3.Clinical Efficacy and Safety of Bianyan Oral Liquids in the Treatment of Acute Pharyngitis

Objective:To observe the clinical efficacy and safety of Bianyan oral liquids in the treatment of acute pharyngitis. Methods:The clinical data of 200 patients with acute pharyngitis in the outpatient department of traditional Chinese medicine in our hospital from September 2010 to June 2012 were analyzed. They were divided into the control group with 100 cases and the observation group with another 100 cases. The control group was given Jinlianhua granules, while the observation group was received Bianyan oral liquids. After 5 days, the symptom score, effective rate, changes in accompanied symptoms and safety index were observed and com-pared. Results:The symptom score and effective rate of the observation group were better than those of the control group (P<0. 05), and the improvement in defecation was better than that of the control group (P<0. 05) as well. There were two cases in the observation group suffered nausea and stomachache, and one case in the control group suffered diarrhea while no untoward effect emerged. Conclu-sion:Bianyan oral liquids with the effects of heat-clearing and detoxicating, nourishing yin and fluid and treatment both manifestation and root cause show significant effect in the treatment of acute pharyngitis.
4.Gram-negative Bacilli Resistance in Anhui Province in 2004: Surveillance and Analysis

OBJECTIVE To investigate the Gram-negative bacterial resistance of clinical isolates in September, 2004 from 13 hospitals which are members of Anhui Center for Surveillance of Bacterial Resistance. METHODS Antimicrobial susceptibility of clinical non-repeated isolates was tested by agar dilution method and the results were analyzed. RESULTS Of 347 clinical isolates from 13 hospitals, 251 were Gram-negative organisms including 33 species. Gram-negative organisms were different resistant to piperacillin, cefotaxime, ceftriaxone, cefuroxime, ciprofloxacin and levofloxacin, respectively, but the resistance rates to lactams and their inhibitors compounds, such as cefoperazone/sulbactam and piperacillin/tazobactam, and imipenem/cilastatin were all under 10%. CONCLUSIONS Bacterial resistance is still or even a more serious clinical problem than before. There is a valuable surveillance of bacterial resistance.
5.Subcellular localization prediction of proteins containing fibronectin domains using collocation of amino acid pairs

BACKGROUND: Proteins containing fibronectin domains play an important role in cell migration, adhesion, growth and differentiation and have been widely applied to a variety of new biological materials. Subcellular localization prediction of proteins containing fibronectin domains can promote protein function research and development of new biomaterials.OBJECTIVE: To realize subcellular localization prediction of proteins containing fibronectin domains. METHODS: A total of 80 human proteins were randomly selected from Uniprot database. The amino acid pairs for each protein were collocated to form 400 dimensional input feature vectors. The feature vectors were then trained and tested using support vector machine and k-nearest neighbor separately. The prediction quality was examined by the jackknife test. RESULTS AND CONCLUSION: The prediction accuracy was 92.5% and 95% for support vector machine and k-nearest neighbor methods respectively. This suggests that support vector machine and k-nearest neighbor methods are of important significance for predicting subcellular localization of proteins containing fibronectin domains and contribute to functional research of such proteins and surface modification of new biomaterials.
6.Resvertrol Suppressing the Rapid Electrical Stimulation Incurred Oxidative Stress Injury in Neonatal Rats Cardiomyocytes With its Possible Mechanisms

Objective: To explore the protective mechanism of resvertrol (RSV) suppressing the rapid electrical stimulation incurred oxidative stress injury in neonatal rats cardiomyocytes. Methods: The neonatal rats cardiac ifbroblasts and myocytes were isolated by double enzyme digestion and differential adhesion method, and the cardiomyocytes were randomly divided into 5 groups:①Control (CTR) group,②Rapid electrical stimulation (RES) group,③RES+APO group, cells were pretreated with NADPH oxidase inhibitor APO,④RES+RSV group,⑤RES+AIP group, cells were pretreated with CaMKII inhibitor AIP. In order to conifrm whether RSV protection was via MsrA-OX-CaMKⅡpathway, the cells were further divided into another 3 groups:①DMSO control group,②RSV+DMSO group,③RES+ RSV+DMSO group. The best dose of RSV was measured with Kit-8 by cardiomyocytes surviving condition, the optimal electrical stimulation time was detected with ELISA by Ang II level in conditioned medium. The level of reactive oxygen species (ROS) in cardiomyocytes was detected by flow cytometry, the protein expressions of MsrA, Nox4, Nox2, P22phox, OX-CaMK II and apoptosis related cleaved caspase-3 were observed by Western blot analysis.Results:①Compared with CTR group, RES group showed increased AngII secretion with increased protein expressions of Nox4, Nox2, P22phox, OX-CaMK II and cleaved caspase-3.②Compared with RES group, the RES+APO, RES+RSV, RES+AIP groups had decreased ROS level, the ROS was even lower in RES+RSV group.③Compared with RES group, the RES+APO, RES+RSV groups presented decreased protein expressions of Nox4, Nox2, P22phox, OX-CaMK II and cleaved caspase-3, while RES+AIP group only had decreased Nox2, OX-CaMK II and cleaved caspase-3.④Compared with DMSO control group, RES+ RSV+DMSO group had the lower level of cleaved caspase-3 expression. Conclusion: RSV has protective effect on rapid electrical stimulation incurred oxidative stress injury in neonatal rats cardiomyocytes, which might be via NADPH oxidase with the increased MsrA expression .
7.Influences of percutaneous coronary intervention on myocardial activity in myocardial infarction patients with different viable myocardium

Objective To evaluate the effect of percutaneous coronary intervention (PCI) on left ventricular function in patients with different types of myocardial infarction and to explore the correlation factors for the left ventricular function.Methods A total of 43 patients diagnosed as acute myocardial iffarction were enrolled in this study.The perfusion and delayed enhancement magnetic resonance imaging (DE-MRI) was applied to observe the following parameters before the PCI and at month 6 after the procedure:infarct mass,left ventricular ejection fraction (LVEF) and abnormal wall motion score.The subjects were divided into the following three groups by the transmural extent of myocardial infarction manifested in the DE-MRI:the transmural enhancement group,the nontransmural group and the mixed group.Laboratory test was done to detect the level of endothelin (ET),matrix metal enzyme 9 (MMP-9) and high sensitive C reactive protein (hsCRP) before PCI and at month 6 after the procedure.The t test was used to compare the differences among the groups and the multiple regression analysis was taken to explore the correlation factors for the left ventricular function.Results Compared with the parameters before PCI,the infarct mass after PCI significantly decreased in the nontransmural group and the mixed group [(4.0 ± 2.9) g/cm3 vs (9.8 ±5.6) g/cm3 and (6.0 ±3.5) g/cm3 vs (11.8 ±6.2) g/cm3,all P <0.05],while LVEF was significantly improved after PCI in both groups [(52.6 ± 15.4) % vs (41.9 ± 16.3) %,(45.6 ± 15.4)% vs (38.9 ± 16.3)%,all P <0.05].The infarct mass was an independent correlation factor for LVEF before PCI (RR =0.318,P <0.05) and LVEF after PCI(RR =0.293,P <0.05).LVEF before PCI was independently correlated with the level of hsCRP (RR =0.318,P < 0.05).Conclusion The effect of PCI on the improvement of left ventricular function differs in patients with different extent of myocardial infarction,which is correlated with the amount of survival myocardium and the inflammatory factors.
8.Comparative Analysis of MRI, X-Ray and CT in Diagnosis of Scheuermann Disease

Objective To study the imaging features of Scheuermann disease and compare the value of MRI, X-Ray and CT images in diagnosis of this disease. Methods The images of 14 Scheuermann disease patients were analyzed retrospectively. Results The pathological changed vertebraes have wedge-shaped changes in different degree. The edges of the vertebraes manifest roughness and have become step-shaped, or Schmorl′s nodes were demonstrated. Conclusion The multi-vertebraes wedge-shaped changes and the Schmorl′s nodes are the main imaging features of Scheuermann disease. The pathological change occurs in the different spine segments, and the imaging features are not completely same. X-Ray and MRI images are more important in diagnosis of this disease. MRI Multi-direction imaging displays clearly the Schmorl′s nodes, marrow dropsy, the fat accumulation and the intervertebral disc denaturation. What′s more important is that MRI has its own superiority in the differential diagnosis of this disease.
9.Analysis of equity and efficiency of public health resource allocation in China:Based on HRAD and DEA

Objective:The aim of this paper is to analyze and evaluate the equity and utilization efficiency of public health resource allocation in 31 provinces and cities in China, and provides reference and basis for public health resource allocation planning. Methods:The allocation of public health resources was evaluated by cluster anal-ysis and used data envelopment analysis was to explore the efficiency of public health resource allocation. Results:The degree of aggregation of public health resources in densely populated and economically developed provinces and cities were greater than or close to 1 , and the difference with the population concentration was mostly less than 0 . The degrees of aggregation in less economically backward provinces were less than 1 , and the difference with the popula-tion concentration was mostly more than 0 . The average technical efficiency of public health resource allocation was 0. 44, and the mean of pure technical efficiency and scale efficiency were 0. 74 and 0. 64 respectively. Conclusion:The degrees of aggregation of public health resources in economically developed provinces and cities are higher, but the corresponding population is poorly distributed based on population distribution. For economically backward prov-inces and cities, the geographical availability of public health resources is poor. In addition, due to unreasonable re-source allocation structure and proportion, public health resources configuration efficiency is relatively low.
10.The value of PCT, CRP, TNF-α and free DNA in predicting the development of MODS in patients with multiple trauma

Objective To explore the value of peripheral blood serum levels of PCT,CRP,TNF-α and free DNA of cells in predicting the development of MODS in patients with multiple trauma.Methods Complete detail clinical data of 54 casualties with multiple trauma admitted within 24 hours after accident from January 2011 through January 2012 were collected for retrospective study.The patients were divided into MODS group and non-MODS group according the criteria set forth by the Chinese Society of Critical Care and Emergency Medicine in 1995 national conference.The data of two groups are comparable,and data of another 20 healthy subjects undertaking routine annual physical examination were taken as control.The peripheral blood levels of PCT,CRP,TNF-α and free DNA of patients of two groups were determined 1 d,2 d,3 d,and 5 days after admission.Then the results were analyzed and compared between groups.Results Compared with non MODS group,the levels of PCT,CRP,free DNA of cells in MODS group were significantly higher (P < 0.05),but there was no deference in TNF-α between MODS group and non-MODS group (P > 0.05).When the relative risks of increased PCT (PCT≥6 mg/L),increased CRP (CRP≥ 130 mg/L)、and increased free DNA of cells (free DNA ≥ 10 0005/L) were analyzed,the presence of these 3 biomarkers with high levels occurred at the same time was the most accurate way to predicts MODS in 6.00 relative risk (RR),and the positive predictive value was 100%.Conclusions PCT,CRP,free DNA of cells could be the predictors of MODS in patients with severe multiple trauma,and the presence of high levels of these three biomarkers appearing together had high sensitivity and specificity for prediction.
