Objective To investigate the role of central and peripheral sensitization in remifentanil-induced hyperalgesia in a rat model of inflammatory pain. Methods Twenty-one male SD rats weighing 200-300 g were used in this study. Inflammatory pain was induced by intraplantar injection of 1 % carrageenan 100 fd in the left hindpaw in all animals. The animals were then randomly divided into 3 groups ( n = 7 each): control group (group C) and two remifentanil groups (group R1 , R2) . In R1 and R2 groups remifentanil was infused iv at a rate of 10 and 30 μg-kg-1·min-2 respectively starting from 5 min before till 25 min after carrageenan injection, while in group C normal saline was infused iv instead of remifentanil. Bilateral paw withdrawal threshold to mechanical stimulation with von Frey filament (PWT) was measured before (baseline) and at 1 h, 3 h and 1-7 d after carrageenan injection. Bilateral paw withdrawal latency to noxious thermal stimuli (PWL) was measured before and at 2 h, 4 h and 1-7 d after carrageenan injection. The thickness of the plantar surface of left hindpaw was measured before and at 1 h, 4 h and 1-7 d after carrageenan injection. Results Bilateral PWT was significantly lower at day 1 after carrageenan injection in R, and R2 groups than in group C. The right PWT was significantly lower at 2 d and 4-7 d after carrageenan injection in group R2 than in group R, . There was no significant difference in PWL and thickness of the plantar surface of left hindpaw among the 3 groups. Conclusion Central sensitization is involved in developing and maintaining the remifentanil-induced hyperalgesia in a rat model of inflammatory pain, while peripheral sensitization is not.

Objective:To explore the role and mechanism of 4-1BBL and B7-1 molecules in the activation and proliferation of human peripheral blood T lymphocyte. Methods:T lymphocyte was purified by magnetic beads negative selecting. The proliferation of T cells in primary allogenic MLR was evaluated by using 3H-TdR incorporation; The immunophenotype of T cells was analyzed by FACS and the quantitative measurement of IL-2 in culture supernatant were performed by ELISA.Results:After negative selection, the purity of CD3+ T cells was over 90%; 4-1BBL and B7-1 molecules stably expressed on XG cells after 4-1BBL and B7-1 cDNA transfection; 4-1BBL or B7-1 gene transfected XG cells could more effectively mediate the activation, proliferation and IL-2 secretion of T cells than XG cells did, and the results also showed that the costimulatory molecules had a co-activating effect on T cells. Conclusion:Human multiple myeloma cells failed to induced antitumor immunoresponse because of the lack or weak expression of costimulatory molecules. The costimulatory molecules of 4-1BBL and B7-1 has a synthetic effect on endowing myeloma cells with enhancing the activation、proliferation and IL-2 secretion of T cells in vitro. [

Objective To study the clinical effect of orthopedic-orthodontic treatment in crossbite in deciduous dentition through cephalmetric analysis. Methods 12 children with crossbite in deciduous dentition were enrolled in treatment group. Control group included 8 patients with normal deciduous dentition. Modified reverse headgear was used in each patient in the treatment group. Orthodontic force was a-bout 2. 94-4. 90 N (300-500 g) , and the direction of force was under occlusion plane for 37 degree. Each patient wore reverse headgear for 10 hour per day. Orthodontic treatment lasted for 6 months, and then orthodontic treatment began at once. Orthodontic treatment was not over until crossbite was corrected. The cephalmetric change was compared between two groups before treatment (T1) , after orthopedic treatment (T2)and after orthodontic treatment (T3). The effect of orthopedic-orthodontic treatment was analyzed in two group. Results Crossbite was corrected successfully in all patients in the treatment group. After treatment, A point moved forward by 2. 8mm, Is-FHp increased 8. 38 mm, <SNA increased 2. 65, incisors in mandible moved backward a little bit. All these changes were statistical different as compared with those in the control group. Conclusion Skeletal and dental deformity are improved progressively after combined orthopedic-orthodontic treatment. It is recommended to perform orthopedic-orthodontic therapy in skeletal crossbite in deciduous dentition.

OBJECTIVE:To systematically evaluate the efficacy and safety of glutathione (GSH) in preventing oxaliplatin-in-duced peripheral neurotoxicity(OIPN),and to provide evidence-based reference in the clinic. METHODS:Randomized controlled trials (RCTs) about therapeutic efficacy of GSH vs. placebo/no any measures (called placbo group) or other drug in preventing OIPN were retrieved from PubMed,EMBase,Cochrane library,CJFD,Wanfang database and VIP. Meta-analysis was performed with Rev Man 5.3 statistical software after data extraction and quality evaluation with Jadad scale. RESULTS:18 RCTs were in-cluded,involving 1200 patients. The results of Meta-analysis showed that:total incidence of oxaliplatin-induced chronic peripheral neurotoxicity (OICPN)[RR=0.71,95%CI(0.59,0.87),P<0.001] and the incidence of severe OICPN [RR=0.50,95%CI(0.42, 0.60),P<0.001] in GSH group were significantly lower than placebo group, with statistical significance;there was statistical signif-icance in the incidence of oxaliplatin-induced acute peripheral neurotoxicity(OIAPN)[RR=0.89,95%CI(0.72,1.09),P=0.25]. The incidence of severe OICPN in GSH group was significantly higher than mecobalamine group, with statistical significance [RR=2.06,95%CI(1.07,3.99),P=0.03]. There was no statistical significance in the incidence of OICPN[RR=1.38,95%CI(0.83,2.31), P=0.21] and severe OICPN [RR=1.91,95%CI(0.85,4.30),P=0.12] between GSH group and Ca+Mg mixture group. CONCLU-SIONS:GSH can effectively prevent the occurrence of OICPN,however, its therapeutic efficacy is equivalent to Ca+Mg mixture and inferior to mecobalamine in preventing severe OICPN.

Objective We evaluate if supplementary grading system can refine patient selection and accurately predict neurological outcome in BAVM. Methods We retrospectively study 221 BAVM patients who were treated micro?surgically by our hospital. The score of pre and post operation mRS and relative clinical, radiology data were collected. Two different logistic models (Spetzler-Martin, Supplementary Spetzler-Martin grading model) were constructed to com?pare the area under ROC. Results Some factors are significant different between worse outcome patients and good out?come patients:Non-hemorrhagic presentations prior surgery, AVM bigger than 3cm, diffuse shape of AVM and the elder patients. Predictive accuracy was higher for the supplementary model (ROC area, 0.91), than the Spetzler-Martin model (ROC area, 0.774). So the predictive accuracy of supplementary model was significantly better than that of the Spet?zler-Martin model (P=0.0362). Conclusions Supplementary Spetzler-Martin model can improve preoperative risk pre?diction and subgroup the patients more efficiently. When the score less than 5(including 5) in supplementary Spet?zler-Martin patients seem to have lower risk relative to surgery.
