1.Survey and Analysis of Drug Use that Goes Beyond the Scope of Package Inserts in Pediatrics

OBJECTIVE:To probe into the situation of drug use that goes beyond the scope of package inserts in pediatrics of our hospital and to promote clinical rational drug use.METHODS:A total of 3 142 prescriptions between Mar.and Apr.2006 collected from the pediatric out-patient department of our hospital were analyzed in accordance with the contents of the related drug package inserts.RESULTS:Of the total 3 142 prescriptions,345(11.0%) involved drug use that goes beyond the scope of package inserts,leading the list were prescriptions of infants,accounting for 71.3% of the total.The problems such as age(52.2%) and indication(36.5%) were the main type;respiration system drugs(48.4%) and the antibiotics(40.6%) were the main drug kinds involved.CONCLUSIONS:It's widespread for drug use that goes beyond the package inserts in the pe-diatrics,which conforms to evidence-based medicine and the spirit of philosophy and ethics,but against the principle of law,which thus should be given fully attention in the clinic.

HDL or apolipoprotein A-Ⅰ,the major protein of HDL particles,protect host against vascular endothelial dysfunction.HDL binding to scavenger receptor-BI activates eNOS for increasing NO.SR-BI and Endothelial lipase(EL) play a critical role in the regulation of plasma HDL.Incubation of HDL with vascular endothelial cells promotes prostacyclin synthesis.Some proteins such as paraoxonase that cotransport with HDL in plasma,have antioxidant properties.HDL inhibits TNF-? dependent expression of adhesion molecules in vascular endothelial cells and interferes with apoptosis,proliferation and migration of vascular endothelial cells.
3.The Clinical Value of Combined Detection of Anti-HCG and Estradiol and Progesterone in Infertile Women

Objective To evaluate the clinical value of combined detection serum anti-HCG and estradiol and progesterone in infertile women.Methods Serum estradiol(E2) and progesterone(P) in infertile women were detected by ECLIA , Anti-HCG were detected by ELISA.Results the levels of estradiol(E2) and progesterone(P) in anti-HCG positive group were significantly lower than those in anti-HCG negative group.Conclusions The combined detection of anti-HCG and estradiol and progesterone were of important clinical value in diagnosis of infertile women.
4.Inter-hand transfer of the effects of unilateral isometric resistance training for wrist flexion and extension

Objective To investigate the effects on one hand of training the other with isometric wrist extension and flexion training and its underlying mechanism.Methods Twenty healthy young girls were recruited and randomized into a training group and a control group with 10 subjects in each,using a random number table.The subjects in the training group were accepted isometric training of the wrist extensors and flexors on the right side once every other day,4 days a week for 6 weeks according to a pre-programmed protocol,while those in the control group had no intervention.Peak torque and surface electromyography (sEMG) were recorded and assessed before and after 6 weeks of training.Results In the training group,the average peak torque of right wrist flexion and extension were both significantly higher than before training (within-group comparison) and higher than in the control group (between-group comparison) after 6 weeks of training.The average peak torques of the left wrist in the training group in flexion and extension were (12.9±2.0) Nm and (6.4 ± 1.3) Nm after training,both significantly higher than before the training and stronger than the control group.In the sEMG traces during wrist extension,after training the ascend velocity of the right extensor carpi ulnaris and the integrated area of the left extensor carpi ulnaris in the training group were significantly smaller than before training and in the control group.Conclusion Unilateral isometric resistance training of the wrist muscles can transfer to the contralateral side,probably by altering muscle recruitment.
5.Cross-education after unilateral resistance training for forearm rotation

Objective To investigate the extent of cross-education between the pronation and supination muscles of the right and left forearms after unilateral isometric training.Methods Twenty healthy young girls were randomized into a training group and a control group using a random number table.The training group underwent isometric training of their right forearms for six weeks,while the control group continued ordinary life without exercises.Pronation peak torque (PPT) and supination peak torque (SPT) were assessed before and after the training for both groups.Results The differences in PPT or SPT between the training group and the control group were not significant before the training program.When the training had been completed,however,the average PPT and SPT on right side of those trained of course had increased significantly compared to before training or to the control group.More significantly,the average PPT and SPT on the left side in the training group were also significantly better than before training or in the control group after training.Conclusion Unilateral isometric training of the forearm pronation and supination muscles for six weeks can significantly increase muscle strength bilaterally,indicating good cross-education.
6.Analysis of cell death induced by xCT deficiency based on differential proteomics technology

Objective To identify the mechanisms underlying xCT deficiency by high-resolution proteomic analysis of differential protein expression in Sut and wild melanocytes .Methods The proteins,extracted from Sut and wild melano-cytes,were analyzed by two dimensional electrophoresis and PDQuest software .Subsequent MALDI-TOF mass spectrometry analysis was carried out .The protein identification was based on peptide mass fingerprint combined with the pI and the rela -tive molecular mass ( Mr) .Sequence coverage was performed with the Peptldent software on the NCBnlm website .Also,the autophagy marker protein LC3-Ⅱ, and the autophagic cell death marker protein Beclin 1 were detected by Western blotting . Results and Conclusion Twenty apparently upregulated or downregulated proteins were identified .Strikingly, important modifications in regulators of trafficking and organization of vesicles and autophagy ( Anxa3; Hist 1h2bk, NDRG1, and CaM) and in regulators of invasion and metastasis of carcinoma (S100A-4;S100A-6 and vimentin)are likely to account for dysfunctions in cell viability and cell-extracellular adhesion .These results indicate that the proteins regulating autophagy , vesicles trafficking and MAP kinase related pathways are activated and play a role in xCT-deficiency .
7.Association study between simple febrile seizures and gabbr1 gene and gabbr2 gene

Objective To investigate whether the gamma-aminobutyric acid (GABAB) receptor genes,including gabbr1 gene and gabbr2 gene,are the susceptible genes for simple febrile seizures (sFS) by screening mutation of gabbr1 gene and gabbr2 gene and to study the possible association between sFS and the 2 genes.Methods All exons and flanking introns of gabbr1 gene and gabbr2 gene were amplified with polymerase chain reaction (PCR) and sequenced to screen the possible mutation on 60 children with sFS in the northern China in Han nationality population.One hundred and one healthy children from the same area were selected as controls,and the genotypes of single nucleotiole polymorphisms (SNPS) (rs29220,rs29230,rs29267 on gabbr1 gene and rs1000440,rs3205936,rs2304391 on gabbr2 gene) were typed by PCR-restriction fragment length polymorphism.EH1.20 software was used to estimate haplotype frequency to study the association with the haplotype as genetic marker between case group and control group.Results No mutation associated with sFS was found in the 60 sFS cases.In all sequenced regions,23 SNPs were identified in both genes:6 SNPs in gabbr1 gene and 17 SNPs in gabbr2 gene.The frequencies of the 6 SNPs were complied well with the Hardy-weinberg equilibrium in sFS group and normal group.Genotype proportions and allele frequencies of 6 SNPs were not significantly different between both groups.The haplotypes of 3 SNPs in gabbr1 gene and in gabbr2 gene distributions were not significantly different between 2 groups.Conclusions No mutations and associations were identified between sFS with both GABAB receptor genes(gabbr1 gene and gabbr2 gene).They may not be the susceptibility gene for simple febrile seizures in Han nationality population in northern China.
8.Homing time window of allogeneic bone marrow stem cells transplanted into Chinese miniature pigs with myocardial infarction

BACKGROUND:Due to the different transplantation time after myocardial infarction, the homing ability of bone marrow mesenchymal stem cels to damaged tissues as wel as the repairing role wil be very different. OBJECTIVE:To explore the optimal window time for the homing of bone marrow mesenchymal stem cels to the myocardial tissue after myocardial infarction. METHODS: Eighteen Chinese miniature pigs were modeled by the ligation of left anterior descending coronary artery. BrdU-labeled bone marrow mesenchymal stem cels were injectedvia the coronary artery at 1 day, 3 days, 1 week, 2 weeks, 3 weeks, 4 weeks after modeling, respectively. Then, the animals were kiled at 3 days after cel transplantation to detect the home of bone marrow mesenchymal stem cels in the infarct zone. RESULTS AND CONCLUSION:BrdU-labeled positive cels with brown nuclei were visible at 1 day, 3 days, 1 week, 2 weeks, 3 weeks, and 4 weeks after myocardial infarction, especialy at 1 week after myocardial infarction (P < 0.05). It indicates that the best homing window for bone marrow mesenchymal stem cels was at week after myocardial infarction, when the stem cel transplantation is given to be able to promote myocardial repair.
9.Ten cases analysis of small vessel vasculitis with myocardial involvement

Objective This study was designed to investigate the clinical characteristics of systemic vasculitis with cardiac involvement.Methods Clinical information of 10 patients with systemic vasculitis complicated by myocardial vasculitis,selected from 181 small vessel vasculitis patients and 114 systemic vasculitis patients were analyzed.Results The clinical manifestations were varied significantly dependent on the etiology of small vessel vasculitis.It is usually djfficult to make the diagnosis because of the insidious onset,varied etiology and the undifferentiated manifestations of heart involvement.Echocardiography is commonly used in detecting and monitoring cardiac involvement.Glueocortieoid therapy can improve left ventricular systolic function dramatically when used properly.Conclusions The cardiac involvement of systemic vasculitis is quite rare.Dyspnea of various degrees and left ventricular systolic dysfunction ale the most common clinical findings.The earlier the establishment of diagnosis and institution of appopriate treatment,the better the prognosis.
10.The clinical,neuroimaging and electrophysiological presentations in 62 cases with multiple system atrophy

Objective To evaluate the clinical,neuroimaging and electrophysiology features of 62 patients with multiple system atrophy(MSA).Methods Sixty-two cases with diagnosis of probable MSA were recruited in a retrospective studied.Clinical,neuroimage and external anal sphincter electromyography (EAS-EMG)data was retrospectively analyzed.Results In 62 cases(44 male and 18 female),the onset age was between 37 and 76.Among them,29 cases(46.8 % )were MSA-A,with orthostatie hypotension as the main clinical manifestation;24 cases(38.7 % )were MSA-C,with cerebellar ataxia ag the main chnical manifestation;9 cases(14.5 % )were MSA-P,with extrapyramidal symptoms as the main clinical manifestation.MRI showed that main lesion of MSA-A was in the cerebellum:that of MSA-C was in the cerebellum,pons and medulla;and that of MSA-P was in the putamen.Fifty-one cases did EAS-EMG and 46 cases showed neurogenie impairments.Nineteen cases were initially misdiagnosed with other diseases.Conclusions MSA is easy to be omitted or misdiagnosed at early stage.The diagnostie rate of MSA can be increased by the combination of clinical expressions,neuroimage,EAS-EMG and other necessary examinations.
