1.Analysis about reason of congenital infantile coxa vara being misdiagnosed as infantile dislocation of hip join

[Objective]To analyze reason of congenital infantile coxa vara to be misdiagnosed as infantile dislocation of hip joint.[Method]There were 6 patients(6 hip join)congenital infantile coxa vara being misdiagnosed as infantile dislocation of hip join in 54 patients(73 hip join)and treated by expectant treatment as infantile dislocation of hip join.The curative effect of 6 patients were not good,after serious medical examination,careful reading image data,paying attention to curative effect,6 patients were confirmed to be diagnosed as infantile dislocation of hip join and not infantile dislocation of hip join.[Result]The features for diagaosis of congenital infantile coxa vara were:the big greater trochanter of femur and its position moving up,short spacious neck of femur,spacious medial blank in hip joint,superposed neck of femur and lesser trochanter of femur,praeter propter exponent of acetabulum,acceptable succession of Shenton’s line,enlarged angle of femoral head epiphysis,late center of ossification,incorporated abnormity such as breves femoral bone,bowing of femoral bone,abortive clavicle and bones of cranium,equines.[Conclusion]The second center of ossification of femoral head is appeared 6~9 monthes after birth,and it is difficult to discriminate congenital infantile coxa vara and infantile dislocation of hip join before this.Early to realize symptom,physical sign and character of image of congenital infantile coxa vara is significant for correct diagnosis and can avoid misdiagnosis and mistreatment.
2.Mental disorder and its risk factors in first degree relatives of patients with type 2 diabetes mellitus

Objective To investigate the incidence and the risk factors of anxiety or depression and in the first degree relatives (FDRs) of patients with type 2 diabetes mellitus ( T2DM ). Methods A total of 102 T2DM patients who visited our hospital from May to July 2010 and their FDRs completed SAS,SDS,and life satisfaction evaluation. The medical history of T2DM patients was collected and compared with their FDRs with or without mental disorders. The correlations of risk factors with anxiety or depression were analyzed, t test or X2 test were used for statistical analysis and a P value less than 0. 05 was considered statistically significant. Results The incidence of anxiety or depression of FDRs was 31.4% and 17.6%,respectively. The life satisfaction score of FDRs with mental disorders was significantly decreased ( t = 4. 26 and 4. 09, P<0. 05 ). Fasting glucose level of T2DM patients with anxious or depressive FDRs was significantly increased ( t = -2. 48 and -2. 15, P<0. 05 ). Higher medical cost and multiple treatment strategies were positively correlated with SAS and SDS score of the FDRs. Conclusion Higher incidence of mental disorders and lower satisfaction scores could be found in the FDRs of T2DM patients. Better control of blood glucose and reduced treatment cost and strategies may be useful in improving metal condition and life satisfaction of the FDRs of T2DM patients.
3.Factors influencing the efficacy of endoscopic mucosal resection for early colorectal cancer

Objective To investigate the clinical risk factors ot endoscopic mucosal resection (EMR) for early colorectal cancer.Methods The clinical data of 140 patients with early colorectal cancer who received EMR at the Changzheng Hospital of Second Military Medical University from January 2005 to June 2012 were retrospectively analyzed.EMR was applied for patients with early colorectal cancer with diameter lesser than 20 mm (protrusive lesions) or 5 mm (depressed lesions).If the diameter of the early colorectal cancer was above 3 cm and can not be resected at one time,endoscopic piecemeal mucosal resection was adopted.Clinicopathological factors including gender,age,tumor location,number,diameter,shape,resection type,depth of tumor invasion,tissue differentiation,lymphatic or vascular invasion,excision degree and curability were retrospectively analyzed.All the patients were divided into the complete cure group and the incomplete cure group.Univariate and multivariate analysis were done using the chi-square test and Logistic regression model,respectively.Results Of the 143 lesions,en-bloc resection was done in 125 lesions,and piecemeal resection in 18 lesions.There were 90 intramucosal carcinomas and 53 submucosal carcinomas.The numbers of high,moderate and low differentiated colorectal cancer were 96,37 and 10.Lymphatic or vascular invasion was found in 8 lesions.The numbers of R0,R1,R2 and Rx resection were 118,15,8 and 2.Two patients with Rx resection were excluded from this study.There were 98 patients (101 lesions) in the complete cure group,and 40 patients (40 lesions) in the incomplete cure group.The results of univariate analysis showed that tumor location,shape and resection type were the risk factors influencing the efficacy of EMR for early colorectal cancer (x2=7.523,16.280,4.156,P <0.05).The results of multivariate analysis showed that tumor located at the proximal colon,superficial protrusive and depressed type and piecemeal resection were the independently risk factors influencing the efficacy of EMR (OR =7.219,6.115,5.211,P <0.05).Conclusions EMR is not easy to achieve complete cure for early colorectal cancer with superficial protrusive and depressed type or located at the proximal colon.Endoscopic piecemeal mucosal resection should try to avoid.
4.Research on the respiratory frequency changes of 4DCT reconstructed image

Objective To study the impact of respiratory frequency and amplitude of patients with the thoracic or abdomen tumor on 4DCT' s reconstructed image.Methods 75 thoracic or abdomen tumor patients who performed a scanning with belly belt-type respiratory gating on 4DCT between November 2012 to March 2013 in our hospital were retrieved.Reviewed their respiratory frequency,amplitude and fault distance of 4DCT' s reconstructed image and analyzed the correlation fault distance of the reconstructed image.The correlation analysis was used by bivariate Spearman method.Results Among these reconstructed images,fault distance more than 6 mm could be seen in 11 cases,fault distance ranging from 3 mm to 6 mm could be found in 46 cases,and distance less than 3 mm could be detected in 18 case.Respiratory frequency correlated significantly with the amplitude (rs =0.369,P =0.000);the respiratory rate had a tight association with the fault distance of the reconstructed image (r,=0.273,P=0.018);and respiratory amplitude was associated obviously with the fault distance of the reconstructed image (r,=0.696,P=0.000).Conclusions Both 4DCT reconstructed image and respiratory amplitude are significantly correlated to the continuity of the respiratory frequency.The smaller the respiratory frequence and amplitude change are,the better continuity will be.
5.Clinical study of the three mesial root canals of mandibular first molars

Objective:To investigate the clinical incidence of the three mesial root canals of mandibular first molars. Methods: Incidence and morphology of the three mesial root canals of mandibular first molars were determined by probing the fissure or grooves between the mesiobuccal and mesiolingual root canal orifices and X-ray analysis. Results: Of the 222 mandibular first molars, 13 of the molars had three mesial root canals. The incidence rate was 5.85 %. Conclusion: Although the incidence rate of the three mesial root canals of mandibular first molars was very low, the dental operative microscope was helpful for the diagnosis.
6.The effects of Xinjia-Liangfu formula combined 5-Fu on survivin and caspase-3 expression of gastric cancer xenografts in nude mice

Objective To observe the effect of Xinjia-Liangfu(XJLF)formula on human gastric cancer(SGC-7901)tumor growth in nude mice and its impact on survivinand Caspase-3 expression in tumor tissue. Methods The animal model of SGC-7901 xenografted nude mice was established, and all the mice were randomly divided into 6 groups, namely model control group, 5-Fu group, XJLF formula high-dose group, XJLF formula medium-dose group , XJLF formula low-dose group and drug combination group(XJLFformula medium-dose and 5-Fu). After 10 days of continuous treatment, tumor inhibition rate was calculated and the expression of Survivin and Caspase-3 was detected by immunohistochemistry techniques. Results The tumor inhibition rates of 5-Fu group, XJLF formula high-dose group, XJLF formula medium-dose group, XJLF formula low-dose group and drug combination group were 55.03%, 56.38%, 41.23%, 23.09%and 60.28%respectively, and the tumor inhibition rate of drug combination group was significantly increased compared to the model control group(P<0.05), 5-Fu group(P<0.05)and XJLF high-dose group(P<0.05) Meanwhile, the Caspase-3 expression was significantly up-regulated and the Survivin expression was significantly down-regulated in drug combination group(Caspase-3 was 77 539.74±50 912.5, Suvivin was 35 709.1±10404.4)compared to the model control group(Caspase-3 was 18 464.71±7 537.01,Suvivin was 63 449.4±50 498.8), P<0.05 or 0.01, 5-Fu groupCaspase-3 was (47 198.89±10 751.2), Suvivin was(37 016.8±6 024.4), P<0.05 or 0.01and XJLF formula high-dose groupCaspase-3was (44213.57±7041.6), Suvivin was (38811.1±7313.0)respectively. Conclusion The tumor inhibition rate of drug combination group was higher than XJLF formula high-dose group and 5-Fu group. Also combination group hadstronger effect compared to model control group in terms of down-regulation of Caspase-3 expression and up-regulation of Survivin expression, which indicates a benefit ofXJLF and 5-Fu combination treatment.
7.A study on risk factors for falling down in elderly people of rural areas in Laizhou City

Objective To study the status and risk factors for falling down in elderly people in rural areas, and to provide evidences for preventions. Methods A total of 1 003 farmers aged 60 years old and over were surveyed to study the falls occurred during the previous 12 months before March 2001 in the elderly in a town of Laizhou City by cluster sampling method. Results The average incidence of falling down was 22.6%, 15.9% for men and 28.4% for women, respectively. The risk factors of falls included gender, age, single-living, falling history in the previous year, at least combined two kinds of chronic diseases, manifesting at least two symptoms, gait abnormalities, as well as complaining poor physical status and poorer general conditions than the previous year. Less social actions was negatively associated with the falls. Conclusions The falling in the elderly walkers results from an outcome of combination of multifactors. Since the complex pathogenesis, it should be prevented by a combination of several methods.
8.Preparation and characterization of microbubbles loaded with hydrogen sulfide.

OBJECTIVETo prepare a phospholipid-coated microbubble loaded with hydrogen sulfide (HSMB) and evaluate its physicochemical and acoustic properties.
METHODSHydrogen sulfide and perfluoropropane were mixed at the ratios of 4:0, 3:1, 2:2, 1:3, and 0:4 to prepare hydrogen sulfide-loaded microbubbles (termed HSMB4:0, HSMB3:1, HSMB2:2, HSMB1:3, and HSMB0:4, respectively). The microbubble concentration and diameter were investigated and their stability were evaluated. The optimal ratio of hydrogen sulfide and perfluoropropane was determined according to the changes of microbubble concentration. The changes of dissolved hydrogen sulfide and concentration of the microbubbles were investigated after exposure to ultrasound, and their acoustic enhancement effects in the myocardium and kidney were observed after intravenous injection in rats.
RESULTSHSMBs were milky in color and spherical in shape without aggregations. The concentrations of HSMB4:0 and HSMB3:1 were lower than that of HSMB2:2 and decreased with time. HSMB2:2, HSMB1:3 and HSMB0:4 showed comparable concentrations and were stable within 72 h. After exposure to ultrasound, the concentration of HSMB2:2 decreased while the dissolved hydrogen sulfide increased significantly. Intravenous injection of HSMB2:2 produced a satisfactory contrast-enhancing effect in the myocardium and kidney of rats.
CONCLUSIONHSMB prepared with the hydrogen sulfide to perfluoropropane ratio of 2:2 has excellent contrast-enhancing effect and is capable of carrying and releasing hydrogen sulfide upon ultrasound exposure to potentially allow visual site-specific delivery of hydrogen sulfide.

9.Systemic Reactions to Dust Mite Subcutaneous Immunotherapy: A 3-Year Follow-up Study.

PURPOSE: The incidence of allergen specific immunotherapy-related systemic reactions (SRs) varies among different studies, and many factors are likely to contribute to SRs. This study aims to investigate the incidence, characteristics, and risk factors of SRs to standardize dust mite-specific subcutaneous immunotherapy (SCIT) in Central China. METHODS: All patients receiving standardized dust mites (100-100,000 SQ-U/mL; Alutard SQ, Hørsholn, Denmark) immunotherapy were followed up. Recorded data included demographics, diagnosis, patient status, pulmonary function testing results before and after each injection, allergen dosage, and details of SRs. RESULTS: From June 2011 to August 2014, a total of 208 patients received 4,369 injections; 27 (13.0%) patients experienced 48 (1.1%) systemic reactions. Most of the SRs were grade 2 reactions (n=30, 62.5%), followed by grade 1 (n=11, 22.9%), grade 3 (n=7, 14.6%), and no fatal reactions occurred. Forty-six SRs (95.8%) occurred within 30 minutes. Higher SR rates were associated with high concentration extracts (100,000 SQ-U/mL), injections with concomitant local reactions (LRs), children, asthma and high sensitivity (skin prick test 3+/4+ and/or sIgE≥17.5 kUA/L) (P<0.05). The estimated odds of SRs increased in children (OR=6.57; 95% CI: 1.88-22.97, P=0.003), asthmatic patients (OR=4.10; 95% CI: 1.72-9.80, P=0.002), and injections with LRs (OR=2.41; 95% CI: 1.33-4.36, P=0.004). CONCLUSIONS: The incidence of SRs to dust mite SCIT was low, and multiple factors were associated with the increased incidence of SRs. Children, asthmatics and patients with concomitant LR may be prone to develop SRs.

10.Survey of the basic situation of Chinese radiotherapists in 2021

Objective:To investigate and analyze the basic occupational situation of Chinese radiotherapists and to provide references for the development and construction of the radiation therapist team in China.Methods:Between May 7, 2021 and June 15, 2021, this survey was conducted using a self-designed questionnaire, which was distributed and collected in the form of online survey to conduct a statistical analysis of the basic situation of radiotherapists in 30 provincial-level administrative regions in China.Results:A total of 2 928 valid questionnaires were collected from 30 provincial-level administrative regions. The ratio of male to female and average age of the participants were 69∶31 and 35 years (18-65), respectively. According to the survey result, 84.7% of the participants came from Tertiary A-level hospitals. In terms of educational background, the participants with doctoral, master, undergraduate, junior-college, and technical-secondary degrees accounted for 0.2%, 6.9%, 76.6%, 14.9%, and 1.4%, respectively. In terms of title, full senior technologists, associate senior technologist, technologist-in-charge, technologist, and technician in the participants accounted for 0.5%, 5.8%, 32.5%, 52.2%, and 8.2%, respectively. The participants with years of employment of 0-, 5-, 10-, 20-y accounted for 33.74%, 25.79%, 25.51%, and 14.96%, respectively. The majors of the participants included medical imaging/imaging technology (45.9%), clinical medicine (19.1%), and biomedical engineering (11.9%).Conclusions:Chinese radiotherapists tend to be younger and are rapidly increasing in number. Employers should attach importance to the career planningand continuing education of radiotherapists to improve their technical skills, in order to fully satisfy the demand for radiotherapists in the new era.
