1.Proposal for building a disaster-medicine rescue system in Hainan Province

Based on the analysis of the status quo and development trends of disaster medicine system abroad along with a summary of the present conditions and inadequacies of the same system in China, this paper proposes to build such a system in Hainan Province. It studied, with reference to the specifics of Hainan Province, the characteristics of such a system in the province, involving such aspects as hospital setup, human resource development, construction of research centers, professional training,management of disease control centers, improvement of the hospital self-rescue system, reliable supply of drugs and medical devices, maintenance of disaster medicine information system, logistics support,improvement of assessment system, establishment of rescue-oriented culture, and psychology crisis intervention and promotions.
2.Laryngopharyngeal reflux effect on expression of COX-2mRNA in glottic carcinoma.

OBJECTIVE:
To investigate laryngopharyngeal reflux effect on expression of COX-2mRNA in glottis carcinoma lesion mucosa.
METHOD:
Forty cases with glottic laryngeal cancer were examined by electronic naspharyngeal laryngoscope and scored by the reflux symptom index (RSI) and the reflux finding score(RFS). Based on the scores, they were divided into two groups:glottic laryngeal cancer with positive reflux(20 cases) and glottic laryn geal cancer with negative reflux (20 cases). Ten cases with adjacent normal membrane were used as control group. The mRNA expression of CoX-2 from 40 patients was examined by reverse transcription polymerase chain reaction (RT-PCR).
RESULT:
The expression of COX 2mRNA in tumor samples was significantly higher than that in normal tissues (P < 0.05); the expression of COX-2mRNA in glottic laryngeal cancer with positive reflux was significantly higher than that in glottic laryngeal cancer with negative reflux (P < 0.05).
CONCLUSION

3.Influence of nursing intervention on psychological state of transfusion patients in outpatient and e-mergency department

ObjectiveTo discuss the influence of nursing intervention on psychological state of transfusion patients in outpatient and emergency department. MethodsSCL-90 questionnaires were used to investigate 600 transfusion patients in outpatient and emergency department to understand their psychological state. Psychological nursing was given to them before, during and after transfusion, SCL- 90 questionnaire was used again for investigation at the last day of transfusion,χ2 test was used to evaluate the results. ResultsAfter psychological care, the somatization, interpersonal relationships, anxiety, depres-sion, hostility and terrorism and other items significantly alleviated compared with those before interven-fion. ConclusionsTo grasp the patients' psychological state and give psychological care to them can ef-fectively reduce the adverse psychological reaction of patients, improve the quality of care and safety and promote the treatment and rehabilitation of diseases.
4.The reliability and validity of the Chinese version of dynamic occupational therapy cognitive assessment for children

Objective To study the reliability and validity of the Chinese version of the dynamic occupational therapy cognitive assessment for children (DOTCA-Ch).Methods The English version of the DOTCA-Ch was translated into Chinese. One hundred and nine students from the city of Jiamusi were assessed with the Chinese Binet intelligence test and the DOTCA-Ch. Intra-class correlation coefficients (ICCs) were used in inter-rater and intra-rater reliability assessments. Cronbach's alpha coefficient was used to assess internal consistency. Factor analysis was used to assess construct validity. Pearson correlation coefficients were used in assessing criterion-related validity. Results The total ICC of the DOTCA-Ch was 0. 892. Cronbach's alpha coefficients for the subtests were orientation 0. 713 ; spatial perception 0. 793 ; praxis 0. 850 ; visuomotor construction 0. 844 and thinking operations 0. 630. The ICC for intra-rater reliability was 0. 991 and for inter-rater reliability it was 0. 989. Using the Binet intelligence test as a benchmark, the Pearson correlation coefficient between the IQ values and the total DOTCA-Ch scores was 0. 872 for 6 year-olds, but 0. 870 for 11 and 12 yearolds. Factor analysis revealed an acceptable set of 5 common factors. Their cumulative contribution was 67%.Conclusions The Chinese version of the DOTCA-Ch is reliable and valid. It can be used as an effective measuring tool to evaluate children's cognitive functioning.

Pseudomonas XN 1 has the ability of degradation for organic pollutant nitrobenzene, and the resistance to ampicillin In this paper, the plasmid was detected and isolated from the cell of Pseudomonas XN 1, and plasmid pXN 1 whose size is about 22 kb was gained The experiment of plasmid elimination confirmed that the plasmid has some relation to the ability of nitrobenzene degradation, while it is irrelevant to the resistance to ampicillin The feature difference between XN 1and its spontaneous mutants was corresponding to the difference of the plasmids detection, and a derivative plasmid pXN 13 was obtained from mutant XN 13 which is a few kb smaller than pXN 1
6.Application of 3D ultrasound VOCAL technique in the detection of fetal thorax

Objective To establish the normal data of the fetal thorax,and to evaluate its values in the diagnosis of fetal thorax malformation.Methods Totally 398 normal singleton fetuses at 16 to 36 gestational weeks(GW) were enrolled,2D-US and 3D-US VOCAL technique were used to measure the 2D data and 3D volumes on the transverse section at the level of the four-chamber view,and the correlation among all measurements with GW was analyzed.Thirty fetuses collected randomly were examined to analyze the reliability.Nine fetuses with congenital thoracic dysplasia (CTD) and 10 fetuses with congenital diaphragmatic hernia (CDH) were assessed and compared with the normal fetuses.Results ① In healthy controls,the fetal thoracic 2D measurements and 3D volumes increased along with the growth of the GW.The regression equations were listed as follows:thoracic transverse diameter (cm) =-0.002 GW2 + 0.301 GW-1.510;thoracic anteroposterior diameter (cm) =0.003GW2 + 0.046GW + 0.666;thoracic area (cm2) =0.071GW2-1.466 GW + 14.728;thoracic circumference (cm) =0.01GW2 + 0.313GW + 3.341;thoracic volume (cm3) =0.285 GW2-7.797GW + 66.592;lung volume (cm3) =0.178 GW2-5.317GW + 45.539;the ratio of lung volume to thoracic volume =0.005GW + 0.396.② The reliabilities of the data obtained by the same/two different operators were good.③ CTD group was obviously lower than the healthy controls in all thoracic measurements (all P <0.01).There was no statistical difference in the 2D data between the CDH group and healthy controls (P >0.05),while the 3D volumes and the ratio of lung volume to thoracic volume were obviously lower than those in the healthy controls (P <0.01).Conclusions 2D-US can evaluate the fetal thoracic development and malformation preliminarily,but 3D-US VOCAL technology plays an important role in distinguishing different types of thoracic malformations.
7.Effects of Wrist Hand Orthoses on Cerebral Palsy Children with Adduction of Thumb

ObjectiveTo observe the effects of wrist hand orthoses on cerebral palsy children with adduction of thumb. MethodsChildren with spastic hemiplegia were divided into experimental group (14 cases) and control group (15 cases). Children in the both groups received routine rehabilitation for 3 months. Children in experimental group wore wrist hand orthoses 4~8 h/d in addition. ResultsThe passive range of motion (PROM) and scores of Fine Motor Function Measure Scale (FMFM) significantly increased in both groups after treatment. The PROM and FMFM scores in experimental groups increased more than those in the control group after treatment (P<0.05).ConclusionWrist hand orthoses can improve the passive adduction angle of carpometacarpal joints of thumb and the fine motor function in spastic hemiplegia children after cerebral palsy.
8.Clinical observation on therapeutic effect of myopia in children treated with abdominal acupuncture.

OBJECTIVETo observe the clinical effects of myopia in children treated with abdominal acupuncture.
METHODSNinety children with myopia were randomly divided into an abdominal acupuncture group (45 cases with 90 ill eyes) and an auricular point group(45 cases with 90 ill eyes). In the abdominal acupuncture group, Zhongwan (CV 12), Shangqu(KI 17), Xiawan(CV 10), Tianshu(ST 25), Qihai(CV 6), Guanyuan(CV 4) were treated with tapping method. The needles were not retained and down to the lower in accordance with the order from Zhongwan (CV 12) to Guanyuan(CV 4) for 5 min,and the erubescence skin without blood was proper. In the auricular point group, vaccaria seeds were pasted at gan(CO12), shen(CO10),pi(CO13), wei(CO4), yan(LO5), pingjianqian(TG2i), pingjianhou(ATli) and zhen(AT3); one ear was chosen every time and the other ear the second time with pressing of patient's own for 5 min every day. The treatment was given twice a week and 10 treatments were considered as one course in the two groups. The changes of the visual levels in visual chart before treatment, 20 min after the first treatment, after 5 weeks' and 3 months' treatment were compared between the two groups,and the clinicall efficacy was compared between the two groups as well.
RESULTS(1) The visual levels in visual chart were improved in the abdominal acupuncture group and auricular point group, and the effects 20 min after the first treatment, after 5 weeks' and 3 months' treatment in the abdominal acupuncture group were superior to those in the auricular point group(4. 78±0. 16 vs 4. 69±0. 22, P<0. 01; 4. 85±0. 16 vs 4. 79±0. 21, P<0. 05; 4. 89±0. 13 vs 4. 71±0. 25, P<0. 01). (2)The effective rates 20 min after the first treatment, after 5 weeks' and 3 months' treatment in the abdominal acupuncture group were better than those in the auricular point group[31. 4% (27/86) vs 0% (0/82). 81.4%(70/86) vs 60. 0%(49/82); 88. 4%(76/86) vs 51. 2%(42/82), all P<0. 01].
CONCLUSIONAbdominal acupuncture for improving the visual levels of myopia in children is superior to auricular point sticking.

9.Factors affecting the quality of life in lung cancer patients measured by EORTC QLQ questionnaire.

BACKGROUNDTo evaluate the quality of life (QoL) of lung cancer patients and to investigate the differences in QoL with respect to social and medical characteristics.
METHODSStructured questionnaires (EORTC QLQ-C30 and QLQ-LC13) were used in 106 consecutive lung cancer patients for data collection during January 1, 2002 to December 31, 2002. The t test and one-way analysis of variance (ANOVA) were used to compare the differences in QoL at a 5% level of significance.
RESULTSThe QoL of lung cancer patients were worse than reference value. The QoL of young, male and married patient groups was better than that in old, female and single groups. The QoL in patients with lower education or income was worse than that in patients with high education and income. Small cell lung cancer patients showed poorer QoL than non-small cell lung cancer patients did. The locally advanced or metastatic patients had worse QoL. All the kinds of treatments could worsen the QoL, however, the patients receiving surgical treatment had best QoL.
CONCLUSIONSThe results suggest that the establishment of QoL judgment standard in Chinese lung cancer patients might be useful for clinicians, researchers and policy-makers.
10.Main issues, causes and strategies for AIDS prevention and control in health care systems

Objective:To summarize the main issues, analyze the causes and offer a strategy for AIDS preven-tion and control for all types of healthcare facilities in the health care system. Methods:184 leaders and experts of tertiary-level healthcare facilities ( provinces, cities and counties) from 30 provinces who had been engaged in AIDS prevention and control for more than five years were studied through a questionnaire including the issues and causes in AIDS prevention and control. Results: According to the results of the respondents and experts’ demonstration, the main issues were identified, including a lack of human resources in the system of disease control facilities, a lack of relevant agencies involved in AIDS response, the difficulty for AIDS patients to receive surgery and follow-up, poor detection of HIV, shortcomings in medical insurance, and so on. Conclusion: Based on the analysis of issues and causes, the paper offers strategic solutions: Clearly define the responsibilities of various health institutions in AIDS prevention and control;promote the participation of primary health institutions in AIDS prevention and control; im-prove the medical service system for AIDS patients;improve AIDS medical security policies;reform the management system of antiretroviral therapy drugs;and strengthen technical support from disease control facilities.
