1.Treatment of sequential purification in rescuing acute grave intoxication:a report of 28 cases

Objective:To investigate the therapeutic effect of sequential blood purification on rescuing acute grave intoxication.Methods:Twenty eight patients with acute intoxication were treated by HP+CVVH during 2002 and compared them with thirty patients without hemopurgation from 1998 to 2002.Results:The group of hemopurgation was better than non-hemopurgation group in the time of revive,the rate of recovery and death,and the total dose of atropine.Conclusion:Hemopurgation has decisively effect on rescuing all kinds of intoxication with toxins and drugs.It could quickly ameliorate symptom,raise success rate and reduce mortality;Hp combining with CVVH are suitable for the patients with toxicsis who had cardiac and respiratory failure,brain edema,and dielectric disorder.

Objective:To investigate the effect of vitamin E supplementation on oxidative stress in patients with MHD(maintenance hemodialysis).Methods:58 patients with MHD and 20 healthy volunteers were involved in this study.The patients were administered 400mg/d of vitamin E for 6 weeks.The plasma levels of oxidative parameters including vitamin E(VitE),malondialdehyde(MDA),advanced oxidation protein product(AOPP),superoxide dismutase(SOD),activity of glutathione peroxidase(GSH-PX) were measured at the initial and the end of VitE supplementation respectively.Results:Plasma levels of AOPP and MDA(187.27?54.9?mol/L vs99.05?26.2?mol/L,7.74?2.17nmol/ml vs3.06?1.70nmol/ml)were significantly higher in patients with MHD compared with healthy controls(P

Pediatric Critical Care Medicine have developed rapidly and played a big role in the clinical treatment in China in recent years.But we axe now still in lack of unified documents as a guidline because of our late development and difference between regions.We should pay more efforts to formulate and perfect the standardization constructions of pediatric intensive care unit,with the help of The Construction and Management of Critical Care Medicine.In this article,we stated the present situation of development,operational mode,personnel and equipment configuration,monitoring and treatment technology,process specification and quality index in pediatric intensive care unit.And we also offerd some proposal about that.

The progress and development of extracorporeal life support (ECLS) is suprising and on-going,especially extracorporeal membrane oxygenation (ECMO) has become an accepted therapeutic modality for patients who have failed conventional therapy with respiratory and/or cardiac support.Blood purification developed from hemodialysis is designed to remove substances from the circulation and now includes hemodialysis,hemofiltration,immunoadsorption,continous blood purification and peritoneal dialysis.Plasma exchange and hemoperfusion play an important role in critically system diseases,i.e.,some kidney diseases,neurologic diseases,liver failure,metabolic diseases,connective tissue disease,infection diseases,intoxication and so on.

OBJECTIVE To improve the competition ability of the hospital,emphasis of the importance of hospital infection management is necessary and urgent.METHODS Our experience on improving culture level in hospital infection management was introduced.RESULTS We improved management efficiency by fluent information,altered the concept of value by bettered the management means,and brought the renovation idea by lifting the qualification of management staff.CONCLUSIONS Correctly making the most use of the unique principles of culture in hospital infection management could be benefit for creating a good atmosphere,establishing nice profession image,and setting up norm criterion,which could facilitate the controlling hospital infection.
8.Study on Hepatitis B Virus Infection Status in Plancetas of Hepatitis B Surface Antigen Positive Pregnant Women During Middle and Late Period of Pregnancy

Objective To investigate Hepatitis B Virus (HBV) infection status in placentas of Hepatitis B Surface Antigen (HBsAg) positive pregnant women during middle and late period of pregnancy. Methods One hundred and sixty seven placentas from HBsAg positive pregnant women were collected, including 158 term placentas and 9 aborted second trimester placentas. HBsAg and hepatitis B core antibody (HBcAg) in placental cytotypes were detected by immunohistochemistry. Results Of the 158 term placentas, HBV infection rates in decidual cells (DC), trophoblastic cells (TC), villous mesenchymal cells (VMC) and villous capillary endothelial cells (VCEC) were 66 46% (105/158), 58 23% (92/158), 27 22% (43/158) and 12 66% (20/158), respectively. The HBV infection rates gradually decreased from maternal side to fetal side of placentas. Both HBsAg and HBcAg were mainly located in the cytoplasm of infected cells. The positive cells were mostly in focal distribution. One out of 9 aborted second trimester placentas was infected. Conclusions HBV infection was found in different cells of term placentas. The positive rates for HBsAg and HBcAg in placentas were gradually decreased from maternal side to fetal side. The positive rate was low in aborted second trimester placentas.

Background and purpose:MicroRNAs (miRNAs, miR) are directly involved in cancer initiation, progression and metastasis. Alterations of miRNAs expression in cancer tissue may be reflected in circulation. We attempted to investigate the expression and clinical signiifcance of plasma miR-221 in patients with small cell lung cancer (SCLC). The plasma and tissues levels of miR-221 in 51 SCLC patients and 20 controls were evaluated and compared among various clinicopathological characteristics. Methods: Confirmation of higher miR-221 levels in primary SCLC tissues than normal lung tissues. Evaluation of plasma miR-221 concentrations by comparing results from 51 consecutive SCLC patients and 20 healthy volunteers. Evaluation of the assay for monitoring tumour dynamics in SCLC patients. Results:Expression of miR-221 was signiifcantly higher in SCLC tissues than in para-carcinoma tissues and normal lung tissues. Plasma miR-221 concentrations were signiifcantly higher in SCLC patients than those in normal people (P<0.05). The expression of plasma miR-221 was not associated with gender, and age (P>0.05), correlated with significantly chemosensitivity, overall survival and clinical stage (P<0.05). Cox regression analysis indicated that plasma miR-221 expression and disease stage were found to be significantly independent prognostic factors for the SCLC patients. Conclusion: Plasma miR-221 could be a useful biomarker for cancer detection, monitoring tumour dynamics and predicting malignant outcomes in SCLC patients, and may contribute to clinical decision making in treatments.
