1.Activatable ultrasmall gold nanorods for theranostics in breast cancer cells

Objective To develop a fluorescence signal activatable multifunctional molecular probe with theranostics function through combining ultrasmall gold nanorods(UGNRs) with fluorescein,and to evaluate its therapeutic effect on photothermal therapy (PTT) in breast cancer cells.Methods The UGNRs were synthesized by the one-pot seedless method,then the functionalized modification of UGNRs were conducted using cysteamine.Finally,the activatable ultrasmall gold nanorods (AUGNRs) were synthesized by amide condensation of —NH2 of cysteamine and —COOH of carboxylated fluorescein CyS.The cell uptake ability and GSH-mediated imaging ability of AUGNRs were studied using breast cancer 4T1 cells.4T1 cells co-cultured with AUGNRs were irradiated with 808 nm excitation light,and the PTT effects were assessed by MTT colorimetric staining and calcein-AM/PI staining.Results The AUGNRs were synthesized successfully,which could be uptaken by 4T1 cells quickly and efficiently,and could achieve intracellular glutathione (GSH) triggered fluorescence recovery.No obvious cytotoxicity of AUGNRs to 4T1 cells was observed in the co-cultivation.Moreover,obvious PTT effects could be induced by 808 nm laser,which could effectively kill 4T1 cancer cells.Conclusion The fluorescence signals of AUGNRs can be induced by intracellular GSH,and tumor cell destruction can be achieved by 808 laser-excitated PTT effects.
2.Antioxidative effect of Tripterygium wilfordii polyglycosides on diabetic rats

OBJECTIVE ToinvestigatethepossibleprotectiveeffectofTripterygiumwilfordiipolyg-lycoside (TWP ) on experi mental diabetic nephropathy (D N ) rats and its possible mechanis m. METHODS Thediabeticmodelwasinducedbyasingleintraperitonealinjectionofstreptozotocin (STZ)65 mg·kg -1 .Three weeks after modeling,TWP 4.5,9.0 and 1 8.0 mg·kg -1 was ig given to rats,once daily,for 8 consecutive weeks.During the experiment,the changes of body mass,hair, mental health of rats were observed.Two days before the end of the experi ment,the rats were placed into metabolic cages to collect 24 h urine in order to detect 24 h urinary albu min excretion rate (UAER). The rats were given TWP for 8 weeks and anesthetized with 1 0%chloral hydrate.The blood was collect-ed fro m the heart and centrifuged,seru m creatinine and urine creatinine were measured,and creatinine clearance (Clcr)was calculated.Blood urea nitrogen (BUN)and the serum catalase (CAT)activity were tested by optical method while the level of seru m superoxide oxygen anion(O2÷)was tested by col-orimetry.The level of malondialdehyde(MDA)was determined by thiobarbituric acid condensation,and glutathione peroxidase (GSH-Px)activity was tested by colorimetry.The right kidney was HE stained to observepathologicalchanges.RESULTS Comparedwithnormalcontrolgroup,theratsinmodel control group developed polydipsia,polyuria,polyphagia,body mass loss,unresponsiveness,brown hair,pale tail and apathy clammy.Besides,blood glucose,BUN and 24 h UAER were significantly higher (P<0.01 ),but Clcr was lower (P<0.01 ).The activity of serum CAT and GSH-Px in renal tissue was significantly lower(P<0.01 ),while the level of serum O2÷ and MDA in the renal tissue was significantly higher(P<0.01 ).Compared with model control group,TWP 9.0 and 18.0 mg·kg -1 could improve the general condition of rats.BUN and 24 h UAER were obviously reduced(P<0.01 ),Clcr and serum CAT were increased obviously(P<0.01 ),the level of MDA and O2÷ were reduced obviously(P<0.01 ),and GSH-Px level was increased(P<0.01 ).TWP 9.0 and 18.0 mg·kg -1 could significantly im-prove the renal histopathological changes of rats.TWP 4.5 mg·kg -1 had no significant effect on the aboveindicators.CONCLUSION TWPhasprotectiveeffectontherenalfunctionofexperimentalDN rats.The mechanis m may be related to inhibition of the oxidative stress and enhance ment of the body antioxidant capacity.
3.Relationship between serum cholinesterase and acute-phase proteins in patients with multiple trauma

Objective To investigate the relationship between serum cholinesterase(ChE) and acute-phase proteins in patients with multiple trauma, then to evaluate their significance to judge prognosis. Method It's a prospective observation study. Patients with multiple trauma admitted to emergency intensive care unit,Second Af-filiated Hospital, Zhejiang Universieg, school of medicihe within 24 h after trauma from Oct. 2005 to Oct. 2007 were enrolled. And those with chronic liver disease, touching orgnaophosphorus, active tuberculosis, tumor, in-fection of major organ before trauma, liver injury or age < 18 year were excluded. Among 81 patients, 57 were male and24 female. The average age was (46±18) years, and the average injury severity score was (34.0±11.9).Seventy six healthy were selected as controls, 53 male and 23 female, with an average age of (44±16)years. The exclusion standards were the same as those in patients. Both groups had same gender proportion and age. Senum ChE and acute-phase proteins(APP) including albumin(ALB), prealbumin(PAB), transferrin(TRF),C-reactive protein(CRP) in patients were detected at 1, 3, 7 d after trauma. The acute physiology and chronic health evaluation Ⅲ (APACHEⅢ) was recorded simuhancously. Serum ChE, ALB, PAB, TRF, CRP in the controls were also detected. All of these indexes in the controls were compared with thoses in patients by t test or rank surn test. The dynamic changes of serum ChE and APPs in patients were analyzed by one way repeated mea-sures ANOVA. The relationships between serum ChE and those APPs and the relationships between APACHE Ⅲ and these indexes were analyzed by Pearson correlation analysis. We also compared these indexes between patients with different outcomes by t test or rank sum test. The values of serum ChE and those APPs to judge prognosis were evaluated by logistic regression analysis. Results Patients had lower serum ChE, ALB, PAB, TRF and higher CRP than the healthy at 1, 3, 7 d post trauma. Serum ChE activity in patients was reducod over 25% (42.3%~50.2%) than that in the healthy, and showed a tendency to decrease after trauma, which resembled PAB and TRF. Serum ChE was positively correlated with ALB, PAB and TRF at 1, 3, 7 d, and negatively correlated with CRP at 3, 7 d. At 1, 3, 7 d post trauma, APACHEⅢ in patients was negatively correlated with serum ChE and TRF, but negatively correlated with ALB only at 1 d, and negatively correlated with PAB only at 1,7 d, and posi-tively correlated with CRP only at 7 d. Non-survivors had lower serum ChE activity and TRF than survivors at 1,3,7 d after trauma, but had lower ALB only at 7 d after trauma and had lower PAB only at 1,7 d after trauma than survivors, and had higher CRP than survivors only at 7 d after trauma. Logistic regression analysis showed serum ChE and PAB were the only two independent risk factors to judge prognosis. Conclusions Serum ChE may be in-cluded as negative acute-phase protein, and is better than other APPs in reflecting injury severity and prognosis in patients with multiple trauma.
4.Relationship between the level of plasma homocysteine and coronary calcification in patients with different blood glucose levels

Objective To determine the relationship between the level of plasma homocysteine and coronary calcification in patients with different blood glucose levels. Methods By measuring plasma homocysteine and coronary calcification in 30 cases of diagnosed diabetes (T2D) ,29 cases of diagnosed impaired glucose tolerance (IGT) in patients and 27 cases with normal, we compared the level of plasma homocysteine and coronary calcification in patients with different blood glucose levels. Results We found significant diffieronces among three groups of the level of plasms homocysteine and coronary calcification (P < 0.01). The plasma homocysteine levels were(19.31 ±3.17) μmol/L, (13.85 ± 1.62) μmol/L, (9.80 ± 1.78) μmol/L in the T2D,IGT and normal groups,respectively. The coronary calcification scores were 207.80 ± 154.10,63.24 ± 10.46,14.47 ± 5.16 in the T2D, IGT and normal groups, respectively. The plasma homocysteine level and coronary calcification score increased with the glycosylated hemoglobin rise in the normal,IGT and T2D groups((4.51 ±0.48)%, (6.13 ±0.31)% and (7.69 ±0.81)%, respectively). Conclusions The plasma homocysteine level is a strong independent predictor of type 2 diabetes and also an important factor of coronary artery event occurrence and develepment.
5.Assessment of coronary artery by prospective ECG-triggered 256 multi-slice CT on children with congenital heart diseases

Objective To investigate the image quality and radiation dosage of the prospective ECG-triggered 256 multi-slice computer tomography (MSCT) angiography in accessing coronary artery (CA) on children.Methods Coronary arteries of ninety-three children were evaluated using prospective ECG-triggered 256-MSCT with the same system setting.Seventy-four patients had the heart rate records and a four-point scoring system was applied to study the capability of MSCT in detecting CA.The signal,noise and contrast-to-noise ratio (CNR) were analyzed to investigate the association of image quality with age (n =74).Then volumetric CT dose index (CTDIvol),dose-length product (DLP) and effective dose (E) were utilized to study the association of radiation dosage with age (n =93).Results On 74 patients with heart records,the detection rate for original,proximal,middle,distal and all 11 segments of CA was 100%,97%,92%,78% and 91%,respectively.No influence of age was found on their detection (all P>0.05).E of group A,B,C and D were 1.8,1.3,0.8 and 0.7 mSv,respectively.A strong negative correlation was found between E and age (r =-0.803,P < 0.01).Higher DLP was found on elder patients,though no correlation was found between ages vs.DLP(r =0.124,P > 0.05).No correlation was found between noise vs.age (r =0.041,P > 0.05).Significantly larger signals,CNR and E(t =3.386,2.073,3.825,P < 0.05) were found on younger patients (age < 3 years).Conclusions Prospective ECG-triggered 256-MSCT has considerable performance for the evaluation of coronary arteries on children.However,children under the age of 3 are not recommended for this examinary,but for examination on children ≥ 8-year old,the tube current could be further needed.
6.Repair of major chest wall defects caused by recurrent lesion or radiation ulcer after radical mastectomy with delto-pectoral flap

Objective To evaluate a repair approach to major defects of chest wall caused by recurrent lesion or radiation ulcer after radical mastectomy.Methods The delto-pectoral island flaps were applied to repair major defects of chest wall.The blood supply of the flap was from the 2nd and 3rd anterior perforator of the internal mammary artery.4 patients with defects of chest wall caused by recurrent cancer lesion and 8 patients caused by radiation ulcer received the pedicled flap after resection of lesion or ulcer.Results All the 12 flaps survived,in which flap necrosis at the distal end occurred in 1 case and it was cured after changing dress.After a follow-up of 6 monthls to 4 years,the flap healed and the cosmetic effect was satisfactory.Conclusions This method is an ideal method for repair of stage I major defects of chest wall caused by recurrent lesion and radiation ulcer after radical mastectomy.
7.On the rotation mode of standardized residency training of non-imaging professional residents in the department of radiology

The department of radiology, as one of the public platforms for standardized residency training, is very crucial to the training of high level clinicians and essential to improve quality of medical care. According to the present situation and existing problems of standardized residency training for the non-imaging professional residents in the department of radiology in China, combining with our practice, this article primarily explores the improvement of the rules and regulations, the specific arrangement of the rotation, daily and graduation examination forms, and training quality supervision, etc. We hope to improve the training quality of the non-imaging professional residents' rotation in the department of radiology and explore more suitable and effective medical educative strategies.
8.Preparation and performance of pH-responsive nanocarriers for mucus penetration

Objective To prepare pH-responsive osmotic nanocarriers (pMPPs),observe their distribution in the genital tract mucosa in mice,and evaluate their radiosensitizing effects in tumor cells.Methods Amphiphilic polymers containing pH-sensitive hydrazone bonds were synthesized and pMPPs were prepared by ultrasonic emulsification.At the same time,the hydrophobic polymer polylactic acid-glycolic acid copolymer (PLGA) and the amphiphilic polymer PLGA-polyethylene glycol without hydrazine bond were selected,and the mucoadhesive nanoparticles(MPs) and mucus-penetrating particles (MPPs) were prepared in the same way.Fluorescence microscopy was used to observe the distribution of three kinds of nanocarriers labeled with fluorescent dye Cy5.5 in the genital tract mucosa.The toxicity of nanocarriers to human cervical cancer cell line HeLa was tested by thiazolyl blue assay.The amphiphilic polymer containing pH-sensitive hydrazone bond was combined with oil-soluble gold nanoparticles to form a multi-encapsulated nanocarrier,and its radiotherapy sensitization effect in HeLa cells was evaluated by thiazole blue assay.Results The pMPPs were successfully prepared with relatively uniform particle size and good dispersion.Fluorescence microscopy showed that pMPPs not only had good mucus permeability,but also could improve the endocytosis efficiency of the nanocarriers in reproductive tract mucosa.The results of thiazolyl blue test showed that when the concentration of the carrier reached to 0.80 mg/ml,the survival rate of HeLa cells in the pMPPs group was higher than 90% which was higher than that in the MPs and the MPPs groups,indicating that pMPPs had good biosafety.The HeLa cell survival rate of the CMNa group (0.80 mg/ml) was higher than that of the multi-package nanocarrier group under different doses of X-ray irradiation (4 Gy:82.90% vs.61.79%;8 Gy:64.75 % vs.42.36%).This result indicated that compared with the CMNa,a commonly used clinical radiotherapy sensitizer,the multi-encapsulated nanocarriers can more effectively enhance the sensitivity of tumor cells to radiation therapy,thereby improving the lethality of radiation therapy on tumor cells.Conclusion This study solved the conflict between mucus permeation and endocytosis design of nanocarriers in mucosal tissue application,and provided new insight for the treatment of mucosal tissue diseases.
9.Exploration and practice of external cooperation projects audit of public hospitals affiliated to universities

Public hospitals and other non-profit or profit-oriented institutions can engage in external cooperation by way of management, technology, talents and information, and achieve medical resource sharing, complementary advantages, and maximal benefits. In December 2019, a university carried out the whole process audit on 75 external cooperation projects which were carried out from January 2016 to June 2019 by 4 affiliated public hospitals through the " two overall planning" management mode (i.e., overall planning audit projects and overall planning organization). The audit found that there were some problems in the external cooperation projects of public hospitals, such as lack of consistency and scientificity in the management system and decision-making mechanism, lack of standardization and preciseness in the conclusion of contracts, and the inadequate supervision of the implementation of contracts and the insufficient of project effectiveness evaluation and dynamic adjustmont. It is suggested to establish a comprehensive audit path for external cooperation projects of public hospitals, innovate the audit management mode, scientifically and reasonably allocate audit forces, expand the breadth and depth of audit supervision, and strictly implement problem rectification.
10.Chemokine CXCR4 regulates gastric cancer development through IL-6/STAT3 signaling pathway

The objective of this study is to investigate the effect of chemokine CXCR4 on the apoptosis of gastric cancer cells through IL-6/STAT3 signaling pathway and to explore the related mechanisms.Fluorescence quantitative PCR and Western blot were performed to detect the mRNA and protein expression levels of the chemokine CXCR4 in human gastric cancer tissues and adjacent non-cancerous tissues(PCT).Next,CXCR4 knockdown and overexpression were achieved by transfecting SGC7901 gastric cancer cell line with lentiviral vectors.TUNEL staining was used to evaluate the apoptosis of SGC7901 cells,while MTT assay was employed to measure cell proliferation.Western blot was conducted to determine the expression of apoptosis-related proteins Bax and Bcl2.Further,enzyme-linked immunosorbent assay(ELISA)was employed to measure the secretion levels of inflammatory cytokines.Real-time fluorescent quantitative PCR(RT-PCR)was utilized to quantify the expression of IL-6 mRNA in the IL-6/STAT3 signaling pathway,and Western blot was performed to analyze the expression of STAT3-Ser727 protein.In addition,after knocking down CXCR4 in SGC7901 cells,IL-6/STAT3 signaling pathway agonist lipopolysaccharide(LPS)was transfected,while in CXCR4-overexpressing SGC7901 cells,IL-6/STAT3 signaling pathway inhibitors angoline or bruceantinol were transfected.Then TUNEL staining was used to assess cell apoptosis,and Western blotting was performed to examine the expression levels of apoptosis-related proteins Bax and Bcl2 in these cells.Data showed that the expression of immune chemokine CXCR4 was increased in gastric cancer tissues,as compared with adjacent non-cancerous tissues.Single-cell gel electrophoresis analysis indicated that knockdown or overexpression of CXCR4 do not induce DNA damage in SGC7901 cells.TUNEL staining,MTT cell proliferation assay and Western blotting demonstrated that knockdown of CXCR4 in SGC7901 cells promoted the apoptosis in SGC7901 cells,while overexpression of CXCR4 inhibited the apoptosis.ELISA showed that knockdown of CXCR4 in SGC7901 cells promotes the expression of pro-inflammatory factors IL-1β and TNF-α,while inhibited the expression of anti-inflammatory factors IL-10 and TGF-β.Conversely,overexpression of CXCR4 demonstrated opposite effects.Finally,the activation of the IL-6/STAT3 signaling pathway significantly reduced the apoptosis induced by knocking down CXCR4 in iSGC7901 cells,whereas the inhibition of IL-6/STAT3 signaling pathway can significantly suppressed the induction of SGC7901 cells proliferation induced by CXCR4 overexpress.In conclusion,immunochemokine CXCR4 regulates gastric cancer cell apoptosis and inflammatory cytokines secretion through IL-6/STAT3 signaling pathway.
