2.Surgical treatment of venous leg ulcer

Objective To study the surgical treatment of venous leg ulcer. Methods The surgical treatment of venous leg ulcer in 101 cases from January 1980 to December 2002 in our hospital was retrospectively analyzed. Results Of 72 cases undergoing subfascial communicating vein ligation around the base of the ulcer, 52 cases were healed within 1 to 3 months, 12 cases within 4 to 5 months, 8 cases after skin graft, but 3 cases were complicated with postoperative infection and 2 cases suffered from relapse of ulcer. However, of the 12 cases undergoing peripheral transfixion of the ulcer through the skin, 2 cases were complicated with skin necrosis and infection, 10 cases healed within 6 months to 1 year. All the 11 cases undergoing endoscopic communicating vein dissection were healed without complications within 1~2 months. Conclusion The surgical procedures for venous leg ulcer should be performed to block the regurgitation of the communicating veins by means of lowering the venous leg pressure. Endoscopic communicating vein ligation or spiral dissection of the communicating vein is valuable in clinical application.

4.THE EFFECT OF INTRACAROT1D INFUSION OF PAPARERINE ON BRAIN - BLOOD -BERR1ER AND ITS APPLICATION TO THE TREATMENT OF MALIGNANT GLIOMA WITH BIS-CHLORONITROSOUREA

The intra-tumor MTX concentration was measured in 12 patients with cerebral gliomas and 31 patients of malignant gliomas were treated with intracarotid infusion of papaverine and bis-chloroaitrosourea (BCNU). After intracarotid infusion of papaverine to open up the blood-brain-barrier (BBB), the MTX concentration within tumor tissues was 1854? 512ng/gram, which higher than that without papaverine infusion, which was 1020? 512ng/gram. The diagnosis was glioblastoma in 21 patients and anaplastic astrocytoma in 10. They received the infusion for a mean of 2.1 times with 250mg BCNU each. The mean survival time was 101.6? 32.7 weeks, with 104 weeks of median survival time. Fourteen were still living, including 11 glioblastoma and 3 anaplastic astrocytoma. Of those, four have lived over three years. We considered combination of papaverine and BCNU infusion for chemotherapy of malignant cerebral glioma is simple, safe and effective, so long as tumors were susceptible to the chemotherapeutic drugs.
5.To strengthen the detection of biomarkers in the synchronous metastatic axillary lymph nodes of primary breast cancer

Postoperative adjuvant therapy for primary invasive breast cancer patients with synchronous ax-illary metastases is mainly based on the characteristics of biomarker of the primary tumor. Recently, some studies have showed the discordance and clinical significance of estrogen receptor (ER), progesterone receptor (PR) and human epidermal growth factor receptor 2 (HER2) status between primary breast cancer and synchronous axillary lymph node metastases. As local metastasis, the synchronous axillary metastases may represent the potentially metastatic breast cancer cells much better than the primary tumor. Hence, determination of biomarkers status should be performed in synchronous axillary metastasis, together with primary tumor, to guide therapy management and evaluate the prognosis of primary invasive breast cancer patients with synchronous axillary metastases.
6.Clinical significance of hormone receptor status detection in simultaneous axillary metastasis for hormone receptor-negative primary breast cancer patients

Objective:This study aimed to investigate the clinical significance of hormone receptor status detection in simultane-ous axillary metastasis for hormone receptor-negative primary breast cancer patients. Methods:Using immunohistochemical methods, hormone receptor status detection in axillary lymph node metastasis was performed among breast cancer patients with simultaneous ax-illary metastasis in the First Affiliated Hospital of Chongqing Medical University. The subjects comprised patients who visited the clin-ic for follow up or those who were hospitalized from July 2012 to January 2013. Endocrine therapy was given for patients diagnosed with positive hormone receptor in their simultaneous axillary metastasis. Results:Out of 56 patients with hormone receptor-negative primary breast cancer, 14.3%gained estrogen receptor (ER), 3.6%gained progesterone receptor (PR), and 5.4%gained ER and PR in their simultaneous axillary metastasis, and then underwent endocrine therapy. The discordance rate of hormone receptor expression be-tween primary tumor and axillary metastasis was 23.3%. Conclusion:Some discordance rates of hormone receptor status between pri-mary tumor and simultaneous axillary metastasis were observed. Through hormone receptor status detection in simultaneous axillary metastasis, we may possibly distinguish patients with negative ER and PR, among whom endocrine therapy may be active.

Breast cancer and dyslipidemia are common diseases in women.It is reported that breast cancer and dyslipidemia have some relevance.Breast cancer patients have a relatively high incidence of dyslipidemia,which is easily undiagnosed for lacking of obvious clinical manifestations and will affect the treatment and prognosis of breast cancer.In breast cancer patients,screening and diagnosis of dyslipidemia through blood lipid detection should be strengthened.

10.Protection of ovarian function for female breast cancer patients undergoing chemotherapy

Breast cancer is one of the most common malignant tumors in females.Recent years,surgery,chemotherapy as well as other systemic therapy had greatly improved the prognosis of the patients.However,damage of ovarian function by chemotherapy lowered life quality,especially for young females.At present,there are several methods to protect the ovarian function of female patients undergoing chemotherapy,such as administration of a gonadotropin-releasing hormone (GnRH) analogs,ovarian cryopreservation,unfertilized ova cryopreservation,embryo cryopreservation,inhibitors of apoptosis,etc.Each method has its advantage,disadvantage and indications.Issues related to ovarian protection are reviewed here.
