1.Clinical Significance of Antinucleosome antibody in systemic lupus erythematosus

Objective To investigate the clinical significance of antinucleosome antibody (AnuA) in the patients with lupus erythematosus (LE) through detecting the serum level of AnuA in different types of LE patients before and after treatment. Methods Enzyme-linked immunosorbent assay(ELISA) was used to detect the serum level of AnuA in 31 SLE patients, 26 SCLE patients, 7 DLE patients, 6 scleroderma patients, 5 dermatomyositis patients and 30 healthy controls. Among them, the serum levels of AnuA before and after treatment were detected in 15 SLE patients and 10 SCLE patients. Clinical data, laboratory test results and the scores of SLE disease activity index (SLEDAI) were analyzed with SPSS11.0 software. 2.Effects of different target effect-site concentrations of remifentanil on the sedative effect of propofol

Objective To investigate the effects of different target effect-site concentrations (Ces) of remifentanil on the sedative effect of propofol. Methods Fifty ASA Ⅰ or Ⅱ patients aged 20-55 yr weighing 48-86 kg with body mass index < 30 kg/m2 were randomly divided into 5 groups ( n = 10 each) . Anesthesia was induced with TCI of remifentanil (Ce = 0, 2, 4, 6 and 8 ng/ml in groups R0-R4 respectively) and propofol. The initial Ce of propofol was 2.0 μg/ml in the 5 groups, and then the Ce of propofol increased by 0.5 μg/ml every 1 min until BIS value decreased to 50. BIS value and Ce of propofol were recorded as the patient lost consciousness. The effect-site concentration and consumption of propofol and the time required were recorded when BIS value decreased to 50.Results BIS value was significantly increased, while the effect-site concentration of propofol was significantly decreased as the patient lost consciousness, and the effect-site concentration and consumption of propofol were significantly decreased and the time required was shortened when BIS value decreased to 50 in R2-R4 groups compared with group R0 (P < 0.05 or 0.01) . Conclusion The sedative effect of propofol can be enhanced when the Ce of remifentanil reaches 4 ng/ml, and the effects are comparable when the Ce of remifentanil reaches 4, 6 and 8 ng/ml.
3.Nursing of locally preserved skin of female patients for neurosurgical cranioctomy

Objective To summarize the experience in nursing locally preserved skin of female patients for neurosurgical cranioctomy. Method The nursing key points were given to 66 female patients undergoing local preparation of skin for cranioctomy as follous preoperative preparation, observation over incision and head nursing. Results The time for skin preparation ranged between 3 to 6 minutes. Postoperatively, no drain tube was placed. No intracranial infections occurred. The incisions in all patients were healed at the first class. Conclusion The nursing measures of good preoperative preparation, close observation of incisions and care to the head for female patients undergoing local preparation of skin for cranioctomy can enhance their satisfaction and propel development of standardized nursing.
4.The expression and significance of E-cadherin and β-catenin in type Ⅰ endometrial carcinoma

Objective To investigate the expression and significance of E-cadherin and β-catenin in type Ⅰ endometrial carcinoma.Methods The expression of E-cadherin and β-catenin were detected by immunohistochemistry in 60 patients with type Ⅰ endometrial carcinoma and 10 normal endometrial tissues.The patients were followed-up postoperatively for the survival rate.Results Expression of E-cadherin and β-catenin in 10 normal endometrial tissues was all high expression,while those of both abnormal expression in type Ⅰ endometrial carcinoma with expression rates with 60.0 % (36/60) and 55.0 % (33/60),respectively,which were difference compared with both in normal endometrial tissues (P < 0.05).The expression rates were markedly decreased in carcinoma with lymph node metastasis than those without lymph node metastasis.The proportions of abnormal expressions of E-cadherin and β-catenin were positively correlated with tumor grade and myometrial invasion in type Ⅰ endometrial carcinoma (P < 0.05).The postoperative two-year survival rate in the patients with abnormal expression of E-cadherin and β-catenin were much lower than those with normal expression (P < 0.05).Conclusions Abnormal high expression of E-cadherin and β-catenin may play important roles on generation and metastasis in type Ⅰ endometrial carcinoma.Both of them may be promising method for predicting invasion and metastasis and can be prognostic predictors in type Ⅰ endometrial carcinoma.
5.Primary Study on Hydrolysation of Centipedes Protein and Evaluation of Its ACE Inhibitory Activity

This study was aimed to analyze the major protein composition of centipedes (Scolopendra subspinipes mutilans L.Koch.), and the ACE inhibitory activity of its hydrolysis. Albumins, gulbulins, coixins and glutelins were sequentially extracted from centipedes flour with corresponding buffer and then quantified by Kjeldahl method and Brandford. Hydrolysation of four kinds of proteins of centipedes and the residues were conducted with pepsin. The hydrolysis was ultrafiltrated (MWCO=3 000) and lyophilized. The peptides (≤3 kD) were obtained to evalu-ate the ACE inhibitory activity by RP-HPLC. The results showed that the total protein content of centipedes was (62.69±1.41)%. Among which the contents of albumins, globulins, coixins, glutelins and residual were account-ing for (6.42±0.31)%, (7.94±0.24)%, (4.31±0.34)%, (40.66±0.56)% and (25.78±0.60)%, respectively. The inhibition rate of hydrolysis of four kinds of protein and residual were 50.28%, 57.37%, 31.15%, 58.99%, 80.81%, respectively. It was concluded that centipedes were rich in protein and the hydrolyzate of all proteins manifested ACE inhibitory activity at different extent. The residual and glutelins indicated strong ACE inhibitory potential by hydrolysis. This research provided valuable sights for exploring hypotensive activity and functional food from centi-pedes.

7.Detection of Herpes simplex Virus,Treponema pallidum and Haemophilus ducreyi by Multiple PCR

OBJECTIVE To establish multiple PCR(M-PCR) assay for simultaneous detection of Treponema pallidum(TP) and herpes simplex virus(HSV),Haemophilus ducreyi(HD).METHODS According to specific gene sequence of the three agents:HSV DNA polymerse gene,TP tpp47 gene,HD 6s rRNA gene;three sets of specific primers were designed and a multiple PCR assay was developed to detect 3 agents in one test.RESULTS The target DNA of 3 agents were specifically amplified by their specific primers,the appropriate size of four DNA fragments was 202bp,252bp,152bp for TP,HSV and HD,respectively.The DNA of other several common microbes of genital tract and human genome could not be amplified by three sets of primers.CONCLUSIONS Primary study indicated that the M-PCR assay can simultaneously,specifically and rapidly detect out HSV,TP and HD.
8.Observation on the effect of psychological intervention combined with nebulized salbutamol on the treatment of severe asthma

Objective To observe the effects of inhalation salbutamol combined with psychological intervention on the treatment of patients with severe asthma, and evaluate its clinical curative effect. Methods 162 cases with severe asthma were selected from February 2014 to February 2017 in hospital of traditional Chinese medicine in Zhoushan as the research object, which were randomly divided into the control group and the study group with 81 cases in each group. All patients were given nebulized salbutamol, the control group in the perioperative were given routine nursing intervention, at this basis, the study group were given targeted psychological nursing. The effect in the two groups was compared. Results The total effective rate in the study group was 91.36% (74/81) was significantly better than that in the control group 76.54% (72/81), the difference was statistically significant (P<0.05). After treatment, cough, shortness of breath, wheezing, chest tightness, lung wheezing were improved significantly , and all the symptoms in the study group was obviously better than those in the control group (P<0.05). Conclusion Aerosol inhalation of salbutamol combined with psychological intervention on the treatment of severe asthma patients has significant curative effect, which can significantly improve the clinical treatment effect, improve the clinical symptoms in a timely manner, and has the value of application and promotion.
9.Analysis of risk factors for adverse pregnancy outcomes in women with gestational diabetes mellitus

Objective To evaluate clinical features,insulin sensitivity and β-cell function of pregnant women with different glucose tolerance status,so as to identify the possible risk factors for adverse pregnancy outcomes in women with gestational diabetes mellitus (GDM).Methods We retrospectively analyzed the clinical data of 360 pregnant women with positive results of 50 g glucose challenge test who received antenatal care and admitted for delivery in the period from January 2009 to June 2012 in Peking Union Medical College Hospital.According to the result of 100 g oral glucose tolerance test (OGTT),the 360 women were divided into GDM group (n =83),impaired glucose tolerance (IGT) group (n =75),and normal glucose tolerance (NGT) group (n =202).The blood glucose level in all those women was controlled in normal range for gestational period.We compared the general clinical data,biochemical indexes,insulin resistance index,insulin sensitivity index,function index of islet β-cell,first-and second-phase insulin secretion,insulin secretion-sensitivity index as well as the pregnancy outcomes of the 3 groups,analyzing the possible risk factors for adverse pregnancy outcomes in women with GDM.Results Compared with the NGT group,the pregnant women in GDM group were older [(33.1 ± 3.7) years vs.(31.7 ± 3.4) years,P =0.008],had higher systolic blood pressure [(115.8 ± 9.7) mmHg vs.(111.4 ± 13.5) mmHg (1 mmHg =0.133 kPa),P =0.031] and diastolic blood pressure in first trimester [(75.4 ±9.0) mmHg vs.(71.8 ±8.8) mmHg,P =0.010],higher positive rate of family history of diabetes in first-degree relatives (37.3% vs.22.3%,P =0.012),positive rate of insulin therapy (10.8% vs.0%,P =0.001),serum triglyceride level [(2.8 ±0.9) mmol/L vs.(2.3 ±0.9) mmol/L,P =0.001],free fatty acid level [(486.7 ± 137.6) μmol/L vs.(438.1 ± 140.7) μmol/L,P =0.033],and C-reactive protein level [(5.7 ± 4.3) mg/L vs.(3.6 ± 3.0) mg/L,P =0.001].The GDM group had a larger pre-pregnancy body mass index [(22.6 ± 2.9) kg/m2] than that in IGT group [(21.3 ± 2.7) kg/m2] (P =0.049) and NGT group [(21.2 ±2.8) kg/m2] (P =0.003).In the order from NGT to IGT to GDM group,the hemoglobin A1c [(5.2 ± 0.3) % vs.(5.3 ± 0.3) % vs.(5.4 ± 0.3) %,P =0.001,P =0.007],the areas under curve of glucose [(20.4±2.0) mmol · h/L vs.(22.9 ± 1.5) mmol · h/L vs.(26.9 ±2.1) mmol · h/L,both P=0.001] and the areas under curve of insulin [(1.7 ±0.9) × 103 pmol · h/L vs.(2.1 ± 1.1) × 103 pmol · h/L vs.(2.7±1.3) ×103 pmol · h/L,P=0.001,P=0.007] increased gradually,while insulin sensitivity index (88.1 ± 52.1 vs.80.0 ± 30.6 vs.50.0 ± 24.1,P =0.001,P =0.014) and insulin secretion-sensitivity index (134 507.0 ± 43 291.0 vs.102 542.0 ± 15 291.0 vs.77 582.0 ± 20 764.0,both P =0.001) decreased gradually.The insulin resistance index in the GDM group (3.3 ± 2.2) was significantly higher than that in IGT (2.2 ± 1.0) and NGT groups (3.0 ± 1.1) (both P =0.001).The function of β-cell,first-and second-phase insulin secretion were not significantly different among the 3 groups.Compared with the NGT group,pregnant women with GDM had shorter gestational age [(38.8 ± 1.1) weeks vs.(39.4 ± 1.1) weeks,P=0.004] and higher incidence of adverse pregnancy outcomes (44.6% vs.21.8%,P =0.001).Seven risk factors predicting adverse pregnancy outcomes in women with GDM were identified,including pre-pregnancy body mass index (P=0.017),0-,1-,and 2-hour blood glucose in 100 g OGTT (P=0.036,P=0.009,P=0.004),3-hour insulin (P =0.014),and hemoglobin A1 c (P =0.002) and C-reactive protein (P =0.005) in second trimester,among which 1-hour blood glucose displayed the highest coefficient (OR =2.767).Conclusions Pregnant women with GDM have elevated blood pressure,dyslipidemia and increased inflammatory cytokine C-reactive protein.Women with GDM and IGT both show insulin resistance and β-cell dysfunction,and these impairments are more severe in women with GDM.Higher pre-pregnancy body mass index and blood glucose levels during pregnancy are associated with adverse pregnancy outcomes in women with GDM.
