1.Comparison of two types of silicone impressions on accuracy of implant model

Objective To compare the different accuracy of implant model made by the two types of silicone impressions.Methods Fifteen impressions of standard abutment were made separately by machine-mixed and hand-mixed,then made into super-stone models and the accuracy of the different impressions was compared.Results The two types of silicone impressions showed significant difference of the margin precision,while differences on the height,diameter and the width of abutment shoulder were not found.Conclusions The two methods both have ideal accuracy,but porcelain-fused-to-metal restorations require high accuracy of the model shoulder,which is not suitable to use machine-mixed silicone impression technology,and hand-mixed method can be used in fixed restoration,removable denture and implant level impression.
2.Comparison and improvement of three methods for primary culture of rat embryonic cortical neurons

Objective To compare three methods for culture of fetal cortical neurons of SD rats and find out the suitable culture conditions of fetal cortical neurons in vitro.Methods The cortex of 16-18-day embryonic rat was used for culture in this study.Mechanical dissociation, trypsin digestion and papain digestion were applied respectively to the neuron culture.The morphological characteristics of neuronal cells at different time points were observed and neuron purity was identified by immunofluorescence staining assay.Results High purity of the fetal rat cortical neurons was successfully achieved by all the three culture methods, and each had distinct morphological characteristics at different time points.The purity of neurons was 96.28%, 95.63%and 97.34%, respectively, with no significant differences among the three groups (P>0.05).Conclusions The three culture methods are improved in our study.Stable neurons with high purity can be obtained by all the three methods respectively, and each of these methods has distinct characteristics.
3.The clinical effect and the preventive analysis of complications on the application of self- prescription soup for eliminating haemorrhoids after patients with moderate or severe hemorrhoids received surgery

Objective:To analyze the clinical effect and the preventive analysis of complications on the application of self-prescription soup for eliminating haemorrhoids after patients with moderate or severe hemorrhoids accepted surgery.Methods: 44 patients with moderate and severe hemorrhoids who once accepted operative treatment were divided into observation group and control group as random number table. The patients of control group received the treatment of hip bath by using potassium permanganate tablets, while the patients of observation group received the treatment of hip bath with fuming and washing by using self-prescription soup for eliminating haemorrhoids. The clinically curative effect, postoperative complication, pain, bleeding, edema, disappearance time of anal pendant expansion and healing time of wound between the two groups were compared and analyzed.Results: The total effective rate (100%) of the observation group was significantly better than that (77.27%)of the control group (x2=5.641,P<0.05). The cases of complication including pain, bleeding, constipation, infection, urinary retention and anal stenosis of observation group were significantly less than that of control group (t=7.333,t=15.655,t=5.500, t=4.247,t=8.193,t=5.500,P<0.05). Besides, the times of pain, bleeding, edema, disappearance of anal pendant expansion and wound healing of observation group were significantly, respectively, less than that of control group (t=3.171, t=5.864,t=4.263,t=6.219, t=10.803,P<0.05).Conclusion: The curative effect of self-prescription soup for eliminating haemorrhoids after patient with moderate or severe hemorrhoids accept surgery is significant, and it can effectively prevent and treat various complication.

The overall rising rate of full recovery and the lowering rate of death of serious and critical patients have shown a higher medical quality.All this attributes largely to the growing attention to medical administra- tion and quality control,tightening infection control,more strict practice of ward visits by three ranks of doctors,more care to serious and critical patients and the trianing of technical personnel.More actions have been taken to answer the call of develoPing the hospital through science and technology with special measures focusing on the basic construction required by the call and technical competitiveness of major departments and on the concentrating attacks on hot problems.
5.X-ray,CT and MR manifestations of spinal giant cell tumor

Objective To explore the X-ray ,CT and MR manifestations of spinal giant cell tumor(GCT) for improving the diag-nosis level of this rare disease .Methods The X-ray ,CT and MR data in 16 cases of spinal GCT were retrospectively analyzed on the location ,size ,morphology ,margin ,CT density/MR signal intensity and enhancement pattern .Results 4 cases located in cervical vertebra ,8 cases in thoracic vertebra and 4 cases in lumbar vertebra ;in which ,6 cases only involved in vertebral body and other 10 cases involved in vertebral body and and appendix .The lesion affected two neighboring vertebrae in 8 cases ,and affected only one vertebra in the remaining 8 cases .16 cases were protruded into the spinal canals and oppressed the spinal cord .The prominent X-ray and CT images showed eccentric growth ,osteolytic and swelling bone destruction .MR showed isointensity or slight hypointensity on T1WI ,and isointensity or slight hyperintensity on T2WI .After intensified scanning ,the solid portion showed marked inhomoge-neous enhancement .Conclusion Spinal GCT has certain imaging characteristics .CT and MR examination could accurately display the extent of lesion and tumor involvement ,which provides more accurate image information for formulating the strategy of clinical therapy .
6.Effect ofIllicium verum Water Extract on Excess-cold Syndrome in Rats

This study was aimed to observe the effect ofIllicium verum water extract on excess-cold syndrome in rats. The intragastric administration of the decoction of gypsum, gentian, phellodendron andrhizoma anemarrhenae (2:1.2:1:1.5) was used to establish the excess-cold rat model. Effects ofI. verum water extract on general body signs of excess-cold syndrome rats were observed. The detection was made on the contents of LD, PA, TG, TSH, T4 in the serum and LD, PA, glycogen in liver tissues. The vitality of the LDH, SDH and ATPas was also detected. The results showed thatI. verum water extract can improve the general body signs of excess-cold syndrome rats. It reduced the content of TG; increased the content of PA, T4, TSH; reduced the content of LD, glycogen, and the vitality of LDH;increased the vitality of LDH, Ca2+-Mg2+-ATPase in liver tissues. It was concluded thatI. verum water extract can improve some general body signs of excess-cold syndrome rats, improve the substance and energy metabolism, and regulate the thyroid axis function, in order to achieve the warmingyang for dispelling cold effect.
7.Effect of perioperative intravenous flurbiprofen axetil on chronic postmastectomy pain

Objective:To explore whether perioperative intravenous flurbiprofen axetil can reduce the incidence and intensity of chronic pain for breast cancer atfer surgical treatment. Methods:This randomized, double-blind, controlled trial enrolled 60 patients undergoing mastectomy and axillary lymph node dissection under general anesthesia. All patients accepted Hospital Anxiety and Depression Scale (HAD) tests the day before the surgery to evaluate depression and anxiety. hTe patients were randomly assigned to receive either 50 mg lfurbiprofen axetil intravenously 15 minutes before the surgical incision and 6 hours later (group F) or intravenous 5 mL intralipid as a control (group C). All patients received patient-controlled intravenous analgesia (PCIA) with fentanyl postoperatively. Peripheral venous blood samples were drawn before the surgery, at 4 and 24 h atfer the surgery to detect the plasma level of PGE2 and tumor necrosis factor-α(TNF-α). Postoperative fentanyl consumption, Numerical Rating Scale (NRS) scores and adverse effects were recorded at 2, 6, 12, 24 and 48 h after the surgery. hTe duration and intensity of pain were followed up by telephone at the 2nd-12th month atfer the surgery. Results:The incidence of pain at 2, 4, 6, and 12 months after the breast surgery was 33%, 20%, 15%, and 10%, respectively, and the average pain score was 0.77, 0.57, 0.28, and 0.18, respectively. Compared with group C, the scores of pain in group F were significantly lower at 2, 4, 6 and 12 months postoperatively (F=7.758, P=0.007). The incidence of pain in group F was significantly lower at 2, 4 and 6 months postoperatively (P<0.05). There was no significant difference in the incidence of pain between the groups at 12 months postoperatively (P>0.05). Preoperatively and at 4 and 24 h atfer the surgery, there was no signiifcant difference in the level of TNF-αbetween the two groups (F=0.530, P=0.470);but plasma concentration of PGE2 in group F was significantly lower than that in group C (F=5.646, P=0.021). No patients developed abnormal bleeding, peptic ulcer, impaired liver or renal function and respiratory depression. Conclusion:Perioperative intravenous infusion of 100 mg flurbiprofen axetil can decrease the intensity and incidence of chronic pain for breast cancer atfer surgical treatment.
8.Emergency treatment of endobronchial stent placement for serious main bronchial stenosis following high-risk orthotopic heart allotransplantation: One case report

BACKGROUND: Ventilation dysfunction caused by bronchomalacia induced bronchostenosis following high-risk heart transplantation is an acute clinical disease, which seriously impairs the function of transplant heart. The case of emergency bronchial stent placement following heart transplantation with high-risk multi-complication has not been reported yet.OBJECTIVE: To investigate the curative effect of emergency stent placement for worse left main bronchial malacia, stenosis and collapse following orthotopic heart allotransplantation.DESIGN: A case analysis.SETTINGS: National Ministry of Health Transplantation Engineering and Technical Research Center, the Third Xiangya Hospital, Central South University; Department of Cardiosurgery, Zhongshan Hospital affiliated to Xiamen University.PARTICIPANTS: An 18-year-old female patient with dilated cardiomyopathy accompanied by moderate to severe pulmonary artery hypertension, who sequentially carried out orthotopic heart allotransplantation, was selected from the Department of Cardiosurgery, Zhongshan Hospital affiliated to Xiamen University in April, 2004. She had suffered from dilated cardiomyopathy for 15 years, and the mean pulmonary artery pressure (MPAP) was 50-51 mm Hg, she was also accompanied by left main bronchial malacia, stenosis and collapse, mixed (mainly central-) sleep apnea syndrome, left inferior pulmonary sequestration, right emphysema, and rheumatoid arthritis for half a year.METHODS: After heart transplantation, bronchus inflammation, congested edema aggravated the severity of bronchial malacia, stenosis and collapse, tenosis reduced to 4/5, and led to obstructive type of ventilation, and the patient was also accompanied by supraventricular tachycardia, ventricular extrasystole, and hypofunction of transplant cardiac systolic function (peak E<peak A, ejection fraction reduced to 40%, inharmonious motion of ventricular wall). Attempted with inotropic agents and ventilator/support were not relieved, which resulted in the aggravation of illness. In order to improve the post-transplant cardiac function, to relieve bronchial collapse and stenosis, and correct the obstructive type of ventilation, an emergency bronchial stent placement surgery was carried out on the sixth day after heart transplantation. Under monitoring of electrocardiogram (EGG) and percutaneaous oxygen saturation (SpO2), patient was awake and in supine to relieve left main bronchial stenosis with a nickel-titanium shape memory alloy stent (Diameter: 12 mm; length: 20 mm) by D20 fiberoptic bronchoscope. Fibrobronchoscopy was used to observe the proximal end of bronchostenosis and set the proximate location mark by using video fluoroscopy; the patency of distal end was explored by stricture, and set the distal location mark; guidewire was inserted into working path of bronchofibroscope and led through the stricture; then loaded the Ni-Ti stent on a special placement apparatus, and led in bronchial stent implantation apparatus along guidewire. When targeting well, the stent was slowly released and adjusted properly. When it was completely released, the stent implantation apparatus was drawn out. Bronchofibroscope was performed postoperatively to observe the adherence of stent; immediately photographed to observe its unfolding. Synchronized intermittent mandatory ventilation (SIMV) was given postoperatively as supportive treatment.MAIN OUTCOME MEASURES: Ameliorations of the cardiac and pulmonary functions of the patient.RESULTS: ①Carbon dioxide retention and hypercapnia were remarkably improved as compared with those preoperatively; hypertensive pulmonary vascular disease was alleviated gradually, and MPAP reduced to 30 mm Hg. One week later, re-examination of bronchofibroscopy was carried out, and the results showed that bronchi of left upper lobe, lingual lobe as well as left lower lobe could be seen distinctly, mucous membrane had slightly congested edema, and lumens were unobstructed.②Supraventricular tachycardia and premature ventricualr contraction disappeared, and the transplant cardiac function recovered well (peak E > peak A, ejection fraction 70%, FS41%), and the heart rate fluctuated at 100-110 beats per minute. ③The chest-radiography and CT postoperatively indicated the relief of left main bronchial stenosis. When the ventilation function of the patient was improved, the parameters of breathing machine were reduced gradually, and replaced by low-flow oxygen. There was no recurrence of obstructive ventilatory disorder. The sleep apnea syndrome of the patient was moderated.CONCLUSION: Emergency treatment with stent placement for bronchial malacia, stenosis and collapse occurring after orthotopic heart allotransplantation cAN improve ventilation dysfunction caused by bronchial malacia and stenosis,and increase the survival rate of heart transplantation.
9.Study on quality control of Minuo Hair Solution

Objective To improve the quality standard of Minuo Hair Solution and to ensure product quality .Methods Thin layer chromatography (TLC) and UV-visible spectroscopy (UV-Vis) method was used for the qualitative identification of Angelicasinensis ,Paeoniae Radix Rubra ,Dictamni Cortex and minoxidil respectively .The Quantitive determination of mi-noxidil was completed by first order derivative spectroscopy .Results The spots in TLC were clear without inference .And the same color spots appeared in the corresponding position of references and reference drugs .The UV-Vis spectral absorption of minoxidil were in (230 ± 3) ,(262 ± 3) ,(286 ± 3) nm .The linear range of minoxidil was obtained between 6 .2 and 18 .6 μg/ml (r=0 .999 7) .The average recovery was 99 .4% (RSD=0 .22% ) .Conclusion The method is simple ,accurate and reproduci-ble ,and can be effectively used to control the quality of Minuo Hair Solution .
10.Serum levels and clinical significance of high mobility group protein B1 in patients with delayed encephalopathy after acute carbon monoxide poisoning.

OBJECTIVETo investigate the changes in serum level of high mobility group protein B1 (HMGB1) in patients with delayed encephalopathy after acute carbon monoxide poisoning and the clinical significance of these changes.
METHODSThirty-four patients with delayed encephalopathy after acute carbon monoxide poisoning (delayed encephalopathy group), 30 normal controls (control group), and 32 cases of acute carbon monoxide poisoning without delayed encephalopathy (carbon monoxide poisoning group) were recruited in this study. The serum HMGB1 level was determined by enzyme-linked immunosorbent assay. The correlation between serum HMGB1 level and scores of the activity of daily living scale (ADL), Information-Memory-Concentration Test (IMCT), and Hasegawa dementia scale (HDS) was determined.
RESULTSIn the acute stage of carbon monoxide poisoning, the serum HMGB1 level of delayed encephalopathy group was significantly higher than those of the carbon monoxide poisoning group and the control group (P < 0.01). In the delayed encephalopathy group, serum HMGB1 level in the convalescent stage was significantly lower than that in the acute stage (P < 0.05); ADL score was higher and HDS and IMCT scores were lower in the acute stage than in the convalescent stage (P < 0.01). In the delayed encephalopathy group, serum HMGB1 level was positively correlated with HDS and ADL scores in both acute stage and convalescent stage (correlation coefficients: 0.612, 0.607, 0.609, and 0.612, P < 0.01).
CONCLUSIONHMGB1, as an important late mediator of inflammation, is involved in the inflammatory reaction in delayed encephalopathy, and is positively correlated with HDS and ADL scores, indicating that it can be used as one of the major indicators in monitoring carbon monoxide poisoning.
