Chinese Journal of Pathophysiology 2000;0(12):-
AIM:To investigate the effect of advanced glycation end products on inflammation in cultured cardiomyocytes.METHODS: Primary cardiomyocytes were isolated from Sprague-Dawley neonatal (1 to 2 days old) rats ventricles. The insulin resistant cardiomyocyte model was established. Neonatal rat ventricular myocytes were exposed to AGEs for 24 hours. TNF-? mRNA and PPAR-? mRNA expressions were determined by RT-PCR. Activation of NF-?B in the cells was examined by using immunocytochemistry. The ultrastructure of the cells was detected by transmission electron microscope.RESULTS: The exprssion of TNF-? mRNA and the activation of NF-?B increased, the expression of PPAR-? mRNA decreased compared with control group (P

The QCC campaign at the emergency center was themed as minimizing incidence of risky transport of critically ill patients, which aimed at continued quality improvement by means of conditions review, problem analysis, measures making, and procedure improvement. The incidence of risky transports dropped from 26% to 0.9% in six months.Another promising outcome was a sharp betterment of clinical nurse management. It has proved that QCC can effectively downsize risky transports of critically ill patients, making it a functional means to improve nursing quality.

Objective To investigate the epidemiology of drug-resistant tuberculosis (TB) in five rural counties of eastern China and analyze the biological, demographic and social risk factors. Methods Subjects of this study were all the diagnosed TB patients registered in the five study sites in Shandong Province,Jiangsu Province and Zhejiang Province during one year of 2008- 2009.Questionnaire interview was conducted in all the subjects to acquire the socio-demographic and clinical information.Sputum samples were collected for culturing and isolating of Mycobacterium tuberculosis (M.TB) strains. All the M.TB isolates were further tested for the susceptibility to first-line drugs including rifampin, isoniazid, ethambutol and strepomycin by proportion method.Mantel-Haenszel chi-square test,Fisher's exact test,ANOVA and nonconditional Logistic regression modeling were applied for data analysis. Results Among the total 380 M.TB isolates,105 were resistant to at least one of the first-line drugs. The total drug resistant TB prevalence was 27.6%.Multidrug-resistant tuberculosis (MDR-TB) was observed in 8.4% of newly treated TB patients, whereas it was 23.3%in previously treated TB patients. After adjusted by county, gender and age of the subjects, multivariate analysis showed that previous treatment history (OR=3.900,95%CI: 1.737-8.704),tuberculosis cavity (OR - 1.987,95 % CI: 1.001 - 3.942) were independent factors influencing the occurrence of MDR-TB.Conclusions The prevalence of drug resistant TB in rural area of eastern China is relatively low compared with the average level in China, while it is still higher than the global average level. The present study highlights that TB patients with previous treatment history, cavitaryTB are correlated with MDR-TB,and elderly patients are at high risk of MDR-TB.

Based on a description of the evolution, main reform practices and effects of public hospital payment system reforms, this paper focused on analyzing the existing problems found in the reforms.Such problems namely include lack of scientific and reasonable design of payment standards, lack of systemic and coordinated system reforms, and failure to fully mobilize the enthusiasm of medical workers to participate in the reforms,as well as other underlying causes.Based on such suggestions were put forward to promote public hospital payment system reforms,as negotiation of the payment standard between purchasers and providers, combined approaches in innovation in payment methods, advancing systemic payment system reform, and establishing a transmission mechanism of incentive to the medical personnel.

The paper presented the payment system theory of the primary health care service in China, the current status of the health service system, and analyzed main challenges for the time being.Based on such studies, the authors made the following policy proposals. The first is adjustment of the fiscal payment method of the government to such institutions in line with the classification guidance principle; the second is to shift the payment method of primary public health services to the post payment of fee-for-service; the third is reform of the performance-based salary system to link payment to medical workers directly with the amount and quality of their services; the fourth is a set of result-oriented performance appraisal indicators, with rising proportion of performance pay; the fifth is to integrate the outpatient clinic fund covered by medical insurance and the primary public health service fund, into a capitation payment.
