1.Risk factors associated with early recurrence of adenocarcinoma of esophagogastric junction (typeⅠ,Ⅱ) after radical resection

Background and purpose:Adenocarcinoma of the esophagogastric junction (AEG) has a high incidence of early recurrence and metastasis after operation. The prognosis of AEG is poor. However, few studies have investigated the recurrence of AEG. The purpose of this study was to clarify the important clinical pathological factors affecting the early recurrence ofⅠ,Ⅱ type of AEG after operation. Thus more active treatment for patients at high risk of recurrence may improve the prognosis.Methods:This study retrospectively reviewed the clinical data from 145 AEG patients who underwent R0 resection during the period from Dec. 2008 to Dec. 2012. Risk factors associated with the early recurrence were analyzed.Results:The mean time to tumor recurrence was 25.4 months after R0 resection, and the 1-year recurrence rate was 38.6%. Univariate analysis showed that the histological grade (degree of tumor differentiation), number of positive lymph nodes, TNM stage and vascular invasion were signiifcantly related with the early recurrence (P<0.05). Logistic multivariate regression analysis showed that only histological grade and vascular invasion were independently related with early tumor recurrence (P<0.05).Conclusion:Histological grade and vascular invasion are independent risk factors for predicting the early tumor recurrence after R0 resection for AEG.
2.Bioinformatics characteristics of lncRNA -uc.167 and its temporal and spatial expression pattern for mouse embryonic development

Objective To explore the basic biological characteristics of lncRNA -uc.1 67,and its spatial dis-tribution,temporal expression pattern during the mouse embryonic development.Methods The UCSC genome browser of ENCODE was used to analyze preliminary bioinformatics of lncRNAs.Real -time (RT)-PCR was applied to detect the expression of uc.1 67 and neighboring genes in the embryonic mouse heart in different stages (P7.5,P1 1 .5,P1 4.5, P1 8.5).Dimethyl sulphoxide was used to induce P1 9 cell differentiation into the cardiomyocytes.RT -PCR was applied to detect the expression changes in uc.1 67 and neighboring genes on differential day 0,4,6,8 and 1 0.Results Full -length of human uc.167 was 201 bp,and human uc.167 was located in the genome 5q14.3 (chr5:88179623 -881 79824,GRCh37 /hg1 9).uc.1 67 mainly expressed in the ventricular muscle tissue.The expression of uc.1 67 was gradually decreased in the mouse embryonic heart development process(P7.5:1 .000 ±0.1 00,P1 1 .5:0.71 4 ±0.1 07, P1 4.5:0.393 ±0.043,P1 8.5:0.1 25 ±0.01 3),while the expression of its neighboring Mef2c gene was gradually in-creased(P7.5:1 .081 ±0.1 1 8,P1 1 .5:2.340 ±0.351 ,P1 4.5:3.958 ±0.542,P1 8.5:9.361 ±0.722),which showed an opposite trend.The expression of uc.1 67 during P1 9 cell differentiation into cardiomyocytes showed a an increase at first and then a decreasepattern,and the highest level expression of uc.1 67 was on differential day 4(d0:1 .071 ± 0.1 1 7,d4:4.71 4 ±0.501 ,d6:3.572 ±0.41 4,d8:2.550 ±0.31 4,d1 0:0.786 ±0.085).The expression of neigh-boring gene Mef2c was in an opposite trend(d0:1 .01 2 ±0.041 ,d4:0.353 ±0.037,d6:2.470 ±0.329,d8:6.706 ± 0.682,d1 0:7.765 ±0.705).Conclusions It is suggested that uc.1 67 may take part in the process of embryonic heart development and may play a role through negatively regulating its neighboring gene Mef2c.
3.Renal and extra-renal rhabdoid tumor:analysis of 4 cases and lit-erature review

Objective:To analyze the clinico-pathological characteristics, pathological diagnosis, and treatment of rhabdoid tu-mor. Methods:The medical records of four rhabdoid tumor patients that were admitted to the Tianjin Medical University Cancer Insti-tute and Hospital since 2000 were analyzed based on existing literature. Results:In one of the four cases, the tumor originated from the kidney, whereas in the other three, the tumor occurred from extra-renal soft tissues. Histologic analysis revealed that the tumor cells were loosely arranged with diffuse growth, vesicular nuclei, dyed cytoplasm, visible eosinophilic inclusions, and more nuclear fission. The results of immunohistochemical staining showed that the vimentin and epithelial membrane antigen were positive, whereas CK, CD99, CD34, and S-100 were positive at different degrees. MyoD1, Desmin, and INI-1 were negative. Conclusion:Rhabdoid tumor is rare and highly aggressive. It occurs mainly in the kidney and can also be found in other systems. The unique pathological form and im-munohistochemical staining observed on the tumor can be used as reference for diagnosis.
4.Study on the effect of pancreatic stone protein in neonatal bacterial infectious diseases

Objective To observe the clinical significance of serum pancreatic stone protein/regenerating protein (PSP/reg) in neonatal bacterial infectious diseases.Methods 92 cases of newborns with bacterial infection in our hospital from June 2016 to May 2017 were enrolled in the study.They were divided into severe infection group (36 cases) and mild infection group (56 cases) according to the severity of the infection.At the same period 58 newborns without infectious diseases in our hospital were also chosen as control group.The serum levels of PSP/reg,procalcitonin (PCT) and C-reactive protein (CRP) were quantitatively measured by enzyme-linked immunosorbent assay (ELISA).Results The serum levels of PSP/reg,PCT and CRP in severe infection group were significantly higher than those in mild infection group (P <0.01) and control group (P < 0.01),while the levels of PSP/reg,PCT and CRP in mild infection group were significantly higher than those in control group (P < 0.01).The levels of PSP/reg 3 day after treatment in both severe and mild infection group were significantly lower than those before treatment (P <0.01),while the level of PSP/reg in severe infection group was higher than that in another group.After treatment,the levels of PSP/reg,PCT and CRP in the two infection groups were significantly decreased (P <0.01),and there was no significant difference between the two groups after healing (P > 0.05).Conclusions The level of PSP/reg is correlated with the severity of neonatal bacterial infectious disease.The change of serum PSP/reg level may be useful in the evaluation of therapy in neonatal bacterial infection.
5.The exposure of antibiotics on the eradication of bismuth quadruple therapy in H.pylori infection

Objective:To analyze the impact of previous exposure to macrolide, quinolones and nitroimidazole antibiotics on eradication rate of bismuth quadruple therapy (BQT) in newly diagnosed patients with Helicobacter pylori( H. pylori). Methods:A total of 469 patients with H. pylori initially treated at the Third Hospital of Peking University from September 2017 to August 2020 were retrospectively recruited. The therapeutic regimens were BQT containing clarithromycin/levofloxacin/metronidazole recommended by Chinese guidelines. Clinical data were collected, including general demographic data, exposure history of antibiotics, CYP2C16 metabolic pattern, endoscopic diagnosis, bacterial density, H.pylori resistance, eradication results, etc. Univariate analysis, Chi-square test, Fisher exact probability test, Kruskal-Wallis H test and Logistic regression model were used as statistical methods. Results:Among different eradication therapies, univariate and multivariate analyses suggested that previous exposure to macrolides ( OR=3.37,95 %CI 1.04-10.98, P<0.05) was relevant to the decreased eradication rate of BQT containing clarithromycin. This may be due to increased resistance to clarithromycin ( OR=6.12,95 %CI 3.99-9.40, P<0.01).The previous exposure to quinolones ( OR=3.65, 95 %CI 1.27-10.49, P<0.05) was relevant to the decreased eradication rate of BQT containing levofloxacin, which was probably explained by the increased resistance to levofloxacin ( OR=2.50, 95 %CI 1.69-3.71, P<0.01). But the previous history of nitroimidazole did not impact the efficacy of BQT containing metronidazole. Conclusions:In patients newly diagnosed with H.pylori infection, the previous exposure to macrolide or quinolones antibiotics is related to lower eradiation rates of H. pylori. Although the exposure to nitroimidazole also indicates drug resistance to metronidazole, the clinical efficacy of BQT with metronidazole 400 mg four times a day is not affected.
