1.Clinical analysis of 12 patients with appendiceal carcinoid

Objective To explore the experience of surgical treatment for appendiceal carcinoid.MethodsThe clinical data of 12 appendiceal carcinoid cases received surgery were retrospectively studied.ResultsPreoperatively,all the 12 cases were considered as appendicitis.The definite diagnosis was verified by pathologic study postoperatively.Among the 12 cases, tumors less than 1 cm 9 cases, between 1 cm to 2cm 2cases, but without serosal involvment or lymph node metastasis(2 cases) recieved simple appendectomy.One case with a tumor larger than 2cm and serosal involvment underwent fight hemicolectomy.ConclusionOwing to its lack of specific symptoms,the diagnosis of appendiceal carcinoid was usually reached after operation.The choice of surgical procedures should based on the size and depth of invasion of the tumor and lymph node metastasis status.
2.Effect of different surgical approaches on postoperative quality of life and survival of patients with Ⅱ, Ⅲ type adenocarcinoma of the esophagogastric junction

Objective To analyze the postoperative quality of life index and survival of Ⅱ,Ⅲ type adenocarcinoma of the esophagogastric junction (AEG) patients who were implemented radical surgery through the transthoracic approach or abdominal transhiatal approach.To explore the reasonable operative approach of Ⅱ,Ⅲ type AEG.Methods 139 cases of patients with Ⅱ,Ⅲ type AEG were prospectively enrolled into the group in Shanxi Cancer Hospital during March 2012 to September 2012.64 cases of them were in transthoracic approach (TT) group,and 75 cases in abdominal transhiatal approach (TH) group.The operative time,the residual incision margin cancer rate,the average number of lymph node dissection,postoperative hospital stay,the average number of days required for gastrointestinal function recovery,the incidence of cardiopulmonary complications,anastomotic leakage rate,postoperative bleeding rate,perioperative mortality,postoperative respiratory function decline rate within 1 year,the average weight loss after 1 year,postoperative reflux rate within 1 year,recurrence and metastasis rate within 1 year,and 1-year survival rate of two groups were respectively compared.Results Statistical analysis was done on the perioperative data of 139 patients,and regular follow-up was carried out for these 139 patients after they discharged from hospital,of which 26 cases were lost,the lost rate was 18.7 %,so the followed-up cases were 113 cases.Postoperative hospital stay in transthoracic approach group was longer than abdominal transhiatal approach group with significant difference,(20.2±8.9) d vs (17.1±6.4) d (P < 0.05),the average number of lymph node dissection in transthoracic approach group (15.2±7.5) was less than abdominal transhiatal approach group (23.0±13.0) with significant difference (P < 0.05).No significant difference were observed in the operative time,the residual incision margin cancer rate,the average number of days required for gastrointestinal function recovery,the incidence of cardiopulmonary complications,anastomotic leakage rate,postoperative bleeding rate,perioperative mortality,postoperative respiratory function decline rate within 1 year,the average weight loss after 1 year,postoperative reflux rate within 1 year,recurrence and metastasis rate within 1 year,and 1-year survival rate of the two operation approaches (all P > 0.05).Conclusions Abdominal approach,through the esophageal hiatus radical surgery is a preferred operative approach of Ⅱ,Ⅲ type AEG.The average number of lymph node dissection in abdominal transhiatal approach group was more than transthoracic approach group.It may have some influence on the prognosis.
3.Astragulus & Angelica mixture enhances nitric oxide production and reduces angiotensin II in the kidney of UUO rats

Objective To investigate the therapeutic effects of Astragalus & Angelica (A&A) , the mixture of Chinese herbs, on different vasoconstrictors and vasodilators in the process of renal interstitial fibrosis in rats with unilateral ureteral obstruction (UUO). Methods Wistar rats were randomly divided into Sham,UUO and UAA (UUO+A&A) groups. Objective To investigate the influence of a combination of Chinese herbs,Astragalus membranaceus var.mongholicus and A ngelica sinensis (A&A)on the production and elimination of reactive oxygen species (ROS)and the underlying mechanism during the process of renal interstitial fibrosis in the obstructive kidneys of rats with unilateral ureteral obstruction(uuo).Method Male Wistar rats were randomly divided into control,sham,UUO and UAA (UUO+A&A)groups.The rats in UAA group were administered with A&A(14 g/kg)by oral gavage once daily:the ones in sham and UUO group were given with equal volumes of water.Three days after setting up models,pathological injury of renal tissue was evaluated.Level of total antioxidant capacity(T-AOC)and activity of CuZn superoxide dismutase(CuZn-SOD)in renal homogenates were measured bv spectrophotometry.Expression of NADPH oxidase subunits p47-phox,p22-phox and nintrotyrosine was analyzed by Western blot. Results Severe interstitial inflammatory cells infiltration, mild tubular atrophy and interstitial fibrosis were found in UUO group, which were alleviated by A&A administration. Compared to sham group, T-AOC of UUO group was not significantly changed, but the expression of the nitrotyrosine, NADPH oxidase subunits p47-phox and p22-phox was increased significantly (P<0.05). After A&A administration, T-AOC level was increased (2.5±1.1 vs 1.5±0.5, P<0.05) and the expression of nitrotyrosine was decreased (P< 0.05) in UAA group compared with UUO group. Additionally, the expression of p47-phox was reduced at day 3 (P<0.05), accompanied with a reduced expression of p22-phox (P< 0.05). CuZn-SOD activity was not significantly changed among the groups. Conclusion The inhibition of A&A in NADPH oxidase subunits p47-phox and p22-phox, which is responsible for reduction of oxidative stress, is associated with the alleviation of renal fibrosis in obstructive rat kidney.
6.Effect of NADPH oxidases inhibition on the development of interstitial fibrosis in unilateral ureter obstruction rats

Objective To clarify whether the NADPH oxidases (NOXs) family contributed to the reactive oxygen species (ROS) production and subsequent interstitial fibrosis in unilateral ureter obstruction (UUO) rats.Methods Male Wistar rats were randomly divided into sham operation group (n =8),sham operation + apocynin treatment group (n =8),UUO operation group (n =8) and UUO operation+apocynin treatment group (n =8).Either vehicle or apocynin (100 mg/kg per day) were given by gavage for 7 days after surgery.Rats were sacrificed at 7th day.ELISA was used to detect the activity of superoxide dismutase (SOD) and catalase (CAT),and the level of 8-iso-prostaglandin F2alpha (8-isoPGF2α) in renal tissue.Western blotting was used to detect the protein expressions of NADPH oxidase subunit NOX2 and NOX4,α-smooth muscle actin(α-SMA),collagen Ⅰ (COL-Ⅰ) and the level of ERK1/ 2 phosphorylation (p-ERK1/2).Results UUO rats with vehicle displayed increased oxidative stress,as measured by renal tissue 8-iso-PGF2α,accompanied with increased renal expression of NADPH oxidases (NOX2,1.5-fold and NOX4,1.7-fold,respectively),compared with sham-operated rats (P <0.05).Furthermore,vehicle treated UUO rats showed increased renal COL-Ⅰ and α-SMA levels,compared with sham-operated rats (P < 0.05).ERK1/2 was also activated as detected by p-ERK1/2 expression in UUO rats with vehicle (P < 0.05).Apocynin treatment significantly decreased renal tissue 8-iso-PGF2α level and expressions of NOX2 (-28.7%) and NOX4 (-31.0%) in UUO rats,respectively,compared with vehicle treated rats (P < 0.05).And significant decrease of COL-Ⅰ (-26.4%) and α-SMA expression (-80.0%) were also observed (P < 0.05).The activation of ERK1/2 in UUO rats was greatly inhibited by apocynin treatment (P < 0.01).Despite the pronounced dysregulation of pro-oxidative NOXs family,no compensatory increase of antioxidative enzyme activities occurred.Conclusion The NOXs family contributes largely to the production of ROS and subsequent interstitial fibrosis after ureter ligation,and inhibition of the NOXs family may be a choice for preventing interstitial fibrosis.
7.Controlled Preparation and Characterization of Aspirin Surface Molecularly Imprinted Separating Medium Based on Halloysite Nanotubes

The molecularly imprinted separating medium with halloysite nanotubes as carrier, which were environmentally friendly natural silica substrate nano-materials, aspirin as template molecule and acrylamide as functional monomer was synthesized using reversible addition fragmentation chain transfer polymerization. The template molecule and monomer were bound to stable composite at 1: 2 using the method of ultraviolet spectroscopy combined with Lamber-Beer theory at molecular level. The morphology and adsorption capacity of imprinted material was studied with Fourier transform infrared spectrometer ( FT-IR ) , transmission electron microscope ( TEM) , static adsorption and selective adsorption. The experimental results showed that a good uniformity of imprinted layer with the thickness of 38 nm was coated steadily on the halloysite nanotubes surface. Compared with the conventional surface imprinted material and the material with silica gel as carrier, our molecularly imprinted material had the characteristics of high adsorption capacity and favorable imprinted effect. Its imprinted factor achieved to 3. 5. The molecularly imprinted material was applied for mimetic intestinal juice diffusion experiment. The experimental results indicated that the imprinted material release the aspirin for 12 h, which was 2 times for non imprinted material for only 6 h, demonstrating excellent drug release result, which provided basic data for potential applications of drug carrier.
8.Clinical study of patients with nasal NK/T cell lymphoma associated hemophagocytic syndrome

Objective To study clinical features, treatment and prognosis of nasal NK/T cell lymphoma associated Hemophagocytic Syndrome (HPS).Methods Retrospectively analysis method was used to analyze the clinical data of 3 patients with nasal NK/T cell lymphoma associated HPS. Results 3 patients with nasal NK/T cell lymphoma fulfilled the criteria of HPS. All patients had adverse prognostic factors of lymphoma.1 patient developed HPS as the main primary manifestations of underlying lymphoma,the other 2 patients developed HPS during lymphoma progression. In three cases, bone marrow was infiltrated with lymphoma cells.When HPS occurred,the disease progressed rapidly.The most obvious clinical features were fever,pancytopenia,hypofibrinogenemia,hyperferritinemia,and hemophagocytosis in bone marrow. After being treated according to the HLH-2004 combined with chemotherapy, all patients showed a clinical response,but with the progression of lymphoma,HPS quickly relapsed,and all patients died of severe hepatic dysfunction,coagulopathy,or DIC.Conclusion Nasal NK/T lymphoma associated HPS is an invariably fatal disease with poor prognosis,typically occurring at advanced stage or the terminal phase of the disease.HLH-2004 based protocol in combination with chemotherapy is helpful for nasal NK/T cell lymphoma associated HPS,which may delay disease progression and provid opportunities for the treatment of primary disease.

10.Renoprotective effects of astragalus and angelica mixture in rats with 5/6 nephrectomy

Objective:To explore whether Chinese herbs Astragalus and Angelica mixture(A&A) have renoprotective effects on chronic renal failure in rats.Methods:5/6 nephrectomized kidney in rats was used as chronic renal failure model.Rats were randomly divided into four groups which included sham,Model,A&A and enalapril(Ena) groups.The remnant kidney was harvested after 2,4,8,12 weeks,and levels of serum creatinine(scr),urea and urinary protein excretion were measured using routine biochemical methods.The pathological lesions were observed by light microscope.The expressions of fibronectin(FN) and collagen Ⅳ(Col-Ⅳ) in glomeruli and interstitium were detected by immnuohistochemistry.Results:5/6 nephrectomized rats were presented with the characteristics of increasing scr and urea,persistent proteinuria at the end of the 2nd,4th,8th,12th weeks,respectively.They had increasing size of glomeruli with progressive proliferation of mesangial cells and mesangial matrix followed by glomerulosclerosis and progressive elevated expressions of FN and Col-Ⅳ.Compared with Model group,the administration of A&A could decrease urinary protein excretion for 34%,44%,25%,45% at the end of the 2nd,4th,8th and 12th weeks,respectively(P
