1.Stress ulcer survey of anti-ulcerative drug use in patients undergoing arthroscopic operation

Objective To investigate anti-ulcerative drug use in patients undergoing arthroscopic operation against stress ulcer. Methosds A total of 297 patients undergoing arthroscopic operation during Jan. to Aug. 2013 in our hospital were recruited and retrospectively analyzed with Excellspreadsheet. Results Among 297 patients in the study, ninety-four patients (31.6%) used AUD before operation, most (94.9%) administered by intravenous, and drug utilization index was 1.35. Ninety-eight point nine patients used proton pump inhibitors (PPIs) in which Lansoprazole was prescribed most. Conclusion This study highlights the overuse of PPIs in patients undergoing arthroscopic operation.
2.Clinical rules for acupoint selection and prescription composition in treatment of polycystic ovary syndrome with acupuncture.

OBJECTIVETo summarize and analyze rules for acupoint selection and prescription composition in clinical literature regarding acupuncture for polycystic ovary syndrome (PCOS).
METHODSCHKD, VIP and Wan-fang databases were retrieved. The clinical literature data included in the study was collected. The descriptive statistical analysis was conducted on the main acupoints, main meridians of main acupoints, distribution of main acupoints, application of special points and rules for acupoint composition.
RESULTSFifty-two articles were included, involving 55 main acupoints with a total frequency of 375. The most frequent acupoints of acupuncture for PCOS were Sanyinjiao (SP 6), Guanyuan (CV 4), Zigong (EX-CA 1), Zhongji (CV 3) and Qihai (CV 6). The meridians of main acupoitns were conception vessel, spleen meridian of foot-taiyin and stomach meridian of foot-yangming. The main acupoints were distributed in the lower abdomen, lower limbs and back. In the special points, the use of front-mu points, five-shu points and back-shu points was more frequent. The prescription was usually consisted of 5 to 7 acupoints, (6. 9 ± 3. 6) acupoints in average.
CONCLUSIONIn the modern treatment of acupuncture for PCOS, Sanyinjiao (SP 6), Guanyuan (CV 4), Zigong (EX-CA 1), Zhongii (CV 3) and Qihai (CV 6) are most used. With the theories of meridian and zang-fu as essential references, the acupoint selection is based on disease differentiation and meridian circulation. Additionally, the methods for selecting the adjacent points and the distant points are adopted.

3.Application of two-dimensional and color Doppler ultrasonography and B-blood flow to quickly screen venous diseases of lower extremities in late pregnant women

Objective To evaluate the value of two-dimensional (2D) and color Doppler ultrasonography and B-blood flow (B-flow) in quick screening for venous diseases of the lower extremities in late pregnant women. Methods Sixty late pregnant women excluding other-cause blood vessel diseases were recruited during September 2007 to January 2008 and underwent 2D and color Doppler ultrasonography and B-blood flow examinations for diameter and blood flow velocity of the veins in their both lower extremities during prenatal and postnatal periods to record intravenous condition, activities of the venous valve and venous valvular regurgitation in detail. Results Diameters of the veins in beth lower extremities of the late pregnant women widened significantly during prenatal period, as compared to that in postnatal period [(11.5±1.5) mm vs. (8.4±1.0) mm, t =7.14, P <0.01] and their blood flow velocity slowed down significantly in prenatal period than that in postnatal period [(11.5±4.0) cm/s vs. (29.7±6.9) cm/s, t =-15.74, P <0.01]. Spontaneous enhancement phenomenon could be shown in their veins in 57 of 60 (95%) pregnant women by 2D and color Doppler ultrasonography, but could not be shown by 2D and color Doppler uhrasonography in the poplitel veins in 21 pregnant women (35%) or in the small and great saphenous veins in 39 pregnant women (65%), which should be activated by B-flow. Venous valvular regurgitation occurred in the great saphenous veins and the superficial femoral veins in 24 percent of pregnant women during prenatal period, and small venous valve bag thrombus formed in 20 percent of the superficial femoral veins and 10 percent of the great saphenous veins, but disappeared two weeks after delivery, without formation of pulmonary embolism. Conclusion Tow-dimentional and color Doppler ultrasonography and B-blood flow examinations can be used in late pregnant women to quickly screen for venous diseases in their lower extremities.
4.Correlation between reduction quality of femoral neck fracture and femoral head necrosis evaluated by three-dimensional measurement method

BACKGROUND:Reduction quality is essential for the healing and function of limb. Traditional X-ray and CT scans have some limitationsin quantitatively and visualy observing the reduction quality of the remnant fracture in three-dimensional displacements. Mimics have advantages in bone rapid modeling and three-dimensional measurement, which offers the chance in quantifying the reduction quality of the displacement in three-dimensional methods.
OBJECTIVE:To measure residual displacement in femoral neck fracture patients with satisfactory reduction by using three-dimensional measurement method, and to evaluate the association between residual displacement and femoral head necrosis.
METHODS:We enroled 141 patients with femoral neck fracture, who were treated in the Third Department of Joint, Affiliated Hospital of Logistics University of People’s Armed Police Force from January 2012 to December 2013. Al patients were divided into undisplaced fracture group (n=44; Garden1&2) and displaced fracture group (n=97; Garden3&4). Based on DICOM data of CT files, three-dimensional models were constructed in each patient. Three parameters were measured: displacement of femoral head’s center, fossa capitis femoris and rotational displacement. Femoral head necrosis was folowed up. The correlation between residual displacement and femoral head necrosis was explored.
RESULTS AND CONCLUSION:(1) The duration of folow-up in those patients ranged from 25 to 43 months. Femoral head necrosis occurred in 7 cases from the undisplaced fracture group and 21 cases from the displaced fracture group. (2) The three parameters (displacement of femoral head’s center, fossa capitis femoris and rotational displacement) were respectively (7.44±2.84) mm, (11.75±4.12) mm and (17.68±8.99)° in the undisplaced group, and (6.51±3.75) mm, (9.34±5.27) mm and (14.67±7.43)° in the displaced group. Except displacement of femoral head’s center, significant differences in the other two parameters were detected between the two groups (P< 0.05). (3) These findings suggest that poor reduction quality is the main factor causing femoral head necrosis. X ray is insufficient to describe the spatial shift and the angle, and has limitations in assessing the reductionquality of femoral neck fracture. Three-dimensional measurement has certain advantages to assess quality of fracture reduction.
5.Antipsychotics combined use pattern among schizophrenic inpatients in Tangshan region

Objective To investigate rates and clinical associates of combined antipsychotic therapy in hospitalized patients with schizophrenia during 1986 ~ 2006. Methods Hospital charts of schizophrenic inpatients who discharged from six psychiatric hospitals in Tangshan in 1986,1996,2001, and 2006 were reviewed. Demographic and clinical character and prescription information at time of discharged of patients' was coded by trained investigators with a special designed form. Results The rates of combination antipsychotic therapy in the four years mentioned above were 13.2%, 18. 1% ,32.7%, and 28.9% ,respectively and the difference was statistically significant( x2 = 76.431, P < 0.01; linear by linear x2 = 25. 542, P < 0.01 ). Combined antipsychotic therapy in schizophrenic inpatients in Tangshan region show a increasing trend. There is significant difference of rate of combined antipsychotic therapy between hospitals. Combined antipsychotic therapy were associated with duration of illness,numbers of prior psychiatric hospital admission,hospital from which patients discharged,and discharged year.
6.Correlation between micro-bleeds and hemorrhagic transformation after ischemic stroke

Objective To investigate the correlation between micro?bleeds and hemorrhagic transformation( HT ) after ischemic stroke. Methods One hundred and forty?five cases patients with acute ischemic stroke who were admitted to Luhe Hospital of Beijing Affiliated to Capital Medical University from 2009 to 2014 were retrospective analyzed. The MBs T2??weighted gradient?echo MRI was performed within 48 hours after admission to examine if there was a micro?bleeds,and the incidence of HT was assessed using follow?up brain MRI or CT in 2 weeks after admission. There were 125 patients with stroke were in without HT group and 20 patients were in HT group. Results There were no significant differences in terms of MBs ( P=0. 170 ) , gender( P=0. 671 ) , age ( P=0. 528 ) , hypertension ( P=0. 597 ) , diabetes ( P=0. 571 ) , hyperlipidaemia ( P=0. 772),smoking(P=0. 672),history of stroke(P=0. 469),coagulation function(P=0. 527),antiplatelet(P=0. 642),anticoagulation(P=0. 611) in HT group and without HT group. Atrial fibrillation((45%(9/20) vs. 22%( 28/125 ) , P = 0. 034 ) , NIHSS score ( 10. 9 ± 0. 5 ) points vs. ( 7. 8 ± 1. 2 ) points, P<0. 001 ) , thrombolysis(20% (4/20) vs.5% (6/125),P = 0.015) have significant differences in two groups.In multivariate logistic regression analysis,atrial fibrillation( OR=2. 421,95%CI 1. 823-3. 256,P=0. 012) ,NIHSS score( OR=9. 303, 95%CI 3. 094-27. 970, P<0. 001 ) were independent predictors for HT after ischemic stroke. Conclusion There is no relationship between MBs and HT after ischemic stroke. Atrial fibrillation,the severe of stroke are independent predictors for HT after ischemic stroke.
7.The relationship between brachial-ankle pulse wave velocity and heart rate variability, cardiac function indexes in essential hypertension

Objective To investigate the relationship between brachial- ankle pulse wave velocity (baPWV) and heart rate variability (HRV), cardiac function in patients with essential hypertension.Methods One hundred and eighty patients with essential hypertensive from January 2014 to August 2016 were collected.The patients were divided into elevated group(baPWV≥1 400 cm/s,100 cases) and normal group (baPWV < 1 400 cm/s, 80 cases) according to the baPWV value. The general data, heart function and HRV of the 2 groups were compared. Results The standard deviation of all normal-to-normal(SDNN),standard deviation of all 5-min mean NN intervals(SDANN)and proportion of number of pairs of adjacent normal-to-normal intervals by more than 50 ms(PNN50%)in elevated group were significantly lower than those in normal group: (116.3 ± 17.0) ms vs. (135.8 ± 19.4) ms, (96.0 ± 21.4) ms vs. (120.0 ± 25.1) ms and (9.3 ± 3.1)% vs. (12.0 ± 4.1)%,and there were statistical differences (P<0.01); there was no statistical difference in root mean square of successive differences (RMSSD)between 2 groups(P>0.05);the left ventricular ejection fraction(LVEF)in elevated group was significantly lower than that in normal group:(57.2 ± 7.0)% vs.(63.0 ± 8.2)%,and there was statistical difference (P<0.01); there were no statistical differences in left ventricular end-diastolic dimension (LVDd) and left ventricular end systolic dimension (LVSd) between 2 groups (P>0.05); the interventricular septal thickness (IVST) in elevated group was significantly higher than that in normal group:(11.4 ± 2.5)mm vs.(9.5 ± 1.4)mm,and there was statistical difference(P<0.01).In patients with essential hypertension,there was a significant negative correlation between baPWV and SDNN,SDANN, PNN50%,LVEF(P<0.01),and there was a significant positive correlation between baPWV and IVST (P<0.05).Conclusions The baPWV measured in patients with essential hypertension can effectively reflect the HRV and cardiac function of the patients.
8. Characteristics of serum cholesterol in sepsis patients and its correlation with prognosis

Objective:
To analyze the characteristics of changes in serum cholesterol level in sepsis patients, and to explore its correlation with prognosis.
Methods:
A retrospective analysis was conducted. Two hundred and six patients with sepsis admitted in the Third People′s Hospital of Dalian from February 2015 to February 2019 were enrolled (observation group), and 206 patients without sepsis hospitalized in the same period were served as control group. The basic clinical data of the two groups were collected, and the results of total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C) and biochemical parameters were compared. The patients with sepsis were divided into death group and survival group, and risk factors influencing the prognosis of patients with sepsis were analyzed with Logistic regression analysis.

9.Prognostic value and correlation analysis of NLR and LMR in patients with advanced non-small cell lung cancer

Objective:To investigate the relationship between neutrophil/lymphocyte ratio (NLR), lymphocyte/monocyte (LMR) ratio and prognosis in patients with advanced non-small cell lung cancer (NSCLC).Methods:Total 96 NSCLC patients with stage Ⅲ B and above confirmed by pathological examination in our hospital from March 2014 to 2016 were selected as the research objects. According to the 24-month follow-up results, the patients were divided into survival group (30 cases) and death group (66 cases). The NLR and LMR values of the two groups before chemotherapy were compared. The critical value of NLR and LMR in predicting the prognosis of patients was analyzed by drawing receiver operating curve (ROC); The clinicopathological characteristics of NSCLC patients with different levels of NLR and LMR were analyzed, and Cox proportional hazard regression model was used to analyze the effect of NLR and LMR on the prognosis of patients.Results:The NLR value of survival group was lower than that of death group ( P<0.05), and there was no significant difference between survival group and death group in LMR value ( P>0.05). The area under curve (AUC) value of NLR was 0.785, and the corresponding critical value was 3.19; the AUC value of LMR was 0.536, and the corresponding critical value was 3.66. There were significant difference in tumor node metastasis (TNM) stage and differentiation between NSCLC patients with different NLR values (NLR≥3.19 group and NLR<3.19 group) ( P<0.05), but there were no significant differences in age, gender, smoking, pathological type and lesion location ( P>0.05). There was no significant difference in TNM stage, differentiation degree, age, gender, smoking, pathological type and lesion location in NSCLC patients with different LMR values (LMR ≥ 3.66 group and LMR<3.66 group) ( P>0.05). The increase of TNM stage, differentiation degree and NRL were independent risk factors of poor prognosis in patients with advanced NSCLC ( P<0.05). Conclusions:Increased NLR values in peripheral blood of patients with advanced NSCLC may increase the risk of poor prognosis in patients.
10.Preliminary establishment of a novel localization method for sacral nerve foramen puncturing

【Objective】 To establish a novel localization method for sacral nerve foramen puncture by analyzing the characteristic of sacral nerve foramen trying to help improve the success rate of sacral foramen puncture. 【Methods】 Clinical data and sacrococcyx CT and three-dimensional reconstruction imaging data of 158 patients who received sacral nerve modulation (SNM) during Jan.2019 and Aug.2022 in our hospital were retrospectively analyzed.The distance between inferior margin of articulatio sacroiliaca and the internal edge of the 3rd neural foramen (D1), and the distance between the internal edge of the 3rd neural foramen and sacral midline (D2) were measured, and the ratio of D1 and D2 was calculated for precise intraoperative positioning. The measurement data characteristic and puncture results were analyzed. 【Results】 A total of 89 males and 69 females were included, with an average age of (49.0±16.9) years.The average D1, D2, and D1/D2 were (29.6±4.9) mm, (13.8±3.2) mm, and (2.2±0.6), respectively.Female patients had greater D1 \[(30.7±5.5) mm vs.(28.7±4.2) mm, P=0.010\] and D1/D2 \[(2.4±0.7) vs.(2.1±0.5), P=0.001\] than male patients.Compared with adults, the adolescents had smaller D1 \[(29.8±4.7) mm vs.(25.7±5.4) mm, P=0.006\].After precise intraoperative positioning using this positioning method, 158 patients were successfully punctured, 152 (96.20%) had excellent intraoperative neural response, 2 (1.27%) had good response, and 4 (2.53%) had average response. 【Conclusion】 The surface projection of sacral nerve was approximately at the middle-inner 1/3 of the inferior margin of articulatio sacroiliaca to sacral midline.Through measuring distance from inferior margin of articulatio sacroiliacato sacral midline via X-ray, sacral nerve situation could be performed in surface, which might be a secure way to accessorily situate electrode implantation site for sacral neuromodulation.
