1.Efficacy and Effect on Recurrence of Adjuvant Therapy with Montelukast for Children with Bronchial Asthma

Objective:To observe adjuvant curative effect and adverse reactions of montelukast in the treatment of children with bronchial asthma and its effect on recurrence. Methods:Totally 180 cases of children with bronchial asthma were randomly divided in-to the control group and the observation group with 90 ones in each. The control group was treated with oxygen and budesonide terbutal-ine inhalation, intravenous methylprednisolone sodium succinate and the other conventional treatment methods, and the observation group was additionally treated with montelukast sodium tablets 5 mg, po, qd. The treatment course was three months, and then expira-tory dyspnea, disappearance time of asthma and wheeze and adverse drug reactions in the two groups were evaluated. After 3-6-month follow-up, the recurrence was compared between the two groups as well. Results: The disappearance time of dyspnea, asthma and wheeze in the observation group was shorter than that in the control group (P<0. 05), the total effective rate in the observation group was 96. 67%, which was significantly higher than that in the control group (73. 33%, P<0. 05). The mean follow-up duration was (5. 1 ± 0. 3) months, and the recurrence rate was 8. 51% in the observation group, which was significantly lower than that in the con-trol group (31. 43%, P<0. 05). During the treatment, no significant adverse drug reaction was appeared in the two groups. Conclu-sion:The conventional drug treatment combined with montelukast can rapidly improve the clinical symptoms and reduce recurrence with promising safety, which is worthy of clinical application.

4.Efficacy observation of the combination of Weimaining capsule and XELOX regimen in the treatment of advanced gastric cancer patients

Objective To evaluate the efficacy and safety of the combination of Weimaining capsule and XELOX regimen (oxaliplatin plus capecitabine) in the treatment of advanced gastric cancer patients.Methods A total of 62 patients with advanced gastric cancer who fulfilled all predetermined criteria were randomly divided into observation group and control group.The 31 patients in observation group received a combination of Weimaining capsule and XELOX regimen,the 31 patients in control group received only XELOX regimen at the same dose intensity.Each patient received at least two cycles (1 cycle =21 days) of treatment,the efficacy was assessed after two cycles.Results The median time-to-progression (TTP) of observation and control groups were 5.5 and 4.8 months,respectively.The difference had no statistical significance (P =0.238).The disease control rates (DCR) of observation group was significantly improved compared with control group [67.7 % (21/31) vs 41.9 % (13/21),P < 0.05],the difference was statistically significant,while the objective response rate (ORR) of the 2 groups had no statistical difference [(32.3 % (10/31) vs 25.8 % (8/31),P > 0.05].In drug safety aspects,there was no Ⅴ grade adverse reactions (deaths caused by drug) among all the patients.Rates of grades Ⅱ-Ⅳ neutrocytopenia and nausea,vomiting in observation group were obviously lower than control group [16.13 % (5/31) vs 38.71% (12/31),P < 0.05;12.90 % (4/31) vs 35.48 % (11/31),P < 0.05],other adverse reactions between the 2 groups had no statistical difference.Conclusions A combination of Weimaining capsule and XELOX regimen in the treatment of advanced gastric cancer patients can improve DCR significantly and reduce part of adverse reaction induced by chemotherapy with satisfactory safety.This method is worthy of application widely.
5.Analgesic effect of diprospan in rats with trigeminal neuralgia.

This study examined the analgesic effect of diprospan in rats with trigeminal neuralgia. Rat model of trigeminal neuralgic pain was established by loosely ligating the left infraorbital branch of the trigeminal nerve. After allodynia developed, the rats were randomly divided into 2 groups (n=20 in each): diprospan group, in which the rats received diprospan (7 mg/mL, 0.1 mL) injected to the left infraorbital foramen area; control group, in which saline (0.1 mL) was administered as the same manner as the diprospan group. The pain threshold (PT) in the left infraorbital area was measured before and 2, 6, and 8 weeks after the administration. The expression of neuropeptides [substance P, preprotachykinin A (PPTA), calcitonin gene-related peptide (CGRP)] in the trigeminal nerve was detected at the same time points as the PT measurement by immunohistochemistry or in situ hybridization method. The results showed that in the diprospan group, the PT was 10.65±1.26, 10.77±1.19 and 14.13±1.34 g 2, 6, and 8 weeks after the administration respectively, significantly higher than that before the administration (PT value: 0.36±0.11) (P<0.05 for each). In the saline group, the PT was 0.37±0.13, 0.66±0.09, 4.45±1.29 and 13.72±1.72 g before and 2, 6, and 8 weeks after the administration respectively with differences being significant between before and 6, 8 weeks after the administration (P<0.01). No significant difference existed in the PT between the diprospan group and the saline group at pre-administration (P>0.05). The PT in the diprospan group was significantly greater than that in the saline group 2 and 6 weeks post-administration (P<0.05). In the diprospan group, the expression levels of neuropeptides were significantly reduced as compared with those in the saline group 2 and 6 weeks post-administration (P<0.05). It was concluded that diprospan has an obvious analgesic effect on the trigeminal neuropathic pain partly by reducing the expression of neuropeptides in the trigeminal ganglia.
6.Psychological Rehabilitation on Dysphagia with Cognitive and Psychological Disorders post Stroke

Objective To explore the clinical effect of psychological rehabilitation on dysphagia with cognitive and psychological disorders post stroke. Methods 120 stroke patients with dysphagia complicated with cognitive and psychological disorders were divided into experimental group (n=60) and control group (n=60). Both groups received neurological routine medical treatment, swallowing function rehabilitation training, feeding training and low-frequency electrical stimulation. Cognitive and psychological therapy was additionally used in the experimental group. Both groups were assessed by Kubota DrinkingWater Test, Mini-Mental State Examination (MMSE), Hamilton Rating Scale for Depression (HAMD) and Hamilton Rating Scale for Anxiety (HAMA). Results 1 month after intervention, the score of Kubota Drinking Water Test improved significantly in both groups, and was higher in the experimental group than in the control group (P<0.001).The rate of clinical improvement was higher in the experimental group than in the control group (P<0.01). The scores of MMSE increased significantly in the experimental group (P<0.001). The scores of HAMD and HAMA decreased significantly in the experimental group (P<0.01). Only the scores of HAMA decreased in the control group (P<0.05). Conclusion Swallowing function rehabilitation training combined with cognitive and psychological therapy may facilitate to improve the cognitive function and psychological state and swallowing function for dysphagia with cognitive and psychological disorders post stroke.
7.Correlation between myc/bcl-2 co-expression and myc/p53 co-expression and prognosis of diffuse large B-cell lymphoma

Objective To explore the relationship between myc/bcl-2 and myc/p53 co-expression and the prognosis of diffuse large B-cell lymphoma (DLBCL). Methods A total of 148 DLBCL cases with the follow-up data in Shanxi Cancer Hospital from January 2010 to October 2014 were selected. The expression of myc, bcl-2 and p53 protein in paraffin samples was detected by immunohistochemistry (IHC). Results The positive expression rate of myc, bcl-2 and p53 was 35.1 % (52/148), 60.1 % (89/148) and 24.3 % (36/148) respectively in 148 patients with DLBCL. There were 37 (25.0%) cases of myc/bcl-2 co-expression, which were correlated with Hans classification (χ2= 4.749, P= 0.029), IPI score (χ2= 4.894, P= 0.027), bone marrow invasion (χ2= 4.751, P= 0.029), and efficacy evaluation (χ2= 9.14, P= 0.003). Myc/p53 co-expression was detected in 17 cases (11.5 %), which was associated with Hans classification (χ2 = 5.349, P= 0.021) andLDH level (χ2= 11.1, P= 0.001). Univariate analysis revealed that myc [overall survival (OS): χ2= 6.044, P= 0.014; progression free survival (PFS):χ2= 6.212, P= 0.013], bcl-2 (OS:χ2= 5.812, P= 0.016; PFS:χ2= 4.878, P= 0.027), p53 (OS:χ2= 20.092, P< 0.0001; PFS:χ2= 18.492, P< 0.0001), myc/bcl-2 co-expression (OS: χ2= 11.277, P= 0.001; PFS:χ2= 9.024, P= 0.003) and myc/p53 co-expression (OS: χ2=21.150, P< 0.0001; PFS: χ2 = 18.655, P< 0.0001) were the adverse prognostic factors. In addition, the survival rate of the co-expression group was lower than that of single expression group, and the survival rate of myc/p53 co-expression group was lower than that of myc/bcl-2 co-expression group. Multivariate analysis showed that among the six independent variables, including myc, bcl-2, p53, myc/bcl-2, myc/p53 and treatment regimen, p53 expression was an independent prognostic affecting factor of OS (95%CI 0.172ˉ0.763, P=0.008) and PFS (95%CI 0.172ˉ0.773, P=0.009). Conclusion Myc, bcl-2 and p53 are the poor prognostic factors in DLBCL. Myc/bcl-2 and myc/p53 co-expression have synergistic effect, indicating the poor prognosis.
8.Changes of accompanying femoral artery and vein blood gas analysis in patients with burn sepsis

Objective To discuss changes of accompanying femoral artery and vein blood gas in patients with burn sep?sis. Methods The retrospective data of twenty-two patients with burn sepsis and forty-three non-sepsis patients (control) was analyzed, and changes of accompanying femoral artery and vein blood gas were compared between two groups. Results Results of femoral artery gas analysis showed that the HCO3-concentration was lower in sepsis group than that of control group. Results of femoral vein blood gas showed that the partial pressure of carbon dioxide [p(CO2)] was higher in sepsis group than that of control group;the oxygen saturation (SvO2) and HCO3-concentration was lower in sepsis group than that of control group (P<0.01). The differences between oxygen partial pressure p(O2), p(CO2), HCO3-concentration, oxygen satura?tion of femoral artery and vein blood gas were significantly higher in sepsis group than those of control group ( P<0.01). Conclusion Results of femoral vein blood gas analysis change obviously in patients with burn sepsis . The detection of accompanying femoral artery and vein blood gas is beneficial to diagnosis of burn sepsis.
9.Gene expressions of the human intrahepatic cholangiocarcinoma RBE cell line

Objective To analyze the differentially expressed genes between the NCAM + c-Kit +RBE and NCAM-c-Kit-RBE of intrahepatic cholangiocarcinoma (ICC) cell lines,and to screen out the differentially expressed genes that are related to the stem cell signaling pathways.Methods Magnetic activated cell sorting was used to isolate the NCAM + c-Kit +/NCAM-c-Kit-subset cells,and then Agilent Whole Human Genome Microarray Kit was used to test the difference in gene expressions between the NCAM + cKit + and NCAM-c-Kit-subset cells.The difference in gene expressions related to the stem cell signaling pathways was analyzed by the SAS system.The result of the microarray was further confirmed by RT-PCR.Results The total differentially expressed genes which could be found through gene microarray were 7270 [foldchange(fc) ≥2 or fc ≤0.5].Compared with the NCAM-c-Kit-RBE,3572 genes were upregulated while 3698 genes were downregulated.The differences in gene expressions related to the stem cell signaling pathways were 421 (fc ≥2 or fc ≤ 0.5),among which 231 genes were upregulated while 190 genes were downregulated.Conclusions High-flux microarray could be used to screen out lots of differentially expressed genes between the NCAM + c-Kit + and NCAM-c-Kit-RBE cells.The differences in gene expression in the stem cell signaling pathways could also be further analyzed using the SAS system.
10.Ultrastructural Dynamic Observation on Murine Schistosomal Hepatic Fibrosis

Objective To explore possible mechanisms of hepatic fibrosis by investigating the ultrastructural dynamic changes of liver tissue, especially several kinds of cells related to hepatic fibrosis.. Methods. Murine schistosomal hepatic fibrosis model was established by infecting mice with Schistosoma japonicum cercariae. Routine transmission electron microscopy was used to observe the liver tissue. H.E. staining was used for examining the pathological changes. . Results . H.E. staining showed that the model was established successfully. Ultrastructural observation showed that at the 6th week after infection, the necrosis of hepatocytes around the acute granulomas occurred; the number of sinusoidal endothelial fenestrae and vitamin A droplets in fat-storing cells decreased; large phagosomes and rough endoplasmic reticulum could be seen in the cytoplasm of Kupffer′s cells. At the 8th week, steatosis was found in some hepatocytes, some microvilli emerged on a few inter-hepatocytic surfaces and the inter-hepatocytic spaces were enlarged. Large collagen fibrillar bundles filled in the perisinusoidal spaces, and capillarization of hepatic sinusoids was observed. Secretory vesicles filled with collagen fibrils appeared in the cytoplasm of fat-storing cells with large amount of collagenous fiber bundles surround the cells. Rough endoplasmic reticulum increased in Kupffer′s cells. At the 10th week, fat-storing cells were activated and transformed into myofibroblasts. At the 12th week, the number of myofibroblasts decreased but that of fibroblasts and fiber cells increased. . Conclusion . Activation of fat-storing cells and transformation from fat-storing cells into myofibroblasts are the critical link in the development of hepatic fibrogenesis following schistosome infection. Kupffer′s cells, necrotic hepatocytes and sinusoidal endothelial cells may relate to the activation of fat-storing cells. Capillarization of hepatic sinusoids possibly accelerates the development of hepatic fibrosis.
