in elderly hospitalized patients

Objective To investigate the prevalence of nutritional risk and nutrition support treatment in elderly hospitalized patients,and provide a basis for nutrient management in elderly hospitalized patients.Methods The Nutritional Risk Screening 2002 (NRS 2002) were administered to 163 patients (74 cases in digestive system and 89 cases in endocrine system) in elderly hospitalized patients from November 2013 to March 2014.Results The prevalence of nutritional risk was higher than in the digestive system (60.8%) in the endocrine system (32.6%) (P<0.05).Conclusions There is a high prevalence of nutritional risk in elderly wards,and the wards with lower prevalence of nutritional risk are more likely to lack of nutrition support treatment.Therefore,it is necessary to pay attention to nutritional risk and nutritional support treatment in departments with low prevalence of nutritional risk.
3.Clinical significance of combined detection of serum NT-proBNP,cTnT and myoglobin in acute myocardial infarction diagnosis

Objective To investigate the clinical significance of combined detection of serum cardiac troponin T (cTnT ) ,N-termi-nal pro-B-type natriuretic peptide(NT-proBNP) and myoglobin(Myo) in acute myocardial infarction diagnosis .Methods 440 pa-tients with main symptoms of acute chest pain were enrolled and divided into AMI group and non-AMI group ,and 220 healthy peo-ple were served as the control group .BECKMAN COULTER Access 2 automated chemiluminescent microparticle immunoassay analyzer was employed to detect their serum NT-proBNP ,cTnT and Myo concentrations .Results The serum concentrations of NT-proBNP ,cTnT and Myo of patients with AMI were (3 546 .12 ± 220 .95) pg/mL ,(0 .13 ± 0 .02)μg/L and (88 .12 ± 14 .27)μg/L , respectively ,which were significantly higher than those in non-AMI group and the control group(P<0 .05) .Areas under the curve (AUC) of receiver operator characteristic(ROC) of NT-proBNP ,cTnT and Myo detection were 0 .809 ,0 .851 and 0 .749 ,respec-tively ,and the ROC curve AUC of cTnT detection was markedly higher than that of Myo (P<0 .05) .The sensitivity ,specificity and diagnostic accuracy of combined detection of NT-proBNP ,cTnT and Myo were 0 .957 ,0 .950 and 0 .948 ,respectively ,which was significantly higher than those in combined detection of any two kinds of indicators (P<0 .05) .Conclusion Combined detection of serum NT-proBNP ,cTnT and Myo can improve the sensitivity ,specificity and diagnostic accuracy in AMI diagnosis .
5.Xanthogranulomatous cholecystitis: a clinical analysis of 78 cases

Objective To study the etiology,diagnosis and treatment of xanthogranulomatous cholecystitis (XGC).Methods Clinical data of 78 cases with xanthogranulomatous cholecystitis (confirmed by postoperative pathology) from January 1985 to December 2012 were reviewed retrospectively.Results All 78 cases underwent ultrasonography,50 cases did CT scan.Preoperative diagnosis included chronic calculous cholecystitis in 60 cases,gallbladder carcinoma with cholelithiasis in 8 cases,gallbladder space-occupying lesions in 10 cases.Among those 68 cases of tentative gallstone disease,67 cases were with gallbladder neck incarcerated stones.Surgery were performed in all 78 cases including cholecystectomy in 48 cases,partial cholecystectomy or subtotal resection in 13 eases,cholecystectomy with partial hepatic wedge resection in 12 cases.Intraoperatively 5 cases were misdiagnosed as the carcinoma of the gallbladder and underwent partial liver resection along with cholecystectomy.17 cases underwent choledocholithotomy.2 cases suffered from hepatic duct injury and received Roux-en-Y hepatojejunal anastomosis.Conclusions XGC is a special type of chronic cholecystitis,and accompanied by yellow granuloma formation.Preoperative diagnosis of XGC is often difficult,the definite diagnosis depends on intraoperative fiozen and postoperative paraffin pathology.
6.Study of carotid arteriosclerosis plaque by ultrasound real-time tissue elastography in patients with TOAST1 style cerebral infarction

Objective To evaluate the vulnerability of carotid arteriosclerosis plaque by ultrasound real-time tissue elastography in patients with SSS-TOAST1 style cerebral infarction, and discussing the value of the technique in assessment of the clinical course after cerebral infarction. Methods There were 113 patients of SSS-TOAST1 style cerebral infarction who had carotid arteriosclerosis plaque and 48 patients of contrast group who had carotid arteriosclerosis plaque selected by ultrasound in Department of Ultrasound, Beijing Tiantan Hospital, Capital University of Medical Sciences. The results between two groups were compared. The cerebral infarction group was divided into two sub-groups according to the clinical course of patients after cerebral infarction, and the difference between them was compared. Results The size had no significant difference between cerebral infarction group and contrast group as well as between aggravated group and non-aggravated group (t=15.61, 10.77, 4.52, P<0.05). The real-time tissue elastography of carotid arteriosclerosis plaques were red-green in most patients of cerebral infarction group. The real-time tissue elastography of carotid arteriosclerosis plaques were green-blue in most patients of in control group. The value of elasticity of plaque, vessel wall and stiffness of carotid arteriosclerosis plaque between cerebral infarction group and control group had significant differences (t=15.61, 10.77, 4.52, P<0.05). The value of real-time tissue elastography between aggravated group and non-aggravated group had significant difference (t=6.39, 2.30, 3.80, P<0.05). Conclusion Real-time tissue elastography could evaluate the stiffness of carotid arteriosclerosis plaque, which was related with the vulnerability of carotid arteriosclerosis plaque. The values of elasticity of plaque, vessel wall and stiffness of carotid arteriosclerosis plaque in patients with SSS-TOAST1 style cerebral infarction were lower, and the vulnerability of carotid arteriosclerosis plaque was higher. Real-time tissue elastography had some worth in evaluating the clinical course of patients after cerebral infarction.
7.OBSERVATION ON SPECIFIC IgG4 ANTIBODY OF SCHISTOSOMIASIS PATIENTS BEFORE AND AFTER TREATMENT

Objective] To observe the alteration of specific IgG4 antibody of schistosomiasis patients before and after treatment. [Methods] ELISA. [Results] The SEA IgG4 and AWA IgG4 positive rates of 27 schistosomiasis cases were 96 3% and 100%, respectively,their average OD values were 1 62 and 0 72. 6 months post treatment 18 cases were followed up, the positive rates were 94 4% and 100%, respectively, their average OD values were 1 06 and 0 56, respectively. 12 months post treatment all cases were followed up, the positive rates of SEA IgG4 and AWA IgG4 were 96 3% and 92 6%, resepectively ,their average OD values were 0 99 and 0 58, respectively. [Conclusion] No obvious changes were found in the SEA IgG4 and AWA IgG4 positive rates of 27 schistosomiasis cases before and after treatment, whereas the antibody level of specific IgG4 was decreased.

9.Expression of lipoprotein(a)and fibrinogen in different genders of patients with coronary heart disease and its clinical significance

Objective To investigate the expression of lipoprotein(a)and fibrinogen different genders of patients with coronary heart disease(CHD)and its clinical significance.Methods 1 500 patients with CHD in the hospital from January 2009 to December 2013 were selected as the observation group,including 954 males and 546 females.Contemporaneous 500 healthy people with physi-cal examination were selected as the control group,including 329 males and 171 females.The myocardial enzymes,serum lipid,blood coagulation function and various proteins were detected.Results Whether males or females,CK,CKMB,AST,LDH,α-HBDH, TC,TG,LDL-C,Fib,D-D,TT,FDP,LPa,cTnI,MYO and HCY in the observation group were significantly higher than those in the control group.HDL-C,APTT,INR and PT in the observation group were significantly lower than those in the control group.The differences were statistically significant(P <0.05 ).Conclusion Whether males or females,the different degrees of abnormal ex-pression of myocardial enzyme spectral indexes,blood lipid indexes,coagulation indexes,apolipoprotein,troponin,myoglobin and ho-mocysteine exist in the patients with CHD.Lipoprotein(a)and fibrinogen have high expression in the CHD patients,which can pro-vide the reliable scientific basis for diagnosis and treatment of the disease.
10.Application of three-dimensional high resolution anorectal manometry and biological feedback therapy in very elderly patients with functional constipation

Objective:To investigate the characteristics of three-dimensional high resolution anorectal manometry and the effect of biological feedback therapy on the improvement of clinical symptoms in very elderly patients with chronic functional constipation.Methods:A total of 68 cases with chronic functional constipation were divided into very elderly group(≥80 years old, n=36)and the elderly group(60-79 years old, n=32)in the retrospective analysis.Patients underwent the three-dimensional high resolution anorectal manometry before and after biological feedback therapy, and the related parameters and scores of constipation symptoms before and after treatment were compared between the two groups.Results:The results of 3D high-resolution anorectal manometry showed that the anus relaxation rate in the simulated defecation test was lower and the bowel threshold in the rectal sensory threshold test was higher in the very elderly group than in the elderly group(2.44±33.81% vs.16.34±16.99%, 103.44±42.01 ml vs.77.22±41.85 ml, t=-2.047 and 2.655, P=0.049 and 0.012). In the very elderly group, the post-biological feedback therapy versus pre-biological feedback therapy showed that anal residual pressure during simulated defecation was decreased, the absolute value of negative anorectal pressure difference was reduced and the anal relaxation rate was increased [57.50±18.88 mmHg(1 mmHg=0.133 kPa) vs.64.84±25.82 mmHg, -29.64±15.98 mmHg vs.-39.47±19.45 mmHg, 10.53±29.35% vs.2.44±33.81%, t=3.342, -4.902 and -3.209, P=0.002, 0.000 and 0.003]. The scores of clinical symptom scale showed that there was no significant difference in the effective rate between the very elderly and elderly groups(66.67% or 24/36 vs.71.88% or 23/32, χ2=0.760, P=0.860). Conclusions:The elderly functional constipation patients with defecation disorder often have rectal propulsive insufficiency and dyscoordination of pelvic floor muscle contraction.The main cause of defecation disorder in very elderly patients is the decrease of anal relaxation rate during simulated defecation.Biological feedback therapy can improve the symptoms of defecation disorder in very elderly patients by reducing the anal residual pressure during simulated defecation, increasing the anal relaxation rate and reducing the absolute value of negative anorectal pressure difference.
