1.Efficacy and prognosis of different surgical approach in siewert Ⅱ and Ⅲ gastroesophageal junction adenocarcinoma

Objective To investigate the surgical approach of Siewert Ⅱ and Ⅲ gastroesophageal junction adenocarcinoma.Methods A total of 148 cases with Siewert Ⅱ,Ⅲ type patients were prospectively studied.The patients were divided into two groups,including transthoracic approach group (58 cases) and transabdominal approach group(90 cases).The results of surgery were compared.Patients were followed up for 2 years and survival rate were compared.Results In transthoracic approach group and transabdominal approach group,operative time ((329.5 ± 84.3) min vs.(202.4± 84.5) min,t =15.431,P < 0.001),the positive rate margin stump (8.62% vs.1.11%,x2 =5.763,P =0.012),pleural effusion (13.79% vs.2.22%,x2 =10.462,P <0.001) and pulmonary infection rate (15.52% vs 1.11%,x2 =12.574,P< 0.001) were significantly higher than transabdominal approach group,and number of lymph node dissection ((16.7 ± 4.3) vs.(22.6± 5.5),t =6.321,P =0.004) was significantly less than transabdominal approach group.In incidence of blood loss,tumor diameter,anastomotic leakage (or bleeding) and discharge time,there was no significant difference (P >0.05).One-year survival rate of transthoracic approach group was 73.24%,and 2-year survival rate was 53.43%.Oneyear survival rate of transabdominal approach group was 78.42%,and 2-year survival rate was 57.51%.Survival rate of two groups showed no significant difference (P =0.453,0.311).Conclusion Transabdominal surgical approach in Siewert Ⅱ,Ⅲ patients is better than transthoracic approach,can better carry out abdominal lymph node dissection,does not destroy the integrity of the chest,and avoid the occurrence of related complications.
2.Clinical Significance of Lymphocyte Subsets Change in Breast Cancer Patients before and after Treatment

Objective To investigate the changes of lymphocyte subsets before and after the treatment with operation and chemotherapy in patients with breast cancer.Methods Using cytometer to measure lymphocyte subsets of CD3+,CD4+, CD8+,CD4+/CD8+ and NK of 25 breast cancer patients before and after treatment.The all 25 patients were treated with operation and chemotherapy after pathological diagnosis.The article compared the figures of T cell subsets before and after treatment.Results Compared with before operation and chemotherapy,the CD8+ statistically decreased after 1~3 months of treatment (t=2.57),and NK statistically increased after 3~6 (t=-2.23).After 6 months of treatment,the patients’ CD3+(t=-2.25),CD4+(t=-2.07)were statistically lower and NK (t=-3.56)was statistically higher.There was no significant difference for other items.Conclusion Lymphocyte subsets test against breast cancer patients can reflect immuni-ty level at a certain level during treatment.But how to explain the figures in different stage against treatment effectiveness and prognosis need further research.
3.Comparative Study on HPV Infection and Hypotype Distribution between Ankang and Yan'an Area of Shaanxi Province

Objective Compare the HPV infection characters between women in different geographic area of Ankang (south of Shaanxi) and Yanan (north of Shaanxi),to provide evidence based reference for cervical cancer control.Methods PCR amplification in vitro combined reverse dot-blot hybridization assay were used to analyze HPV genotypes of 7 912 cases from Ankang City Center Hospital,and 3 543 cases from Yanan Affiliated Hospital of Yanan Medical University from June 2012 to July 2016,and compared HPV infection rate,age and hypotype distribution of the cases.Results The HPV infection rate of Ankang area was 21.75% (1 721/7 912),and Yanan area was 41.41% (1 402/1 721).The percentage of HPV positive women with high-risk hypotype was 81.46% (1 402 1 721) in Ankang and 86.16% (1 264/1 467) in Yanan,with low-risk hypotype was 11.04% (190/1 721) in Ankang and 7.23% (106/1 467) in Yanan,with high and low risk hypotype was 7.50% (129 147) in Ankang and 6.61% (97/1 467) in Yanan.The women more than 60 year old were higher HPV prevalence age group.The HPV infection rate of this age group was 30.41% (104/342) in Ankang,and 50.00 % (60/120) in Yanan.Yanan's HPV infection rate was obviously higher than Ankang.The popular HPV hypotypes in Yanan were same with that in Ankang.The most popular high-risk hypotypes were HPV16,58 and 52,the most popular low-risk hypotypes were 6,11 and 81.Conclusion The HPV infection rate were rather high in all age group.The most popular HPV hypotypes were same between Ankang and Yanan area.But Yanan's HPV infection rate was obviously higher than Ankang (P< 0.01).The causes need further explore.
5.Effects of thymopeptides on cell immune function of patients with malignant tumor under radiotherapy

Objective:To study effects of large dose thymopeptides on T cell subpopulations of patients with malignant tumor under radiotherapy.Methods:Fifty one patients with malignant tumor under radiotherapy were divided into 2 groups with 100 mg and 200 mg thymopeptides respectively.The patients we re given thymopeptides,100 mg/d or 200 mg/d,iv, for 10 days.The positive percent ages of CD4,CD8,CD25 and CD56 in T cells of peripheral blood before and after thymopeptide treatment were determined by flow cytometry.Results:The positive percentages of CD4 and CD25 in T cells of peripheral bl ood after 100 mg/d thymopeptide treatment were significantly higher than those befor e thymopeptide treatment (P<0.05),while those of CD4,CD8,CD25 and CD56 in T cells of peripheral blood after 200 mg/d thymopeptide treatment all increased significantly (P<0.05 or P<0.01). Conclusion:These results suggest that large dose of thymopeptides can increase i mmune function of patients with malignant tumor under radiotherapy,and the curat ive effect of 200 mg/d thymopeptides is better.
6.Effects of Jagged1 on hippocampal radial glial cells’ proliferation and neuronal differentiation

Objective To investigate the effect of Jagged1 on hippocampal radial glial cells (RGCs) proliferation and neuronal differentiation in vitro.Methods Hippocampal RGCs were cultured in vitro, the agonist Jagged1 and(or) inhibitor DAPT of Notch signaling were added into the culture medium , and then the cells were divided into control group , Jagged1 group, Jagged1 combined with DAPT group and DAPT group .CCK-8 regent was used to detect cells ’ vitality;immunofluorescent was used to detect the number of BLBP /Ki67 double labeled cells and differentiated microtubule associated protein-2(MAP-2) positive cells.Results Cell vitality in Jagged1 group was obviously higher than that of the other groups .The number of BLBP/Ki67 double labeled cells and differentiated MAP-2 positive cells were more than other groups.Conclusion Jagged1 promotes the proliferation and neuronal differentiation of hippocampal RGCs in vitro.
7.Dynamic expression of Lhx8 in nerve growth factor induced hippocampal neuroregeneration

Objective To investigate the relationship between the nerve growth factor ( NGF ) induced hippocampal neuroregeneration and homeobox gene Lhx 8.Methods Seventy-two SD rats were divided into control group , transected group, NGF group, transected combined with NGF group after right fimbria-fornix transection and NGF intracerebroventricular injection . Real-time PCR and Western blotting were applied to detect the gene and protein expression of Lhx8 in each group.The choline acetyltransferase ( ChAT)/Lhx8 double labeled cells in subgranular zone ( SGZ) of hippocampus in each group were detected by immunofluorescence .Results The expression of Lhx8 gene and protein in the transected , NGF group and especially in the transected combined with NGF group was obviously higher than in the control group .The number of ChAT/Lhx8 double labeled cells in the NGF group and the transected combined with NGF group was obviously more than in the control group and transected group . Conclusion The hippocampal neuroregeneration which induced by NGF intracerebroventricular injection was associated with the higher expression of Lhx8.
8.Sequence analysis for the 3' end of genomes from enterovirus 71 isolated in Beijing during 2008-2009

Objective To investigate the sequences of the 3' end of genomes from enterovirus 71 isolated from pediatric patients with different symptoms in Beijing during 2008-2009.Methods Clinical specimens were collected from pediatric patients suffering from hand,foot and mouth disease(HFMD)and/or with neurological complications who visited the affiliated Children's Hospital during the epidemic seasons of 2008 and 2009 in Beijing.The samples were inoculated into the Vero cell line,and the virus isolates were further identified by nested reverse transcription-nested PCR(RT-nPCR)assay using universal enterovirus primers,type specific primers for EV71 and CA16.The 3' end of genomes(including 3D and 3' UTR regions)from 10 EV71 strains derived from various clinical presentations were amplified and sequenced.Results Analysis of the nucleotide sequences of the amplified fragments showed that the 3' end of genomes of 10 EV71 isolates include the 3D region of 1386 nucleotides(nt)encoding the 3D polymerase(3Dpol,462 amino acids),the terminator codon TGA and 3' UTR of 81 nt.Nucleotide sequence identities among 3D regions from these 10 EV71 isolates were in the range of 95.8%-99.6%,while the nucleotide sequence identities for 3' UTR were 96.3%-100%.The majority of nucleotide changes were located at the third codon positions which caused silent mutations,thus the amino acid sequence changes of 3Dpol among those 10 EV71isolates were scanty.The residues 140 and 263 which were R and 1 were substituted by K and V,respectively in 3 of 4 neurovirulent strains,whereas only 1 of 6 strains from mild cases had these 2 amino acid changes.The sequences of the 3D and 3' UTR regions of 10 EV71 isolates were compared to the representative strains of known genotypes from the GenBank.The nucleotide and amino acid sequences of 10 EV71 isolates in the 3D region exhibited highest homology to the subgenotype C4 of EV71(92.7%-94.2%and 96.8%-97.6%.respectively).However,3' UTR of 10 EV71 isolates shared the highest nucleotide identity with CA16/G10(88.9%-91.4%).The phylogenetic analysis based on the 3D regions demonstrated that 10 EV71 isolates had the closest genetic relationship with the representative isolate of CA sub-genotype of EV71 and shared more homology with CA16/G10 than other known genotypes of EV71.Conclusion Genetic analysis of the 3' end of genomes from 10 EV71 strains indicated that the 3' end of genome may play a role in the evolution of EV71.

10.The Correlation of ACE I/D Gene Polymorphisms with the Antihypertensive Efficacy of Irbesartan Among Hypertensive Patients

Objective To analyze the correlationship of ACE I/D gene polymerphisms with the anti-hypertensive efficacy of irbesartan among essential hypertension (EH) patients in Yunnan han people.Methods One hundred EH patients hypertension were treated with irbesartan 150mg once daily for 4 weeks,and anti-hypertensive efficacy were monitored during the treatment.Results There was a significant difference between anti-hypertensive efficacy of irbesartan among different genotype groups,DD group >ID group >Ⅱ group (P < 0.05) Conclusions There is different therapy responsiveness in diffenent genotypes.ACE I/D polymorphism may be an important hereditary factor that impacts the efficacy of irbesartan.
