Multiple symmetric lipomatosis (MSL) is an extremely uncommon disorder. In the medical literatures about 200 cases have been reported. MSL is not associated with other generalized lipomatous disorders, nor are these patient to be necessarily obese. The cause of MSL is unknown. The disorder usually occurs in middle-aged males and there is frequently a history of alcoholism. Some instances of familial occurrence have been reported, but the majority of cases are sporadic. Two cases of MSL are presented.

3.CT diagnosis of the fat containing mediastinal masses

Fat containing masses, except mediastinal lipomatosis, of the thorax are uncommon. In spite of uncommonoccurences, as CT can detect not only the fat content but also the presence and character of the non-fattycomponent of the mass, accurate preoperative diagnosis can usually be made in most cases of fat containingmediastinal masses. Authors report 6 cases of fat containing mediastinal masses, that were diagnosedpreoperatively by CT scan, comprising 2 cases of limpoma(combined with plexiform neurofibroma in a case), 1liposarcoma, 1 thymolipoma, and 2 cases of omental hernia through Morgagni foramen.

Multiple symmetric lipomatosis (MSL), or Madelung's disease, is a rare disease of unknown etiology. It is characterized by the presence of loose adipose tissue deposits localized in the cervical region and upper body. MSL presenting as bilateral huge gynecomastia is an extremely rare phenomenon. The present report describes a case of MSL in a 66-year-old man. The patients presented with bilateral breast bulging. He had a history of cigarette and alcohol use. His condition was treated with a bilateral nipple-sparing mastectomy. MSL can present as a form of gynecomastia, for its accurate diagnosis and proper treatment of MSL, increasing awareness of the clinical characteristics of the disease is required, especially amongst breast surgeons. Herein, we review the literature and discuss the clinical characteristics, pathology, and surgical treatment of MSL.

Gastric lipomas are rare benign submucosal tumor, usually solitary, and comprise about 3% of gastric benign tumors. The occurrence of gastric lipomatosis is extremely rare. Although most gastric lipomas are usually detected incidentally, they can cause severe symptoms such as obstruction, invagination, and life-threatening hemorrhage. The diagnosis and differentation from malignant tumors and other submucosal tumors are difficult with conventional diagnostic modalities such as X-ray or endoscopic examination. But endoscopic ultrasonography allows us to visualize the structures underlying the gastrointestinal wall in a noninvasive maneuver, and can contribute to make differential diagnosis and decision of management. The treatment of lipoma is still controversial. We reported a 70-year-old male who had gastric lipomatosis in entire stomach, which could be diagnosed with endoscopic ultrasonography and was proven by endoscopic lumpectomy, pathologically.

Benign symmetrical lipomatosis (BSL), or Madelung's disease, is a rare disease characterized by he progressive growth of diffuse, painless, non-enveloped symmetric lipomas at typical sites in the body. The etiology of this disease remains unknown. A disturbance of lipid metabolism is involved, and there is a proven connection with chronic alcoholism. Problems with the differential diagnosis of the characteristic clinical picture arise from the fact that this disease is not widely recognized. Surgical lipectomy in several sessions can relieve pressure and provide satisfactory patient functional recovery as well as cosmetic disfiguration.

8.Intestinal lipoma near the ileocecal calve Report of 3 cases and review of the literature.

Three cases of intestinal lipoma near the ileocecal valve are described. In patients of cecal and ileocecal valve lipoma, they had right lower quadrant abdominal pain secondary to partial intestinal obstruction. In rarely developed segmental lipomatosis of the ileum, the patient had right abdominal mass and pain to fecal impaction of the diverticula. Diagnosis may be made by an abnormal roentgenographic pattern and confirmed by colonoscopy. Surgical removal of the affected segment results in cure in symptomatic patients.

The 15 year-old female patient was admitted with the complaints of moon face, obesity and short stature. She has received steroid therapy since 11 years ago at home to control joint pain due to rheumatoid arthritis. The diagnosis was confirmed by history of long-term steroid therapy, characteristic clinical features, biochemical studies and radiological studies. In radiological studies, superior mediastinal widening, aseptic necrosis of both hip an diffuse osteoporosis in various skeleton were revealed. Brief review of literature was presented.

Madelung's disease, also known as multiple symmetric lipomatosis (MSL) is a rare disease characterized by the presence of multiple, symmetric, loose adipose tissues distributed around the neck, shoulder, back or chest. MSL is different from simple obesity, which is characterized by the presence of well-distributed total body fat. It is rarely reported in Asia and most commonly seen among middle-aged men. Although the etiology of MSL is unknown, the disease is associated with alcoholism in 60% to 90% of patients. The diagnosis is usually made on the basis of the history, and clinical appearance. Also, imaging such as CT or MRI could confirm the diagnosis. Herein, we experienced an uncommon case of MSL in middle-aged women with alcoholism.
