1.Sexual psychology changes and analysis of its related factors for ovarian cancer patients after operation

Objective To evaluated the sexual psychology changes and marriage quality for ovarian cancer patients after operation and supply selective psychological nursing intervention for patients and their family members.Methods Self-designed questionnaire was used to investigated ovarian cancer patients under 60 years old one month after operation to understand the sexual psychology changes and its related factors.Nursing intervention was adopted according to different conditions of patients.Results Factors that influenced the sexual psychology of patients included:age,education degree,chemotherapy process,clinical symptom,anxiety and depression status,marital status and degree of nursing intervention.Conclusions Sexual psychology Was influenced by many factors.We should adopt various kinds of nursing intervention to promote the physical and mental health of patients,facihtate the recovery of sexual function and improve their life quality.
2.Survey and analysis of community healtfa service institutions across the country

A survey and analysis were conducted at the end of 2002 of urban community health service institutions in 31 provinces, autonomous regions and municipalities directly under the Central Government so as to find out about the basie situation of community health service institutions across the country. The result indicated that by the end of 2002, the number of cities that had conducted community health service work reached 358. The coverage of community health services in cities of the prefecture level, areas under the jurisdiction of a city and cities of the county level was respectively 92. 1%, 80. 3% and 28.0% . 12 132 community health service institutions were set up in 2002, marking an increase of 31.6% as compared with 2001. The growth of community health services was fast and yet uneven in various regions. It is suggested that more efforts be made to strengthen community health services in medium-sized and small cities and improve the management of institutions and the system for statistical analysis of in-formation.
3.Cloning of the prostate cancer related genes with mRNA differential display

Objective Cloning of some specific genes related to the prostate cancer. Methods Specimen from a patient with prostate cancer and from a normal adult were studied by the improved mRNA differential display and the differentially expressed sequence-tags were cloned,sequenced and analized.Reverse transcriptive-polymerase chain reaction(RT-PCR) was used to examine the expression level of Clusterin in prostate cancer tissues,prostate cancer cell line and normal prostate tissues. Results Significant difference was observed between the two kinds of tissues in gene expression and seven differentially expressed sequence-tags were found through analysis in GenBank.Among them,five were proved to be new gene tags,one shared 100% homology with Clusterin,which was highly expressed in the prostate cancer tissue and the other one shared 97% homology with superoxide dismutase 1 (SOD1),which was highly expressed in the normal prostate tissue.It was proved by RT-PCR that to compare with the internal marker gene ?-actin,the expression level of clusterin in prostate cancer is much higher than that of normal prostate. Conclusions Five new gene tags,clusterin and SOD1 were found to be differentially expressed between tissues of prostate cancer and normal prostate and may play important roles in carcinogenesis and development of prostate cancer.
4.E-cadherin expression in prostate cancer and its correlation with PSA

Objective To investigate the expression of E-cadherin in prostate cancer and its relationship with PSA. Methods E-cadherin expression of 56 prostate cancer samples were studied by immunohistochemical stain and its expression level was analyzed with respect to T-PSA, F-PSA and F/T ratio. Results 24 patients (43%) were normal and 32 patients (57%) were aberrant in E-cadherin expression. E-cadherin was related significantly to the grade and stage of cancer and the changes of F/T ratio.There were no significant relationship between E-cadherin expression and T-PSA or F-PSA. Conclusions E-cadherin expression may act as a marker to the malignant degree and the prognosis of the prostate cancer.
5.Risk factors of under staging and under grading of patients undergoing radical prostatectomy

Objective To determine the relevant factors of under staging and under grading in patients undergoing radical prostatectomy and to define important predictors of under staging in this population. Methods The clinical T stages and biopsy Gleason scores were compared with the pathological T stages and prostatectomy Gleason scores in 55 patients. Results Among the 55 cases,50 were preoperatively classified as clinical stages T 1~T 2,of whom 21(42%) were postoperatively classified as pathological stages T 3~T 4.Of the 26 cases who preoperatively had the Gleason scores of 2 to 6,11(42%) had postoperatively pathological grades of 7 to 10.Logistic regression analysis revealed that PSA( P =0.0159)at diagnosis and the percentage of positive biopsies of prostate ≥50%( P =0.0013) were significant predictors for preoperative clinical staging lower than postoperative pathological staging. Conclusions The results demonstrate that patients who were classified as clinical stages T 1~T 2 with PSA≥20 ng/ml and percentage of positive biopsies≥50% may have high possibility of under staging.
6.Changes in clinical features of inpatients with prostate cancer in the past 11 years

Objective:To investigate changes of the stage,age of onset,and prostate specific antigen(PSA) level of prostate cancer with socioeconomic development and medicare promotion.Methods:The medical records of 784 inpatients with prostate cancer were analyzed retrospectively,who were diagnosed in Peking University First Hospital from 1997 to 2007.According to the time of diagnosis,all the patients were sorted into three groups:earlier group(1997-2001),intermediate group(2002-2004),and contemporary group(2005-2007).The tumor stages,ages,Gleason scores,and PSA levels of each group and of the three groups were compared to determine whether the discrepancies were significant.Results:The discrepancies of ages,Gleason scores,and stages between earlier and intermediate groups were not significant.The differences of ages and stages between intermediate and contemporary groups were not significant,but the change of Gleason scores was significant.The discrepancies of Gleason scores and stages between earlier and contemporary groups were meaningful,but the change of ages was not significant.Conclusion:As time passes,PSA levels and tumor stages of prostate cancer patients show a descending trend,but the discrepancy of ages between the three groups is meaningless.The weight of low risk and intermediate risk groups in localized prostate cancer is becoming heavy.
7.Application of laparoscopic techniques in the diagnosis and treatment of abdominal emergency during pregnancy

Objective To assess the value of laparoscopy in the diagnosis and treatment of abdominal emergency during pregnancy.Methods Clinical data of 40 cases of abdominal emergency at 7~17 gestational weeks(mean,13.2 weeks) from June 1997 to October 2004 were retrospectively reviewed.There were 16 cases of uterine with ectopic pregnancy,3 cases of twisted ovary,5 cases of pedicel torsion of ovarian cyst,5 cases of acute cholecystitis,and 11 cases of acute appendicitis.The diagnosis and treatment were conducted under laparoscope. Results The confirmative diagnosis was clarified under laparoscope in all the cases.Except 1 case of conversion to open surgery,the operation was accomplished laparoscopically in all cases. The operation time was 25~70 min(mean,38.2 min).Surgical complications occurred in 1 case.During postoperative follow-up observations,there were 3 cases of threatened abortion,3 cases of inevitable abortion,and 1 case of premature delivery. Conclusions Laparoscopic operation is safe and effective for acute abdominal emergency during early-to-middle pregnancy.
8.Comparative Study for Risk Factors and Features in Patients With Premature Coronary Artery Disease and Mature Coronary Artery Disease

To explore the risk factors and features in patients with premature coronary artery disease (CAD) and mature CAD. Methods: General and clinical information was collected from 747 patients who received coronary angiography (CAG) in our hospital from 2015-01 to 2016-02 and the patients were divided into 3 groups based on CAG findings: Premature CAD group, n=138, Mature CAD group, n=364 and non-CAD group, n=245. CAD diagnosis was defined by at least one major coronary artery stenosis ≥50%; premature CAD was defined by the onset age in male<55 years, in female<65 years. Risk factors and features were compared between premature CAD and mature CAD patients. Results: Compared with Mature CAD group, Premature CAD group showed the higher incidences of family history of hypertension, abnormal blood glucose and BMI, increased blood levels of TC, TG, LDL-C, elevated apolipoprotein A and B (APOA and APOB); while the lower ratio for combining hypertension, P<0.05. Multinomial Logistic regression analysis presented the following parameters had the increased risk for premature CAD occurrence: combining hypertension and diabetes (OR=2.98, 95% CI 1.04-8.57), family history of hypertension (OR=3.50, 95% CI 1.28-9.57), abnormal blood glucose (OR=1.98, 95% CI 1.04-3.80) and elevated APOB (OR=36.67, 95% CI 3.51-99.83). The following parameters had the increased risk for mature CAD occurrence: advanced age (OR=1.20, 95% CI 1.15-1.24), male gender (OR=6.22, 95% CI 3.31-11.69), combining hypertension (OR=1.75, 95% CI 1.08-2.82), concomitant hypertension and diabetes (OR=3.25, 95% CI 1.42-7.46) and elevated APOB (OR=16.39, 95% CI 1.74-99.44). Compared with Mature CAD group, Premature CAD group presented the higher ratio of double vessel disease (38.4% vs 22.3%) and lower ratio of multi vessel disease (31.2% vs 48.1%), all P<0.05. Conclusion: The incidences as family history of hypertension, abnormal blood glucose and BMI, elevated APOB were higher in premature CAD patients than mature CAD patients. It is important to conduct targeted prevention to control relevant risk factors.
9.Introspection on the implementation of national essential drug system in three public community hospitals in Beijing

The national essential drug system is designed to solve the problem of covering hospital expenses with medicine revenue, and the problem of difficulty and high expense to see a doctor. This paper investigated whether the national essential drug system worked in three public community hospitals in Beijing. The result showed that the implementation of the program was now well underway, but left some problems. This paper put forward some suggestion for bidding of essential drug, improving the enthusiasm of the medical staff and improving the function of community hospitals.
10.Expression of dihydrodiol dehydrogenase 2 in prostate cancer and its significance

Objective To study the expression of dihydrodiol dehydrogenase (DD) in prostate cancer and its significance. Methods With reference to expression of ?-actin gene,the expression level of a human dihydrodiol dehydrogenase isoform (DD2) mRNA was examined in prostate cancer tissues (11 cases) and normal prostate tissues (10 cases) by reverse transcriptive-polymerase chain reaction(RT-PCR). Quantitative determination of relevant band densities was performed using densitometry-scanning techniques. Results Strong expression of DD2 mRNA was detected in prostate cancer tissures with absorbance in the range of 0.550 to 1.018 (median,0.726),and low expression of DD2 mRNA in normal prostate tissues with absorbance in the range of 0.248 to 0.420 (median,0.333).
