Objective To investigate the relationship between of the serum homocysteine (Hcy) level and the occurrence and severity of diseased coronary arteries in patients. Methods The total serum Hcy content was determined in sixty-seven patients with diseased coronary arteries and forty-three normal subjects. They were all subjected to coronary artery angiography for evaluation. Using the Gensini's scoring system the degree of severity of of pathology the coronary arteries were assessed. Other risk factors were analyzed in addition to Hcy and Gemini's scores. Nuclear factor-kappa B (NF-κB) is a key regulator in epithelial-mesenchymal transition (EMT) of cancer cells.EMT of cancer,one of the reasons of drug resistance,enhances the infiltration and distant metastases ability of cancer cells.Recent researches show that Snail,Slug,Twist and Zeb play important roles in regulating EMT of cancer cells.Drugs and targeted therapies that inhibit NF-κB activities can reverse the EMT of cancer cells.NF-κB may become an effective therapeutic target in cancers in the future.

Objective To observe the impact of different culture conditions on the growth of the endothelial progenitor cells(EPCs) from rat peripheral blood. Methods Mononuclear cells obtained from rat peripheral blood were isolated by using density gradient centrifugation. According to the culture medium added with vascular endothelial growth factor (VEGF), basic fibroblast growth factor (bFGF) and the dishes were precoated with fibronectin or not, the mononuclear cells were divided into different groups and cultured under different conditions in vitro to compare the differences of the growth conditions. The different growth conditions were recorded and the result was calculated by statistics software. At last, the cells were identified by immunohistochemistry and immunofluorescence. Results The mononuclear cells from rat peripheral blood grew in the manner of keeping close to the wall in vitro. The cells and cell colonies cultured for 7 days hinted:under the same culture conditions, precoating FN was beneficial to the adherent proliferation of EPCs (t=4.43, P＜0.05; t=3.70, P＜0.05). Excluding the impact of fibronectin, growth factors could promote mononuclear cells differentiation to EPCs, indicating that growth factors enhanced proliferation of EPCs (t =-13.22, P＜0.01; t =-10.96, P＜0.01). Immunohistochemistry and immunofluorescence showed that, at day 4, 7, 10 of cells cultured, CD34 and CD133 expressions were increased gradually [(35.7±4.2)%, (60.1±3.8)%, (81.8±6.4)%; (3.2±0.9)%, (18.4±7.3)%,(32±3.8)%, respectively]; at day 14, both were decreased [(32.1±5.4)%, (1.9±2.7)%]; but Flk-1was increased at day 4, 7,10, 14 [(31.2±3.5)%, (40.6±5.3)%, (71.2±8.4)%, (81.5±4.1)%].Conclusions Fibronectin is conducive to adhesion and proliferation of EPCs. VEGF and bFGF play an important role in the differentiation of EPCs. The success culture of EPCs in vitro will provide a sufficient number of seed cells for its application in vascular tissue engineering, and offer new ideas for peripheral blood stem cell transplantation for the treatment of various diseases.

Objective In order to know the proper prevention and nursing measures for postoperative hemorrhage among patients with tyroid surgery. Methods Analized the medical history of 19 patients with postoperative hemorrhage after thyroid surgery retrospectively. Results The 19 patients with postoperative hemorrhage were all recovery smoothly by careful nursing and proper treatment. Conclusions Uncomplete intraoperative hemostasis, intraoperative breathholding and improper placement of drainage structures are the risk factors for postoperative hemorrhage. Careful postoperative observation and nursing can avoid certain postoperative hemorrhage.

Objective To characterize the clinical features and the prognosis of paediatric lobar pneumonia. Methods Clinical data of 45 cases with paediatric lobar pneumonia,such as clinical manifestations,symptom, pathogens and prognosis were retrospectively analyzed.Results Of the 45 patients,45 cases had cough,43 cases with fever,15 cases with pain in the chest,13 cases wtih cutaneous lesions,35 cases wtih antibody of mycoplasma positive, 6 cases wtih Staphylococcus aureus infection,21 cases wtih Streptococcus pneumonia infection,cardiovascular and digestive system were involved besides lung,secondly nervous system involved.After treatment,29 cases were cured, 15 cases improved,1 case transferred other hospital.Conclusion Paediatric lobar pneumonia caused by decreased gradually,but increased by virus and other bacteria.Cases of paediatric pneumonia with fever,cough,pulmonary symptom should be tested by laboratory finding,especially antibody and X ray or CT in order to identify pathogenesis of disease,thus to be treated correctly,dosage and course of antibiotics should be also enough.

Objective To compare the renal function after neurosurgery performed under closed-circuit anesthesia with sevoflurane and desflurane and determine the safety of the closed-circuit sevoflurane anesthesia. Methods 36 ASA Ⅰ or Ⅱ patients aged 18-60 years were randomly divided into 2 groups to receive either sevoflurane (group S, n = 18) or desflurane (group D, n = 18) . The duration of surgery was expected to be ≥90 min. The patients were premedicated with intramuscular midazolam 0.07 mg?kg-1 . Anesthesia was induced with midazolam 0.1 mg?kg-1, fentanyl 1.5-2.0 ?g?kg-1 , propofol 1-2 mg?kg-1 and vecuronium 0.1-0.15 mg? kg-1. The patients were mechanically ventilated (VT = 8-12 ml?kg-1) after tracheal intubation. PET CO2 was maintained a 35-45 mm Hg. Fresh gas flow (FGF) was first set at 1-5 L?min-1 for 2-3 min, then reduced to 0.18-0.3 L?min-1. In group D end-tidal desflurane concentration was maintained at 6%-10% ; whereas in group S the end-tidal sevoflurane concentration was maintained at 2.7%-4.0% . BP, HR, ECG, SpO2 PET CO2, inspiratory and expiratory O2 , desflurane and sevoflurane concentrations and the temperature in the center of soda-lime canister were continuously monitored during the operation. Gases were collected from breathing circuit for determination of compound A concentration (by gas chromatography) before anesthesia, at the end of 2-3 min wash-in, every 30 min during maintenance of anesthesia and at the end of anesthesia. Venous blood samples were taken before anesthesia (baseline) , at the end of surgery (T1) and at 2, 24 and 72 h after operation (T2-4) for determination of serum F-, creatinine (Cr) and blood urea nitrogen (BUN) concentrations. Urine specimens were taken for determination of total protein (TP), ?2-microglobulin (?2-MG) and ?-N-acetyl-glucuronidase (NAG) levels, before operation (baseline) at the end of operation and at 24, 48 and 72 h after operation. TP/Cr, ?2-MG/ Grand NAG/Cr were calculated to eliminate the influence of the volume of urine.Results There were no significant differences between the two groups with respect to age, sex, body weight, height, duration of anesthesia and temperature in the center of soda-lime canister. The inspiratory O2 concentration was ≥ 75% during anesthesia. There was no significant difference in serum Cr and BUN concentrations and NAG/Cr in urine before and after operation as well as between the two groups. TP/Cr and ?2-MG/Cr in urine were significantly increased after operation compared to the baseline values but there was no significant difference between the two groups. There was no significant correlation between postoperative TP/Cr, ?-MG/Cr and compound A. Conclusion Closed-circuit sevoflurane ansthesia does not affect postoperative renal function.

Objective To discuss the efficacy of applying management-by-objectives(MBO) in the hospital-level nursing quality control group.Methods Goals of objective management and assessment details were set by leaders of quality control groups along with the nursing department at the beginning of the year.The nine subordinate groups performed upon the required criteria,completed the quality inspection on time and recorded group activities.At the end of the year,group leaders reported to the hospital-level nursing quality committee and head nurses about their performances,where several major honors were rewarded to the best performing groups.SPSS 17.0 software was used for data statistics.The measurement data were analyzed by paired t test data while enumeration data by chi-square test.Results Scores of assessment items were significantly raised after implementing MBO(P＜0.05).Scores of key items,such as high quality care,emergency management,nursing skill,were increased to (98.1 ± 2.2),(97.7 ±2.7),(99.6±0.6),(96.1 ±2.1)from (93.6 ±2.5)(P=0.009),(93.0 ± 2.6) (P=0.009),(96.2 ± 1.3) (P=0.000) and (89.3 ± 4.5) (P--0.000),respectively.Care complaints were sharply dropped for all inpatient wards while numbers of management innovationand process reengineering were greatly increased (x2=8.884,P=0.031).Condusion Introduction of MBO in the hospital-level nursing quality control group does facilitate the continuous improvement of care quality.
