Competition of teaching skills is regarded as an effective means to develop teacher's ability in teaching.This article briefly described a teaching competition at a medical school from participant identification,requirement clarification,training before competition,evaluation index screening and interviewer selection.Meanwhile the article further analyzed and analyzed the problems arising from the competition and proposed measures to promote teachers' awareness and ability of teaching.

3.Clinical study of reversed lamellar keratoplasty

Objective To evaluate the therapeutic efficiency of reversed lamellar keratoplasty for bullous keratopathy, Terrien's marginal degeneration and ocular chemical burns.Methods Twenty eyes of patients with bullous keratopathy, twelve eyes with Terrien's marginal degeneration, ten eyes with chemical burns were recruited in this study.Diseased tissues were removed by means of dissection.Reversed lamellae corneal tissues which had been prepared by cryopreservation were thawed and rinsed. Then the prepared posterior corneal lamellae was turned over, endothelium facing upwards. The diameter of the graft should be 0.5~1.0mm larger than that of the recipient bed. The reversed donor posterior lamellae was sutured into the host keratectomy bed with 10-0 nylon sutures continuously or interruptedly.Results Allograft reaction episode occurred only one (chemical burn) of the total 42 eyes enrolled.The grafts of the rest 41 eyes maintained clear or semitransparent for 1~5 years. Graft edema could be observed within 2~4 weeks postoperatively. Postoperative interface separation was not found. For all 20 patients with bullous keratopathy, the symptoms of pain disappeared. The visual acuity improved 2~3 lines of Snellen among eleven of twelve patients with Terrien's marginal degeneration. The visual acuity improved 2~3 lines of Snellen with four patients with chemical burns.Conclusion The technique of reversed lamellar keratoplasty can be applied successfully for tectonic reconstruction of corneal surface such as bullous keratopathy, Terrien' s marginal degeneration and also chemical burns.The effects of therapy was satisfactory.
4. The treatment of 23 patients with macrostomia by modified modiolus reconstruction

Objective:
To explore a method for macrostomia repairment, in order to to achieve better physiological and aesthetic outcomes.
Methods:
23 patients with macrostomia were treated by standard procedure: including orbicularis oris reconstruction, perioral expression muscles fixation, modiolus reconstruction, commissure point shift, and " w" shape incision.
Results:
22 cases obtained both static and dynamic symmetrical outcomes. One case developed asymmetrically.
Conclusions
The macrostomia repairment following present standard procedure, is likely to achieve both static and dynamic satisfactory outcomes, due to balanced perioral muscles.
5.Unilateral congenital microphthalmia in newborn: a case report

Here,we reported a case of term infant with unilateral congenital microphthalmia (CM).Physical examination conducted on 2 d after birth showed that the left eye was barely open and the eye socket was deeper than the right.Meanwhile,the cornea of the left eye was not completely exposed and the light reflex could not be elicited.Ophthalmology consultation,ultrasound and CT scan were conducted,and CM was finally diagnosed since the infant was found to have a small eyeball and shortened axial length,accompanying by pathological changes in the lens,vitreous body and retina.This case suggested that detailed physical examination should be carried out,especially for the eyes,including orbit and eyelid,presence or absence of secretion or concealed eyeball,together with medical imaging techniques technology,to ensure an early detection and diagnosis of CM in neonates.
6.Mediating effect of rehabilitation self-efficacy on perceived social support and rehabilitation exercise adherence in hospitalized patients after hip/knee arthroplasty

Objective·To explore the correlation among perceived social support,rehabilitation self-efficacy and rehabilitation exercise adherence of in-patients after hip/knee arthroplasty,and examine the mediating role of rehabilitation self-efficacy in the correlation between perceived social support and rehabilitation exercise adherence.Methods·A convenience sampling method was used,and 141 post-total hip/knee arthroplasty inpatients in the bone and joint ward of the First Affiliated Hospital of Naval Medical University(Shanghai Changhai Hospital)from October 2021 to February 2023 were selected as the study subjects.The general demographic data,such as age and gender,and the information of disease and operation were assessed by using General Data Questionnaire.The adherence to rehabilitation exercise in patients after hip/knee arthroplasty was assessed by using the Rehabilitation Exercise Adherence Scale(REAS);the self-efficacy of rehabilitation exercise was assessed by using the Self-Efficacy for Rehabilitation Outcome Scale(SER);the social support level of individual subjective feelings of the patients was assessed by using the Perceived Social Support Scale(PSSS).Pearson's correlation analysis was used to test the correlation between the variables,and Bootstrap method was used to investigate the mediating role of rehabilitation self-efficacy in the correlation between perceived social support and rehabilitation exercise adherence.Results·A total of 167 questionnaires were distributed and 141 valid questionnaires were recovered,with a valid recovery rate of 84.4%.The age range of the 141 patients after hip/knee arthroplasty was 26?84 years old,and the average age was(64.75±10.74)years old.The total score of SER of the patients after hip/knee arthroplasty was(100.45±21.71),the total score of PSSS was(68.29±10.89),and the total score of REAS was(11.93±2.29).Pearson's correlation analysis results showed that there was a significant correlation between SER score and PSSS score,SER score and REAS score,and PSSS score and REAS score in patients after hip/knee arthroplasty.The mediation effect analysis showed that the indirect effect of perceived social support on rehabilitation exercise adherence was significant(effect value 0.033,95%CI 0.017?0.058,P<0.05),and the direct effect of perceived social support on rehabilitation exercise adherence was not significant(effect value 0.027,95%CI-0.008 ? 0.065,P>0.05).Conclusion·Perceived social support positively predicts rehabilitation exercise adherence,and rehabilitation self-efficacy fully mediates the correlation.Direct intervention in rehabilitation self-efficacy is superior to moderating the level of perceived social support.
7.One case of subcutaneous human sparganosis disease of thigh misdiagnosed as lipoma

Human sparganosis is a rare parasitic disease. Pleroceroid can infiltrate brain, mammary gland and other organs, with a high misdiagnosis rate. A case of subcutaneous sparganosis disease of thigh was diagnosed and treated in the Burn, Plastic Surgery and wound Repair Department of Longyan First Hospital Affiliated of Fujian Medical University in January 2022. The patient was followed up for lipoma in other hospitals for many times with no clinical treatment. Combined with the medical history and characteristic of the mass, we also treated the mass as "lipoma" and surgical resection was performed in author’s hospital. Live worms were found intraoperatively, and finally confirmed as pleroceroid by pathology and expert consultation. No local recurrence was found in 6 months of follow-up. In order to reduce the misdiagnosis rate of the disease, the diagnosis and treatment methods were reviewed and discussed. This case may provide more clinical experience for the prevention and treatment of the disease.
8.Effects of Different Habitual Foot Strike Patterns During Running on Achilles Tendon Morphology and Mechanical Loading

Objective To investigate the effects of different foot strike patterns during running on Achilles tendon(AT)morphology and mechanical loading.Methods Fourteen habitual rearfoot strike runners and 14 habitual forefoot strike runners were recruited.Morphological characteristics(tendon length,cross-sectional area,and thickness)of the AT were collected using ultrasound imaging.The AT loading characteristics(plantar flexion moment,tendon force,load rate,impulse,and stress)of subjects wearing cushioned running shoes while running at a speed of 10 km/h were collected and calculated using a three-dimensional force measurement treadmill.Results Compared to habitual rearfoot strike runners,habitual forefoot strike runners showed a significant increase in peak plantar flexion moment of ankle joint,AT peak force,average loading rate,and peak loading rate(P<0.05).However,the differences in AT length,cross-sectional area,and thickness between the two groups were not statistically significant(P>0.05).Conclusions Long-term forefoot strike patterns can adaptively enhance the mechanical loading characteristics of the AT during repetitive stretch-shortening cycles.

10.Application of actively vascular catheters management via a professional team with clinical nurse ;specialist as the core among patients receiving chemo-radiotherapy

Objective To explore the effect of actively vascular catheters management via a professional team with clinical nurse specialist as the core among patients receiving chemo-radiotherapy. Methods Totally 762 tumor patients with chemo-radiotherapy in Sihucan cancer hospital were selected as the research object by convenient sampling method. 300 patients from January to December 2014 was taken as the control group ( patients received routine management ) and 462 patients from January to December 2015 was taken as the experimental group (patients received professional management with clinical nurse specialist as the core). The incidences of complications and indwelling time in two groups were compared and analyzed by independent-samples T test and chi-square test.Results There were 11 cases of thrombus, 13 cases of tube plugging and 16 cases of phlebitis in the control group, while there were 5 cases of thrombus, 3 cases of tube plugging and 8 cases of phlebitis in the experimental group. The incidences of thrombus, tube plugging and phlebitis in the experimental group were obviously lower than those in the control group (χ2=4.676,12.009,8.876;P<0.05). There were 8 cases of unplanned extubations in the control group, but none in the experimental group. The indwelling time of central venous catheter (CVC) and peripherally inserted central catheter (PICC) were (21.2± 11.5) d and (55.8±26.3) d in the control group, while they were (25.6±13.1)d and (63.8±22.1)d in the experimental group (t=-2.57,-2.97;P<0.01).Conclusions The actively vascular catheters management via a professional team with clinical nurse specialist as the core among patients receiving chemo-radiotherapy, is likely to reduce some complications after intravenous indwelling catheter, and avoid unplanned extubation and extend the indwelling time.
