1.A contrast between the cognitive function of patients with simple leukoaraeosis and subcortical arterioselerotic encephalopathy

BACKGROUND: Cognitive function of simple leukoaraeosis (LA) and subcortical arterioselerotic encephalopathy (SAE) is a part of research in cerebrovascular neuropsychology, however, there is no contrast study on cognitive function between the two diseases. OBJECTIVE: To investigate the different features in cognitive function among patients with simple leukoaraeosis, subcortical arterioselerotic encephalopathy and simple leukoaraeosis combined with cerebral infarction. DESIGN: Randomized controlled observation SETTING: Department of Neurological Medicine, Qilu Hospital, ShandongUniversity. PARTICIPANTS: A total of 91 cases of patients with cerebrovasculardisease diagnosed in Department of Neurological Medicine of Qilu Hospital of Shandong University from March 1997 to May 2000 were selected. All the patients participated in the observation voluntarily. They were divided into 3 groups according to the type of disease, with 27 cases in simple leukoaraeosis group, 33 cases in subcortical arterioselerotic encephalopathygroup and 31 cases in simple leukoaraeosis + cerebral infarction group.Additionally, 30 healthy cases were selected as control group. All the subjects in the above groups participated in the observation voluntarily. METHODS: Assessment on cognitive function and memory ability was conducted on the subjects in each group, and contrast analysis was performed. Mini-mental state examination was used for detecting cognitive function. Patients whose score was less than 17, 20, 22, 23 respectively according to different educational degree (illiteracy, primary school, middle school, university), would be diagnosed as dementia. Clinical me mory scale A edited by Psychology Institute of Chinese Academy of Medical Sciences was used for detecting memory ability including associative learning, directive memory, recognition of nonsense figure, image free recall and portrait characteristics associative recall. Conversed the above 5 items of detecting results to scores of scales and calculated out the memory quotients. MAIN OUTCOME MEASURES: State of cognitive disorder and scoresof mini-mental state examination and clinical memory scale of subjects in each group. RESULTS: All the 91 cases of patients with cerebrovascular disease and 30 healthy control cases entered results analysis without any drop out. ① Comparison of the state of cognitive disorder among subjects in each group: There were 21 cases of mild cognitive disorder (77.8%) in simpleleukoaraeosis group, 8 cases of moderate cognitive disorder (24.2%) and 25 cases of dementia (75.8%) in subcortical arterioselerotic encephalopathy group, 6 cases of moderate cognitive disorder (19.4%) and 24 cases of dementia (77.4%) in simple leukoaraeosis + cerebral infarction group. ② Comparison of the scores of mini-mental state examination among and clinical memory scale: Scores of the two scales in simple leukoaraeosis group, subcortical arterioselerotic encephalopathy group and simple leukoaraeosis + cerebral infarction group were significantly lower than those in healthy control group (t=2.14-3.81, P < 0.05-0.01). The scores in subcortical arterioselerotic encephalopathy group and simple leukoaraeosis+ cerebral infarction group were significantly lower than those in simple leukoaraeosis group (t=2.13-3.37, P < 0.05-0.01).CONCLUSION: ① The cognitive impairment in simple leukoaraeosis is mainly mild cognitive disorder. ② The cognitive impairments in subcortical arterioselerotic encephalopathy and simple leukoaraeosis + cerebral infarction are mainly moderate cognitive disorder and dementia which are obviously severer than mild cognitive disorder. 2.Clinical study of Paroxetine combined with psychotherapy in treating post-stroke emotion disorder

Objective To investigate the incidence of the post-stroke emotion disorder, the relationship between post-stroke emotion disorder and the location of stroke, and the effects of Paroxetine combined with psychotherapy in treating post-stroke emotion disorder. Methods 181 patients with acute brain stroke were assessed by SDS and SAS. Of these patients, 54 patients met the criteria of depression and anxiety after acute brain stroke. These 54 patients were divided into 2 groups randomly. They were treated with routine anti-stroke medication, routine anti-stroke medication with Paroxetine combined with psychotherapy for 6 weeks, respectively. The rating scales such as SSS, HAMD, HAMA, TESS and so on were used to evaluate the improvement of symptoms and side effects. Results According to the evaluation of SDS or SAS, there were 81 patients (44.75%) with post-stroke emotion disorder. The rate of post-stroke depression with anxiety was 66.67%. 3.Strategy of postoperative TR -Band tourniquet to stop bleedingin coronary interventional treatment

Objective To observe the different methods of hemostasis and comfort of the TR -Band com-pression hemostasis device after coronary intervention and to find the best strategy for TR -Band to stop the bleeding. Methods 150 patients with coronary artery disease were randomly divided into three groups according to the digital table method.The initial time of TR -Band compression hemostasis of group A was 3h,(once per hour,2mL per time,total compression time was 10h),while that of group B was 2h(once in every two hours,3mL per time,total com-pression time was 8h),and that of group C was 1h(once per hour,2mL per time,total compression time was 6 h). Thumb oxygen saturation,complications and comfort level in the three groups of patients were observed and compared. Results The differences between the three groups of oppression hemostasis before and after 2 h thumb oxygen saturation were normal,the three groups had no statistical significance (P >0.05).lesions,hematoma,bruising and radial artery occlusion occurred rate of group A were respectively 4,3,5,2 cases,those of group B were 3,4,6, 1 cases,and those of group C were 2,5,6,0 cases.Lesions,hematoma,bruising,radial artery occlusion of local com-plication incidence of the three groups showed no statistical significance (P >0.05).The comfort level (numbness, swelling,pain)of the three groups had statistically significant differences (Fnumbness =8.215,Fswel ing =7.657,Fpain =7.985,all P <0.01).Conclusion TR -Band compression hemostasis device can achieve effective hemostasis in the treatment of coronary artery intervention.And the reasonable adjustment of different bleeding time,compression time and the amount of bleeding can affect the comfort of patients.
4.A preliminary study on abnormal expression of long noncoding RNA CTD-2012K14.6 in placentas and its role in inducing macrosomia

Objective To explore the role of long noncoding RNA ( lncRNA) CTD-2012K14. 6 in the development of gestational diabetes mellitus (GDM) related macrosomia. Methods The quantitative real-time PCR ( qRT-PCR) was performed to measure the expression of CTD-2012K14.6 in placentas of women with or without GDM, and the quantity of CTD-2012K14. 6 expression and its association with fetal weights were analyzed; Bioinformatic analysis was performed to predict the downstream molecules. CTD-2012K14. 6 over-expressing lentiviral and siRNA was constructed in human trophoblastic cell line HTR-8/SVneo cells, qRT-PCR and Western blot (WB) were used to invest its effect in modulating the expression of downstream molecules. Results The expression of CTD-2012K14.6 in GDM placentas was significantly higher than that in normal controls (1.70 ± 0.63 vs 1.00 ± 0.56,t=3.68,P<0.01), and positively correlated with fetal weight (r=0.8501, P<0.01); on-line analysis showed that CTD-2012K14.6 was located at chr16:67,549,214-67,563,958, which was located in the intron of CCCTC-binding factor( CTCF); Up-regulating CTD-2012K14.6 could significantly reduce the expression of CTCF mRNA and protein, and increase the expression of insulin-like growth factor-Ⅱ( IGF-Ⅱ) mRNA and protein, while down-regulating CTD-2012K14.6 could significantly increase the expression of CTCF mRNA and protein, and reduce the expression of IGF-ⅡmRNA and protein. Conclusion The CTD-2012K14. 6 may play an important role in the pathogenesis of GDM related macrosomia by upregulating the expression of CTCF and IGF-Ⅱ.

6.Clinical Study on Effect of Baohe Granules on Chronic Heart Failure Patients with TCM Syndrome and Gastrointestinal Hormone Secretion

This article was aimed to study the effect of Baohe granules for patients with chronic heart failure (CHF) of traditional Chinese medicine (TCM) syndrome and the effect of gastrointestinal hormone secretion. Patients with CHF were used as the object of study. A total of 80 selected patients were divided into 2 groups. The control group was treated with western medicine anti-heart failure therapy. The treatment group was treated with the combination of Baohe granules. All patients were compared in aspects of TCM efficacy, symptom score and serum gastrin, motilin secretion after two-week medication. The results showed that both treatments can significantly improve the TCM syn-drome and symptom total score of CHF patients. And the effect of the treatment group with Baohe granules was bet-ter. The TCM single symptom integral was also obviously improved in the treatment group combined with Baohe granules, which included heart palpitations, shortness of breath, abdominal distension, and loss of appetite. The GAS and MTL levels of CHF patients were significantly increased in the treatment group. It was concluded that the treat-ment combined with Baohe granules can improve CHF patients with TCM syndromes, symptoms and gastrointestinal hormone secretion. Thus, it contributed to the stability of the disease condition. It can slow the disease progression and improve prognosis. So it is worth using widely in the clinical practice.
7.The diagnosis, treatment and clinicopathologic analysis on gastrointestinal neuroendocrine carcinomas

Objective To investigate the clinicopathologic features of gastrointestinal neuroendocrine carcinomas. MethodsThe diagnosis and treatment results of 45 cases were studied, and clinicopathologic features and immunohistochemical expressions of NSE, Syn and CgA were detected.ResultsMicroscopically carcinomas were divided into three types: type Ⅰ(25 cases), type Ⅱ(10 cases) and type Ⅲ(10 cases). The histologic categories were correlated with lymph node metastasis significantly( P0.05). The 5-year survival rate for type Ⅰ, type Ⅱ and type Ⅲ was 70%, 65% and 52%, respectively.ConclusionsThe combination of NSE, Syn and CgA immunohistochemical stainnig is necessary for the diagnosis of gastrointestinal neuroendocrine carcinomas. The histologic classification is coincident with the requirement of clinical treatment and prognosis.
8.Measurement of optic tracts in normal Chinese adults of the Han nationality based on the high-resolution MRI

Objective To explore the morphological characteristics of optic tracts in healthy Chinese Han adults on the high-resolution MRI and fill the database of Chinese standard brain with morphological data of optic tracts.Methods Cerebral MRI scans with T1 WI 3D MPRAGE sequence of 1000 healthy Chinese volunteers from 15 hospitals were divided into five stages, ranging in age from 18 to 70.With the technique of multi-baseline, structure and morphology of optic tracts were displayed optimally on the images with multiplanar reconstruction.Data were measured as following: transverse distance of the cisternal optic tract (TD1) and peri-crural optic tract (TD2), length from the cisternal optic tract to the peri-crural optic tract (L) ,angle between optic tract(AOT) and height of optic tract from its first segment to plane of anterior commissure (H) including H1, H2, H3, H4 and HS.The measurements of optic tracts between sexualities and among age groups were compared by anasis of covariance; those among five age groups were compared pairwisedly by least significant difference analysis (LSD); and the differences of measurements between left and right optic tracts were analyzed using paired t test.Results (1) Comparisons of optic tract structures between male and female: the mean optic tract length of male [(11.69±1.45),(11.56±1.44) mm] was significant longer than that of female [(10.58±1.29),(10.40±1.34) mm] (F=22.236, 29.703, P=0.000); the mean H1 of male [(2.56±0.28),(2.60±0.29) mm] and female [(2.57±0.31 ), (2.63±0.32) mm] were significantly different ( F =11.130,7.805, P = 0.000, 0.005).No significant differences of the other measurements were found between male and female ( P > 0.05 ).(2) Comparisons among age groups: among 5 age groups, TD1 of both sides [left TD1 :(4.64±0.51 ), (4.64±0.57), (4.55±0.58), (4.39±0.53), (4.36±0.58)mm;right TD1 :(4.84±0.53) ,(4.80±0.60), (4.77±0.65), (4.60±0.59), (4.57±0.59) mm] and the right TD2[(3.33±0.45),(3.34±0.41),(3.33±0.36),(3.23±0.38),(3.23±0.39) mm] had statistical differences ( F = 3.458, 2.735, 4.711, P = 0.008,0.028, 0.001 ).The LSD analysis found that the 50 years old group was the watershed with significant differences ( P < 0.05 ).The results also showed that the L of both sides had statistical differences among 5 age groups ( F = 14.510,14.532, P = 0.000).The LSD analysis found that the 60 years old group was the watershed with significant differences (P <0.05).Left TD2 and the H1-H5 of both sides had no significant differences among age groups ( P >0.05).(3) Comparisons of the measurements between left and right optic tracts: TD1 of bilateral optic tracts were (4.52±0.57)and(4.72±0.60) mm respectively; H1 of bilateral optic tracts were (2.56±0.30)and (2.61±0.30) mm respectively; H2 of bilateral optic tracts were (2.66±0.30)and (2.70±0.30) mm respectively; and L of bilateral optic tracts were ( 11.14±1.47 ) and ( 10.98±1.50 )mm respectively.There were significant differences in these measurements between left and right optic tracts( t =12.460, - 6.013,5.595,4.784, P = 0.000 ), while there were no significant differences in TD2, H3, H4 and H5( P > 0.05 ).Conclusions With high-resolution MR1 and 3D reconstruction, optic tract can be displayed clearly and measured accurately.There are definite differences in anterior segments of optic tracts between sexualities, sides and among ages in normal Chinese Han adults, while the posterior segments of optic tracts keep stable.Normal reference values of optic tracts in Chinese Han adults are provided to clinical practices and scientific researches, which are valuable for building of Chinese standard brain.
9.Measuring the volume of the fourth ventricle in healthy Chinese adults of the Han nationality on the high-resolution MRI

Objective To explore the normal range of the fourth ventricle volume of Chinese adults of the Han nationality and provide morphological data for the construction of database for Chinese Standard Brain.Methods This is a clinical multi-center study.One thousand Chinese healthy volunteers (age range= 18 to 70) recruited from 15 hospitals were divided into 5 groups, i.e., Group A (age range = 18 to 30),B (age range =31 to 40), C (age range =41 to 50), D (age range =51 to 60), and E (age range =61 to 70).Each group contained 100 males and 100 females.All of the volunteers were scanned by MR using T1 weighted three-dimensional magnetization prepared rapid acquisition gradient echo sequence.After three dimension data reconstruction, the volumes of the fourth ventricle were measured at sagittal view by automatic trace of Midobl.2 combined with manual outlining.The difference of volumes of the fourth ventricle between male and female were analyzed by independent sample t test, and among age groups by ANOVA.Pearson's correlation coeffcient was used to characterize the relationship between volumes of the fourth ventricle and age.Results The fourth ventricle volumes of Group A-E were (2.1±0.9), (2.1±0.8), (2.2±0.8), (2.1±1.0) and (2.4±0.8) ml respectively for male; those for female were(2.0±0.7), (1.9±0.6), (18±0.6), (1.9±0.7) and (2.0±0.6) ml respectively.The fourth ventricle volumes of males were significantly larger than those of females ( t = 5.573, P =0.000 ) ; there were no significant differences among the female groups ( F = 1.788, P = 0.130 ) ; there were significant differences among the male groups ( F = 2.639, P = 0.033 ) and multiple comparison found that the 60 years old was the watershed with significant difference ( P < 0.05 ).Correlation between the change of males' volumes and the ages was not strong (r = 0.119, P = 0.008 ), and the females' volumes did not correlated with their ages ( r = 0.041,P = 0.360 ).Conclusion There are gender differences in the fourth ventricle volumes of normal Chinese adults, and changes of the fourth ventricle volume with aging are different between males and females.
10.Study on quality control of Minuo Hair Solution

Objective To improve the quality standard of Minuo Hair Solution and to ensure product quality .Methods Thin layer chromatography (TLC) and UV-visible spectroscopy (UV-Vis) method was used for the qualitative identification of Angelicasinensis ,Paeoniae Radix Rubra ,Dictamni Cortex and minoxidil respectively .The Quantitive determination of mi-noxidil was completed by first order derivative spectroscopy .Results The spots in TLC were clear without inference .And the same color spots appeared in the corresponding position of references and reference drugs .The UV-Vis spectral absorption of minoxidil were in (230 ± 3) ,(262 ± 3) ,(286 ± 3) nm .The linear range of minoxidil was obtained between 6 .2 and 18 .6 μg/ml (r=0 .999 7) .The average recovery was 99 .4% (RSD=0 .22% ) .Conclusion The method is simple ,accurate and reproduci-ble ,and can be effectively used to control the quality of Minuo Hair Solution .
