1.Efficacy comparison between saccharomyces boulardii sachets and tetragenous viable bifidobacterium tablets in the treatment of rotaviral gastroenteritis

Objective To compare the effect between Saccharomyces boulardii sachets and tetragenous viable bifidobacterium tablets in the treatment of rotaviral gastroenteritis of children .Methods A total of 191 patients (below 2 months to 8 years old) with rotaviral gastroenteritis admitted to this hospital from January 2012 to May 2014 were divided randomly into control groups (smectite ,conventional treatment ,number=62) ,tetragenous viable bifidobacterium tablets group (conventional treatment combined with tetragenous viable bifidobacterium tablets ,number= 64) ,saccharomyces boulardii group (conventional treatment combined with saccharomyces boulardii ,number=65) .Therapeutic effects were compared between 3 groups after 3 days and 5 days of treat‐ment .Results The obvious curative rate ,effective ratio and total rate in the groups of saccharomyces boulardii and tetragenous via‐ble bifidobacterium tablets were higher than those in the control group(P<0 .05) .However ,no significant difference was found in the two cases among the groups (P>0 .05) .There was significant difference in 3 days and 5 days therapeutic effect among 3 groups (P<0 .05) .The duration of diarrhea in the group of Saccharomyces boulardii was lower than this in the group of tetragenous viable bifidobacterium tablets (P<0 .05) .Conclusion The therapeutic effect of Saccharomyces boulardii is similar to that of tetragenous viable bifidobacterium tablets in treatment of children with Rotaviral gastroenteritis .The duration of diarrhea in the group of Sac‐charomyces boulardii was lower than that of the group of tetragenous viable bifidobacterium tablets .
2.A Study on Individual Health Risk Appraisal and Risk Factors for Stroke in Communities in Chongqing

Objective To investigate the related risk factors for stroke among residents aged over 26 years old in communities in Chongqing,to set up a quantitative criteria for determining risk for stroke,and to evaluate and predict the risk of the stroke for individuals.Methods 1 034 cases with 1:2 matched controls were chosen from five communities in Chongqing,which were interviewed with a uniform questionnaire.The risk factors of stroke were analyzed with Logistic regression,collected the odds ratios of risk factors for stroke,and different levels of risk exposure factors be converted into a risk scores by using statistical models.The related risk scores were added to obtain a combined risk score to predict the individual's risk of stoke in the future.Results An individual health risk appraisal model of stroke applicable to individuals of different sex,age,health behavior,disease and family history was established.For men,11 risk factors,including hypertension(5.728),coronary heart disease(5.404),family history of stroke(4.599),and so on,were entered the main effect model;for women,hypertension(5.270),family history of stroke(4.866),hyperlipidemia(4.346),and other eight risk factors entered the main effect model.Conclusion Personal diseases,including hypertension,CHD and hyperlipidemia,etc.,are very important risk factors for stoke.The stroke prevalence can be effectively reduced by changing the unhealthy lifestyle and curing the related individual diseases.The evaluation model of the risk score for stroke based on the main risk factors could be useful in the future health education and community health service on stroke prevention.

4.Inhibitory effect of specific inhibitor of p38 mitogen activated protein kinase on hyper secretion of airway mucus in asthma

Objective To explore the inhibitory effects of SB203580, a specific inhibitor of p38 mitogen activated protein kinase (p38 MAPK) on hyper secretion of airway mucus in mice with asthma. Methods Forty BABL/C mice were randomly allocated into asthma group, SB203580 treatment group, dexamethasone treatment group and control group (sham-challenged group). Mice in SB203580 treatment group were intraperitoneally injected with SB203580 (10mg/kg), while those in dexamethasone treatment group were injected with dexamethasone (1mg/kg) before provocation. The number of goblet cells in small bronchi of all groups was determined after Alcian blue-periodic acid Schiff (AB-PAS) staining. The contents of MUC5AC in bronchoalveolar lavarge fluid (BALF) were evaluated with ELISA asssy. The levels of MUC5AC mRNA expression were determined with reverse transcription-polymerase chain reaction (RT-PCR) assay, and gray-scale analysis was done with image analysis method. 5.The use of a self-made airway occluder in bronchoscopic lung volume reduction

Objective To explore the therapeutic efficacy of a self-made occluder used for bronchoscopic lung volume reduction(BLVR).Methods Two patients with severe chronic obstructive pulmonary disease(COPD)in stable phase were selected for the present study.One or two nitinol occluders were placed,guided by brochoscopy,into the target bronchi of the two patients.Examination of pulmonary functions,chest computed tomography,complications,blood gas analysis,and evaluation of life quality with St George's respiratory questionnaire(SGRQ)were carried out before and 7 months after the bronchoscopic lung volume reduction.Results No dislodging or shifting of all the inserted occluders occurred.Only slight proliferation of granulation tissue was found in mucous membrane around the occluder,and atelectasis was observed at the target sites of lung.Before and 7 months after BLVR,ventilatory capacity(VC)of case 1 rose from 3.08L to 3.57L,and that of case 2 was elevated from 2.19L to 2.58L.Forced vital capacity(FVC)of case 1 was elevated from 2.87L to 3.57L,and that of case 2 from 2.60L to 2.58L.SGRQ showed a remarkable improvement of patients' dyspnea with a decrease of scores from 62 to 26 in case 1 and from 48 to 23 in case 2.No evident changes were found on arterial oxygen blood partial pressure(PaO2)in both patients,and no severe complications such as obstructive pneumonia or pneumothorax occurred yet in either case 1 or case 2.Conclusion The present study has demonstrated that the self-made occluder is convenient for insertion with satisfactory stability and therapeutic efficacy.Application of such occluder is safe with few complications,and may serve the purpose of reducing lung volume and improving lung function.
6.Mutation of HPV 18 E6 inhibits the growth of Hela cells and induces apoptosis

Objective To construct pcDNA 3.1(+)/HPV 18 E 6 fusion gene and a single-codon mutation pcDNA 3.1(+)/E6 F49R or pcDNA 3.1(+)/E6 F127R fusion gene in eukaryotic expression vector and study the effects on proliferation and apoptosis of cervical carcinoma cell line Hela. Method HPV 18 E6 gene sequence and the single-point mutation HPV 18 E6 F49R or HPV 18 E6 F127R were amplified from total RNA of Hela cell line by reverse transcription- polymerase chain reaction ( RT- PCR), then the gene sequences were respectively inserted into pcDNA 3.1(+) vector to reconstruct recombinant plasmids which were transfected transiently into Hela cells. MTT and RT-PCR were used to test the expression levels of HPV 18 E6 and the growth of HeLa cells after transfected about 48 h. The proliferation and apoptosis of Hela cells were detected respectively by cell counting and AO/EB fluorescent vital staining. Results The pcDNA 3.1(+)/HPV 18 E6, pcDNA 3.1(+)/E6 F49R and pcDNA 3.1(+)/E6 F127R eukaryotic expression vectors were successfully constructed. The gene of HPV 18 E6 was discriminably detected in the HeLa cells which were transfected with the recombinant plasmids. After several days, the proliferation of Hela cells transfected with pcDNA 3.1(+)/E6 F49R or pcDNA 3.1(+)/E6 F127R plasmid were obviously inhibited and the apoptotic rates were significantly increased, then the proliferation of cells transfected with pcDNA(+)/HPV 18 E6 was rather increased slightly, and we could observe the phenomena of early apoptosis and the formation of thekaryopyknosis by fluorescent microscope in the cells transfected with pcDNA 3.1(+)/E6 F49R or pcDNA 3.1(+)/E6 F127R. Conclusion The eukaryotic expressing vectors encoding HPV 18 E6 F49R and HPV 18 E6 F127R provide fundamental basis for the further study on HPV 18 E6 mechanism as well as prevention and treatment of uterine cancer.
7.Reasons for perioperative death in treatment of acute myocardial infarction by emergent percutaneous coronary intervention

ObjectiveTo analyze the dead reasons in treatment of acute myocardial infarction (AMI) by emergent percutaneous coronary intervention (EPCI) during perioperation. Methods The clinical characteristics of the dead patients,results of EPCI and reasons of death were retrospectively analyzed in 473 patients with AMI who received EPCI treatment during March 2004 to March 2011. ResultsAmong the 18 cases(3.8%)dead patients,there were 8 case with three-vessel lesions,5 cases with two-vessel lesions, 2 cases with single vessel lesion and 3 cases with left main lesion. Nine patients were accompanied with hypertensions,5 cases with diabetes mellitus, 1 case with old myocardial infarction, 2 cases with old cerebral infarction, 1 case with chronic obstructive pulmonary disease complicated by severe pneumonia, and 1 case with chronic renal insufficiency. Nine patients died of cardiogenic shocks , 4 cases (22. 2%) of heart ruptures, 2 cases (11.1%) of noreflow,1 case (5.6%) of massive hemorrhage of the upper alimentary tract, 1 case (5.6%) of respiratory failure and 1 cases (5.6%) of refractory ventricular fibrillation.ConclusionsPrimary reasons for perioperative death were cardiac shock and heart rupture in treatment of acute myocardial infarction by EPCI. Multi-vessel lesion, complications with hypertensions and diabetes mellitus may serve as succumbed factors of death.
8.Observation on curative effect of pulse-radiofrequency combined with epiduralspace block in cervicogenic headache

Objective To observe the curative effect of cervicogenic headache(CEH)treatment through the combination of pulse radio frequency(PRF)on the C2 dorsal rootganglion and continuous epidural space block.Methods Sixty patients with CEH in our hospital were randomly divided into groups A and B,30 cases in each group.The group A was treated with combination of PRF on cervical dorsal root ganglion and continuous epidural space block.The group B was treated with PRF on cervical dorsal root ganglion method.The pain VAS scores before treatment and at 1 week,3,6 months after treatment were compared between the two groups.Results Compared with pretreatment,the VSA scores at 1 week,3,6 wonths after treatment in the two groups had statistical difference(P<0.05),moreover,the VAS score decrease in the group A was better than the group B.All the patients had no nerve and artery injury or infection complications.Conclusion It is safe and effective to treat cervical CEH through the combination of ganglion PRF on the cervical 2 dorsal root and continuous epidural space block.
9.Investigation on long-term complications of glucocorticoid in treatment of Diamond-Blackfan anemia

Objective To collect and summarize long-term complications of glucocorticoid (GC) in treatment of Diamond-Blackfan anemia (DBA).Methods A total of 17 DBA patients,treated with GC more than 1 year from December 2009 to November 2012 in Being Children's Hospital,Capital Medical University,were retrospectively investigated.The data of general information,drug treatment,treatment response,height and body mass in different therapy periods,and therapy related adverse reaction were collected.The data entry and the statistical analysis were performed using SPSS 16.0 software.Results Seventeen cases which fulfilled the research criteria were enrolled.The 58.8 percent of cases (10/17) began GC therapy from the age younger than 6 months.The 76.4 percent of the cases (14/17) started prednisone therapy with the dosage ≥2 mg/(kg · d),and the median time of maintenance therapy with this dosage was 2 months (1-5 months).The median time of prednisone dosage greater than 0.5 mg/(kg · d) was 6 months (3-48 months).Patients were divided into 2 groups at the beginning of therapy according to whether their age was younger or older than 6 months.The median height of younger age group was-1.0 SD (-3.5-1.0 SD) of corresponded age-sex-standard height at the beginning of prednisone therapy,and was dropped to-3.5 SD (-3.5--2.0SD)afterhalfyeartreatment.For older age group,it was0.0 SD(-1.5-2.0 SD)and-0.5 SD (-1.5-0.5 SD) respectively.During the therapy,there were 1 fracture,2 measles pneumonia,3 pneumonia,3 hirsuitisms,5 thrushes,and 12 central obesity cases.Conclusions GC related adverse reaction might appear when applying prednisone for the treatment of DBA in long term.It was suggested that GC therapy should start after 6 months old if possible,and the duration of 0.5 mg/(kg · d) GC treatment should be reduced as short as possible.
10.Effect of Acupuncture at Acupoint Neiguan (PC 6) on the Myocardial c-fos Gene Expression of Acute Myocardial Ischemia in Rats

Objective: To investigate the mechanism of improvement effect of electro-acupuncture at acupoint Neiguan (PC 6) on acute myocardial ischemia (AMI). Methods: Ninety-six rats were randomly divided into sham-operation group, myocardial ischemia model group and myocardial ischemia model plus electro-acupuncture group, the rat model of AMI was established by occlusion of the anterior descending branch of the left coronary artery, then electro-acupuncture therapy was performed at bilateral acupoint Neiguan (PC 6). The myocardial enzyme activities in plasma was detected with biochemical method, and the myocardial c-fos gene expression was detected with reverse transcription polymerase chain reaction(RT-PCR) method. Results: The myocardial enzyme activities in plasma and the myocardial c-fos gene expression in myocardial ischemia model group were obviously higher than those in sham group(P< 0.05), after electro-acupuncture treatment, the myocardial enzyme activities in plasma and the myocardial c-fos gene expression were decreased,there was a significant difference (P< 0.05). Conclusion: The mechanism of improvement effect of electro-acupuncture at acupoint Neiguan (PC 6) on AMI is related to down-regulation of the myocardial c-fos mRNA expression.
