1.Establishment of chromatin immunoprecipitation assay to analyze transcriptional regulation of gene expression

Objective To establish a Chromatin immunoprecipitation assay to analyze transcriptional regulation of gene expression in hepatocytes. Methods The PEPCK gene was used as a target gene, HPRT, MBP and IgG were used as internal reference, the chromatin immunoprecipitation and PCR were performed with antibody specific for CREB to verify bindings of transcription factor CREB to the PEPCK promoter under various metabolic conditions. Results Transcription factor CREB occupies its target sites in the PEPCK promoter and this binding is increased by 4-fold during fasting in the anti-CREB antibody immunoprecipitated DNA fragments. Conclusion The results confirm that CREB binds to the PEPCK promoter and is involved in transcriptional regulation of PEPCK gene in hepatocytes.
2.The effect of preconditioning with buflomedil hydrochloride on expression of Bcl-2 and Bax on neuron cells of DRG after the sciatic nerve lesion model in rats

Objective:To explore protective mechanism of buflomedil Hydrochloride on the neuron of damaged Sciatic nerve in rats. Methods:SD rats were divided into normal, model,buflomedil hydrochloride group in random. Preconditioned for 14 days,the sciatic nerve lesion models were performed, the Bcl-2 and Bax positive cells were counted by immmunohistochemistry at the end of 6h,12h,24h,48h,72h,96h,7days respectively. Results:(1) At the end of 12h after the model perfomed,The expressions of Bcl-2 in buflomedil hydrochloride group increased obvionsly than the normal and model groups,peaked at 24~48h,and at the end of 7days,the expressions of Bcl-2 in buflomedil hydrochloride group increased obviously compared with model group.(2)The expressions of Bax in buflomedil hydrochloride groug were lower than the normal and model groups in the same periods,and that was the lowest at the end of 7 days,It was different significantly than that of model group(P
3.Clinical analysis on complicated arrhythmia after 104 cases lung resection

Objective:To analyze the reasons of complicated arrhythmia after resection of lung. Methods:The relation of arrhythmia with age, operation mode,preoperative lung function and postoperative blood gas of 562 cases of lung resection were analyzed. Results:There were 104 of 562 cases (18.5%) complicated with arrhythmia postoperatively. 38.77% of them were older than 70 years. 27.72% had operation of pneumonectomy.The parameters of FEV 1,MVV,FVC lower than 50% were 67.24%,46.53% and 67.93% respectively.Postoperative blood gas analysis of the 23 cases showed that Po 2 was lower than 60 mmHg,Pco 2 was higher than 45 mmHg,So 2 was lower than 96%.21 of them developed postoperative arrhythmia.After tracheotomy and immediately correcting anoxia,all the 21 patients recovered from arrhythmia within 3 days.4 patients died in this group(3.84%).Conclusion:Pneumonectomy,in patients older than 70,results in the parameters of FEV 1,MVV and FVC lower than 50%.Significant anoxia is the cause of arrhythmia.
4.The application of VATS in the diagnosis and treatment of small lung neoplasm

Objective:To discuss the method of Video Assisted Thoracoscopy(VATS) in diagnosis and treatment of small peripheral lung neoplasm.Methods:A wedge-shaped excision with VATS had been performed on 20 patients with small peripheral lung neoplasm.The sample had been sent to pathology during surgery.Lobectomy was done with the malignant tumor.Results:14 cases(70%) were malignant in stage IA ,6 cases were benign.The mean follow-up time postoperatively was 24 months(12~36months),there was no relapse case.Conclusion:For those patients,the diameter of neoplasm is smaller than 15mm,and antibiotic treatment is ineffective for 2~3 weeks,VATS can be available for diagnosis and treatment.
5.Comparative study on different methods for detecting plasma fibrinogen by automated coagulation analyzers

Objective To comparatively analyze the results of the PT derivative method and the Clauss method for detecting fi-brinogen(Fib) by the automated coagulation analyzer and to explore their accuracy and clinical value .Methods Fib was detected in 156 blood samples selected from the patients in this hospital from March to May 2013 ,25 normal control samples and 13 positive samples of D-dimer (DD) by the TP derivative method and the Clauss method .Results The Clauss method and the PT derivative method by Sysmex CA-1500 coagulation analyzer had good repetitive rate in measuring Fib for 25 times ;when Fib<1 .0 g/L ,the results detected by these two methods had statistically significant difference (P<0 .05);when Fib in the range of 1 .0 -2 .0 g/L , 2 .1-3 .0 g/L ,3 .1-4 .0 g/L ,the detection results had no statistically significant difference between the two methods (P>0 .05);when Fib in the range of 4 .1 -5 .0 g/L ,the detection results had statistically significant difference (P<0 .05);when Fib>5 .0 g/L ,the detection results had statistically significant difference (P<0 .05);in 13 DD positive samples ,the detection results of the PT derivative method were significantly higher than those of the Clauss method with statistically significant difference (t=8 .530 , P<0 .05) .Conclusion The difference exists between the PT derivative method and the Clauss method for detecting Fib ,but the ac-curacy of the Clauss method is higher than its specificity ,the Clauss method should be the first choice for the clinical determination of plasma Fib .
6.Effects and Mechanisms of Pioglitazone on Expression of Glucose Transporter-4 in Cardiac Muscle Tissue of Rats with Diabetes Mellitus

OBJECTIVE: To observe the effects of long-term administration of pioglitazone at different doses on the expression of glucose transporter-4(GLUT-4) of cardiac muscle tissue in rats and to discuss the possible mechanisms.METHODS: A total of 40 Wistar rats were enrolled,of which,10 were randomly assigned to normal control group(NC),and the other 30 were injected intraperitoneally with streptozotocin to establish diabetes mellitus(DM) rat model.After successful modeling,the rats were assigned to DM model group(DM,normal saline),high and low dose pioglitazone group(HP,LP;0.75,0.25 mg?kg-1?d-1).The NC groups received normal saline for 12 weeks.Blood glucose(BG) level before grouping(baseline level) and prior to death was determined by tail-cutting method.GLUT-4 level in cardiac muscle tissue and both protein and mRNA expression of AMPK? were determined by RT-PCR and western blot,respectively.RESULTS: As compared with NC group,the BG level in DM group was significantly increased but the expression of GLUT-4 and AMPK? decreased significantly(P

ObjectiveStudy on the effect of nasal decontamination hefor aerosol inhalation in elderly patients with general anesthesia postoperation.Methods130 elderly patients with general anesthesia postoperation were randomly divided into routine treatment group and nasal decontamination group.The time of first sputum,the time of postoperative sputum changing characters,respiratoty-related indicators,sputum cuhure positive rate of bacterial,the incidence of postoperative pulmonary complications was examined.ResultsCompared with routine group,nasal decontamination group obviously shorten sputum character changing time ( P < 0.05 ).Postoperative nasal decontamination group and conventional group sputum Staphylococcus aureus positive rate were 7.7% (5 cases) and 20.0% ( 13 cases) respectively,the difference was significant(x2 =4.13,P <0.05).ConclusionGiven nasal decontamination before aerosol inhalation could improve character of sputum and reduce sputum bacterial culture positive rate,and had good efficacy and safety.

9.Influence of nursing intervention on sleep quality of postoperative elderly patients

Objective To study the impact of nursing intervention on the sleep quality of postoperative elderly patients who had sleep disorder. Methods 260 cases of postoperative elderly patients with sleep dis-orders were randomly divided into the experimental groupand the control group with 130 cases in each group. Conventional care was given to the control group, the experimental group received appropriate nursing interven-tion such as psychological nursing, rehabilitation,etc. Pittsburgh sleep quality index (PSQI) was used to evaluat-ed sleep quality of two groups before and after intervention, t test was adopted. Results No difference was seen in each sleep index in the two groups. While each index evidently alleviated after intervention in the ex-perimental group and was better than those of the control group. Conclusions Nursing interventions have certain effects on improving sleep quality of postoperative elderly patients with sleep disorders.
10.Research in application of modified venipuncture method in clinic and emergency infusion room

Objective To discuss the effect of modified venipuncture method on success rate of vcnipuncture, in order to alleviate the suffering of patients with emergency transfusion. Methods 380 cases of transfusion patients were randomly divided into the routine group and the modified group, rou-tine and modified venipuncture methods were applied to each group respectively. The success rate and pain response of each group were compared between the two groups by χ2 test. Results The success rate of venipuncture of the modified group was 93.50%, which was significantly higher than that of the routine group, and the rate of pain of the modified group was lighter than that of the routine group. Conclusions The modified venipuncture method can alleviate pain of patients, improve work efficien-cy and was worthy of clinical nursing application.
