1.Curative efficacy of L -glutamine gung shimron acid sodium in combination with esomeprazole in the treat-ment of pediatric peptic ulcer and its effects on immunological function and intestinal flora

Objective To investigate the clinical effect of L -glutamine gung shimron acid sodium in combi-nation with esomeprazole in the treatment of pediatric peptic ulcer and its effects on immunological function and intes-tinal flora.Methods 120 patients of pediatric peptic ulcer meeting the inclusion criteria were randomly divided into two groups equally.Besides conventional treatments,the control group was treated with esomeprazole,and the observa-tion group was treated with L -glutamine gung shimron acid sodium in combination with esomeprazole.After the course of treatment,the curative efficacy was compared after the eight -week follow -up.Before and after the treat-ment,T lymphocyte subpopulation (CD +4 ,CD +8 ,CD +4 /CD +8 )reflecting cellular immune function,intestinal flora and adverse reactions were compared.Results After the eight -week follow -up,the total effective rate of the observa-tion group was 96.7%,which was significantly higher than 86.7% of the control group (χ2 =3.93,P <0.05).After treatment,levels of CD +4 [(43.5 ±6.6)% vs (40.9 ±6.2)%],CD +4 /CD +8 [(1.72 ±0.38)vs (1.38 ±0.45)] were statistically higher and level of CD +8 [(26.2 ±3.7)% vs (30.1 ±5.0)%]was statistically lower in the obser-vation group in comparison with the control group (t =2.22,P <0.05;t =4.47,t =4.86,P <0.01).As to intestinal flora,the observation group had statistically higher concentration of lactobacillus [(7.5 ±1.3 )LonN /g feces vs (5.8 ±1.2)LonN /g feces],bifidobacterium[(8.3 ±1.1)LonN /g feces vs (6.6 ±1.4)LonN /g feces]in the feces in comparison with the control group after treatment (t =7.44,t =7.40,both P <0.05).During the treatment,there were no severe adverse reactions.Conclusion L -glutamine gung shimron acid sodium in combination with esome-prazole is effective for pediatric peptic ulcer,which can significantly increase curative efficacy,enhance cellular immune function and effectively regulate intestinal flora with minor adverse reactions.
2.Effect evaluation of admission test combined with artificial rupture of membrane to predict delivery and fetal outcome

Objective To evaluate the value of admission test of fetal heart rate and artificial rup-ture of membrane in predicting fetal distress in labor. Methods 658 pregnant women who had been pregnant for 33+5 to 43 weeks and delivered spontaneously in 24 hours were divided into low risk group(598 cases) and hign risk group(60 cases) according to their admission test. The admission test, result of deliv-ery, amniotic fluid contamination and Apgar scores of newborns were analyzed. Results 515 cases were reactive type(78.27%), 72 cases were equivocal type(10.94%) and 71 cases were ominous type(10.79%). Total incidence of neonatal asphyxia were 6.53%,and it was 1.94% and 33.08% in reactive type subjects and ominous type subjects, respectively. The rates of anmiotic fluid contamination were 20. 97% and 67.61% in reactive type subjects and ominous type subjects (P<0.01). The incidence of reactive type and equivocal type of amniotic fluid contamination and neonatal asphyxia were significantly lower in the low risk group than those in the high-risk group (P<0.05). Conclusions Reactive type of admission test can ensure safety of fetus during the subsequent four to five hours. Admission test of fetal heart rate and artifi-cial rupture of membrane can be used to predict fetal outcome in labor.
3.Reducing radiation dose with electrocardiogram-pulsing technique in 64-row multi-slice spiral CT coronary angiography

Objective To evaluate the value of reducing radiation dose with ECG-pulsing and image quality in 64-row multi-alice CT coronary angiography.Methods Fifty-nine consecutive patients whose heart rates were less than 80 beat per minute and cardiac rhythm was regular were randomly divided into two groups from October 26, 2007 to March 12, 2008.Conventional technique of CT coronary angiography was employed in group 1, while ECG-pulsing technique was applied in group 2.CT dose index volume (CTDIvol) and dose length product (DLP) were obtained automatically, and then the corresponding effective dose (ED) were calculated.The quality scores were performed on obtained imaging by using double blind method.Student t-test was applied in the comparison of value of CTDIvol, value of ED and quality of imagiugbetween two groups.Results The value of CTDIvol was (70.0±1.1) and (39.0±2.7) mGy, the value of ED was (16.8±2.0) and (9.5±1.7) rosy in group 1 and group 2, respectively, which reached statistically significant differences between the groups(t=57.675,15.346 ,P <0.01, respectively). The quality scores of coronary images were 3.8±0.2 and 3.8±0.1 in the groups, which did not reach the statistical significance (t=-0.222, P 0.05).Conclusions The proper application of ECG-Pulsing technology in 64-slice spiral CT coronary angiography can reduce radiation dose significantly while having no influence on the quality of the imaging.
4.Comparison of BRAF mutation detection in patients with papillary thyroid microcarcinoma by ARMS and direct sequencing

Objective:To investigate the sensitivity and the specificity of scorpions amplification refractory mutation system ( ARMS) in comparing with that of direct DNA sequencing in the detection of BRAF gene mutations in patients with papillary thyroid microcarcinoma.Methods:Direct sequencing and ARMS were used simultaneously to detect BRAF mutation status in 56 patients with PTMC.Results:BRAF mutations were identified in 46 cases with a mutation rate of 82.9%by ARMS,while in 18 cases with a mutation rate of 32.1%by direct sequencing.Besides,the sensitivity of ARMS was 100%and that of direct sequencing was 39.1%.There were significant differences of both mutation rate and sensitivity between two methods ( P<0.01 ).Conclusion: Compared to direct sequencing,ARMS gains a higher sensitivity in the detection of BRAF mutations in samples with tiny lesions.
5.EBV DNA detection in the diagnosis of Epstein-Barr virus infection realated diseases in children

Objective To investigate the clinical significance of Epstein-Barr virus EBV DNA in children with Epstein-Barr virus infection realated diseases.Methods A retrospective cohort study was performed.Totally 222 blood samples were collected from children who were diagnosed as EBV infection in Shandong Provincial Hospital from June 2012 to August 2013.Fluorescent quantitative PCR( FQ-PCR) was used to analyze the EBV DNA in peripheral blood lymphocytes.ELISA was used to analyze the four EBV serology antibodies in the serum.Two groups of tested results were compared.Heart, hepatic impairment and renal function were analyzed through detecting AST, ALT, BUN, CREA, CK, CKMB.The results were grouped by EBV DNA copy number, and then non-parametric test together with correlation analysis was performed using SPSS21.0 analytics software.Results The positive rate of EBV-CA IgM and EBV DNA was 51.35%(114/222) and 72.97% (162/222) respectively, χ2 =24.01, P<0.001.The EBV DNA copy number was significant difference (χ2 =11.79,P<0.05) in children at different stages of infection ( no previous infection:6.30 ×103 -1.20 ×104 copies/ml;early infection:5.56 ×103 -3.92 ×106 copies/ml;acute infection:6.58 ×103 -1.73 ×106 copies /ml;chronic infection or recurrent infection:8.92 ×103 -2.34 ×104 copies/ml;late infection or recovery:5.20 ×103 -1.12 ×107 copies/ml;past infection:5.46 × 103 -1.33 ×104 copies/ml).Each biochemical targets were divided into five groups by EBV DNA copy number (Ⅰ>1 ×106 copies/ml,Ⅱ1 ×105 -1 ×106 copies/ml, Ⅲ1 ×104 -1 ×105 copies/ml, Ⅳ5 × 103 -1 ×104 copies/ml, Ⅴ<5 ×103 copies/ml), and ALT(χ2 =10.14,P<0.05), BUN(χ2 =18.17, P<0.05), CK(χ2 =13.09,P<0.05), CKMB(χ2 =17.93,P<0.01) had a statistically significant difference between each group.Well, the log value of EBV DNA copy number had a positive correlation relationship with AST(r=0.357,P=0.001), ALT(r=0.376,P=0.001), BUN(r=0.329,P=0.000), CK(r=0.235,P=0.035).Conclusions Detection of EBV DNA can be used for the early diagnosis and assessment of process of the EBV infection related disease in children.The detection of liver, kidney function and myocardial enzymes can be used for evaluating the severity of EBV infection.
6.Clinical Value of CD44 mRNA and CD24 mRNA and Protein Expression Levels in Placental Tissue of Patients with Severe Preeclampsia

Objective To explore clinical value of the expression levels of cell surface transmembrane glycoprotein molecule 44(CD44)mRNA,cell surface transmembrane glycoprotein molecule 24(CD24)mRNA,and protein in the placenta of severe preeclampsia(SPE)patients.Methods The SPE patients who were delivered by cesarean section in the Second People's Hospital of Liaocheng from June 2019 to June 2022 were further divided into 45 patients in early onset SPE group(gestational age≤34 weeks)and 55 patients in late onset SPE group(gestational age>34 weeks)according to the different gestational age.The control group consisted of 100 normal cases in the same period.The expression of CD44 and CD24 in placenta of SPE patients was detected by fluorescent quantitative PCR and immunohistochemistry,Pearson method was used to analyze the difference of their expression levels and their correlation with the clinical characteristics of SPE disease,and multivariate logistic regression was used to analyze the influencing factors of SPE.Results Compared with the control group,the expression levels of CD44 mRNA(0.55±0.12 vs 1.02±0.33)and CD24 mRNA(0.68±0.19 vs 1.05±0.11)in SPE placental tissues decreased significantly,the differences were statistically significant(t=13.385,16.853,P<0.05).The immunohistochemical staining results showed that CD44 and CD24 were mostly negative or weakly positive in the SPE group placental tissue,while they were mostly positive in the control group,the positive rates of CD44 and CD24 in the SPE placental tissue were lower than those in the control group,and the differences were statistically significant(χ2=9.696,14.346,P<0.05).Compared to the early onset SPE group,the expression levels of CD44(0.65±0.17 vs 0.42±0.11)and CD24(0.77±0.23 vs 0.58±0.13)mRNA in placental tissue of late onset SPE were higher,and the differences were statistically significant(t=7.830,4.932,P<0.05).Compared with the control group,the BMI,systolic blood pressure,diastolic blood pressure,urinary protein,Cr,LDH and BUN were significantly increased in SPE group(t=5.360~30.241,all P<0.05).In SPE group,the gestational age was earlier,the MPV and ALB were lower,the newborn's birth length was shorter,and the body weight than control group,the differences were statistically great(t=3.232~11.109,all P<0.05).The expression of CD44 and CD24 in SPE placenta was positively correlated(r=0.698,P<0.05),the expression of CD44 in SPE placenta was positively correlated with CD24,gestational week of delivery,MPV and neonatal birth length(r=0.611,0.639,0.612,0.465,all P<0.05),and was negatively correlated with systolic blood pressure,urinary protein and LDH(r=-0.604,-0.569,-0.593,all P<0.05).The expression of CD24 was positively correlated with gestational age,MPV and newborn birth length(r=0.605,0.584,0.640,all P<0.05),and was negatively correlated with systolic blood pressure,urinary protein and LDH(r=-0.637,-0.593,-0.561,all P<0.05).The results of logistic regression analysis showed that MPV(95%CI:1.429~4.350),urinary protein(95%CI:1.529~2.709),and LDH(95%CI:1.425~3.932)were all independent risk factors for SPE(all P<0.05).High levels of CD44(95%CI:0.561~0.940)and CD24(95%CI:0.495~0.814)were independent protective factors for SPE(P<0.05).Conclusion The low expression levels of CD44 and CD24 in placenta of SPE patients are independent protective factors of SPE,which can provide direction for the follow-up treatment of SPE.
7.Genotyping and traceability analysis of Brucella isolated from Himalayan marmot in Qinghai-Tibet Plateau by MLVA

Objective:To observe multiple locus variable-number tandem repeat analysis (MLVA) typing of Brucella isolated from Himalayan marmot in Qinghai-Tibet Plateau of Qinghai Province, and to explore the relationship between the strains and strains previous isolated from Qinghai Province. Methods:Blood samples of Himalayan marmot were collected in Qinghai-Tibet Plateau of Qinghai Province from March 2019 to October 2020. Pathogens were isolated and cultured from Brucella antibody positive samples identified by using the rose bengal test (RBT). Conventional biological methods and molecular biological methods (BCSP31-PCR and AMOS-PCR) were used for strain identification. At the same time, MLVA method was used to genotype the isolated strains, and cluster analysis was used to analyze the genetic relationships between the strains based on the genotype of 70 Brucella isolated from different hosts in Qinghai Province. Results:A total of 1 466 blood samples of Himalayan marmot were collected from Qinghai-Tibet Plateau. Two strains of Brucella were isolated and cultured from 64 RBT-positive samples, named QH2013054 and QH2013062, respectively. They were identified as Brucella ovis biotype Ⅲ by conventional and molecular biological methods. The MLVA genotyping results showed that QH2013054 and QH2013062 were different at the Bru16 locus, indicating different MLVA genotypes. Cluster analysis showed that strain QH2013054 had the same MLVA genotype as 7 strains, among which 6 strains were from 3 farmers and 3 sheep from the same family in Gonghe County, and 1 strain was from a farmer in Menyuan Hui Autonomous County. The strain QH2013062 had the same MLVA genotype as 4 strains, including 3 strains from 3 farmers in Menyuan Hui Autonomous County and 1 strain from a farmer in Tu Autonomous County of Huzhu. Conclusions:The strains of Brucella isolated from Himalayan marmot in Qinghai-Tibet Plateau of Qinghai Province have the same MLVA genotype as some strains of Brucella isolated from humans and sheep in Qinghai Province. It is speculated that the host humans, sheep and Himalayan marmot in Qinghai-Tibet Plateau may have a common source of infection.
8.Changes of serum sTREM-1, NF-κB and IL-8 levels in senile patients with hospital-acquired pneumonia

Objective To observe the changes of soluble triggering receptor expressed on myeloid cell-1 (sTREM-1)and inflammatory mediators in serum of senile patients with hospital-acquired pneumonia and explored the state of local inflammatory response in HAP. Methods Twenty senile patients with HAP and twenty healthy controls were in-cluded. Serum sTREM-1, nuclear factor-κB (NF-κB) and interleukin-8 (IL-8) were detected on day 1, 5, 10. Com-bined with CPIS score to analyze the correlation of these indicators and the severity and prognosis of pneumonia. Re-sults The concentrations of sTREM-1, NF-κB and IL-8 in serum were significantly higher than those in controls. There was a significant positive correlation between the sTREM-1, NF-κB and IL-8 and CPIS score. Conclusion sTREM-1, NF-κB and IL-8 is a good marker in diagnosing pneumonia. The levels of sTREM-1, NF-κB and IL-8 have a positive correlation with severity of HAP.
