1.Clinical molecular epidemiological study of human parvovirus B19 infection in patients with leukopenia

Objective To explore the distribution of parvovirus B19 (HPVB19) infection in patients with leukopenia.Methods Patients who visited the Affiliated Hospital of Hangzhou Normal University from January 2015 to June 2016 were analyzed.Patients with peripheral leucocytes count less than 3.5×109/L were included in experiment group and healthy people were included in control group.HPVB19 IgG and IgM were detected by enzyme-linked immunosorbent assay, and HPVB19 DNA was detected by quantitative polymerase chain reaction.Differences in continuous data between two groups were compared with two-sample t test and those in categorical data were compared with Chi-square test.Results A total of 79 patients were included in experiment group, including 32 males and 47 females.Ages ranged from 24 to 62 years old.And 126 healthy individuals were included in control group, including 55 males and 71 females.Ages ranged from 28 to 67 years old.The positive rates of HPVB19 IgG, IgM and DNA in experiment group were 34.2%, 5.1% and 3.8%, respectively, while those in control group were 36.5%, 0 and 0, respectively.The detection rates of HPVB19 IgM and DNA between two groups were significantly different (χ2=6.507, P=0.011 and χ2=4.856, P=0.028, respectively).Sequence analysis for 3 of the HPVB19 DNA positive samples showed that there were two single nucleotide polymorphisms in VP1/VP2 sequence from one patient, which contributed to the 153rd (L/H) and 219th (N/Y) amino acids mutations, respectively.Phylogenetic analysis found that two strains belong to genotype 1a and one strain belongs to genotype 1b.Conclusions Detection rate of parvovirus HPVB19 infection (positive rates of HPVB19 IgM and DNA) in leukopenic patients is significantly higher than healthy controls.HPVB19 should be detected before considering transfusion in leukopenic patients in clinical practice.
2.Expression of nuclear factor ?B and the effect of topical tacrolimus ointment on lesional atopic dermatitis skin

Objective: To investigate the role of nuclear factor ?B (Rel/NF-?B) in pathogenesis of atopic dermatitis(AD) and the effect of topical 0.1%(mass fraction) or 0.03%(mass fraction) tacrolimus ointment on expression of NF-?B in lesional AD skin. Methods: Immunohistochemistry has been employed to study the expression of NF-?B in normal skin and lesional AD skin before and after using topical tacrolimus ointment. Results: The expressions of NF-?Bp50 and NF-?Bp65 were scattering or negative in normal keratinocytes. NF-?Bp50 was overexpressed on nuclear of basal and suprabasal keratinocytes in 9 cases of AD, NF-?Bp65 was overexpressed in cytoplasm and perinuclear of basal and suprabasal keratinocytes. After using topical tacrolimus ointment for three weeks , nuclear NF-?Bp50 expressed on basal and suprabasal keratinocytes were lost and NF-?Bp50 was expressed sparsely on basal keratinocytes cytoplasm or nuclear. NF-?Bp65 was expressed sparsely on basal and suprabasal keratinocytes cytoplasm. Conclusion: These data suggest that increased NF-?B activity may represent the basis of initiation or maintenance of the skin inflammatory response in atopic dermatitis. Topical tacrolimus may directly or indirectly inhibit NF-?B nuclear expression in keratinocytes and inhibit skin innate immuno-inflammatory response in atopic dermatitis that related to NF-?B.
3.Effect of the 20° trendelenburg position on hypotension during induction of general anesthesia of patients undergoing gastrectomy

Objective To explore the effect of 20°trendelenburg position on hemodynamics during induction of general anesthesia of patients under gastrectomy.Methods One hundred and eighteen patients (77 males and 41 females,aged 50-60 years,ASA physical status Ⅰ or Ⅱ),scheduled for elective radical gastrectomy,were randomly divided into group A (n=40),group B (n=38) and group C (n=40).In group A,patients were placed in the supine position during induction of anesthesia,and vasopressors was administered when hypotension occurred.In group B,patients were placed in the supine position during induction of anesthesia and was placed in the 20°trendelenburg position when hypotension occurred,and vasopressor would be administered if blood pressure was not restored.In group C,patients were placed in the 20°trendelenburg position during induction of anesthesia,and vasopressors was administered when hypotension occurred.In all groups Lactated Ringer′s was infused at 15 ml·kg-1·h-1in 30 min before induction.Anesthesia was induced with propofol 2 mg/kg,sufentanil 0.5 μg/kg,rocuronium 0.9 mg/kg.The occurrence of hypotension and the total amounts of drug administrations were doccumented.Results Nineteen patients (47.5%) in group A,fourteen patients (36.8%) in group B and six patients (15.0%) in group C developed hypotention.The incidence of hypotension in group A and B was significantly higher than that in group C (P<0.05).In all groups the total dosage of ephedrine was used.Phenylephine was only used in the group A at a dose of 26.5±0.14 μg.The amounts of drug administrations in group B and C were significantly lower than that in group A (P<0.05).Two patients in group A went into a refractory hypotensive state.However,no patients in all groups fell into a serious arrhythmia state,so atropine was not used.Conclusion The 20°trendelenburg position during induction of general anesthesia of patients during gastrectomy can prevent hypotension during induction.This position is effective in the management of hypotension after the induction of general anesthesia and can decrease the amounts of drug administrations during induction of general anesthesia.
4.Characteristics of the spatial distribution of myopia prevalence for Chinese children and adolescents aged 7-18 years from 2005 to 2014

Objective:
To explore the spatial distribution pattern and characteristics of myopia prevalence for Chinese children and adolescents aged 7-18 years old from 2005 to 2014, and to provide scientific basis for regional myopia prevention and control.
Methods:
Using data from the National Survey on the Constitution and Health of Chinese Students from 2005 to 2014,children and adolescents aged 7-18 yeaer old with Han and Tibetan ethnicity were included into analysis by using the Arc-GIS 10.2 software.
Results:
From 2005 to 2014, the overall Moran’s index were 0.13,0.02,-0.06(P>0.05), there was no spatial autocorrelation. Local autocorrelation analysis showed that Shandong, Jiangsu, Shanghai, and Zhejiang Province were "high" aggregation areas, while Hainan and Guangxi Province were "low" aggregation areas.
Conclusion
The myopia prevalence of children and adolescents in 31 provinces, municipalities, and autonomous regions shows significant local spatial autocorrelation, most of which are positive correlation; the number of high-high aggregation gradually decreases, while the number of low-low and low-high aggregation gradually increases.
5.Early predictors of refractory epilepsy: a case-control study

ObjectiveTo identify the early predictors of refractory epilepsy (RE). MethodsAll 173 epileptic patients with correct diagnosis and reasonable treatment were enrolled. The 106 patients were classified as drug non-responsive epilepsy (DNR-EP). The remaining 63 patients were classified as drugresponsive epilepsy (DR-EP). With multiple logistic regression, the clinical characteristics between the two groups were compared to identify the early predictors of RE. ResultsMultiple logistic regression analysis demonstrated that more than 10 seizures before treatment (OR =4. 46, 95% CI 1.60-12. 40, P =0. 004),mental retardation at early time ( OR =19. 87, 95% CI 3. 60-109. 78, P =0. 001 ) and abnormal electroencephalogram(EEG) with epileptiform wave after treatment ( OR =7.57, 95% CI 2. 54-22. 56,P <0. 01 ) were independent predictors of RE.Response to initial therapy was a protective factor of RE (OR=0.05, 95% CI 0.018-0. 139, P<0.01). ConclusionPatients who have many seizures before treatment, mental retardation at the early time, epileptiform abnormality in EEG after treatment and who are resistant to initial therapy are likely to develop into refractory epilepsy.
6.The correlation study of allergic rhinitis and chronic rhinosinusitis with nasal polyps in Ningxia region.

OBJECTIVE:
To investigate the correlation of allergic rhinitis and chronic rhinosinusitis with nasal polyps in the Ningxia region.
METHOD:
One hundred and sixteen patients diagnosed with allergic rhinitis, chronic rhinosinusitis with nasal polyps, allergic rhinitis combined with chronic rhinosinusitis with nasal polyps in the department of Otorhinolaryngology Head and Neck Surgery, Ningxia Medical University Hospital from 2009 January to December was analysed in this study, followed by grouping the experimental group I: allergic rhinitis(AR) group (46 cases), the experimental group II: chronic rhinosinusitis with nasal polyps group (46 cases), the experimental group III: allergic rhinitis combined with chronic rhinosinusitis with nasal polyps group (24 cases), and the normal control group (31 cases). The skin prick test was used and the serum specific IgE concentration of selected objects was measured by enzyme linked immunosorbent assay.
RESULT:
(1) The positive rate of specific IgE in the experimental group I was 83.3%, the concentrated for specific IgE mainly graded in 0 to 3 level, The total positive rate of specific IgE in the experimental group II and III were 75.0% and 87.5%. (2) The specific IgE concentration in the experimental group I, II , III and control group were 10.33 +/- 2.12 (kU/L), 8.43 +/- 2.23 (kU/L), 5.28 +/- 2.16 (kU/L), 1.03 +/- 3.96 (kU/L), respectively. The data analyzed using ANOVA showed that the specific IgE concentration in the experimental group I, II, III compared with the control group was significantly different;
CONCLUSION
(1) The specific IgE concentration in the Ningxia region of allergic rhinitis, chronic rhinosinusitis with nasal polyps were graded in 0 to 3 level, mainly graded in 1 level and lower than other regions. The positive rates of the skin prick test about Wormwood, German cockroach, house dust mites and pollen IV allergen were relatively higher in Ningxia region. (2) The specific IgE concentration was relatively higher in the allergic rhinitis patients combined with chronic rhinosinusitis with nasal polyps. Allergic rhinitis may promote the development of chronic rhinosinusitis with nasal polyps.

7.Construction of subtractive cDNA library of apoptosis-related genes in NB4 cells treated by arsenic trioxide.

OBJECTIVEConstruct the gene library of apoptosis related genes in acute promyelocytic leukemia (APL) cell line NB4 cells treated by arsenic trioxide to clarify the apoptotic mechanism of NB4 cells.
METHODAPL cell line NB4 cells treated with or without arsenic trioxide for 24 hours. Total RNA was extracted and suppress subtractive hybridization (SSH) was conducted according to the manual. With the cDNA of the apoptosis cells as the tester and that of control cells as the driver, forward and reverse hybridization was performed. Differentially expressed genes were linked with pGEM-Teasy cloning vector and transformed into E. coli DH5alpha. The positive clones were screened by blue and white spot. PCR were used to amplify these genes.
RESULTThe subtractive cDNA libraries related with apoptosis of NB4 cells were successfully constructed.
CONCLUSIONThe constructed subtractive libraries are suitable for further study on the functional genes associated with apoptosis ofNB4 cells induced by arsenic trioxide.

Objective To evaluate the efficacy and safety of 0.03% tacrolimus ointment for the treatment of atopic dermatitis (AD) in Chinese children. Methods A total of 139 children, 2 to17 years of age, with moderate to severe AD from 5 study centres were enrolled in this multicentre, randomized, double-blind, vehicle-controlled, parallel group study. Treatment with 0.03% tacrolimus ointment or vehicle was applied twice daily to the affected areas for 3 weeks. Visits were scheduled on day 1 (base line, before treatment) and 1, 2, 3 weeks after the treatment. The main therapeutic parameter was the efficacy rate at the end of the treatment. 9.Application of standardized patient teaching based on Calgary-Cambridge communication model in nurse patient communication training for junior college interns

Objective:To explore the effect of applying standardized patient teaching based on Calgary-Cambridge communication model in nurse patient communication training for junior college interns, and to enrich the teaching methods of nurse patient communication training in domestic medical institutions.Methods:This study was a quasi-experimental study. In July 2022, 78 students were selected from 335 junior college interns in the First Affiliated Hospital of Shandong First Medical University by random coding method, and were randomly divided into the experimental group (39 students) and the control group (39 students) by lot. The control group received routine training. The experimental group received standardized patient teaching based on Calgary Cambridge communication model: teaching the key points of communication, guiding demonstration based on Calgary Cambridge communication model, and guiding reflection and exploration. The nurse patient communication ability, nurse patient communication practice skills, communication self-efficacy, and teaching satisfaction of the two groups of interns were compared between the two groups after 8 weeks of training.Results:After training, the total score of nurse patient communication ability evaluation in the experimental group was (91.41 ± 5.35) points, higher than that in the control group (88.08 ± 7.40) points, there was significant difference ( t=2.24, P<0.05); after training, the communication self-efficacy score of the experimental group was (30.21 ± 4.28) points, higher than that of the control group (27.94 ± 5.09) points, there was significant difference ( t=2.09, P<0.05); the total score of communication practice skills in the experimental group was (173.59 ± 18.48) points, higher than that in the control group (158.44 ± 15.57) points, there was significant difference ( t=3.82, P<0.05); the total score of communication teaching and training satisfaction in the experimental group was (16.77 ± 2.94) points, higher than that in the control group (15.22 ± 1.90) points, and there was significant difference ( t=2.68, P<0.05). Conclusions:The standardized patient teaching based on Calgary Cambridge communication model can effectively improve the practical skills of nurse patient communication of junior college interns, and promote the improvement of their nurse patient communication self-efficacy, which is conducive to the improvement of nurse patient communication ability junior college intern.
10.Preliminary study of the avian influenza knowledge-attitude-practice scale for Community Residents

Objective To develop the avian influenza Knowledge-Attitude-Practice ( KAP ) scale for community residents, and provide an objective evidence tool for community nursing. Methods The scale was based on the frame work of KAP theory and reference to the relevant literature and expert′s advice. A total of 454 community residents in Fuzhou were selected by convenient sampling and we analyzed the scale reliability and validity. Results The scale was composed of 3 domains and 46 items. The text-retest reliability of three domains were 0. 784, 0. 907 and 0. 881. The Cronbach α value for the overall scale was 0. 744 and the each domains was between 0. 678 and 0. 732. The content validity index (CVI) for the overall scale was 0. 78 and the each domain ranged from 0. 75 to 0. 84. Exploratory factor analysis revealed 17 common factors, which account for 59. 839% of the total variance. Conclusions The structure of the scale is in line with theory. The scale has good stability and reliability, therefore it can be used as a KAP scale of the avian influenza in community residents as survey tool.
