1.Quality of life of elderly person in aged-care at home and influencing factors in Shanghai Minhang district

ObjectiveTo understand the quality of life and influencing factors of elderly person in agedcare at home.MethodsThe elderly person in aged-care at home in 3 towns and a street of Shanghai Minhang District were enrolled by stratified cluster sampling.To investigate quality of life for elderly person by adopting SF-36 form.Meanwhile,to find out the principal influencing factors on quality of life by adopting both single-factor and multiple-factor analysis.ResultsThe score of life quality of senior citizen in local was as following:43.63 ± 30.04 (Physical Function),55.57 ± 48.96 (Role Physical),81.75 ± 20.49 (Bodily Pain),42.07 ± 18.84 ( General Health),58.57 ± 18.96 (Vitality),66.94 ± 26.79 ( Social Function),73.33 ± 43.33 ( Role Emotional),70.92 ± 19.02 ( Mental Health),61.60 ± 21.06 ( Weighted average).There were two aspects of the factors that influencing elderly persons'the quality of life:disease-related (coronary heart disease,cerebrovascular accident) and non-disease-related ( whether to go out for a walk every day,loneliness,living space,gender and on the current life satisfaction etc).ConclusionQuality of life of elderly person in aged-care at home is relatively low,special in the general health and physical function dimensions.The controllable factors are diseases,whether to go out for a walk every day,loneliness,on the current life satisfaction etc.
2.Activities of Daily Living and Related Factors of the Elderly Homecare Receivers

Objective To understand the activities of daily living and related factors of the elderly homecare receivers. Methods The elderlyhomecare receivers in 3 towns and 1 street of Shanghai Minhang District were enrolled by stratified cluster sampling, 830 elderly personswere investigated with Activities of Daily Living Scale (ADL). Results The score of the elderly in local areas was as following: ADLscore was (25.59 ± 11.16), Physical Self-maintenance Scale (PSMS) score was (8.74 ± 4.29), Instrumental Activity of Daily Living Scale(IADL) score was (16.85±7.53). PSMS impairment or dysfunction accounted for 55.19%, and IADL accounted for 79.64%. ADL score >16points had 593 people, accounting for 71.45%, ≥22 points had 458 people, accounting for 55.18%. Conclusion The elderly homecare receivershave relatively poor ability of daily living. Related factors were whether to go out for a walk every day, monthly income, cerebrovascularaccident, medical insurance, educational level, diabetes, the current life satisfaction, disease and loss of spouse.
3.Brain protection of adenosine preconditioning in infants undergoing radical correction for fallot tetrad

Objective To investigate the brain protection of adenosine preconditioning in infants with tetralngy of Fallot (TOF) undergoing open heart surgery. Methods 120 patients with TOF undergoing open heart surgery were randomly divided into two groups: adenosine group and control group. Total dosage of 1.5 mg/kg adenosine was intravenously dripped in the adenosine group before the cardiopulmonary bypass. Serum S-100β and neurone specific enolase (NSE) were measured before the cardipulminary bypass, 30 min and 12 hours after the aorta off clamping. Results At 30 min and 12 hour after opening the aorta, the S-100β[(0. 346±0. 162)μg/L vs. (0.717±0.260) μg/L and (0.541±0.272) μg/L vs. (1.140±0.567) μg/L,P <0.001 ]and NSE[(10.43±5.54) μg/L vs. (15.75±7.96)μg/Land (15.54±7. 98)μg/L vs. (23.67±8.00)μg/L,P<0.001]were both lower in adenosine group than that in control group. Conclusion Adenosine preconditioning can reduce the S-100β and NES in infants with TOF undergoing open heart surgery and has brain protection.
4.Clinical Application of ~1H-MRS in Breast Tumors

Objective To investigate the clinical values and influencing factors of1H magnetic resonance spectroscopy(1H-MRS) in breast tumors.Methods Thirty-eight patients with breast tumors confirmed pathologically,including 20 cases of breast cancers and 18 cases of benign tumors examined by dynamic contrast-enhanced MRI and1H-MRS were studied retrospectively in comparison with pathology.The value of1H-MRS in detecting breast cancers was evaluated.Results On1H-MRS,11 breast cancers had increased choline peaks and 3 benign tumors presented increased choline peaks.The sensitivity,specificity and accuracy were 55.0%,83.3%and 68.4% respectively by using choline peak to detect breast cancers.Conclusion 1H-MRS has high diagnostic specificity for breast cancer.The low sensitivity of Choline to be detected rate in breast cancer is mainly dues to the lesions and technique factors.
5.A retrospective study on 256 cases of assessment for body impairment of the injured children and teenagers in road traffic accidents

Objective Review the epidemiology, injuries and assessments for body impairment on the injured children and teenagers in road traffic accidents (RTAs). Methods The data of 256 cases of assessment for body impairment of the injured children and teenagers in RATs was studied retro spectively. Results Boys were injured more than girls, mainly 6-18 years of age. RATs occurred mostly in 10-12a.m and 4-6p.m. The types of vehicles were mostly car and wagon. The commonest accidents were injury of pedestrians. The sites of injures were mainly head and extremities, the commonest injures were fracture, contusion and wound, the degree of internal organ injuries was relatively slight, the fracture of lower extremities was far more often than the upper extremities. Most of the injuries were assessed to be of medial injury. In most of the cases the rate of disability was evaluated to be lighe (33.2%), in which Ⅸ , Ⅹ grade were the commonest. Conclusion The criterion of assessment for body impairment of the injured in RTAs should be broadened properly, some special items for children and teenagers should be added.
6.An investigation in 370 cases of child neglect

BACKGROUND: Child neglect is one of the most important aspects of child abuse and has become a serious problem influencing children's development all over the world. Therefore, it is necessary to nderstand the current conditions and influential factors of child neglect and establish effective evaluation methods in China.OBJECTIVE: To investigate the current conditions and influential factors of urban child neglect in Suzhou city.DESIGN: A comparative analysis of sampling survey.SETTING: Department of Children's Health Care, Soochow University Affiliated Children's Hospital.PARTICIPANTS: This study was conducted in one public kindergarten and two private kindergartens of urban areas in Suzhou city between March and October 2003. Totally 370 children (196 boys and 174 girls) aged between 4 to 6 years old without obvious physical and mental diseases were voluntarily investigated, and they had been living with the person that answered the questionnaire regarding to child neglect in China in the recent 1 year. METHODS: Questionnaire regarding to child neglect in China was sent to each volunteer.he questionnaire evaluates the current conditions of child neglect by 20 items of 5 aspects including physical care (buy fruit for child, consider the nutritional need of child, care about the body height and body mass of child), emotional care (chat with child, play and tour with child, encourage child to do the things whatever they can, listen to what the child feels, introduce the child to the guests when they come to your home, let him or her stop immediately when child cries, care about the activities in kindergarten), education care (buy books for child, tell stories to child, recognize and correct the bad behavior of child, answer the questions asked by child seriously), safety care (protections against fire and electricity at home, warn child against matches, knives and other sharp things, teach child how to cross the road) and health care (prepare the common medicine for child at home, urge child to brush teeth, correct the bad posture). The volunteers were asked to answer each question objectively according to the conditions of child within the recent 1 year, and each question should be answered by selecting only one choice (never, occasionally, usually, always), and anyone should not be omitted. Meanwhile,the educational background and occupation of the parents were also investigated.MAIN OUTCOME MEASURES: The general status of child neglect and the effects of different family educational background and occupation of parents on child neglect were observed. RESULTS: Totally 370 questionnaires were sent out, and 370 qualified ones were recollected, the effective rate was 100%. ① In Suzhou city, the children were given better regards and good care in the aspects of education, medicine and safety, but neglects on children's physical and mental care still exists in a few families. ② Families of different educational background could think highly of the medical care, safety and psychical care of children, but neglects on education and emotional development more commonly existed in the families with the educational background under university. ③ All the families with different occupation could think highly of the medical care, safety and physical care of the children, but education and emotional development were better in the cadre families than in the ordinary families.CONCLUSION: In Suzhou city, the children are given better regards and good care in the aspects of education, medicine and safety, but some bad manifestations of child neglect exist in education and emotion, which are associated with the educational background and occupation of the parents.Further studies about the evaluation, prevention and intervention are needed for child neglect in China.
7.Comparisons of the life quality and its influential factors among the senior citizens in communities, nursing homes and elderly wards in Shanghai Minhang district

Objective To understand the life quality of elderly person in communities, nursing homes and elderly wards, and to find out the related influencing factors for providing references for policy-makers to improve the life quality of the elderly. Methods The senior citizens in 9 communities, 3 villages, 12 nursing homes and 5 elderly wards in Shanghai Minhang District were enrolled by stratified cluster sampling. And the life quality and its influential factors among the inhabitants in communities, nursing homes and elderly wards were compared by adopting Short-Form 36 (SF-36) questionnaire. Results Regarding to the life qualities of senior citizens, community showed that physical functioning was (71. 9 ± 23. 3)scores, role physical was (73. 6 ± 43. 0) scores, bodily pain was (80. 2±20. 3)scores, general health was (53. 2± 19. 1)scores, vitality was (70. 4± 15. 7)scores, social function was ( 83. 8 ± 20. 2) scores, role emotional was ( 81. 1 ± 37. 2 ) scores, mental health was (78. 1 ± 15. 4) scores, weighted average was (74. 1 ± 17. 5) scores. Nursing home showed that physical functioning was (42. 1 ±30. 7) scores, role physical was (57. 9 ±48. 1) scores, bodily pain was (78.0±23.0)scores, general health was (50. 2± 19. 5)scores, vitality was (66. 0± 19. 5)scores, social function was (70. 8 ± 26. 3) scores, role emotional was (66. 8 ± 45. 7) scores, mental health was (73. 6 ± 19. 6) scores, weighted average was (63. 2 ± 21. 0) scores. Elderly ward showed that physical functioning was (20. 4 ± 26. 0) scores, role physical was (36. 1 ± 47. 0) scores, bodily pain was (65. 4±29. 5)scores, general health was (43. 1±17. 1)scores, vitality was (59. 0± 18. 0)scores, social function was (57. 9 ± 25. 3) scores, role emotional was (49. 5 ± 48. 7) scores, mental health was (66. 2 ± 18. 3) scores, weighted average was (49. 7 ±18. 3) scores. The major influential factors on the life quality were gender, education, marriage, profession, age, disease, residence, entertainment, physical exercise for community; Age, disease, residence, entertainment, physical exercise for nursing home; Age, etc for elderly ward. Conclusions The influences of demographic factors on life quality of the senior citizens in communities are more significant than in nursing home and elderly wards. Variations on the degree of satisfaction (in residence, entertainment, health care and transportation) and the situation of physical exercise have made vast difference in the life quality of the senior citizens both in communities and nursing homes.
8.Comparison of the effects of bismuth-containing quadruple and standard triple therapy on Hp positive peptic ulcer

Objective To study the changes of serum NO, TGF-β1, IL-10 and IL-17 Levels and study of the efficacy of Bismuth - containing quadruple and standard triple therapy for patients with Hp positive peptic ulcer. Methods 109 cases of peptic ulcer treated in our hospital were retrospectively selected, and Hp test was positive. According to different dosing regimen, the patients were divided into two groups. There were 54 patients in the control group, using standard triple therapy, and the regimen was lansoprazole, amoxicillin, and metronidazole. 55 patients in the study group were treated with bismuth-based western therapy with bismuth potassium citrate, rabeprazole, amoxicillin, and furazolidone. Two weeks after treatment were regarded as the observation period, and the levels of serum NO, TGF-β1, IL-10 and IL-17 in the two groups were compared before and after treatment. The effective rate of ulcer healing, Hp clearance rate and incidence of adverse reactions were compared. Results The level of serum NO, TGF-β1, IL-10 and IL-17 was significantly lower than that in the control group (P <0.05). The effective rates of ulcer healing and Hp clearance in the study group were significantly higher than those in the control group (P <0.05). There was no significant differences between the study group and the control group in the incidence of adverse reactions. Conclusion Western medicine with bismuth in the treatment of Hp peptic ulcer patients, the serum NO, TGF-β1, IL-10 and IL-17 levels of indicators have significantly improved, and has obvious advantages in clinical treatment, and it is a reasonable dosing regimen of high efficiency and low recurrence.
9.Optimization of Solid Dispersion Prescription of the Extract of Fructus Cnidii

Objective To obtain the optimal solid dispersion prescription of the extract of Fructus Cnidii.Methods The release rate t63.2 was used as index and uniform design was used to investigate the effect of PEG,PVP,and Poloxamer on the release of the solid dispersion of the extract of Fructus Cnidii.Results PVP,PEG can delay the release of osthole from the solid dispersion,and tween-80 can promote its release.Conclusion The optimal solid dispersion prescription consists of tween-80(10.2 %),Poloxamer(79.8 %) and the extract(10 %).
10.Determination of Ephedrine Hydrochloride in Antiviral Oral Liquid by HPLC

OBJECTIVE: To establish a HPLC method for the determination of the content of Ephedrine hydrochloride in Antiviral oral liquid. METHODS: The determination was performed on Diamonsil C18(250mm?4.6mm,5?m). The mobile phase consisted of acetonitrile-0.1% phosphate (6∶94) with flow rate of 1.0mL?min-1. The detection wavelength was 207nm and the column temperature was set at room temperature. RESULTS: The linear range of Ephedrine hydrochloride was 5.8~29.0?g (r=0.999 6), with average recovery at 98.13%,RSD=1.4%(n=9). CONCLUSION: This method was highly specific, sensitive and reproducible, and applicable for the quality control of Antiviral oral liquid.
